Abstract

Objective To evaluate the impact of coadministering statins with direct oral anticoagulants (DOACs) on the risk of major bleeding events in patients with venous thromboembolism (VTE).Design Observational cohort analysis based on a multicentre international registry.Setting Data were extracted from the Registro Informatizado de Enfermedad TromboEmbolica Registry, which involves 205 centres across 27 countries.Participants A total of 73 659 patients diagnosed with VTE were classified based on their anticoagulant therapy (DOACs) versus low-molecular-weight heparin (LMWH) or vitamin K antagonists (VKAs) and concurrent use of statins.Methods Multivariable Cox proportional hazards models adjusted for confounding variables to assess the risk of major bleeding events stratified by the type of anticoagulant use and statin use.Results From October 2013 to February 2023, 73 659 patients were recruited: 2573 were statin users on DOACs, 14 090 were statin users on LMWH or VKA therapy, 10 088 were non-statin users on DOACs and 46 908 were non-statin users on LMWH or VKA therapy. Statin users were 10 years older and more likely to have hypertension, diabetes, renal failure or prior artery disease. During anticoagulation (median, 187 days), 1917 patients (2.6%) suffered major bleeding. Rates of major bleeding per 100 patient-years were 2.33 (95% CI 1.72 to 3.09), 3.75 (95% CI 3.43 to 4.10), 1.39 (95% CI 1.13 to 1.69) and 3.10 (95% CI 2.93 to 3.27), respectively. On multivariable analysis, patients treated with DOACs had a significantly lower risk of major bleeding compared with those on LMWH or VKA therapy (adjusted HR 0.59; 95% CI 0.48 to 0.74). The adjusted HR in statin users versus non-users was 1.03 (95% CI 0.92 to 1.14), while in statin users on DOACs versus the rest of patients, it was 1.18 (95% CI 0.79 to 1.76).Conclusions In patients with VTE receiving statins, long-term anticoagulation with DOACs was associated with a reduced risk of major bleeding, regardless of the statin use. These findings support the safety profile of DOACs over VKAs or LMWH in the management of VTE in patients requiring statins.

Abstract

This paper presents an optoelectronic design for high-definition imaging in low-light underwater environments. By integrating a high-performance camera with a custom-designed electronic system, we obtain a photogrammetry system capable of being integrated into an ROV or operated by divers to obtain high quality images of marine habitats and species. Abstract

Background: With the arrival of disease-modifying treatments, it is mandatory to find new cognitive markers that are sensitive to Alzheimer's disease (AD) pathology in preclinical stages. Objective: To determine the utility of a newly developed Learning and Associative Memory face test: LAM test. This study examined the relationship between AD cerebrospinal fluid (CSF) biomarkers and performance on LAM test, and assessed its potential clinical applicability to detect subtle changes in cognitively healthy subjects at risk for AD. Methods: We studied eighty cognitively healthy volunteers from the Valdecilla cohort. 61% were women and the mean age was 67.34 years (±6.416). All participants underwent a lumbar puncture for determination of CSF biomarkers and an extensive neuropsychological assessment, including performance on learning and associative memory indices of the LAM-test after 30 min and after 1 week, and two classic word lists to assess verbal episodic memory: the Rey Auditory Verbal Learning Test (RAVLT) and the Free and Cued Selective Reminding Test (FCSRT). We analyzed cognitive performance according to amyloid status (A+ versus A–) and to ATN model (A–T–N–; A+T–N–; A+T+N–/A+T+N+). Results: Performance on the LAM-test was significantly correlated with CSF Aβ ratio. A+ participants performed worse on both learning (mean difference = 2.19, p = 0.002) and memory LAM measures than A– (mean difference = 2.19, p = 0.004). A decline in performance was observed along the Alzheimer's continuum, with significant differences between ATN groups. Conclusions: Our findings suggest that LAM test could be a useful tool for the early detection of subjects within the AD continuum, outperforming classical memory tests. [ABSTRACT FROM AUTHOR]

Abstract

Abstract In this work, a novel optical fiber sensor capable of measuring both the liquid level and its refractive index is designed, manufactured and demonstrated through simulations and experimentally. For this, a silica capillary hollow-core fiber is used. The fiber, with a sensing length of 1.55 mm, has been processed with a femtosecond laser, so that it incorporates four holes in its structure. In this way, the liquid enters the air core, and it is possible to perform the sensing through the Fabry–Perot cavities that the liquid generates. The detection mode is in reflection. With a resolution of 4 μm (liquid level), it is in the state of the art of this type of sensor. The system is designed so that in the future it will be capable of measuring the level of immiscible liquids, that is, liquids that form stratified layers. It can be useful to determine the presence of impurities in tanks.

