Abstract

BACKGROUND: We hypothesized that treatment delays might be an effect modifier regarding risks and benefits of intravenous thrombolysis (IVT) before mechanical thrombectomy (MT). METHODS: We used the dataset of the SWIFT-DIRECT trial, which randomized 408 patients to IVT+MT or MT alone. Potential interactions between assignment to IVT+MT and expected time from onset-to-needle (OTN) as well as expected time from door-to-needle (DTN) were included in regression models. The primary outcome was functional independence (modified Rankin Scale (mRS) 0-2) at 3 months. Secondary outcomes included mRS shift, mortality, recanalization rates, and (symptomatic) intracranial hemorrhage at 24 hours. RESULTS: We included 408 patients (IVT+MT 207, MT 201, median age 72 years (IQR 64-81), 209 (51.2%) female). The expected median OTN and DTN were 142 min and 54 min in the IVT+MT group and 129 min and 51 min in the MT alone group. Overall, there was no significant interaction between OTN and bridging IVT assignment regarding either the functional (adjusted OR (aOR) 0.76, 95% CI 0.45 to 1.30) and safety outcomes or the recanalization rates. Analysis of in-hospital delays showed no significant interaction between DTN and bridging IVT assignment regarding the dichotomized functional outcome (aOR 0.48, 95% CI 0.14 to 1.62), but the shift and mortality analyses suggested a greater benefit of IVT when in-hospital delays were short. CONCLUSIONS: We found no evidence that the effect of bridging IVT on functional independence is modified by overall or in-hospital treatment delays. Considering its low power, this subgroup analysis could have missed a clinically important effect, and exploratory analysis of secondary clinical outcomes indicated a potentially favorable effect of IVT with shorter in-hospital delays. Heterogeneity of the IVT effect size before MT should be further analyzed in individual patient meta-analysis of comparable trials. TRIAL REGISTRATION NUMBER: URL: https://www. Abstract

Abstract Background In the perioperative setting, the most accurate way to continuously measure arterial blood pressure (ABP) is using an arterial catheter. Surrogate methods such as finger cuff have been developed to allow non-invasive measurements and are increasingly used, but need further evaluation. The aim of this study is to evaluate the accuracy and clinical concordance between two devices for the measurement of ABP during neuroradiological procedure. Methods This is a prospective, monocentric, observational study. All consecutive patients undergoing a neuroradiological procedure were eligible. Patients who needed arterial catheter for blood pressure measurement were included. During neuroradiological procedure, ABP (systolic, mean and diatolic blood pressure) was measured with two different technologies: radial artery catheter and Nexfin. Bland-Altman and error grid analyses were performed to evaluate the accuracy and clinical concordance between devices. Results From March 2022 to November 2022, we included 50 patients, mostly ASA 3 (60%) and required a cerebral embolization (94%) under general anaesthesia (96%). Error grid analysis showed that 99% of non-invasive ABP measures obtained with the Nexfin were located in the risk zone A or B. However, 65.7% of hypertension events and 41% of hypotensive events were respectively not detected by Nexfin. Abstract

In spite of the high performance and reliability of deep learning algorithms in a wide range of everyday applications, many investigations tend to show that a lot of models exhibit biases, discriminating against specific subgroups of the population (e.g. gender, ethnicity). This urges the practitioner to develop fair systems with a uniform/comparable performance across sensitive groups. In this work, we investigate the gender bias of deep Face Recognition networks. In order to measure this bias, we introduce two new metrics, $\mathrm{BFAR}$ and $\mathrm{BFRR}$, that better reflect the inherent deployment needs of Face Recognition systems. Motivated by geometric considerations, we mitigate gender bias through a new post-processing methodology which transforms the deep embeddings of a pre-trained model to give more representation power to discriminated subgroups. It consists in training a shallow neural network by minimizing a Fair von Mises-Fisher loss whose hyperparameters account for the intra-class variance of each gender. Interestingly, we empirically observe that these hyperparameters are correlated with our fairness metrics. In fact, extensive numerical experiments on a variety of datasets show that a careful selection significantly reduces gender bias. Abstract

