PurposeFeasibility of a highly reduced-dose lumbar spine CT protocol using iterative reconstruction (IR) in a human cadaver model.Materials and methodsThe lumbar spine of 20 human cadavers was repeatedly examined using three different reduced-dose protocols (RDCT) with decreasing reference tube current-exposure time products (RDCT-1: 50 mAs; RDCT-2: 30 mAs; RDCT-3: 10 mAs) at a constant tube voltage of 140 kV. A clinical standard-dose protocol (SDCT) served as the reference (reference tube current-exposure time product: 70 mAs; tube voltage: 140 kV). Images were reconstructed using filtered back projection (FBP) and two increasing levels of IR: IRL4 and IRL6. A five-point scale was used by two observers to assess the diagnostic quality of anatomical structures (cortical and trabecular bone, intervertebral foramina, pedicles and intervertebral joints, spinous and transverse processes). Objective image noise (OIN) was measured. Results were interpreted using a linear mixed-effects regression model.ResultsRDCT-2 with IRL6 (1.2 ± 0.5mSv) was the lowest reduced-dose protocol which provided diagnostically acceptable and equivalent image quality compared to the SDCT (2.3 ± 1.1mSV) with FBP (p > 0.05). All RDCT protocols achieved a significant reduction of the mean (±SD) effective radiation doses (RDCT-1: 1.7±0.9mSv; RDCT-2: 1.2±0.5mSv; RDCT-3: 0.4±0.2mSv; p < 0.05) compared to SDCT. OIN was lower in all RDCT protocols with the application of IRL4 and IRL6, compared to the SDCT with FBP (p < 0.05).ConclusionHighly reduced-dose lumbar spine CT providing diagnostically acceptable image quality is feasible using IR in this cadaver model and may be transferred into a clinical setting.

The aim was to determine which of the diagnostic instruments (VC, DLCO, TCT) are relevant for the early detection of asbestos-related pleural and pulmonary fibrosis. The records of 111 formerly asbestos-exposed workers with TCT, VC, DLCO were reviewed; substantial comorbidities (cardiac, malignant) and pulmonary emphysema were excluded. By that, data of 41 males (mean 69.8 years ±6.9) were included. TCT findings for pleural and pulmonary changes were coded according to ICOERD and scores were determined. In all 41 males the VC showed only minor limitations (mean 96.5 ±18.0% pred.), DLCO was slightly reduced (76.4 ±16.6%), DLCO/VA was within reference values (102 ±22%). In the TCT 27 workers showed pleural asbestos-related findings, 24 pulmonary fibrosis. Statistical analysis provided low correlations of VC (ρ = -0.12; 0.10, resp.) and moderate correlations of DLCO (-0.25, -0.23, resp.) with pleural plaque extension and ICOERD-score for pulmonary fibrosis. DLCO had the highest accuracy with 73.2% and Cohens κ with 0.45. DLCO/VA showed no correlations to ICOERD-score. A newly developed score, regarding the extent of pleurovisceral plaques, shows a moderate correlation with DLCO (ρ = -0.35, p Lung function alterations and TCT findings correlated highly and showed highest accuracy using DLCO and ICOERD-score considering parenchymal ligaments, subpleural curvilinear lines, roundatelectases and pleural effusion in addition to pleural plaque extension, also in regard to pulmonary findings. VC showed only weaker correlations. Besides TCT, the determination of VC and DLCO is mandatory for the early detection and assessment of functional asbestos-related deficiencies.

