Abstract

Background: Autosomal dominant polycystic kidney disease (ADPKD) is the most common monogenic cause of renal failure. For several decades, ADPKD was regarded as an adult-onset disease. In the past decade, it has become more widely appreciated that the disease course begins in childhood. However, evidence-based guidelines on how to manage and approach children diagnosed with or at risk of ADPKD are lacking. Also, scoring systems to stratify patients into risk categories have been established only for adults. Overall, there are insufficient data on the clinical course during childhood. We therefore initiated the global ADPedKD project to establish a large international pediatric ADPKD cohort for deep characterization. Methods: Global ADPedKD is an international multicenter observational study focusing on childhood-diagnosed ADPKD. This collaborative project is based on interoperable Web-based databases, comprising 7 regional and independent but uniformly organized chapters, namely Africa, Asia, Australia, Europe, North America, South America, and the United Kingdom. In the database, a detailed basic data questionnaire, including genetics, is used in combination with data entry from follow-up visits, to provide both retrospective and prospective longitudinal data on clinical, radiologic, and laboratory findings, as well as therapeutic interventions. Discussion: The global ADPedKD initiative aims to characterize in detail the most extensive international pediatric ADPKD cohort reported to date, providing evidence for the development of unified diagnostic, follow-up, and treatment recommendations regarding modifiable disease factors. Moreover, this registry will serve as a platform for the development of clinical and/or biochemical markers predicting the risk of early and progressive disease. Abstract

The number of immigrants and refugees in the United States is growing, yet many trainees and clinicians feel unprepared to manage the diverse needs of this population. This perspective piece describes the development of the Immigrant Partnership and Advocacy Curricular Kit (I-PACK) by the Midwest Consortium of Global Child Health Educators. I-PACK is an adjunct to the Consortiums sugarprep.org global health curricular materials. Using Kerns six-step approach to curriculum development, they developed eight modules in immigrant and refugee health that incorporate interactive learning activities. The I-PACK was launched as an open-access resource in September 2020. As of September 2021, the curriculum has been freely available at sugarprep.org/i-pack and downloaded from educators in 15 countries. The I-PACK curriculum can address a growing need in medical education to empower learners and clinicians to provide competent and compassionate care for immigrants and refugees.

Abstract

This brief communication analyzes the statistics and methods Lotka used to derive his inverse square law of scientific productivity from the standpoint of modern theory. It finds that he violated the norms of this theory by extremely truncating his data on the right. It also proves that Lotka himself played an important role in establishing the commonly used method of identifying power-law behavior by the R^2 fit to a regression line on a log-log plot that modern theory considers unreliable by basing the derivation of his law on this very method., Comment: 12 pages, 5 figures, 2 tables

Abstract

This paper presents proof that Google Scholar (GS) can construct documentary sets relevant for evaluating researchers' works. Nobelists in economics were the researchers under analysis, and two types of tests of the GS cites to their works were performed: distributional and semantic. Distributional tests found that the GS cites to the laureates' works conformed to the power-law model with an asymptote or "tail" conterminous with their h-index demarcating their core oeuvre, validating both GS and the h-index. Semantic tests revealed that their works highest in GS cites were on topics for which they were awarded the prize.

Due to differences in the disciplines and prizes, it finds that the work of chemistry Nobelists is better incorporated than that of Fields medalists.

Abstract

ObjectivesChronic endobronchial infections with Pseudomonas aeruginosa contribute to bronchiectasis and progressive loss of lung function in patients with cystic fibrosis. This study aimed to evaluate the therapeutic potential of a novel macrocyclic peptide, rhesus θ-defensin-1 (RTD-1), by characterizing its in vitro antipseudomonal activity and in vivo efficacy in a murine model of chronic Pseudomonas lung infection.MethodsAntibacterial testing of RTD-1 was performed on 41 clinical isolates of P. aeruginosa obtained from cystic fibrosis patients. MIC, MBC, time-kill and post-antibiotic effects were evaluated following CLSI-recommended methodology, but using anion-depleted Mueller-Hinton broth. RTD-1 was nebulized daily for 7 days to cystic fibrosis transmembrane conductance regulator (CFTR) F508del-homozygous mice infected using the agar bead model of chronic P. aeruginosa lung infection. In vivo activity was evaluated by change in lung bacterial burden, airway leucocytes and body weight.ResultsRTD-1 exhibited potent in vitro bactericidal activity against mucoid and non-mucoid strains of P. aeruginosa (MIC90 = 8 mg/L). Cross-resistance was not observed when tested against MDR and colistin-resistant isolates. Time-kill studies indicated very rapid, concentration-dependent bactericidal activity of RTD-1 with ≥3 log10 cfu/mL reductions at concentrations ≥4× MIC. No post-antibiotic effect was observed. In vivo, nebulized treatment with RTD-1 significantly decreased lung P. aeruginosa burden (mean difference of -1.30 log10 cfu; P = 0.0061), airway leucocytes (mean difference of -0.37 log10; P = 0.0012) and weight loss (mean difference of -12.62% at day 7; P