Abstract

3D reconstruction and virtual reality (VR) technology provide many opportunities for the documentation and dissemination of underwater cultural heritage. Advances in the development of underwater exploration technology have allowed for the first time to accurately reconstruct a complete 3D model of the cargo Río Miera in the Cantabrian Sea. Sunk on December 6, 1951 after a strong collision, the cargo ship Río Miera rests on a sandy bottom about 40 meters deep, very close to the Cantabrian coast. Located in an area of strong currents is a classic objective of the region for the most experienced divers. The survey was carried out this summer in R/V Ramón Margalef of the IEO, acquiring acoustic data with multibeam echo sounders and hundreds of images acquired by a remotely piloted underwater vehicle. The campaign is part of the PhotoMARE project - Underwater Photogrammetry for MArine Renewable Energy. This work describes the workflow regarding the survey, images and acoustic data acquisition, data processing, optic 3D point cloud color enhancement and acoustic and optic dataset merging procedure to obtain a complete 3D model of wreck Río Miera in Cantabrian Sea. Through this project, Spanish Institute of Oceanography – IEO have advanced – combining acoustic and image methods - in the generation of 3D models of archaeological sites and submerged structures.

Abstract

Background: The use of low-molecular-weight heparin (LMWH) for long-term therapy of venous thromboembolism (VTE) in patients without cancer has not been consistently evaluated. Methods: We used the data in the RIETE registry to compare the 3-month outcomes (VTE recurrences, major bleeding or death) in non-cancer patients with VTE, according to long-term therapy with LMWH or vitamin K antagonists (VKAs). Results: As of March 2018, 14,582 non-cancer patients with VTE had received initial therapy with LMWH and then switched to VKAs, while 9151 were prescribed LMWH for initial and long-term therapy. Overall, 11,494 had initially presented with pulmonary embolism (PE) and 12,239 with isolated deep vein thrombosis (DVT). Among 11,494 patients initially presenting with PE, 84 had VTE recurrences, 204 major bleeding and 406 died. Among 12,239 patients with isolated DVT, 133 developed VTE recurrences, 137 bled and 289 died. On propensity score analysis, PE patients on long-term LMWH therapy were at increased risk for PE recurrences (OR: 3.30; 95%CI: 1.67–6.48), major bleeding (OR: 1.68; 95%CI: 1.21–2.32) or death (OR: 3.16; 95%CI: 2.43–4.09) compared with those receiving VKAs. In patients with DVT, those on long-term LMWH also were at increased risk for PE recurrences (OR: 2.31; 95%CI: 1.13–4.73), major bleeding (OR 2.28; 95%CI: 1.51–3.44) or death (OR: 2.32; 95%CI: 1.54–3.51). Conclusions: In the RIETE non-cancer patients with VTE, long-term therapy with VKAs was associated with a lower risk for recurrences, major bleeding or death.

Abstract

Thanks to their lower losses and sharper resonances compared to their metallic counterparts, all-dielectric metasurfaces are attracting a quickly growing research interest. The application of such metasurfaces in the field of refractive index sensing is extremely attractive, especially due to the expected high performance and the simplicity of the sensing element excitation and readout. Herein, we report on an all-dielectric silicon metasurface based on complementary split-ring resonators (CSRRs) optimized for refractive index sensing. A quasi-bound state in the continuum (quasi-BIC) with an ultra-high quality factor can be excited in the near-infrared (NIR) window by violating the structure symmetry. By using the three-dimensional finite element method (3D-FEM), a refractive index sensor for biomedical applications with an ultra-high figure of merit (FoM > 100,000 RIU−1) has been designed, exploiting the quasi-BIC resonance. The proposed design strategy opens new avenues for developing flat biochemical sensors that are accurate and responsive in real time.

