PURPOSE COVID-19 is a condition that can lead to other chronic conditions. These conditions are frequently diagnosed in the primary care setting. We used a novel primary care registry to quantify the burden of post-COVID conditions among adult patients with a COVID-19 diagnosis across the United States. METHODS We used the American Family Cohort, a national primary care registry, to identify study patients. After propensity score matching, we assessed the prevalence of 17 condition categories individually and cumulatively, comparing patients having COVID-19 in 2020-2021 with (1) historical control patients having influenza-like illness in 2018 and (2) contemporaneous control patients seen for wellness or preventive visits in 2020- 2021. RESULTS We identified 28,215 patients with a COVID-19 diagnosis and 235,953 historical control patients with influenza-like illness. The COVID-19 group had higher prevalences of breathing difficulties (4.2% vs 1.9%), type 2 diabetes (12.0% vs 10.2%), fatigue (3.9% vs 2.2%), and sleep disturbances (3.5% vs 2.4%). There were no differences, however, in the postdiagnosis monthly trend in cumulative morbidity between the COVID-19 patients (trend = 0.026; 95% CI, 0.025-0.027) and the patients with influenza- like illness (trend = 0.026; 95% CI, 0.023-0.027). Relative to contemporaneous wellness control patients, COVID-19 patients had higher prevalences of breathing difficulties and type 2 diabetes. CONCLUSIONS Our findings show a moderate burden of post-COVID conditions in primary care, including breathing difficulties, fatigue, and sleep disturbances. Based on clinical registry data, the prevalence of post-COVID conditions in primary care practices is lower than that reported in subspecialty and hospital settings. Background: Studies that were carried out previously on learning outcomes focused mainly on the student's cognitive domain while identifying factors that predicted it. More so, most of the learner's assessments in school are largely dependent on the score obtained from specific subjects by the learner, and efforts to address other domains of instruction such as affective and psychomotor domains have been minimal or absent in regard to the variables selected for the study. This study therefore sought to address that gap by finding out the relative and composite contribution of academic optimism and capital indicators to the learning outcomes (of students. Methods: The study adopted a correlational design with a multistage sampling technique to select a total of 534 senior secondary class II students. Two research instruments, the Academic Optimism and Capital Indicators Scale (AOCIS) and the Learning Outcomes Scale (LOS), were used for data collection. Exploratory and confirmatory factors analysis were used to assess the dimensionality of the items and factor structure of the scales. The psychometric properties obtained for scale were adequate for the instrument to be adjudged valid and reliable.The collected data were analysed using the hierarchical regression approach (HRA). Results: The findings of the study revealed that academic emphasis, collective efficacy, faculty trust, social capital, economic capital, and cultural capital, relatively and jointly, predict overall learning outcomes (cognitive, affective, and psychomotor construct). The result showed that there was an increased proportion of variance with each addition of a predictor to the model. Social capital reduced the percentage change at the initial time, but with the addition of economic capital, the proportion of change increased among others in the subsequent model examination. Conclusion: /implication: The study provides knowledge and empirical evidence that academic optimism and capital indicators, with their dimensions, affect composite learning outcomes among students. This study will help school ministries, policymakers, and curriculum planners make sure that the educational objectives, philosophies, and programmes are planned to reflect the total learner in order to produce the total learner that will effect changes in the society. This study has provided evidence that academic inputs and capital indicators are crucial indices of their learning outcomes in the three area of learning. The ability of the school to emphasise academics, ensure that all school agents are committed to instructional delivery, and gain the trust of parents is crucial for adequate support to enhance students learning outcomes. The outcome has implication for policy development and providing a climate that can stimulate equity, trust and motivation.

Purpose: This study aims to find out whether the use of MOODLE in English lessons as a tool for blended instruction makes a significant difference to the students' language journey. Research Methods: This is quasi-experimental research by nature, utilizing a mixed-method approach as the most appropriate tradition. In this study, 44 students ranging in age from 16 to 18 selected on the basis of a convenience sampling technique participated. After a fifteen-week treatment of MOODLE with the experimental group, paired samples T-test was applied to see if there was any difference between the two groups. A questionnaire and semi-structured interviews were also conducted to gain a deeper insight into students' experience with MOODLE. Findings: Quantitative data revealed that there was statistically significant difference between the experimental group and the control group, suggesting potential contribution of MOODLE to learners' language achievement in blended EFL lessons. In the same vein, the qualitative data affirmed that most of the students were satisfied with using MOODLE to support English lessons. Implications for Research and Practice: Since there is a statistically significant difference between the 1st and 2nd English exam scores of the students in the experimental group (t=-3.085 sig=0.005), MOODLE proves to be an effective online learning tool supporting blended learning. The findings of the study also provide empirical evidence for integrating blended-instruction with MOODLE in foreign EFL lessons and valuable information to the Ministry of National Education (MONE) about employing blended-instruction with MOODLE in EFL classes.

