Abstract

Introduction: There is not much information about the care of infants with hypoxic-ischaemic encephalopathy (HIE) treated with therapeutic hypothermia (TH) in Spain. This includes whether protocols are routinely used, the type of neuro-monitoring performed, and how information on the neurological prognosis is presented to families. The answers to these would allow to detect and implement areas of improvement. Method: A cross-sectional analysis was performed on the responses to structured questionnaires sent to all the Spanish neonatal units that were performing TH in June 2015. Questions were divided into 5 sections: (1) the availability of protocols and technological resources, (2) the use of neuro-monitoring tools, (3) the knowledge and training of the professionals; (4) the prognostic information given to the parents; and (5) the discharge report and the follow-up plan. Results: Most centres (95%) use servo controlled whole-body cooling methods and have specific management protocols. Sedation is used in 70% of centres, and in 68% of them the onset of enteral feeding is delayed until the end of the cooling period. Amplitude-integrated electroencephalography monitoring is used in more than 80% of the centres, although only in 50% are nurses able to interpret it. Cerebral oxygen saturation is not often monitored (16%). As regards diagnostic-prognostic studies, neuroimaging is universal, but brain damage biomarkers are hardly used (29%). Prognostic information is offered within the first 72 posnatal hours in 21% of the centres, and is given without the presence of the nurse in 70% of the centres. Follow-up is performed by a neuro-paediatrician (84%), with an uneven duration between centres. Conclusions: The care of infants with HIE treated with TH in Spain is generally adequate, although there are areas for improvement in neuromonitoring, sedation, prognostic information, teamwork, and duration of follow-up. Método: Estudio transversal de los 57 hospitales españoles que realizaban HT en 2015, mediante cuestionario sobre: 1) la disponibilidad de protocolos y de recursos tecnológicos; 2) el uso de herramientas de neuromonitorización; 3) los conocimientos de los profesionales; 4) la información pronóstica que se da los padres, y 5) el informe al alta y del plan de seguimiento. Resultados: El 95% utiliza enfriamiento corporal-total servocontrolado y dispone de protocolos específicos de actuación. El 70% utiliza sedación y el 68% deja al paciente a dieta absoluta. La monitorización con electroencefalografía integrada por amplitud se utiliza en más del 80% de los centros, aunque solo en el 50% la enfermera es capaz de interpretarlo. La saturación de oxígeno cerebral es escasamente monitorizada (16%). Entre los estudios diagnóstico-pronósticos, la neuroimagen es universal, pero los neurobiomarcadores apenas se utilizan (29%). Solo el 21% ofrece información pronóstica antes de las 72 h de vida; sin presencia de la enfermera en el 70%. El seguimiento lo realiza el neuropediatra (84%), con una duración desigual entre centros. Conclusiones: La asistencia del RN con EHI en España es adecuada, con áreas de mejora en: neuromonitorización, sedación, marco temporal de la información pronóstica, trabajo en equipo y estandarización de la duración del seguimiento.

Abstract

Introduction: The process of care and assistance from birth to the starting of therapeutic hypothermia (TH) is crucial in order to improve its effectiveness and prevent the worsening of hypoxic-ischaemic injury. Methods: A national cross-sectional study carried out in 2015 by use of a questionnaire sent to all level III units on the care of the newborn ≥ 35 weeks gestation within the first hours of life after a perinatal asphyxia event. According to clinical practice guidelines, the quality of care was compared between the hospitals that carried out or did not carry out TH, and according to the level of care. Results: A total of 89/90 hospitals participated, of which 57/90 performed TH. They all used resuscitation protocols and turned off the radiant warmer after stabilisation. All of them performed glucose and blood gas analysis, monitored the central temperature, put the newborn on a diet, and performed at least two examinations for the diagnosis of hypoxic-ischaemic encephalopathy. Greater than one-third (35%) of hospitals did not have amplitude-integrated electroencephalogram, and 6/57 were TH-hospitals. The quality of care among hospitals with and without TH was similar, childbirth being better in those that performed TH, and those with a higher level of care. Level IIIc hospitals had higher scores than the others. The TH-hospitals mentioned not always having neonatologists with experience in neurological assessment and interpretation of amplitude-integrated electroencephalogram (25%), or in brain ultrasound (62%). Conclusions: In response to the recommendations of the asphyxiated newborn, there is a proper national health care standard with differences according to the level of care and whether TH is offered. More amplitude-integrated electroencephalogram devices are necessary, as well as more neonatologists trained in the evaluations that will be required by the newborn with hypoxic-ischaemic encephalopathy. Resumen: Introducción: El proceso asistencial hasta el inicio de la hipotermia terapéutica (HT) es crucial para mejorar su efectividad y prevenir el agravamiento del daño hipóxico-isquémico. Método: Estudio transversal nacional realizado en 2015 mediante cuestionario a todas las unidades nivel iii sobre la asistencia al recién nacido (RN) con asfixia perinatal en las primeras horas de vida. Se comparó la calidad asistencial entre los hospitales que realizaban o no HT y según el nivel asistencial, de acuerdo a las guías de práctica clínica. Resultados: Participaron 89/90 hospitales, 57/90 realizaban HT. Todos utilizaban protocolos de reanimación y apagaban la cuna tras estabilización. Fue universal realizar medición de glucemia y gasometría, monitorizar la temperatura, dejar al RN a dieta y realizar al menos 2 exploraciones para el diagnóstico de encefalopatía hipóxico-isquémica. El 35% no disponía de electroencefalograma integrado por amplitud; 6/57 eran hospitales que realizaban HT. La calidad asistencial entre los hospitales con/sin HT fue similar, siendo mejor la del parto en los que hacían HT, y la de aquellos con mayor nivel asistencial. El 25% de aquellos que realizaban HT no tenían neonatólogos con experiencia en la exploración neurológica o en la interpretación del electroencefalograma integrado por amplitud; ni en la realización de ecografía cerebral en el 62%. Conclusiones: Atendiendo a las recomendaciones del RN asfíctico, existe un adecuado estándar asistencial nacional, con diferencias según el nivel asistencial y si realizan o no hipotermia. Son necesarios más equipos de electroencefalograma integrado por amplitud y formación de los neonatólogos en las evaluaciones que requerirá el RN con encefalopatía hipóxico-isquémica.

