9. Awareness of the diagnosis, treatment, and control of diabetes mellitus in Brazil

ABSTRACT OBJECTIVE To estimate the proportions of awareness, treatment, and control of diabetes mellitus (DM) in the Brazilian adult population. METHOD This is a cross-sectional study, with data from a representative sample of the Brazilian population, taken from the National Health Survey(PNS 2014/2015). Outcomes were defined based on glycated hemoglobin (HbA1c) measurements, self-reported DM diagnosis, and use of hypoglycemic agents or insulin. The proportion of DM awareness, treatment, and control was estimated according to sociodemographic characteristics, health conditions, and access to health services, and their respective 95% confidence intervals. RESULTS DM prevalence in the Brazilian population was of 8.6% (95%CI: 7.8–9.3): 68.2% (95%CI: 63.9–72.3) were aware of their diagnosis, 92.2% (95%CI: 88.6–94.7) of those who were aware were undergoing drug treatments, and, of these, 35.8% (95%CI: 30.5–41.6) had controlled HbA1c levels. The proportions of DM awareness, control, and treatment were lower in men aged 18 to 39 years, individuals with low education, without health insurance, and beneficiaries of the Bolsa Família program. CONCLUSION Approximately one in ten Brazilians has DM. A little more than half of this population is aware of their diagnosis, a condition measured by HbA1c dosage and clinical diagnosis. Among those who know, the vast majority are undergoing drug treatments. However, less than half of these have their HbA1c levels controlled. Worse scenarios were found in subgroups with high social vulnerability.

Abstract Background It has been hypothesized that the coronavirus disease 2019 (COVID-19) pandemic may have changed the conduct of obstetric practices at the time of labor, delivery, and birth. In Brazil, many practices lacking scientific evidence are implemented in this care, which is charcaterized by excessive use of unnecessary interventions. This scenario may have been worsened by the pandemic. Thus, we analyzed the effects of the pandemic on care during prenatal care and delivery by comparing the results of two surveys (one was administered before the pandemic and the other during the pandemic) in public hospitals in Belo Horizonte - Minas Gerais (MG), Brazil. Methods This cross-sectional and comparative study analyzed preliminary data from the study “Childbirth and breastfeeding in children of mothers infected with SARS-CoV-2”, which was conducted in three referral maternity hospitals in Belo Horizonte - MG during the pandemic in the first half of 2020 in Brazil. The final sample consisted of 1532 eligible women. These results were compared with data from 390 puerperae who gave birth in the three public hospitals in the study “Birth in Belo Horizonte: labor and birth survey”, conducted before the pandemic to investigate the changes in practices of labor and delivery care for the mother and her newborn, with or without COVID-19 infection, before and during the pandemic. In this research, “Birth in Belo Horizonte: labor and birth survey”, data collection was performed between November 2011 and March 2013 by previously trained nurses. Between study comparisons were performed using Pearson’s chi-square test, with a confidence level of 95%, and using Stata statistical program. Results We found a significant increase in practices recommended by the World Health Organization during the pandemic including the following: diet offering (48.90 to 98.65%), non-pharmacological pain relief (43.84 to 67.57%), and breastfeeding in the newborn´s first hour of life (60.31 to 77.98%) (p