The Stanford Population Health Sciences Data Ecosystem was created to facilitate the use of large datasets containing health records from hundreds of millions of individuals. This necessitated technical solutions optimized for an academic medical center to manage and share high-risk data at scale. Through collaboration with internal and external partners, we have built a Data Ecosystem to host, curate, and share data with hundreds of users in a secure and compliant manner. This platform has enabled us to host unique data assets and serve the needs of researchers across Stanford University, and the technology and approach were designed to be replicable and portable to other institutions. We have found, however, that though these technological advances are necessary, they are not sufficient. Challenges around making data Findable, Accessible, Interoperable, and Reusable remain. Our experience has demonstrated that there is a high demand for access to real-world data, and that if the appropriate tools and structures are in place, translational research can be advanced considerably. Together, technological solutions, management structures, and education to support researcher, data science, and community collaborations offer more impactful processes over the long-term for supporting translational research with real-world data.

The COVID-19 pandemic represented a huge obstacle for public health and demonstrated weaknesses in surveillance and health promotion systems around the world. Its etiological agent, SARS-CoV-2, of zoonotic origin, has been the target of several studies related to the control and prevention of outbreaks and epidemics of COVID-19 not only for humans but also for animals. Domestic animals, such as dogs and cats, have extensive contact with humans and can acquire the infection both naturally and directly from humans. The objective of this article was to summarize the seroprevalence findings of SARS-CoV-2 in dogs and cats and correlate them with the strength of infection risk between each of them. This is a systematic review and meta-analysis following the recommendations of PRISMA 2020. The search and selection of papers was carried out using in vivo experimental works with animals using the descriptors (MeSH/DeCS) “Animal”, “Public Health”, “SARS-CoV-2” and “Pandemic” (together with AND) in English, Portuguese or Spanish for Science Direct, PUBMED, LILACS and SciELO databases. The ARRIVE checklist was used for methodological evaluation and the Comprehensive Meta-Analysis v2.2 software with the Difference Risk (RD) test to evaluate statistical inferences (with subgroups by continent). Cats showed greater susceptibility to SARS-CoV-2 compared to dogs both in a joint analysis of studies (RD = 0.017; 95% CI = 0.008–0.025; p < 0.0001) and in the American subgroup (RD = 0.053; 95% CI = 0.032–0.073; p < 0.0001), unlike the lack of significant difference on the European continent (RD = 0.009; 95% CI = −0.001–0.018; p = 0.066). Therefore, it was observed that cats have a greater interest in health surveillance due to the set of biological and ecological aspects of these animals, but also that there are a set of factors that can influence the spread and possible spillover events of the virus thanks to the anthropozoonotic context.

Many stool-based gut microbiome studies have highlighted the importance of the microbiome. However, we hypothesized that stool is a poor proxy for the inner-colonic microbiome and that studying stool samples may be inadequate to capture the true inner-colonic microbiome. To test this hypothesis, we conducted prospective clinical studies with up to 20 patients undergoing an FDA-cleared gravity-fed colonic lavage without oral purgative pre-consumption. The objective of this study was to present the analysis of inner-colonic microbiota obtained non-invasively during the lavage and how these results differ from stool samples. The inner-colonic samples represented the descending, transverse, and ascending colon. All samples were analyzed for 16S rRNA and shotgun metagenomic sequences. The taxonomic, phylogenetic, and biosynthetic gene cluster analyses showed a distinctive biogeographic gradient and revealed differences between the sample types, especially in the proximal colon. The high percentage of unique information found only in the inner-colonic effluent highlights the importance of these samples and likewise the importance of collecting them using a method that can preserve these distinctive signatures. We proposed that these samples are imperative for developing future biomarkers, targeted therapeutics, and personalized medicine.

