Background. Nowadays, hematology is a dynamically developing science due to the in-depth study of the molecular mechanisms of a particular disease. A better understanding of oncohematological diseases biology makes it possible to synthesize new targeted drugs, which have a favorable therapeutic effect. In particular, in multiple myeloma, after the introduction of proteasome inhibitors and immunomodulatory drugs into clinical practice, an improvement in overall survival was observed. However, characteristics of the mechanisms of transformation normal plasma cells into malignant ones are still difficult; therefore, the study of the pathobiological basis of multiple myeloma is currently an urgent task.The objective: to evaluate the possible influence of MAGE-C1 gene expression and the presence of mage-c1 protein in patients with newly diagnosed multiple myeloma on the anti-tumor response after bortezomib-containing therapy.Materials and methods. A prospective study included 33 multiple myeloma patients. The diagnosis was established according to International Myeloma Working Group criteria (IMWG, 2014). In 32 patients the induction therapy included bortezomib-containing courses, in one patient lenalidomide was included in the first-line regimens. The MAGE-C1 gene expression by real-time polymerase chain reaction and mage-c1 protein by immunohistochemistry in plasma cells bone marrow, were determined for all patients at the debut of multiple myeloma. As a control group was examined the bone marrow material of healthy donors.Results. When assessment the statistical relationship between the expression of MAGE-C1 gene and mage-c1 protein, it was found that there was no high expression of mage-c1 protein at low values of MAGE-C1 gene expression. At the same time, high expression of the gene was always associated with protein expression above normal values. The analysis aimed at finding the relationship between MAGE-C1 gene and mage-c1 protein detection and the degree of antitumor response after 6 courses of induction therapy showed that high expression of the studied parameters was associated with a worse response to bortezomib-containing treatment.Conclusion. We confirmed that the results of the two methods were comparable. Single factor analysis showed that patients with decreased MAGE-C1 gene and mage-c1 protein expression levels achieved a significantly higher antitumor response to bortezomib-containing regimens, while high expression was accompanied by refractoriness to bortezomib.

Food safety and quality are especially important in the dairy industry. Mass spectrometry is an effective tool of state control in this sphere. The research objective was to study the prospects for gas mass spectrometry with smart mathematical processing in assessing the composition and quality of dairy products. The study featured 11 samples of fresh and acidified yoghurts from different manufacturers and with various starters, functional food additives, etc. These samples and their packaging were evaluated using a small-sized quadrupole gas mass spectrometer MS7-200 with electron impact ionization developed at the Institute for Analytical Instrumentation, Russian Academy of Sciences. The data obtained were mathematically processed by the method of principal components. Based on the ratios of peak intensities at m/z = 55, 56, 57, 58, 59, 60, 61, 64, 67, 69, 70, 71, 72, 73, 74, 84, 85, and 88 Da, the fresh and expired samples were classified not only by the degree of freshness, but also by the composition and quality of the microbiological starters, raw materials, food additives, etc. In addition, some packaging materials proved to be of poor quality. In this study, the method of gas mass spectrometry was supplemented by the authentic methods for selecting gas emissions from yoghurts and packaging, accelerated acidification, and smart mathematical processing. The approach proved to be time-saving, sensitive, selective, available, and cost-effective. As a result, it demonstrated good potential as a means to control the composition and quality of dairy products and their packaging.

Aim. To evaluate the effectiveness and safety of Extimia® BIOCAD (international non-proprietary name: empegfilgrastim) to reduce the incidence and duration of neutropenia, the incidence of febrile neutropenia (FN) and infections associated with FN in patients with lymphoproliferative diseases receiving myelosuppressive therapy. Materials and methods. The paper presents the results of a multicenter retrospective-and-prospective observational post-marketing study of the safety and effectiveness of Extimia® BIOCAD (international non-proprietary name: empegfilgrastim) in patients with lymphoproliferative diseases receiving cytotoxic therapy. Initially, the study was defined as retrospective-and-prospective. However, only 2 of 671 (0.3%) patients were included retrospectively. Thus, the study is based on the evaluation of prospectively collected data. The analysis describes the characteristics and treatment of 671 patients diagnosed with lymphoma who received one or more cycles of chemotherapy in the LEGERITY study. The endpoints of interest included the incidence of grade 3–4 neutropenia, FN, and grade 3–4 infectious complications, the frequency of antibiotic therapy, and the incidence of all adverse reactions in patients who received at least one dose of the study medication. Results. A total of 671 patients were enrolled in the study, the majority having B-cell non-Hodgkin lymphoma (428/671, 64%) and Hodgkin lymphoma (211/671, 31%). The mean age was 54 years (18–84); 35% (236/671) of patients were elderly (over 60). The number of injections of empegfilgrastim for the entire observation period was 3093 (n=671). Adverse events were reported in 57/3093 (1.8%) cases. The most common adverse events were mild to moderate ossalgia and myalgia (27/671, 4%), back pain, and arthralgia (12/671, 1.8%), which did not require drug therapy. Two patients developed serious adverse reactions – CTCAE v.5 grade 4 hypotension events. 529 (78.8%) patients received at least 4 courses of chemotherapy. FN was reported in 14 (2.6%) patients and did not require antimicrobial therapy. Conclusion. The final analysis results confirm the high clinical efficacy and safety of the Russian original pegylated granulocyte colony-stimulating factor empegfilgrastim (Extimia®) in patients with indolent and aggressive lymphomas. The data obtained from real-world clinical practice demonstrate a favorable safety and tolerability profile of empegfilgrastim in all age groups, including the elderly population.