O tumor venéreo transmissível (TVT) é também conhecido como tumor de Sticker, condiloma canino ou linfossarcoma venéreo, referenciado como um neoplasma contagioso, constituído por células redondas, primariamente encontrado na genitália externa, mas também podem ocorrer neoplasmas primários extragenitais e metastáticos. Podem ser únicos ou múltiplos, de até 10 cm de diâmetro, com uma superfície avermelhada, ulcerada e de nódulos múltiplos. Este trabalho realizou levantamento de tumor venéreo transmissível observados no projeto de controle populacional, no município de Salinas, Norte de Minas Gerais. A pesquisa foi realizada no Hospital Veterinário do Instituto Federal de Educação, Ciência e Tecnologia do Norte de Minas Gerais, Campus Salinas, Minas Gerais, com 31 cães no total, no período de julho a dezembro de 2022, provenientes do canil municipal de Salinas. Foram realizadas as técnicas de swab e imprint, vaginal ou peniano dos cães. A frequência de TVT nos cães avaliados foi 54,84%. Do total de 17 animais positivos para TVT, somente 3 deles (17,65%), apresentaram lesões macroscópicas com massas neoplásicas presentes na região genital e os outros 14 (82,35%), não as apresentavam. Com base nos exames citomorfológicos, a maior frequência foi do tipo TVT plasmocitoide com 64,70%. De acordo com a distribuição pelo sexo, avaliando-se de forma geral, houve uma maior frequência de TVT em fêmeas (64,70%), em relação aos machos (35,30%). Todavia, o maior número de animais participantes do projeto nesse período foi de fêmeas, sendo 22 fêmeas e nove machos. Em relação a distribuição da frequência de TVT, quanto ao sexo, no que tange a citomorfologia das células tumorais, avaliou-se uma maior frequência de TVT plasmocitoide tanto em fêmeas (63,64%), como em machos (66,66%). O presente estudo é o primeiro a realizar um levantamento de casos de TVT em cães no município de Salinas, Norte de Minas Gerais. Os dados obtidos sugerem uma frequência elevada, além de acometer ambos os sexos. Um dos fatores que favorecem a disseminação do TVT, é a falta de informação quanto aos cuidados com os animais, posse responsável e projetos de castração, evitando a disseminação não só de TVT, como outras doenças de saúde pública, sendo de extrema importância a orientação da sociedade. Propõe-se a realização de novos estudos municipais ou regionais a fim de comparar a ocorrência de TVT, visto que a cultura da região ainda é manter seus animais semidomiciliados e há uma grande quantidade de cães errantes, justificando assim os dados encontrados.

Objective: to analyze the health conditions of the population affected by the mining dam collapse from the perspective of healthcare professionals and managers. Method: this is a qualitative case study, conducted using public domain documents and interviews with healthcare professionals and managers, from March to May 2022, totaling 14 participants. The data were analyzed according to the theoretical propositions strategy based on the Social Determinants of Health. Results: the health conditions of this population have worsened in terms of respiratory problems, gastroenteritis, obesity, arboviruses, increased use of alcohol and drugs, and mental illnesses. The loss of employment, home, and community belonging of those affected is highlighted. Conclusion: the socio-environmental disaster was a conditioning factor for changes in the health conditions of those affected, exposing them to new scenarios and overlapping risks capable of modifying their quality of life and aggravating their biopsychosocial health conditions.

Beginning December 2016, sylvatic yellow fever (YF) outbreaks spread into southeastern Brazil, and Minas Gerais state experienced two sylvatic YF waves (2017 and 2018). Following these massive YF waves, we screened 187 free-living non-human primate (NHPs) carcasses collected throughout the state between January 2019 and June 2021 for YF virus (YFV) using RTqPCR. One sample belonging to a Callithrix, collected in June 2020, was positive for YFV. The viral strain belonged to the same lineage associated with 2017-2018 outbreaks, showing the continued enzootic circulation of YFV in the state. Next, using data from 781 NHPs carcasses collected in 2017-18, we used generalized additive mixed models (GAMMs) to identify the spatiotemporal and host-level drivers of YFV infection and intensity (an estimation of genomic viral load in the liver of infected NHP). Our GAMMs explained 65% and 68% of variation in virus infection and intensity, respectively, and uncovered strong temporal and spatial patterns for YFV infection and intensity. NHP infection was higher in the eastern part of Minas Gerais state, where 2017-2018 outbreaks affecting humans and NHPs were concentrated. The odds of YFV infection were significantly lower in NHPs from urban areas than from urban-rural or rural areas, while infection intensity was significantly lower in NHPs from urban areas or the urban-rural interface relative to rural areas. Both YFV infection and intensity were higher during the warm/rainy season compared to the cold/dry season. The higher YFV intensity in NHPs in warm/rainy periods could be a result of higher exposure to vectors and/or higher virus titers in vectors during this time resulting in the delivery of a higher virus dose and higher viral replication levels within NHPs. Further studies are needed to better test this hypothesis and further compare the dynamics of YFV enzootic cycles between different seasons.