Abstract

In this invited review, we provide an overview of the recent advances in biomedical photonic sensors within the last five years. This review is focused on works using optical-fibre technology, employing diverse optical fibres, sensing techniques, and configurations applied in several medical fields. We identified technical innovations and advancements with increased implementations of optical-fibre sensors, multiparameter sensors, and control systems in real applications. Examples of outstanding optical-fibre sensor performances for physical and biochemical parameters are covered, including diverse sensing strategies and fibre-optical probes for integration into medical instruments such as catheters, needles, or endoscopes.

One of their main virtues is that, based on their data and by applying machine learning algorithms and techniques, it is possible to perform body tracking and make an accurate three-dimensional representation of body movement. Specifically, this paper will use the Kinect v2 device, which incorporates a random forest algorithm for 25 joints detection in the human body. However, although Kinect v2 is a powerful tool, there are circumstances in which the device’s design does not allow the extraction of such data or the accuracy of the data is low, as is usually the case with foot position. We propose a method of acquiring this data in circumstances where the Kinect v2 device does not recognize the body when only the lower limbs are visible, improving the ankle angle’s precision employing projection lines. Using a region-based convolutional neural network (Mask RCNN) for body recognition, raw data extraction for automatic ankle angle measurement has been achieved. All angles have been evaluated by inertial measurement units (IMUs) as gold standard. For the six tests carried out at different fixed distances between 0.5 and 4 m to the Kinect, we have obtained (mean ± SD) a Pearson’s coefficient, r = 0.89 ± 0.04, a Spearman’s coefficient, ρ = 0.83 ± 0.09, a root mean square error, RMSE = 10.7 ± 2.6 deg and a mean absolute error, MAE = 7.5 ± 1.8 deg. For the walking test, or variable distance test, we have obtained a Pearson’s coefficient, r = 0.74, a Spearman’s coefficient, ρ = 0.72, an RMSE = 6.4 deg and an MAE = 4.7 deg.

Abstract

The consolidation of laser micro/nano processing technologies has led to a continuous increase in the complexity of optical fiber sensors. This new avenue offers novel possibilities for advanced sensing in a wide set of application sectors and, especially in the industrial and medical fields. In this review, the most important transducing structures carried out by laser processing in optical fiber are shown. The work covers different types of fiber Bragg gratings with an emphasis in the direct-write technique and their most interesting inscription configurations. Along with gratings, cladding waveguide structures in optical fibers have reached notable importance in the development of new optical fiber transducers. That is why a detailed study is made of the different laser inscription configurations that can be adopted, as well as their current applications. Microcavities manufactured in optical fibers can be used as both optical transducer and hybrid structure to reach advanced soft-matter optical sensing approaches based on optofluidic concepts. These in-fiber cavities manufactured by femtosecond laser irradiation followed by chemical etching are promising tools for biophotonic devices. Finally, the enhanced Rayleigh backscattering fibers by femtosecond laser dots inscription are also discussed, as a consequence of the new sensing possibilities they enable.

Abstract

Imaging fibers are used to perform real-time fluorescence endomicroscopy, in vivo, in situ, with the goal of increasing diagnostic information for a plethora of organ systems and diseases. Widefield fiber endomicroscopy systems are simple, cost-effective, and come with fast image acquisition times. However, alternative approaches such as scanning systems produce higher contrast images with intrinsic optical sectioning, improving the visibility of histological features, albeit at the expense of simplicity, cost, and acquisition rate. We developed a selective plane illumination microscopy endoscopic fiber platform, consisting of an ultrafast laser fabricated end-cap, integrated with a polymer coherent fiber bundle, and an epifluorescence microscope. Polymer fibers are known to fluoresce when pumped with blue light, enhancing the background and noise in images. Our end-cap design circumvents this challenge. We demonstrate a reduction of out-of-focus features, along with improved contrast of in-focus features, in images of a tissue phantom. Moreover, we demonstrate the utility of our platform for endomicroscopy using a whole, ex vivo human lung model.