Background Asbestos-related lung diseases are one of the leading diagnoses of the recognized occupational diseases in Germany, both in terms of their number and their socio-economic costs. The aim of this study was to determine whether pulmonary function testing (spirometry and CO diffusion measurement (DLCO)) and computed tomography of the thorax (TCT) are relevant for the early detection of asbestos-related pleural and pulmonary fibrosis and the assessment of the functional deficiency. Methods The records of 111 formerly asbestos-exposed workers who had been examined at the Institute for Occupational and Maritime Medicine, Hamburg, Germany, with data on spirometry, DLCO and TCT were reviewed. Workers with substantial comorbidities (cardiac, malignant, silicosis) and/or pulmonary emphysema (pulmonary hyperinflation and/or TCT findings), which, like asbestosis, can lead to a diffusion disorder were excluded. The remaining data of 41 male workers (mean 69.8 years ±6.9) were evaluated. The TCT changes were coded according to the International Classification of High-resolution Computed Tomography for Occupational and Environmental Respiratory Diseases (ICOERD) by radiologists and ICOERD-scores for pleural and pulmonary changes were determined. Correlations (ρ), Cohens κ and accuracy were calculated. Results In all 41 males the vital capacity (VC in % of the predicted value (% pred.)) showed only minor limitations (mean 96.5 ± 18.0%). The DLCO (in % pred.) was slightly reduced (mean 76.4 ± 16.6%; median 80.1%); the alveolar volume related value (DLCO/VA) was within reference value (mean 102 ± 22%). In the TCT of 27 workers pleural asbestos-related findings were diagnosed whereof 24 were classified as pulmonary fibrosis (only one case with honey-combing). Statistical analysis provided low correlations of VC (ρ = − 0.12) and moderate correlations of DLCO (− 0.25) with pleural plaque extension. The ICOERD-score for pulmonary fibrosis correlated low with VC (0.10) and moderate with DLCO (− 0.23); DLCO had the highest accuracy with 73.2% and Cohens κ with 0.45. DLCO/VA showed no correlations to the ICOERD-score. Besides TCT the determination of both lung function parameters (VC and DLCO) is mandatory for the early detection and assessment of functional deficiencies in workers formerly exposed to asbestos.

The purpose of the study was to assess the impact of iterative model reconstruction (IMR) on reader confidence with respect to stone detection and image quality in comparison to filtered back-projection (FBP) and iDose level 4 (iDose4) in abdominal MDCT with radiation doses below 2 mSv.For 32 consecutive patients with suspected ureteral stone disease, the raw data of unenhanced 256 slice MDCT (120 kV, 40 reference mAs, mean CTDIvol: 2.7 ± 0.8 mGy, mean DLP: 126 ± 38 mGy × cm) were reconstructed using a prototype version of IMR (levels 1 - 3), iDose4 (level 4) and FBP at a 3 mm slice thickness. Image analysis was independently performed by two radiologists in a blinded fashion. The reader confidence level with respect to stone detection was recorded based on a 5-point scale (1 - certain exclusion; 5 - concrement definitely present) as well as for the evaluation of image quality regarding the depiction of anatomical details (1 - poor; 5 - excellent). A clinical reference standard for stone detection was not established. Statistical evaluation included weighted kappa analysis and Wilcoxon test.17 pelvic and ureteral stones were found. 11 further concrements were located within the ostium of the urinary bladder or the bladder itself. Applying IMR, a distinct improvement in image quality was observed at every level (mean value for FBP, 2.0; iDose4, 2.9; IMR L1, 4.2; IMR L2, 4.0; IMR L3, 3.9; all p 0.001). Applying the higher IMR levels L2 and L3, a certain level of so-called "blotchiness" of anatomical contours was observed. Reader confidence was significantly improved and was independent of IMR level (certain stone detection FBP, 69 %; iDose4, 81 %; IMR L1 to L3, 95 %; all p 0.001). With increasing IMR levels, the reduction in streak artifacts was quantified by a decrease in image noise. A loss of anatomical information was not observed. The sensitivity rates for stone detection were equivalent for all MDCTs reconstructed with FBP, iDose4 and IMR. A mean effective dose of 1.9 ± 0.6 mSv was calculated.In comparison to FBP and iDose4, a significant increase in mean image quality, reduction in image noise and improvement in