BackgroundClinicians in areas where malaria and typhoid fever are co-endemic often treat infected patients irrationally, which may lead to the emergence of drug resistance and extra cost to patients. This study determined the proportion of febrile conditions attributable to either malaria and/or typhoid fever and the susceptibility patterns of Salmonella spp. isolates to commonly used antimicrobial agents in Ghana.MethodsOne hundred and fifty-seven (157) febrile patients attending the Ga West Municipal Hospital, Ghana, from February to May 2017 were sampled. Blood samples were collected for cultivation of pathogenic bacteria and the susceptibility of the Salmonella isolates to antimicrobial agents was performed using the Kirby-Bauer disk diffusion method with antibiotic discs on Müller Hinton agar plates. For each sample, conventional Widal test for the detection of Salmonella spp was done as well as blood film preparation for detection of Plasmodium spp. Data on the socio-demographic and clinical characteristics of the study participants were collected using an android technology software kobo-collect by interview.ResultsOf the total number of patients aged 2-37 years (median age = 6 years, IQR 3-11), 82 (52.2%) were females. The proportion of febrile patients with falciparum malaria was 57/157 (36.3%), while Salmonella typhi O and H antigens were detected in 23/157 (14.6%) of the samples. The detection rate of Salmonella spp in febrile patients was 10/157 (6.4%). Malaria and typhoid fever coinfection using Widal test and blood culture was 9 (5.7%) and 3 (1.9%), respectively. The isolates were highly susceptible to cefotaxime, ceftriaxone, ciprofloxacin, and amikacin but resistant to ampicillin, tetracycline, co-trimoxazole, gentamicin, cefuroxime, chloramphenicol, and meropenem.ConclusionPlasmodium falciparum and Salmonella spp coinfections were only up to 1.9%, while malaria and typhoid fever, individually, were responsible for 36.3% and 6.4%, respectively. Treatment of febrile conditions must be based on laboratory findings in order not to expose patients to unnecessary side effects of antibiotics and reduce the emergence and spread of drug resistance against antibiotics.

Abstract Background Artisanal mining creates enabling breeding ground for the vector of malaria parasites. There is paucity of data on the effects of artisanal mining on malaria. This study assessed burden of malaria and caregivers’ health-seeking behaviour for children under five in artisanal mining communities in East Akim District in Ghana. Methods A cross-sectional study involving caregivers and their children under five was conducted in three artisanal mining communities in the East Akim District in Ghana. Caregivers were interviewed using a structured questionnaire. Finger prick blood samples were collected and analysed for haemoglobin concentration using a rapid diagnostic test, and thick and thin blood smears were analysed to confirm the presence of malaria parasites. Results Of the 372 children under 5 years included in the study, 197 (53.1%) were male, with a mean age (± SD) of 23.0 ± 12.7 months. En febrero del 2020, la Organización Mundial de la Salud alertó de una infodemia masiva, es decir, de una gran cantidad de información que en algunos casos puede ser correcta (en otros casos no), y que puede dificultar a que las personas encuentren fuentes confiables y orientación fiable cuando las necesiten. La infodemia se propaga extremadamente rápido, particularmente con el auge de las redes sociales. La infodemiología es la ciencia de la distribución y los determinantes de la información en medios electrónicos, como la Internet, o en una población, con el fin de informar sobre la salud y las políticas públicas. Durante el periodo del 2002 al 2021 se registraron 484 publicaciones sobre infodemiología en la base de datos MEDLINE (vía PubMed), y se observó que el 71.9 % de los trabajos (348/484) se publicaron durante los años 2020 y 2021. Así como se puede controlar que las epidemias se propaguen rápidamente si se actúa a tiempo, también se puede mitigar la infodemia. Sin embargo, existen desafíos que agregan una carga adicional en el manejo de la pandemia, y por ello urge la necesidad de educar a la población e implementar programas de formación a los trabajadores de salud para gestionar eficientemente la infodemia. El objetivo del artículo especial es describir los desafíos para la gestión de la infodemia en salud en tiempos de la COVID-19.