Abstract Although heterogeneity of FAP+ Cancer-Associated Fibroblasts (CAF) has been described in breast cancer, their plasticity and spatial distribution remain poorly understood. Here, we analyze trajectory inference, deconvolute spatial transcriptomics at single-cell level and perform functional assays to generate a high-resolution integrated map of breast cancer (BC), with a focus on inflammatory and myofibroblastic (iCAF/myCAF) FAP+ CAF clusters. We identify 10 spatially-organized FAP+ CAF-related cellular niches, called EcoCellTypes, which are differentially localized within tumors. Consistent with their spatial organization, cancer cells drive the transition of detoxification-associated iCAF (Detox-iCAF) towards immunosuppressive extracellular matrix (ECM)-producing myCAF (ECM-myCAF) via a DPP4- and YAP-dependent mechanism. In turn, ECM-myCAF polarize TREM2+ macrophages, regulatory NK and T cells to induce immunosuppressive EcoCellTypes, while Detox-iCAF are associated with FOLR2+ macrophages in an immuno-protective EcoCellType. FAP+ CAF subpopulations accumulate differently according to the invasive BC status and predict invasive recurrence of ductal carcinoma in situ (DCIS), which could help in identifying low-risk DCIS patients eligible for therapeutic de-escalation.

Abstract

Abstract Although cancer-associated fibroblast (CAF) heterogeneity is well-established, the impact of chemotherapy on CAF populations remains poorly understood. Here we address this question in high-grade serous ovarian cancer (HGSOC), in which we previously identified 4 CAF populations. While the global content in stroma increases in HGSOC after chemotherapy, the proportion of FAP+ CAF (also called CAF-S1) decreases. Still, maintenance of high residual CAF-S1 content after chemotherapy is associated with reduced CD8+ T lymphocyte density and poor patient prognosis, emphasizing the importance of CAF-S1 reduction upon treatment. Single cell analysis, spatial transcriptomics and immunohistochemistry reveal that the content in the ECM-producing ANTXR1+ CAF-S1 cluster (ECM-myCAF) is the most affected by chemotherapy. Moreover, functional assays demonstrate that ECM-myCAF isolated from HGSOC reduce CD8+ T-cell cytotoxicity through a Yes Associated Protein 1 (YAP1)-dependent mechanism. Thus, efficient inhibition after treatment of YAP1-signaling pathway in the ECM-myCAF cluster could enhance CD8+ T-cell cytotoxicity. Altogether, these data pave the way for therapy targeting YAP1 in ECM-myCAF in HGSOC.

Abstract

This study aims to assess the diagnostic accuracy of non-contrast-enhanced 4D MR angiography (NCE-4D-MRA) compared to contrast-enhanced 4D MR angiography (CE-4D-MRA) for the detection and angioarchitectural characterisation of brain arteriovenous malformations (bAVMs). Utilising a retrospective design, we examined 54 MRA pairs from 43 patients with bAVMs, using digital subtraction angiography (DSA) as the reference standard. Both NCE-4D-MRA and CE-4D-MRA were performed using a 3-T MR imaging system. The primary objectives were to evaluate the diagnostic performance of NCE-4D-MRA against CE-4D-MRA and DSA and to assess concordance between imaging modalities in grading bAVMs according to four main scales: Spetzler–Martin, Buffalo, AVM embocure score (AVMES), and R2eDAVM. Our results demonstrated that NCE-4D-MRA had a higher accuracy and specificity compared to CE-4D-MRA (0.85 vs. 0.83 and 95% vs. 85%, respectively) and similar agreement, with DSA detecting shunts in bAVMs or residuals. Concordance in grading bAVMs was substantial between NCE-4D-MRA and DSA, particularly for the Spetzler–Martin and Buffalo scales, with CE-4D-MRA showing slightly higher kappa values for interobserver agreement. The study highlights the potential of NCE-4D-MRA as a diagnostic tool for bAVMs, offering comparable accuracy to CE-4D-MRA while avoiding the risks associated with gadolinium-based contrast agents. The safety profile of imaging techniques is a significant concern in the long-term follow up of bAVMs, and further prospective research should focus on NCE-4D-MRA protocol improvement for clinical use.