Yellow fever virus (YFV) is the agent of yellow fever (YF), which affects both humans and non-human primates (NHP). Neotropical NHP are highly susceptible to YFV and considered sentinels for YFV circulation. Brazil faced a significant YF outbreak in 2017–2018, with over 2000 human cases and 2000 epizootics cases, mainly in the State of Minas Gerais, Brazil. This study aimed to investigate whether YFV circulation persisted in NHP after the human outbreak had subsided. To this end, NHP carcass samples collected in Minas Gerais from 2021 to 2023 were screened for YFV. RNA was extracted from tissue fragments and used in RT-qPCR targeting the YFV 5’UTR. Liver and lung samples from 166 animals were tested, and the detection of the β-actin mRNA was used to ensure adequacy of RNA isolation. YFV RNA was detected in the liver of 18 NHP carcasses collected mainly from urban areas in 2021 and 2022. YFV positive NHP were mostly represented by Callithrix, from 5 out of the 12 grouped municipalities (mesoregions) in Minas Gerais state. These findings reveal the continued YFV circulation in NHP in urban areas of Minas Gerais during 2021 and 2022, with the attendant risk of re-establishing the urban YFV cycle.

25. Integrative taxonomy supports two new species of Chimarra Stephens, 1829 from Brazil (Trichoptera: Philopotamidae)

Chimarra Stephens, 1829 is the largest genus of the Philopotamidae with about 930 species and cosmopolitan distribution. Recent taxonomic revisions have subdivided the genus into four subgenera: Chimarra, Curgia Walker, 1860, Chimarrita Blahnik, 1997, and Otarrha Blahnik, 2002, the last three restricted to the New World. In this paper, we describe and illustrate two new species of Chimarra from Brazil, C. (Otarrha) paraodonta sp. nov. from Rio de Janeiro State and C. (Chimarrita) truncata sp. nov. from Amazonas and Pará states. Partial sequences of cytochrome oxidase subunit I (COI, DNA barcodes) were generated and integrated with morphological evidence to delimit the new species and evaluate their phylogenetic relationships within the genus. A maximum likelihood analysis of 48 COI sequences representing 19 species of Chimarra corroborated their subgeneric assignment based on morphology and highlighted their putative sister species. Both new species showed high K2P divergences when compared to their sister species: Chimarra (O.) paraodonta sp. nov. and C. (O.) odonta (17.4–21.3%) and Chimarra (C.) truncata sp. nov. and C. (C.) simpliciforma (20.0–21.3%). These distances are comparable to the range of interspecific distances calculated for the whole genus (13.6–22.7%), adding support to their description as new species. This analysis was especially important because of the high morphological similarity of C. paraodonta sp. nov. and C. odonta. Finally, analysis of the sequences of Chimarra odonta suggests that the nominal species may represent a complex of cryptic species with high intraspecific divergences (up to 18.1%), with at least two of those lineages co-occurring with C. paraodonta sp. nov. at Parque Nacional do Itatiaia.

26. Surgical Approach to Odontogenic Maxillary Sinusitis Caused by Tooth Root Displacement into the Maxillary Sinus: A Case Report

Paranasal sinuses affected by foreign bodies are uncommon clinical situations. Foreign body displacement into facial sinuses is a potential iatrogenic complication, that may cause an oroantral fistula, sinusitis, cellulitis, and subdural emphysema. The maxillary sinus is usually the most affected by tooth displacements, due to its proximity. A dental fragment in the maxillary sinus may cause Odontogenic Maxillary Sinusitis (OMS). The present case report presents a case from the diagnosis to treatment of OMS originated by a fragment of tooth root displacement into the sinus during a previous dental extraction. A female patient, 13-year-old, had characteristic signs of sinusitis, reporting rhinorrhoea, headache, eye pain, and pain on the left side of the face. The clinical examination showed pain on palpation in the buccal region of the upper left first molar, which was absent. The Cone Beam Computed Tomography (CBCT) showed a foreign body inside the maxillary sinus, complete opacification, and bone defect on the sinus floor. Given the final diagnosis of OMS, the treatment selected was the combination of antibiotic therapy and fragment removal from the sinus. The diagnosis, planning, and correct use of surgical techniques described in the literature are essential for preventing iatrogenesis and actively ensuring patient health. In this sense, patients need to be informed about any complication, which should be written in the medical records along with resolution measures.