Introduction. When designing pipeline crossings for various purposes, the solution of many engineering problems is associated with the calculation of velocity distribution and estimation of hydraulic resistance created by them. Since it is very difficult to estimate the value of hydrodynamic resistance coefficients and lifting force by theoretical means, experimental studies are usually resorted to. Pipelines can be positioned in different ways in relation to the flow, and in practice pipelines are also buried in the bottom of the watercourse. In physical experiments, a partially buried pipeline is often modelled by truncating the buried section of the pipeline. This experimental setup is more suitable for a pipeline located in an impermeable bottom. In reality, erosion-prone bottoms are often porous and permeable.Materials and methods. Hydrodynamic forces acting on a pipeline with uneven depth on both sides, on a permeable bottom, are numerically studied. Two-dimensional Navier – Stokes equations averaged over Reynolds with k–e turbulence model are used to simulate fluid flow. The seepage flow at the permeable bottom is assumed to obey Darcy’s law, the Laplace equation is solved to calculate the pore pressure assuming an isotropic and homogeneous bottom. The flow structure and pressure distribution around the pipeline are considered. The ANSYS Fluent software package is used for numerical modelling.Results. It was found that the flow structure around the pipeline is asymmetric due to the difference in bottom levels on the two sides of the pipeline. The process of scouring of the sandy bottom in the area of the pipeline location was modelled in ANSYS Fluent. Comparison of calculation results at different flow rates was made. It was found that there is an obvious difference between the hydrodynamic forces experienced by the pipeline due to the asymmetric flow structure around the pipeline.Conclusions. Peak values of external forces and lift force decreasing as the value of depth into the bottom behind the pipeline increases (e2/D). The maximum error of the drag and lift forces calculated using sixth order Fourier series is about 4 %.

Quality assurance of transfusion media is a fundamental principle of the functioning of all blood service institutions, allowing guaranteeing the safety of the recipient. An integral part of the procuring process of blood components is their laboratory testing, which results reliability and reproducibility can be proved by carrying out of validation of analytical methods. The aim of the study was to define the rules for planning and carrying out validation tests of methods for quality control of blood components, including verification of their compliance with established acceptance criteria. Material and methods. The analysis of the literature about the safety of blood components and the quality of laboratory tests was carried out with an assessment of the applied significance of the information proved. Results and discussion. As a result of the work done, a unified model for confirming of the accuracy of determining of the safety indicators of transfusion media was created, which contributes to improving of the quality of donor biomaterial and the effectiveness of therapy. Conclusions. The presented validation design is relevant for medical organizations working in the sphere of blood donation and its components.