Ecosystem services (ES) are benefits nature provides to humans; these services change in space and time and are largely dependent on context. Coastal habitat that provides key ES are blue carbon ecosystems, namely seagrass and mangroves. One important ES they provide is the provisioning of seafood, which benefits coastal populations with livelihoods and food security. We employed a social-ecological approach that draws from the vulnerability literature for social, ecological, and economic criteria to map ES provision in ten communities on Busuanga Island, Palawan Province, Philippines. We assess the spatial dynamics of ES provision for small-scale fisheries in seagrass and mangroves, in relation to local beneficiaries. Using a mixed-methods approach with ecological assessments of seagrass beds, spatial analysis, landing surveys, household and key informant interviews, we overlaid biophysical variables on social data, mapping sensitivities and adaptive capacities to compare communities’ social vulnerabilities. Spatial analysis revealed healthy blue carbon ecosystems in ten local communities (barangays) as measured by proportion of coastline covered, low patchiness and high continuity along the coastline, and the presence of adjacent habitat. We found seagrass ecosystems were more vulnerable than mangroves. Rural barangays had less exposure and lower sensitivity to blue carbon ecosystem loss than urban barangays. Blue carbon ecosystem fisheries are highly sensitive fisheries, due to their catch composition and low catch per unit effort, with mangrove fisheries having a slightly lower sensitivity than seagrass fisheries due to greater catch per unit effort. Diversified livelihoods and the presence of NGOs and People’s Organizations (POs) increased adaptive capacity and reduced overall vulnerability. We aim to highlight a coastal human community’s relationship with blue carbon ecosystems using context-specific vulnerability criteria. Our site-specific social vulnerability assessment may be adapted for use in other coastal communities within the coral triangle. This work suggests opportunities for conservation interventions to manage local communities’ sensitivities and adaptive capacity around the use of blue carbon ecosystems.

Namun tidak ada dilakukannya evaluasi Tata Kelola TI yang sudah dijalankan selama 3 tahun perusahaan beroperasi, oleh karena itu sangat perlu untuk dilakukannya evaluasi dan penilaian Tata Kelola TI di perusahaan. Penelitian pada kali ini bertujuan untuk mengukur tingkat kapabilitas pada Tata Kelola TI di PT XYZ dengan menggunakan Process Assesment Model (PAM) COBIT 5, dengan menggunakan data primer yaitu data wawancara serta data sekunder melalui pengamatan lingkungan kerja dan dokumen organisasi. Berdasakan hasil dari penelitian serta kesepakatan pihak perusahaan proses domain yang akan diukur adalah proses BAI01 Mengelola Program dan Proyek. Hasil pegukuran tingakat kapabilitas pada proses BAI01 Mengelola Program dan Proyek adalah berada pada level 2 (managed process). Atherosclerosis is a progressive disease characterized by chronic inflammation of the arterial walls, associated with genetic and infectious factors. The present study investigated the involvement of Chlamydia trachomatis and Chlamydia pneumoniae infections and immunological markers (C-reactive protein, CRP, TNF-α, IL-6, IL-8, and IL-10) in the process of atherosclerosis. The evaluation included 159 patients for surgical revascularization (CAD) and 71 patients for surgical heart valve disease (HVD) at three hospitals in Belém, Brazil. The control group (CG) comprised 300 healthy individuals. Blood samples collected before surgery were used for antibodies detection (enzyme immunoassay), CRP (immunoturbidimetry) and IL-6 levels (enzyme immunoassay). Tissue fragments (atheroma plaque, heart valve and ascending aorta) were collected during surgery and subjected to qPCR for detection of bacterial DNA. Promoter region polymorphisms of each marker and relative quantification of TNF-α, IL-8, and IL-10 gene expression were performed. Demography and social information were similar to the general population involved with both diseases. Antibody prevalence to C. trachomatis was 30.6, 20.3, and 36.7% (in the CAD, HVD, and CG, respectively) and to C. pneumoniae was 83.6, 84.5, and 80.3% (in the CAD, HVD, and CG, respectively). C. trachomatis cryptic plasmid DNA was detected in 7.4% of the samples. Frequency of IL6−174G>C polymorphism was higher in CAD and HVD than in CG regardless of previous exposure to Chlamydia. Previous C. trachomatis infection showed involvement in HVD and CAD. Significant association between disease and previous C. pneumoniae infection was found only among HVD. GG genotype of IL6−174G>C is apparently a risk factor for heart disease, whereas AT genotype of IL8−251A>T was mainly involved in valvulopathies, including patients with prior exposure to C. pneumoniae.