The objective of this study was to evaluate the growth and quality of arabica coffee seedlings produced in tubes under different sources of phosphorus. The experiment was conducted in a greenhouse in a randomized block design with eleven treatments and four replications, three plants per plot. The dose of organomineral, MAP and MAP coated was based on the recommendation of P2O5 for the formation of coffee seedlings.. For Osmocote, the manufacturer's recommendation was used depending on the amount, in dm³, of substrate. After 180 days of transplanting, the growth characteristics of the plants (leaf dry matter, stem and roots) were evaluated, and the aerial part and root system ratio and Dickson's quality index were calculated, as well as regression equations were adjusted for the evaluated parameters. For the growth and quality of seedlings, it was observed that in fertilizations with pelleted and branched organomineral, coated MAP and Osmocote, the highest averages were obtained for the contents of dry matter, height, diameter of the collection and quality indexes, when compared to the other sources of P2O5.. The dose that provides the maximum growth and quality of coffee seedlings for the treatments with organomineral was calculated, this being equal to 6 g/dm³ of P2O5 for bran and pelleted organomineral. According to the results, the sources that promoted the formation of a vigorous seedling, with an adequate supply of nutrients and a good quality standard are pelleted organonineral fertilization and MAP, reducing the risk of losses and consequently the need to discard the seedlings in the nursery.

Objetivo: realizar a tradução, adaptação cultural e validação do instrumento “Instrumento de Autoavaliação em Diabetes” para aplicação no contexto brasileiro. Métodos: estudo metodológico realizado com 132 profissionais, entre os anos de 2016 e 2018, em seis etapas: 1- Tradução inicial; 2- Síntese da tradução; 3- Retrotradução (back-translation); 4- Avaliação pelo comitê de juízes; 5- Adequação cultural (pré-teste); 6- Reprodutibilidade. Houve participação de profissionais de equipes multiprofissionais envolvidas no tratamento do diabetes, por meio da plataforma e-surv. Resultados: entre os participantes, predominaram o sexo feminino (73,5%), profissionais com especialização (pós-graduação Lato Sensu) (51,5%) e com experiência na assistência a pessoas com diabetes (84,4%). O Índice de Validade de Conteúdo (IVC) foi satisfatório (média de 0,850). O instrumento apresentou boa consistência interna (Alfa de Cronbach=0,878). A análise de confiabilidade do instrumento, realizada pelo cálculo do coeficiente de correlação intraclasse (CCI), indicou concordância adequada em todas as medidas, 0,878 (IC 95%: 0,864 – 0,891), com Kappa Ponderado médio de 0,714 e índices acima de 0,60 em 85% os itens, mostrando boa concordância teste e reteste. Conclusão: a versão traduzida e adaptada culturalmente do Instrumento de Autoavaliação em Diabetes apresentou boa confiabilidade, aceitabilidade e estabilidade temporal satisfatórias conforme os parâmetros internacionais, podendo ser utilizada para autoavaliação em diabetes pelos profissionais da saúde.