subjective reader confidence can be achieved by applying IMR even at significantly reduced dose settings below 2 mSv. Bei der Anwendung von IMR wurde mit jeder Iterationsstufe eine deutliche Verbesserung der Bildqualität beobachtet (Mittelwert für FBP, 2,0, iDose4, 2,9, IMR L1, 4,2, IMR L2, 4,0, IMR L3, 3,9, alle p 0,001). Mit den höheren IMR-Stufen L2 und L3 wurde ein gewisses Maß an sogenannter „blotchiness“ (Unschärfe) anatomischer Konturen beobachtet. Die Sicherheit des Radiologen wurde so unabhängig vom IMR-Level signifikant verbessert (sichere Steindetektion FBP, 69 %; iDose4, 81 %; IMR L1 bis L3, 95 %, alle p 0,001). Mit zunehmenden IMR-Werten wurde die Reduktion von Streifenartefakten durch eine Abnahme des Bildrauschens quantifiziert. Ein Verlust anatomischen Informationen wurde nicht beobachtet. Die Sensitivität der Steindetektion waren äquivalent für alle MDCTs, die mit FBP, iDose4 oder IMR rekonstruiert wurden. PURPOSE To compare T2 * relaxation times of the tibiotalar cartilage between professional football players and matched healthy male volunteers. MATERIALS AND METHODS Twenty-two ankles of professional football players (24.3 ± 3.8 years) and 20 age- and body mass index-matched healthy individuals (25.6 ± 2.4 years) were investigated. The study protocol consisted of multiplanar T1 -weighted, fat-saturated proton-density weighted (Pdw) and a 3D multiecho T2 * sequence with 22 echo times (4.6-53.6 msec). The articular cartilage was subdivided into six segments. Regions of interest were manually drawn in three zones (lateral, central, medial). Differences and confidence intervals were estimated applying a random effects models. Fixed effects were professional football players versus healthy individuals and areas. The random effect was defined as the person cluster of the different individuals. RESULTS T2 * values were significantly prolonged in football players compared to male volunteers in all predefined cartilage segments (mean, 17.5 vs. 15.5 msec; P < 0.001). In both groups, the highest relaxation times were found in the lateral zone, with statistically higher relaxation times in professional football players (18.5 vs. 16.5 msec, P = 0.003). Separate evaluation revealed the longest relaxation times in the posterior tibiotalar cartilage, with 21.0 msec for professional football players compared to 19.4 msec for healthy volunteers (P = 0.064). CONCLUSION Based on these initial results, T2 * values of the tibiotalar cartilage seem to be elevated in professional football players compared to healthy volunteers. Prospective longitudinal studies should be encouraged to show if these results represent early subtle cartilage lesions prior to clinical manifestation or rather temporary adaptation related to daily high-level loading. Background According to the as low as reasonably achievable (ALARA) principle, radiation exposure in computed tomography (CT) should be minimized while maintaining adequate image quality. Dedicated CT protocols combined with iterative reconstruction (IR) can reduce radiation dose and/or improve image quality. Purpose To investigate the feasibility of extremely reduced-dose (RD) CT of the thoracic spine in human cadavers using a standard-dose (SD) and three different RDCT protocols reconstructed with filtered back projection (FBP) and IR. Material and Methods The thoracic spines of 11 cadavers were examined using different RDCT protocols with decreasing reference tube currents (RDCT-1: 50 mAs; RDCT-2: 30 mAs; RDCT-3: 10 mAs) at 140 kV. A clinical SDCT (70 mAs, 140 kV) served as reference. Raw data were reconstructed using FBP and two increasing levels of IR (IRL4 and IRL6). Images were evaluated for image quality, diagnostic acceptability, and visibility of anatomical structures according to a 5-point-scale. Results Regardless of the reconstruction technique, image quality was rated as diagnostically acceptable for all cadavers in SDCT and RDCT-1. Image quality of reconstructions with FBP were generally rated lower. Application of IR improved image quality ratings in SDCT and RDCT. RDCT-2 with IR was the most reduced-dose CT protocol which enabled diagnostically acceptable image quality in all cadavers. Compared to SDCT, RDCT protocols resulted in significantly reduced effective radiation doses (SDCT: 4.1 ± 1.5 mSv; RDCT-1: 2.9 ± 1.1 mSv; 2:1.7 ± 0.6 mSv; 3:0.6 ± 0.1 mSv; P = 0.001). Conclusion Diagnostically acceptable RDCT of the thoracic spine with 1.7 mSv is feasible using IR.