14. KomaMRI.jl: An Open-Source Framework for General MRI Simulations with GPU Acceleration

Abstract

Purpose: To develop an open-source, high-performance, easy-to-use, extensible, cross-platform, and general MRI simulation framework (Koma). Methods: Koma was developed using the Julia programming language. Like other MRI simulators, it solves the Bloch equations with CPU and GPU parallelization. The inputs are the scanner parameters, the phantom, and the pulse sequence that is Pulseq-compatible. The raw data is stored in the ISMRMRD format. For the reconstruction, MRIReco.jl is used. A graphical user interface utilizing web technologies was also designed. Two types of experiments were performed: one to compare the quality of the results and the execution speed, and the second to compare its usability. Finally, the use of Koma in quantitative imaging was demonstrated by simulating Magnetic Resonance Fingerprinting (MRF) acquisitions. Results: Koma was compared to two well-known open-source MRI simulators, JEMRIS and MRiLab. Highly accurate results (with MAEs below 0.1% compared to JEMRIS) and better GPU performance than MRiLab were demonstrated. In an experiment with students, Koma was proved to be easy to use, eight times faster on personal computers than JEMRIS, and 65% of them recommended it. The potential for designing acquisition and reconstruction techniques was also shown through the simulation of MRF acquisitions, with conclusions that agree with the literature. Conclusions: Koma's speed and flexibility have the potential to make simulations more accessible for education and research. Koma is expected to be used for designing and testing novel pulse sequences before implementing them in the scanner with Pulseq files, and for creating synthetic data to train machine learning models., Comment: 13 pages, 7 figures, 2 tables

First-pass perfusion cardiac magnetic resonance (FPP-CMR) is becoming an essential non-invasive imaging method for detecting deficits of myocardial blood flow, allowing the assessment of coronary heart disease. Nevertheless, acquisitions suffer from relatively low spatial resolution and limited heart coverage. Compressed sensing (CS) methods have been proposed to accelerate FPP-CMR and achieve higher spatial resolution. However, the long reconstruction times have limited the widespread clinical use of CS in FPP-CMR. Deep learning techniques based on supervised learning have emerged as alternatives for speeding up reconstructions. However, these approaches require fully sampled data for training, which is not possible to obtain, particularly high-resolution FPP-CMR images. Here, we propose a physics-informed self-supervised deep learning FPP-CMR reconstruction approach for accelerating FPP-CMR scans and hence facilitate high spatial resolution imaging. 28. Teachers' Beliefs about the Role of Digital Educational Resources in Educational Practice: A Qualitative Study

Abstract

Information and Communication Technologies and Digital Educational Resources have undergone a rapid evolution and have been swiftly introduced into educational contexts. Teachers play a key role in integrating these technological resources into the classroom. The objective of the present study was to determine the value that teachers attribute to digital resources in their educational practice. Based on a qualitative methodology, the necessary information was obtained via an open-ended interview, in which a Spanish school's Early Childhood and Primary Education teachers participated. The results revealed that teachers value the integration of digital resources into the classroom, though no consensus was reached as to the suitable level of integration. Use satisfaction was mainly related to student motivation. Certain problems or limitations also came to light, however, linked to students' digital training. An important conclusion according to the perception of teachers is that the integration of digital resources in their educational practice was significant and improved the quality of the educational process.