The study analyzed the influence of temperature on the depth of cure of bulk-fill composite resins. Three discs (ISO 4049/2000) from each group were made, and four restorative materials were investigated: Opus Bulk-Fill Flow, Opus Bulk-Fill APS, Filtek™ One Bulk-Fill, and Filtek™ Bulk-Fill Flow. They were light-cured (Gnatus ± 1,200 mW/cm2) for 20 seconds, varying the temperature (23°C and 5°C) and simulating the use of the product both at room temperature and under refrigeration. The materials were inserted in aluminum matrices with 10 mm in depth and 4 mm in diameter. A clear film strip and a glass microscope slide were positioned at the top and bottom surfaces. The material was condensed and light-cured with the tip of the light source in close contact, for 20 seconds on the top surface. Immediately after irradiation, the samples were removed from the mold, and the uncured part was removed with a plastic spatula. The measurement was performed with a micrometer ± 0.1 mm, and the value was divided by 2. The data were analyzed with two-way ANOVA at a significance of 0.05. Objetivo: estimar intervalos de referência (IR) de creatinina e hemoglobina glicosilada (HbA1c) na população adulta brasileira. Métodos: estudo transversal, utilizando na base de dados Pesquisa Nacional de Saúde (PNS), entre 2014-2015, composta por 8.952 adultos. Para estabelecer IR, aplicaram-se critérios de exclusão, removeram-se outliers e foi feita estratificação. Após esses procedimentos, a amostra constitui-se de 2.723 adultos para HbA1c e de 2.738 adultos para creatinina. Avaliaram-se diferenças pelos testes Mann Withney e Kruskal Wallis (p≤0,05). Resultados: homens (IR 0,69-1,25; mediana 0,95 mg/dL) apresentaram maiores IR para creatinina que mulheres (IR 0,53-1,05; mediana 0,74 mg/dL) e tiveram maiores valores de limites inferiores (LI) e mediana de HbA1c (sexo masculino: IR: 4,55-5,97; mediana 5,3%; sexo feminino: IR 4,49-5,97; mediana 5,20%) (p ≤ 0,05). Nas mulheres, IR para creatinina foram mais elevados entre 45 a 59 anos (IR: 0,55-1,04; mediana 0,77 mg/dL) e a partir dos 60 anos (IR: 0,54-0,98; mediana 0,77 mg/dL (p ≤ 0,05). Para HbA1c, homens apresentaram IR mais elevados a partir de 60 anos (IR 4,65-6,07; mediana 5,44%) e mulheres a partir de 45 anos (45 a 59 anos: IR 4,61-6,05; mediana 5,40%; e 60 anos ou mais: IR 4,82-6,03; mediana 5,50%) (p ≤ 0,05). Para creatina, foram observados menores LI dos IR e mediana mais proeminente nos adultos de raça/cor branca (IR: 0,56-1,19; mediana 0,85%) em comparação com a parda (IR: 0,55-1,19; mediana 0,84%) (p ≤ 0,05). Conclusão: IR próprios possibilitam desvelar as condições de saúde dos adultos brasileiros e podem subsidiar a identificação adequada de doença renal crônica e diabetes.

Abstract Mechanisms involved in the acute responses to renal denervation (RDN) have yet to be fully understood. We assessed urinary volume, autonomic control and aorta vascular reactivity after acute RDN. Male normotensive Wistar rats and spontaneously hypertensive rats (SHR) were divided into normotensive + RDN (ND) or sham surgery (NS), and hypertensive + RDN (HD) or sham surgery (HS). Metabolic parameters and hemodynamic measurements were recorded 72h and 4 days after intervention, respectively. Aortic rings were studied 7 days post RDN in an isometric myograph. Resumo: Introdução: A educação interprofissional (EIP) procura desenvolver habilidades colaborativas dos profissionais de saúde para a melhoria do cuidado ao paciente. Objetivo: Essa revisão explora as estratégias educacionais não pontuais utilizadas na formação interprofissional, na graduação em saúde, identificando seus potenciais e suas fragilidades. Método: A busca incluiu artigos publicados nas bases de dados BVS (LILACS), Cochrane, CINAHL, Embase e MEDLINE. Definiu-se a questão de pesquisa pelo anagrama PICO: selecionaram-se estudos que incluíssem, pelo menos, dois cursos de graduação em saúde, sendo um deles de Medicina, e que relatassem estratégia educacional mínima de 15 horas e sua avaliação. Resumos publicados em congressos, opiniões, editoriais e revisões sistemáticas foram excluídos. Resultado: Avaliaram-se 28 estudos publicados entre 2005 e 2019, sendo 31% no último biênio. Prevaleceram a simulação (36%) ou o uso de métodos combinados (29%) na avaliação de atitudes dos alunos, a compreensão dos papéis dos profissionais de saúde, o trabalho em equipe, a comunicação e o conhecimento em resposta à intervenções de EIP. Predominaram estudos nos domínios: papéis e responsabilidades (75%) e trabalho em equipe (64%). A abordagem de valores e ética (32%) e de comunicação (28%) foi menos frequente. Dos artigos, 18 (64%) apresentavam dois ou mais objetivos e seis (18%) buscavam estudar, em conjunto, os quatro domínios da EIP. Entre as intervenções utilizadas como estratégias de ensino, 36% (dez estudos) eram de simulação; 29% (oito), métodos combinados; 18% (cinco), prática clínica (trabalho colaborativo em unidades ambulatoriais ou enfermarias); 14% (quatro), observação direta (shadowing); 11%, aprendizagem baseada em problemas; e dois, aprendizado on-line (e-learning) e workshop. A qualidade geral dos estudos incluídos foi baixa, atendendo de dois a cinco dos seis critérios de qualidade. O cegamento do avaliador não foi citado em 25 publicações. O trabalho colaborativo em cenários reais é descrito como o mais eficiente. Conclusão: A EIP vem sendo incorporada ao processo de formação na saúde, e múltiplas estratégias focadas em resultados e baseadas em competências otimizam a construção de relações efetivas e o desenvolvimento de habilidades para a prática colaborativa. A fragilidade dos artigos aponta que a EIP de estudantes ainda constitui grande desafio para as instituições formadoras.