Aim. To assess effectiveness and safety of Extimia BIOCAD (INN: empegfilgrastim) used to decrease the rate and duration of neutropenia, the rate of febrile neutropenia and infections that manifest in febrile neutropenia in patients with lymphoproliferative diseases who receive myelosuppressive therapy. Materials and methods. The paper presents the results of the second interim analysis of multicenter retrospective-and-prospective observational post-marketing study of effectiveness and safety of Extimia BIOCAD (INN: empegfilgrastim) in patients with lymphoproliferative diseases who receive cytotoxic therapy. The second stage of the interim analysis describes patient characteristics, therapy used in 221 patients with morphologically confirmed lymphoma who received one or more cycles of chemotherapy as part of LEGERITY study. The endpoints of interest included the rate of Grade 3/4 neutropenia in patients, after the first cycle of any therapy line; the rate of febrile neutropenia; the rate of Grade 3/4 infectious complications; the rate of antibacterial therapy prescriptions; and the assessment of the relative dose-intensity of received chemotherapy. Additionally, the incidence rate of all adverse drug reactions (ADRs) was assessed in patients who received at least one dose of the study drug; the incidence rate of all serious ADRs was assessed in patients who received at least one dose of the study drug; the incidence rate of Common Terminology Criteria for Adverse Events (CTCAE) 5.0 Grade 3/4 ADRs in patients who received at least one dose of the study drug; the rate of study drug discontinuations due to ADRs was also assessed. Results. As of the second interim analysis, LEGERITY included 221 patients with various indolent and aggressive lymphomas. Median age of the patients was 53 years (1982). A group of older patients (over 60 years of age) accounted for 34% of the study population. Patients received 1 injection of Extimia per chemotherapy cycle. Grade 3/4 neutropenia was registered in 6.7% (n=14) patients. Overall, all-grade neutropenia was reported in 21.4% (n=44) patients. Febrile neutropenia was reported in 2.9% cases. Severe infections and the use of antimicrobials were reported in no patients throughout the CT period and after each cycle of therapy. Most commonly reported adverse reactions included mild-to-moderate ossalgias (6.8%) and myalgias, back pain, and arthralgias (3.2%), that did not require pharmaceutical therapy. One (0.5%) patient had a severe adverse drug reaction a CTCAE 5.0 Grade 4 hypotension episode. Conclusion. Interim analysis results support high effectiveness and safety of the Russian original pegylated granulocyte colony-stimulating factor of empegfilgrastim (Extimia) in patients both with indolent and aggressive lymphomas. Real world evidence demonstrates a favourable safety and tolerability profile of empegfilgrastim in all age groups, including in the aging population. As of the moment of the interim analysis publication, the study is ongoing. Final conclusions on the safety and effectiveness of empegfilgrastim (Extimia) are to be drawn upon the study completion.

Background. The relevance of the work is due to the medical sorting of victims in emergency situations, providing them with medical assistance directly in the emergency zone, their medical evacuation in conditions of time shortage accompanying rescue operations, as well as the risks that rescuers and doctors are exposed to when rescuing victims. The aim of the work is to develop an information management medical system for remote correction of the health status of the victim in emergency situations. Materials and methods. An approach to the construction of an information management medical system for remote correction of the health status of victims in emergency situations is presented. Results. The scheme of construction is presented and the main elements of the proposed information management medical system are presented. The purpose and functionality of the unmanned aircraft included in the system are presented. An approach is described to the advance preparation of medical institutions for the placement and provision of medical care to the victims delivered to them through information exchange with the service for optimizing therapeutic and diagnostic medical care under the control of the server of the proposed system. Conclusions. The use of the proposed information and control medical system for remote correction of the health status of victims in emergency situations allows to reduce the time allotted for the provision of medical care and evacuation of victims, as well as to carry out these measures with minimal involvement of doctors and rescuers in the area of rescue operations.

Aim. To assess the efficacy and safety of using the drug Extimia BIOCAD (international nonproprietary name INN: empegfilgrastim) in order to reduce the frequency and duration of neutropenia, the frequency of febrile neutropenia (FN) and infections manifested by FN in patients with lymphoproliferative diseases receiving myelosuppressive therapy. Materials and methods. This publication presents the interim results of a multicenter retrospective prospective observational post-marketing study of the safety and efficacy of the drug Extimia BIOCAD (INN: empegfilgrastim) in patients with lymphoproliferative diseases receiving cytotoxic therapy (LEGERITY). The interim data analysis included 40 patients with lymphoproliferative diseases (Hodgkins lymphoma, diffuse large B-cell lymphoma, multiple myeloma, primary mediastinal large B-cell lymphoma, follicular lymphoma, chronic lymphocytic leukemia, splenic marginal zone lymphoma), who were treated in ten research centers of the Russian Federation (Moscow, St. Petersburg, regional clinics). The median age of patients was 48 (2172) years, 13/40 (32.5%) patients belonged to the older age group (60 years). Patients had functional status on the ECOG scale of 02 and received at least 2 chemotherapy injections against the background of prophylaxis with empegfilgrastim. Empegfilgrastim was administered at a dose of 7.5 mg subcutaneously once 24 hours after the end of the administration of cytotoxic chemotherapeutic agents. Primary endpoint: frequency of neutropenia 35 degrees of severity; secondary endpoints: frequency of FN; frequency of severe infections (34 stages); frequency of antibiotic prescription; relative dose intensity of therapy of the conducted chemotherapy courses; the incidence of all adverse reactions in patients who received at least one dose of the study drug empegfilgrastim; the incidence of all serious adverse reactions in patients who received at least one dose of the study drug empegfilgrastim; the incidence of CTCAE 5.0 grade 34 HP in patients who received at least one dose of the study drug empegfilgrastim; discontinuation rate of study drug empegfilgrastim due to adverse reactions. Results. The results of this study demonstrate that the incidence of neutropenia of 3 degree of severity after the 1st cycle of chemotherapy developed in 2/40 patients (5%) and as a result of high-dose therapy (R-DHAP). Neutropenia of any severity was reported in 5/40 patients (12.5%). Cases of FN development have not been registered. Severe infections (mucositis, enteropathy, pneumonia, etc.), as well as the use of antibacterial and antifungal drugs during 1 cycle of chemotherapy and in the inter-course period after 1 cycle of therapy were not recorded in any patient. The next course of myelosuppressive therapy was not delayed due to the development of neutropenia in any of the patients during the study. Adverse events, according to the researcher, associated with the use of empegfilgrastim, were registered in 2/40 patients (5%): moderate generalized pain syndrome (diffuse pain) of 1 severity and in one case ossalgia of 2 severity. No serious adverse reactions were reported. Conclusion. The results of the interim analysis of the study demonstrate the high efficacy of the first Russian original pegylated granulocyte colony-stimulating factor empegfilgrastim after a single administration of a fixed dose in the treatment of patients with aggressive and indolent lymphomas. The drug has a favorable tolerance profile in any age group of patients, especially in elderly patients. Administration of empegfilgrastim as a prophylaxis of neutropenia in patients with lymphoproliferative diseases receiving myelosuppressive therapy of varying intensity can reduce the burden on medical personnel, improve patient adherence to treatment, and contribute to the implementation of the therapeutic plan.