Recent investigation has underlined the potential of quantitative MR imaging to be used as a complementary tool for the diagnosis of cartilage degeneration at an early state. The presented study analyses T2* relaxation times of articular cartilage of the knee in professional athletes and compares the results to age- and BMI (Body Mass Index)-matched healthy amateur athletes.22 professional football players and 22 age- and BMI-matched individuals were underwent knee Magnetic Resonance Imaging (MRI) at 3T including qualitative and quantitative analysis. Qualitative analysis included e.g. meniscal tears, joint effusion and bone edema. For quantitative analysis T2* (22 ET: 4.6-53.6ms) measurements in 3D data acquisition were performed. Deep and superficial layers of 22 predefined cartilage segments were analysed. All data sets were postprocessed using a dedicated software tool. Statistical analysis included Student t-test, confidence intervals and a random effects model.In both groups, T2* relaxation times were significantly higher in the superficial compared to the deep layers (p0.001). Professional athletes had significantly higher relaxation times in eight superficial and three deep cartilage layers in the predefined cartilage segments (p0.05). Highly significant differences were found in the weight-bearing segments of the lateral superficial femoral condyle (p0.001).Elevated T2* values in cartilage layers of professional football players compared to amateur athletes were noted. The effects seem to predominate in superficial cartilage layers.

Objective To compare both image quality and visibility of normal and abnormal findings at submillisievert chest CT (smSv-CT) using filtered back projection (FBP) and the two different iterative reconstruction (IR) techniques iterative model reconstruction (IMR) and iDose 4 ™. Materials and methods This institutional review board approved study was based on retrospective interpretation of clinically indicated acquired data. The requirement to obtain informed consent was waived. 81 patients with suspected pneumonia underwent smSv-CT (Brilliance iCT, Philips Healthcare; mean effective dose: 0.86±0.2mSv). Data were reconstructed using FBP and two different IR techniques iDose 4 ™ and IMR (Philips Healthcare) at various iteration levels. Objective image noise (OIN) was measured. Two experienced readers independently assessed all images for image noise, image appearance and visibility of normal anatomic and abnormal findings. A random intercept model was used for statistical analysis. Results Compared to FBP and iDose 4 ™, IMR reduced OIN up to 88% and 72%, respectively (p 4 ™ images provided satisfactory to good image quality for visibility of normal and abnormal findings and were superior to FBP (p 4 ™, while being superior for visibility of abnormal findings except for reticular pattern (p Conclusion IMR results for visibility of normal and abnormal lung findings are heterogeneous, indicating that IMR may not represent a priority technique for clinical routine. iDose 4 ™ represents a suitable method for evaluation of lung tissue at submillisievert chest CT.