Abstract This systematic review assessed the effectiveness of ozone (O3) in the color change of in-office tooth bleaching in vital teeth (TB) and the sensitivity control. Only randomized controlled clinical trials were included. Seven databases were used as primary search sources, and three additional sources were searched to capture the "grey literature" partially. The JBI tool was used to assess the risk of bias. TB was assessed using the ΔELab color change metric comparing tooth color pre- and post-bleaching. We meta-analyzed the ΔELab estimates per method and calculated the absolute standardized mean difference using random-effect models. The GRADE approach assessed the certainty of the evidence. The ΔELab estimates ranged from 1.28 when the O3 was used alone to 6.93 when combined with hydrogen peroxide (HP). Two studies compared O3 and HP alone, but their TB was similar (SMD = − 0.02; 95%CI: − 0.54; 0.49). The bleaching effectiveness for the combination of O3 + HP compared to HP was similar (SMD = 0.38; 95%CI: − 0.04; 0.81). Thus, based on the available literature, our findings suggest that O3 is not superior to the conventional technique using HP on the change of tooth color. The O3 did not present sensitivity when used alone. When O3 was used in combination with HP, patients reported hypersensitivity only when O3 was applied before HP, i.e., no sensitivity was perceived when O3 was applied after HP.

Objetivo: analisar a tendência das internações por condições sensíveis à atenção primária em crianças menores de cinco anos de idade, no estado de Minas Gerais. Métodos: estudo ecológico de séries temporais utilizou registros do Sistema de Informação Hospitalar, período de 2008 a 2018. As análises de tendência foram realizadas pelo método de Prais-Winsten para verificar tendências: estacionárias (p > 0,05), decrescentes (p < 0,05 e coeficiente de regressão negativo) ou ascendentes (p < 0,05 e coeficiente de regressão positivo) por região de saúde e por grupo etário (até um ano e de um a quatro anos). Resultados: houve tendência decrescente de internações entre crianças no estado (variação percentual anual = -4,96%; p < 0,05), com redução de internações por gastroenterites infecciosas e complicações, pneumonias bacterianas e asma. Observou-se aumento de internações por anemia; doenças pulmonares e infecções de ouvido, nariz e garganta. Para crianças menores de um ano, foi observado que as internações por doenças preveníveis por imunização e condições sensíveis apresentaram tendência ascendente (ß=5,69 e p < 0,05), com destaque para a sífilis congênita. Conclusões: o cenário de Minas Gerais é similar ao de outros estados brasileiros, nos quais se observa redução de internação de crianças de até cinco anos, com aumento de internações por anemia, doenças pulmonares e infecção de ouvido, nariz e garganta. Ainda que existam melhorias nas práticas e políticas voltadas para a saúde da criança, os achados reforçam o planejamento de ações para o cuidado a agravos preveníveis na atenção primária.