Abstract

Generative Adversarial Networks (GANs) are now capable of producing synthetic face images of exceptionally high visual quality. In parallel to the development of GANs themselves, efforts have been made to develop metrics to objectively assess the characteristics of the synthetic images, mainly focusing on visual quality and the variety of images. Little work has been done, however, to assess overfitting of GANs and their ability to generate new identities. In this paper we apply a state of the art biometric network to various datasets of synthetic images and perform a thorough assessment of their identity-related characteristics. We conclude that GANs can indeed be used to generate new, imagined identities meaning that applications such as anonymisation of image sets and augmentation of training datasets with distractor images are viable applications. We also assess the ability of GANs to disentangle identity from other image characteristics and propose a novel GAN triplet loss that we show to improve this disentanglement., Comment: Presented at IJCB 2020 (oral)

Abstract

Face-morphing attacks have been a cause for concern for a number of years. Striving to remain one step ahead of attackers, researchers have proposed many methods of both creating and detecting morphed images. These detection methods, however, have generally proven to be inadequate. In this work we identify two new, GAN-based methods that an attacker may already have in his arsenal. Each method is evaluated against state-of-the-art facial recognition (FR) algorithms and we demonstrate that improvements to the fidelity of FR algorithms do lead to a reduction in the success rate of attacks provided morphed images are considered when setting operational acceptance thresholds.

Abstract

Abstract Resistance to endocrine treatments and CDK4/6 inhibitors is considered a near-inevitability in most patients with estrogen receptor positive breast cancers (ER + BC). By genomic and metabolomics analyses of patients’ tumours, metastasis-derived patient-derived xenografts (PDX) and isogenic cell lines we demonstrate that a fraction of metastatic ER + BC is highly reliant on oxidative phosphorylation (OXPHOS). Treatment by the OXPHOS inhibitor IACS-010759 strongly inhibits tumour growth in multiple endocrine and palbociclib resistant PDX. Mutations in the PIK3CA/AKT1 genes are significantly associated with response to IACS-010759. At the metabolic level, in vivo response to IACS-010759 is associated with decreased levels of metabolites of the glutathione, glycogen and pentose phosphate pathways in treated tumours. In vitro, endocrine and palbociclib resistant cells show increased OXPHOS dependency and increased ROS levels upon IACS-010759 treatment. Finally, in ER + BC patients, high expression of OXPHOS associated genes predict poor prognosis. In conclusion, these results identify OXPHOS as a promising target for treatment resistant ER + BC patients.

Abstract

Rupture of an intracranial aneurysm leads to subarachnoid hemorrhage, a severe type of stroke. To discover new risk loci and the genetic architecture of intracranial aneurysms, we performed a cross-ancestry, genome-wide association study in 10,754 cases and 306,882 controls of European and East Asian ancestry. We discovered 17 risk loci, 11 of which are new. We reveal a polygenic architecture and explain over half of the disease heritability. We show a high genetic correlation between ruptured and unruptured intracranial aneurysms. We also find a suggestive role for endothelial cells by using gene mapping and heritability enrichment. Drug-target enrichment shows pleiotropy between intracranial aneurysms and antiepileptic and sex hormone drugs, providing insights into intracranial aneurysm pathophysiology. Finally, genetic risks for smoking and high blood pressure, the two main clinical risk factors, play important roles in intracranial aneurysm risk, and drive most of the genetic correlation between intracranial aneurysms and other cerebrovascular traits.

Abstract

Introduction Two recent trials demonstrated a benefit for endovascular thrombectomy (EVT) in the treatment of basilar artery occlusion (BAO). Considering the expected increase in the utilization of EVT for BAO, we sought to understand the technique preferences of neurointerventionalists currently performing EVT for BAO. Methods We conducted an international online survey of physician opinions on the use of EVT in BAO between January to March 2022. The survey was distributed through stroke and neurointerventional organizations. Survey questions examined selection of patients for the procedure and the techniques currently used for EVT in BAO. Results More than 3,000 participants were invited yielding 1,245 respondents, of which 543 were classified as neurointerventionalists across 52 countries and included in this analysis. Most neurointerventionalists would proceed to EVT for occlusions of the V4 segment, the basilar artery, or the PCA, without regard for prior IVT. For BAO of embolic etiology, aspiration only thrombectomy was the preferred method with 50.3% of neurointerventionalists. For BAO of intracranial atherosclerotic disease (ICAD) etiology, combined stent retriever and aspiration thrombectomy was the preferred method with 40.5% of neurointerventionalists (Figure 1). The majority of neurointerventionalists (88.0%) would proceed to stenting after three or fewer failed passes for patients with BAO of ICAD etiology. In patients undergoing stenting, aspirin and clopidogrel was the most common antiplatelet regime (52.4%). Conclusions Amongst the surveyed neurointerventionalists, the most common techniques for EVT of patients with BAO were contact aspiration or combined stent retriever with aspiration thrombectomy. For patients with BAO due to ICAD, the majority of neurointerventionalists were willing to stent and do so most often after three or fewer failed passes and with the use of dual antiplatelet medications. Further study is needed to determine the optimal technique for EVT of BAO with or without ICAD.