Abstract

Logarithmic transformation of the data has been recommended by the literature in the case of highly skewed distributions such as those commonly found in information science. The purpose of the transformation is to make the data conform to the lognormal law of error for inferential purposes. How does this transformation affect the analysis? We factor analyze and visualize the citation environment of the Journal of the American Chemical Society (JACS) before and after a logarithmic transformation. The transformation strongly reduces the variance necessary for classificatory purposes and therefore is counterproductive to the purposes of the descriptive statistics. We recommend against the logarithmic transformation when sets cannot be defined unambiguously. The intellectual organization of the sciences is reflected in the curvilinear parts of the citation distributions, while negative powerlaws fit excellently to the tails of the distributions.

29. Open Doors, Closed Doors: Home-School Partnerships in a Large Bronx Elementary School.

Abstract

Although efforts to support family-school partnerships are a prominent part of strategies for improving education for at-risk students, in communities with wide home/school cultural differences, such "partnerships" often operate on a superficial level involving one-way compliance to school norms. This study examined home-school partnerships in a New York City school, focusing on two families whose children attended the same fifth grade classroom in 1996-97. Members of the Ruiz Vega family and the Arjay family were interviewed and visited at home several times. Clarissa Ruiz had severe reading problems that were overlooked at school because she did not call attention to herself. The study maintains that her parents were not the aggressive advocates that school personnel believed necessary because of their own poor school experiences, lack of skills, and the intimidation of parents inherent in school practices. Luther Arjay had considerable difficulty in completing homework. His mother and school personnel identified the reason for the difficulty and implemented a solution. Mrs. Arjay had a great deal of support from a parent-teacher program sponsored by Lehman College in Luther's fifth grade and was determined to enroll him in the best middle school she could find. The study concluded that the attitudinal changes required to foster the mutual respect necessary to form effective partnerships must be deep and widespread, and must involve transforming the whole school culture. At this school, other reform efforts openly contradicted the effort to foster school-family partnerships. Thus, some families found the door to partnership open, and others found it closed. Abstract

This publication examines a study of cultural exchange within a fifth grade classroom, presenting case studies of three children and their families and the relationships they developed with the teacher, the ethnographer, and the school community. The study examined teachers, students, and families who drew lessons from interacting with one another and using those lessons to improve and enrich the ways in which they approached the world. Researchers examined interchange in the classrooms and school communities, focusing on occasions when students brought their cultural perspectives into the collective discourse or teachers represented their own world views or the knowledge of institutional culture, their sense of school, to students or families. The school under study was located in a very poor district, and most of the students were Hispanic or African American. After regularly observing one fifth grade classroom and visiting with students and families, the researcher concluded that each student had a unique story, though in most of the stories, the process of interchange unfolded in a similar pattern (setting the stage, building trust relationships, getting to know each other, building bridges, and self-expression at school). The process of cultural exchange did not proceed nearly as far with some students as it did with others. (Contains 32 references.) (SM)

Abstract

In the wake of emerging antimicrobial resistance, antibacterial drug development has become more critical. At the same time, development of antibacterial drugs targeting specific pathogens or resistance phenotypes that may have low prevalence presents challenges because it is difficult to conduct large, randomized controlled trials for such drugs. Animal models have increasingly supported clinical development of antibacterials; however, more work is needed to optimize the design and application of these animal models to ensure clear and actionable translation to further human investigation. This review discusses recent case studies of animal infection models used to support antibacterial drug development in order to illuminate considerations for future development of novel antibacterial drugs.

Abstract

Background. Radical surgeries for phlegmons, abscesses, necrotizing infection are often followed by extensive wounds which require an individual approach to their rapid closure by means of plastic surgery methods. In such a case, the optimal wound therapy is primary healing with the use of vacuum therapy.Cases description. The study involved three clinical trials of patients with acute purulent-necrotic diseases of soft tissues of various localizations, volumes and grades, for which vacuum therapy was used as a part of complex therapy. Surgical interventions are proved to be urgently performed in acute purulent-necrotic diseases. Early reconstructive skin and bone plastic surgery aimed at closing post-operative wound defects is also very important.Conclusion. In the demonstrated clinical cases, the purulent process was stopped and wound therapy was performed by means of primary healing, which undoubtedly ensures medical and social rehabilitation of such patients. Notably, vacuum drainage is not an independent method, but an important component in the complex active surgical treatment of purulent wounds.