1556 Objectives Widespread adoption of new imaging technologies requires that diagnostic performance is maintained as technology migrates from expert sites into general use. This Blinded Independent Evaluation (BIE) study was designed to demonstrate that naive readers could be trained to interpret fluciclovine F18 (aka 18F FACBC) images with acceptable diagnostic performance and reproducibility. Methods A reader training program using standardised interpretation methodology was established following an expert consensus meeting facilitated by the SNMMI Clinical Trial Network. The BIE was conducted at the American College of Radiology Clinical Research Centre. 3 independent board-certified radiologists or nuclear medicine physicians were trained and their proficiency assessed, using training and proficiency image sets distinct from those used in the blinded read itself. Fluciclovine F18 PET-CT images (n=121) and corresponding standard of truth data (SoT), from 110 subjects with biochemical relapse of prostate cancer, were collected from Emory University, Atlanta, USA. The images were from a single time-point acquisition undertaken from the symphysis pubis to above the diaphragm starting 5 min post-injection of ~10mCi fluciclovine F18 and were acquired on a GE DLS (2D) or GE D690 (3D). The readers evaluated the images in a random sequence blinded to clinical data. Primary objectives were: i) the diagnostic performance of individual readers and overall interpretation (2/3 reader concordance) compared to SoT (histopathology and/or clinical follow-up); ii) inter-reader reproducibility. Analyses were conducted at Lesion, Region (Prostate {including bed} or Extra-Prostatic) and Subject level. Secondary objectives included comparison to the expert reader at Emory and assessment of intra-reader reproducibility. Results Table 1 shows the diagnostic performance results by reader and overall interpretation on a regional basis, using the composite SoT (histopathology and clinical follow up). The lower specificity in the Prostate region is due to the overall low number of patients with true negative disease in this region. Inter-reader agreement amongst the three readers was 94.7% , 74.4%, 70.3% for the Lesion, Prostate and Extra-Prostatic regions, respectively, with associated Fleiss’ Kappa values of 0.54, 0.50 and 0.57. Intra-reader agreement was 97.8%, 96.9%, 99.1% at the Lesion level, 100%, 100%, 91.7% for the Prostate region and 83.3%, 75.0%, 83.3% for the Extra-Prostatic region. The degree of agreement between the blinded read and the on-site read of the fluciclovine F18 scans was 95.4%, 95.4% , 94.2% at the Lesion level, 90.9%, 90.1%, 76.9% for the Prostate region, 81.8%, 82.6%, 76.9% for the Extra-Prostatic region. Conclusions Use of standardised interpretation methodology and a specific training program for the assessment of fluciclovine F18 PET-CT images enables naive readers to achieve good diagnostic performance and reproducibility when staging recurrent prostate cancer patients. These finding support the wider dissemination of this technology in the future.

To evaluate the influence of the urinary bladder volume on the detectability of urolithiasis at the ureterovesical junction (UVJ) using a low-dose CT (LD-CT) with iterative reconstruction (IR) and a standard-dose CT (SD-CT) without IR in a large cohort. Four hundred patients (278 males (69.5%), mean 44.6 ± 14.7 years) with urolithiasis at the UVJ were investigated either by an LD-CT with IR (n = 289, 72%) or an SD-CT without IR (n = 111, 28%) protocol. The detectability of distal urolithiasis was assessed by a dichotomous assessment (definite or questionable) by two radiologists in consensus and by a quantitative analysis of the signal density distribution across a line drawn parallel to the distal ureter. Based on the resulting graph, minimum/maximum density values and mean/maximum upslopes and downslopes were derived and calculated automatically. In all patients, the total bladder volume was calculated by a slice-by-slice approach on axial CT images. BACKGROUND: The value of computed tomography (CT) of the chest in the management of patients with ARDS is poorly defined. The aim of this study was to assess the clinical utility of thoracic CT scans in subjects with ARDS using the Berlin definition. METHODS: This was a retrospective, observational study in a university hospital ARDS center on all subjects with ARDS in whom a CT scan of the chest was performed immediately before or during an ICU stay between January 1, 2007 and June 30, 2013. RESULTS: During the study period, a total of 1,781 thoracic CT scans were performed, of which 204 cases met inclusion criteria. The most common pathologic findings of the lung parenchyma were consolidations (94.1% of cases) and ground glass opacities (85.3%). Furthermore, CT scans showed pleural effusions (80.4%), mediastinal lymphadenopathy (66.7%), signs of right ventricular strain and pulmonary hypertension (53.9%), pericardial effusion (37.3%), emphysema of the chest wall (12.3%), pneumothorax (11.8%), emphysema of the mediastinum (7.4%), and pulmonary embolism (2.5%). Results of CT scans led to changes in management in 26.5% of cases. Mortality was significantly increased in subjects with involvement of lung parenchyma of >80% ( P = .004). Intrahospital transport was associated with critical incidents in 8.3% of cases. CONCLUSIONS: Systematic evaluation of thoracic CT scans yielded information useful for making a diagnosis, predicting prognosis, and recognizing concomitant disorders requiring therapeutic interventions. Results obtained from CT scans led to changes in management in 26.5% of cases.