Abstract

Background: To compare a model-based iterative reconstruction (MBIR) versus a hybrid iterative reconstruction (HIR) for initial and final Alberta Stroke Program Early Ct Score (ASPECT) scoring in acute ischemic stroke (AIS). We hypothesized that MBIR designed for brain computed tomography (CT) could perform better than HIR for ASPECT scoring. Methods: Among patients who had undergone CT perfusion for AIS between April 2018 and October 2019 with a follow-up imaging within 7 days, we designed a cohort of representative ASPECTS. Two readers assessed regional-cerebral-blood-volume-ASPECT (rCBV-ASPECTS) on the initial exam and final-ASPECTS on the follow-up non-contrast-CT (NCCT) in consensus. Four readers performed independently MBIR and HIR ASPECT scoring on baseline NCCT. Results: In total, 294 hemispheres from 147 participants (average age of 69.59 ± 15.63 SD) were analyzed. Overall raters’ agreement between rCBV-map and MBIR and HIR ranged from moderate to moderate (κ = 0.54 to κ = 0.57) with HIR and moderate to substantial (κ = 0.52 to κ = 0.74) with MBIR. Overall raters’ agreement between follow-up imaging and HIR/MBIR ranged from moderate to moderate (κ = 0.55 to κ = 0.59) with HIR and moderate to almost perfect (κ = 0.48 to κ = 0.82) with MBIR. Conclusions: ASPECT scoring with MBIR more closely matched with initial and final infarct extent than classical HIR NCCT reconstruction.

Abstract

A number of pattern recognition tasks, \textit{e.g.}, face verification, can be boiled down to classification or clustering of unit length directional feature vectors whose distance can be simply computed by their angle. In this paper, we propose the von Mises-Fisher (vMF) mixture model as the theoretical foundation for an effective deep-learning of such directional features and derive a novel vMF Mixture Loss and its corresponding vMF deep features. The proposed vMF feature learning achieves the characteristics of discriminative learning, \textit{i.e.}, compacting the instances of the same class while increasing the distance of instances from different classes. Moreover, it subsumes a number of popular loss functions as well as an effective method in deep learning, namely normalization. We conduct extensive experiments on face verification using 4 different challenging face datasets, \textit{i.e.}, LFW, YouTube faces, CACD and IJB-A. Results show the effectiveness and excellent generalization ability of the proposed approach as it achieves state-of-the-art results on the LFW, YouTube faces and CACD datasets and competitive results on the IJB-A dataset., Comment: Under review

Abstract

Face recognition (FR) methods report significant performance by adopting the convolutional neural network (CNN) based learning methods. Although CNNs are mostly trained by optimizing the softmax loss, the recent trend shows an improvement of accuracy with different strategies, such as task-specific CNN learning with different loss functions, fine-tuning on target dataset, metric learning and concatenating features from multiple CNNs. Incorporating these tasks obviously requires additional efforts. Moreover, it demotivates the discovery of efficient CNN models for FR which are trained only with identity labels. We focus on this fact and propose an easily trainable and single CNN based FR method. Our CNN model exploits the residual learning framework. Additionally, it uses normalized features to compute the loss. Our extensive experiments show excellent generalization on different datasets. Abstract

The aim was to assess the occurrence of magnetic susceptibility artifacts (MSA) following endovascular treatment of intracranial aneurysm by stent using susceptibility weighted imaging (SWI). Imaging and clinical data of 46 patients who underwent stent placement in the case of intracranial aneurysm endovascular treatment (S-Group) were retrospectively analyzed and compared to a control group (C-Group) in which 46 patients had coiling alone. The mean number of MSA was higher in the S-group than in the C-group on postprocedural SWI sequence (8.76, 95%CI [5.76; 11.76] vs. 0.78 [0.32; 1.25], respectively, p < 0.001) with a higher frequency of the appearance of MSA also in the S-group (78.26% vs. 21.74% in the C-group, p < 0.001). In the S-group, in the vascular territory of the treated artery, there was a higher number of MSA than in other vascular territories (mean of 5.18 [3.43; 6.92] vs. 3.08 [1.79; 4.36], p = 0.001). An odds ratio (OR) of 20.98 [5.24; 83.95] suggested a higher proportion of onset of MSA in the S-group than in the C-group (p < 0.001). The appearance of MSA after a treatment by stenting for intracranial aneurysm in patients under antiplatelet therapy was common, particularly in the treated artery territory.