35. Direct Assessment of a Progressive Public Elementary School: Graduates of Central Park East.

Abstract

This report provides an assessment of the Central Park East (CPE) elementary school (in New York City) effort, an effort by teachers who believed that the "progressive education" that worked for the children of the wealthy and privileged in many private and suburban schools could work equally well for poor and working-class children. Study data suggest that graduates of the Central Park East elementary school achieved substantially higher high school completion and college entrance rates than did the population selected for comparison--the total New York City public school population. The report identifies three dimensions (mentioned by the students themselves) for this success. First, teachers helped students develop pathways to learning, starting from interests that students brought from home or discovered in the classroom. Second, members of the school staff, in their personal interactions with students, demonstrated that they cared about the students as human beings. Finally, although CPE was established by teachers searching for an opportunity to put their own ideas about teaching and learning into action, the role of parents as partners and allies increased steadily. A personal account of one student is presented to emphasize these points. Contains 44 references. (GLR)

39. Surgical disconnection of high delineated immature debilitating jejunal fistulas using posterolateral access

Abstract

Objective. The aim of the study was to develop the safest technique for surgical disconnection of high, unformed jejunal fistulas, so as to avoid problems with detection the intestine leading to the fistula and to avoid complications of viscerolysis.Material and methods. The authors present their experience in surgical management of 35 patients with high delimited unformed debilitating jejunal fistulas. Of these, 22 patients were included in the comparison group; their fistulas were closed or disconnected with the known methods. Findings obtained from other 172 patients with adhesive intestinal obstruction after infectious complications of laparotomic wounds and topography of visceroparietal planar adhesions of the peritoneum were analyzed. After studying the nature and location of visceroparietal adhesions, the authors could develop a technique for proximal disconnection of high delimited unformed debilitating jejunal fistulas using posterolateral access which was applied in 13 patients from the studied group.Research results. The proposed technique of unilateral disconnection of high unformed jejunal fistulas reduces duration, damage and risk of prolonged bowel deserosing. As a result, a significant decrease in postoperative mortality from 59.1 ± 9.2% to 23.1 ± 11.2% (t=2.5; pConclusion. The posterolateral access prevented extended viscerolysis and allowed to put a reliable and atraumatic interintestinal anastomosis so as to disconnect the fistula. The unilateral disconnection of the jejunal fistula at postoperative intestinal paresis unloaded the interintestinal anastomosis, similar to Meidl’s definitive jejunostomy. The abovementioned positive aspects of the proposed surgical intervention allowed to significantly decrease postoperative complications and mortality rate, if to compare with outcomes after known surgical interventions with a similar purpose.

Abstract

Allergic bronchopulmonary aspergillosis and invasive fungal diseases represent distinct infectious entities that cause significant morbidity and mortality. Currently, administered inhaled antifungal therapies are unapproved, have suboptimal efficacy, and are associated with considerable adverse reactions. The emergence of resistant pathogens is also a growing concern. Inhaled antifungal development programs are challenged by inadequate nonclinical infection models, highly heterogenous patient populations, low prevalence rates of fungal diseases, difficulties defining clinical trial enrollment criteria, and lack of robust clinical trial endpoints. On 25 September 2020, the US Food and Drug Administration (FDA) convened a workshop with experts in pulmonary medicine and infectious diseases from academia, industry, and other governmental agencies. Abstract

The number of immigrants and refugees in the United States is growing, yet many trainees and clinicians feel unprepared to manage the diverse needs of this population. This perspective piece describes the development of the Immigrant Partnership and Advocacy Curricular Kit (I-PACK) by the Midwest Consortium of Global Child Health Educators. I-PACK is an adjunct to the Consortium’s sugarprep.org global health curricular materials. Using Kern’s six-step approach to curriculum development, they developed eight modules in immigrant and refugee health that incorporate interactive learning activities. The I-PACK was launched as an open-access resource in September 2020. As of September 2021, the curriculum has been freely available at sugarprep.org/i-pack and downloaded from educators in 15 countries. The I-PACK curriculum can address a growing need in medical education to empower learners and clinicians to provide competent and compassionate care for immigrants and refugees.