The article examines the concept of citizen science and theoretical approaches to assessing the effectiveness of e-participation projects. In the stack of smart agriculture tools and technologies, citizen science is an approach that has great potential as a tool that can provide timely and relevant data to improve the well-being of the agricultural sector, by keeping the entire population engaged and informed. The study will be of interest to government and business representatives involved in the development, implementation and improvement of existing information and communication technologies for socio-political participation of citizens.

An expedition was carried out to take soil samples of Eastern Donbass and other districts of the Rostov region. The main goal was to study the distribution of potentially toxic elements (PTE) in soils. The total content of mineral components in solid-phase samples was determined by using the X-ray fluorescence method. It was found that, according to the average concentrations, PTE`s line up in the following series: Fe>Mn>Zn>Cr>V>Cu>Ni>Pb>Co. The contents of Zn and Cu in the soil were compared by seasons. It was determined that PTE concentrations in the upper layer of soils were higher in winter than in summer. Geochemical spectra were constructed. They demonstrated that the concentration Clarks of Cr, Cu, Zn, Co, Pb exceeded the Clarks of these elements in the Earth’s crust. Against this background, Pb stands out with contrast, the concentration in the soil of which exceeds those in the earth’s crust by 2.8 times. The tightness of the relationship between the content of PTE in soil samples was calculated, which turned out to be a high straight line.

Background. The object of the study is to ensure the safety of the flight of unmanned aircraft as part of a heterogeneous group in the emergency zone, designed for the localization and elimination of emergency situations. The subject of the study is the proposed zones, or areas of security, which are built in a basic coordinate system, the beginning of which is tied to a certain point in the emergency zone, around each unmanned aircraft or potentially dangerous object. The purpose of the work is to develop a concept of application (and recommendations for use), including the construction of zones or security areas around each unmanned aircraft and potentially dangerous objects, taking into account the relative position of these zones or security areas, the intersection of which is unacceptable during the flight of an unmanned aircraft, thereby ensuring the protection of an unmanned aircraft from collision with obstacles. Materials and methods. To describe the process of transition from local coordinate systems associated with a specific unmanned aircraft to the base coordinate system, homogeneous transformation matrices are used. The concept of security spheres, as well as adaptive security spheres, dynamic radii of adaptive security spheres is introduced. Results. An approach to ensuring the protection of unmanned aircraft moving as part of a heterogeneous group in an emergency zone from collisions with obstacles, including each other, is proposed. This approach can be used to organize the movement and interaction of objects operating as part of multi-agent systems. Conclusions. The methods used for the transition from local coordinate systems linked to a particular unmanned aircraft (UAV) to the basic coordinate system allow you to organize the movement of a heterogeneous group of UAVs in the emergency zone. The developed concept and recommendations for the construction of an emergency zone, zones, or security areas around each operating as part of a heterogeneous group of UAVs and around potentially dangerous objects, as well as for taking into account the mutual location of these zones, or security areas, protect each UAV from collisions with obstacles, and also increase the traffic safety of a heterogeneous group of UAVs as a whole.