Objective To quantify standard values of the discus interpubicus in healthy subjects and to determine reliability and repeatability using T2 relaxation time measurements at 3T. Methods 20 asymptomatic participants (10 male, 10 female; mean age: 27.3 years ±4.1, BMI: 22.2 ±1.8) underwent a 3T Magnetic Resonance Imaging (MRI) of the pelvic region in a supine position. We included sagittal and para-axial T2w sequences centred over the pubic symphysis in order to identify the complete discus interpubicus. For quantitative analysis, a multi-echo Turbo Spin Echo (TSE) sequence (including 12 echo times between 6.4 and 76.8 ms) was acquired and analysed by using an in-house developed quantification plugin tool (qMapIt) extending ImageJ. Two readers in consensus defined three central slices of the pubic symphysis with the greatest length. For each slice, both readers separately placed three regions-of-interest (ROI) covering the whole discus interpubicus. Both readers repeated the ROI placements in identical fashion after a four-week interval on the original MRI images. Statistical analysis included intraclass correlation coefficient (ICC), nonparametric Wilcoxon test, Fisher exact test and mean relaxation time in ms and 95% confidence intervals. Results T2 relaxation time analysis was performed for all 20 participants. In total, a mean relaxation time of all analysed segments for both observers was 48.6 (±6.3 ms), with a mean relaxation time for observer 1 of 48.7 (±6.0 ms) and for observer 2 of 48.5 ms (±6.6ms). The calculated ICC comparing inter- and intrarater reproducibility was excellent in all segments (≥0.75). Conclusion T2 mapping of the discus interpubicus demonstrates good inter- and intrarater repeatability as well as reliability. Mean relaxation times were calculated with 48.6ms in healthy volunteers.

Diagnosing acute pancreatitis (AP) may be challenging in patients with acute abdominal pain but missing threefold increased serum lipase levels (Lip-). This studyaims both to characterize these patients using clinical, radiological and mortality data, and to assess the group of patients who need contrast-enhanced computed tomography (CECT).In this retrospective, IRB approved study 234 consecutive patients with AP were investigated. Inclusion criteria were single (SAP) and recurrent attacks (RAP) of AP and CECT ≥72h after onset of symptoms. Severity of AP was assessed by C-reactive protein at 48h after hospital admission and using 3 CT-based scores (CTSI, mCTSI, EPIC) by 2 observers. Mortality rates from pancreatic and non-pancreatic causes were noted with regard to lipase increase. Results were compared with paired t-test and Wilcoxon signed-rank test.64/234 (27%) patients belonged to Lip- group and 170/234 (73%) patients were allocated to Lip+ group. Significantly more male patients (78% in Lip- vs. 63% in Lip+, p0.05) with RAP (63% in Lip- vs. 21% in Lip+, p0.001) were observed in the Lip- group. EPIC was significantly lower in Lip- group compared to Lip+ group (3(IQR 2-5) vs. 2(IQR 1-3), p0.001). Mortality from pancreatic and non-pancreatic causes was comparable in Lip- and Lip+ group (pancreatic causes: 9% vs. 6%, p=0.60; nonpancreatic causes: 8% vs. 5%, p=0.58).27% of all patients with AP presented without threefold increase of lipase levels. Thus, they would be underdiagnosed without confirming CECT, which revealed significantly lower counts of pleural effusions and ascites. Male patients with RAP were found significantly more often among the Lip- group. Hence, they would benefit the most from CECT for diagnosing AP.