Abstract

ABSTRACT Quantitation of cytomegalovirus (CMV) DNA load in specimens other than blood such as bronchoalveolar lavages, intestinal biopsies, or urine has become a common practice as an ancillary tool for the diagnosis of CMV pneumonitis, intestinal disease, or congenital infection, respectively. Nevertheless, most commercially available CMV PCR platforms have not been validated for CMV DNA detection in these specimen types. In this study, a laboratory-developed test based on Alinity m CMV (“Alinity LDT”) was evaluated. Reproducibility assessment using spiked bronchial aspirate (BAS) or urine samples showed low standard deviations of 0.08 and 0.27 Log IU/mL, respectively. Evaluating the clinical performance of Alinity LDT in comparison to a laboratory-developed test based on RealTime CMV (“RealTime LDT”) showed good concordance across 200 clinical specimens including respiratory specimens, intestinal biopsies, urine, and stool. A high Pearson's correlation coefficient of r = 0.92, a low mean bias of −0.12 Log IU/mL, a good qualitative agreement of 90%, and a Cohen's kappa value of 0.76 (substantial agreement) were observed. In separate analyses of the sample types BAS, tracheal aspirates, bronchoalveolar lavage, biopsies, and urine, the assay results correlated well between the two platforms with r values between 0.88 and 0.99 and a bias

Los mantenidos por Gregorio Mayans y Siscar a lo largo de su vida con innumerables corresponsales constituyen el mejor ejemplo de la rentabilidad que se puede obtener del análisis de esta fuente histórica. En los más de treinta volúmenes publicados, hasta la fecha, por Antonio Mestre y sus colaboradores sobre la correspondencia del ilustrado de Oliva, hallamos numerosas referencias al problema ocasionado por el paludismo o fiebres tercianas en la España del siglo XVIII. En este artículo se efectúa una primera aproximación a esta cuestión tras analizar varios de sus epistolarios.

Abstract

Abstract Background There is limited evidence over the real‐life secukinumab survival in psoriatic patients, especially in the long‐term bio‐naïve subjects, and to date biological treatments have been assessed in patients with a chronic and recalcitrant psoriasis. It has been hypothesised that early intensive treatment with biological therapies could decrease pathogenic tissue resident memory (TRm) cells migration and potentially modify the natural course of psoriasis and related comorbidities. Objectives To analyze long‐term secukinumab efficacy, safety and survival, and its predictive factors for psoriasis treatment determining also whether early intervention may result in better outcomes for patients. Methods Bio‐naïve psoriatic patients under treatment with secukinumab (n = 128 patients) in a daily practice setting were analyzed in a retrospective multicentric study and followed up to 8 years. Drug survival rate, efficacy, posology and safety were reported. Results The overall secukinumab survival was 81.9% for an average treatment exposure of 147.9 weeks. The approved posology was the most commonly prescribed regimen (78.7%), and 17.7% could be optimized. An absolut‐PASI ≤3 was reached by 86.6%, 82.6% and 91.7% at the weeks 48,156 and 264, respectively. The incidence of arthritis or psoriasis related comorbidities was low. The response was not influenced by weight, age (>65), gender or the presence of arthritis. No severe adverse events were reported. Conclusions In our bio‐naïve series, the high efficacy and long‐term survival rates observed and the low prevalence of arthritis and comorbidities might suggest that early intervention could contribute to modify the course of the disease, but further studies are needed.

Abstract

. How can universities enhance their students’ internationalisation at home? The need to find teaching methodologies that boost students’ intercultural communication skills without travelling abroad has motivated the present study, carried out by two English lecturers from Florida Universitaria in Spain and Jade University of Applied Sciences in Germany. Research was divided into two stages: first, a deep analysis of the intercultural experiences organised in the past at both universities; and second, the implementation of virtual encounters with students from different cultures as tools to promote interculturality at the home university. This paper aims at showing the impact of those exchanges on boosting students’ intercultural communication in English. Two groups of undergraduate students, during their English classes, worked synchronously on topics related to essential aspects of intercultural communication. The main teaching objectives of the Virtual Intercultural Communication (VIC) exchange were to increase participants’ motivation for knowing other cultures, to raise their intercultural awareness, to foster their capacity to change perspectives, and to improve students’ English language skills and their digital abilities. Through the outcomes achieved, students’ self-reported feedback, and lecturers’ observation, it can be concluded that virtual exchanges are effective tools to improve students’ intercultural communication while they meet the English learning objectives.
