PURPOSE: Identifying molecular and immune features to guide immune checkpoint inhibitor (ICI)-based regimens remains an unmet clinical need. EXPERIMENTAL DESIGN: Tissue and longitudinal blood specimens from phase III trial S1400I in patients with metastatic squamous non-small cell carcinoma (SqNSCLC) treated with nivolumab monotherapy (nivo) or nivolumab plus ipilimumab (nivo+ipi) were subjected to multi-omics analyses including multiplex immunofluorescence (mIF), nCounter PanCancer Immune Profiling Panel, whole-exome sequencing, and Olink. RESULTS: Higher immune scores from immune gene expression profiling or immune cell infiltration by mIF were associated with response to ICIs and improved survival, except regulatory T cells, which were associated with worse overall survival (OS) for patients receiving nivo+ipi. Immune cell density and closer proximity of CD8+GZB+ T cells to malignant cells were associated with superior progression-free survival and OS. The cold immune landscape of NSCLC was associated with a higher level of chromosomal copy-number variation (CNV) burden. Patients with LRP1B-mutant tumors had a shorter survival than patients with LRP1B-wild-type tumors. Olink assays revealed soluble proteins such as LAMP3 increased in responders while IL6 and CXCL13 increased in nonresponders. Upregulation of serum CXCL13, MMP12, CSF-1, and IL8 were associated with worse survival before radiologic progression. CONCLUSIONS: The frequency, distribution, and clustering of immune cells relative to malignant ones can impact ICI efficacy in patients with SqNSCLC. High CNV burden may contribute to the cold immune microenvironment. Soluble inflammation/immune-related proteins in the blood have the potential to monitor therapeutic benefit from ICI treatment in patients with SqNSCLC.

Abstract To inform clinical trial design and real-world precision pediatric oncology practice, we classified diagnoses, assessed the landscape of mutations, and identified genomic variants matching trials in a large unselected institutional cohort of solid tumors patients sequenced at Dana-Farber / Boston Children’s Cancer and Blood Disorders Center. Tumors were sequenced with OncoPanel, a targeted next-generation DNA sequencing panel. Diagnoses were classified according to the International Classification of Diseases for Oncology (ICD-O-3.2). Over 6.5 years, 888 pediatric cancer patients with 95 distinct diagnoses had successful tumor sequencing. Overall, 33% (n = 289/888) of patients had at least 1 variant matching a precision oncology trial protocol, and 14% (41/289) were treated with molecularly targeted therapy. This study highlights opportunities to use genomic data from hospital-based sequencing performed either for research or clinical care to inform ongoing and future precision oncology clinical trials. Furthermore, the study results emphasize the importance of data sharing to define the genomic landscape and targeted treatment opportunities for the large group of rare pediatric cancers we encounter in clinical practice.

Objective To investigate the effect of daily iron supplementation for 14 weeks on the serum iron concentration and other markers of iron status in exclusively breastfed infants in Gambia. Methods A placebo-controlled, randomized, double-blind trial was performed in rural Gambia between 3 August 2021 and 9 March 2022. Overall, 101 healthy, exclusively breastfed infants aged 6 to 10 weeks were recruited at vaccination clinics and through community health workers. Infants were randomized to receive iron supplementation (7.5 mg/day as ferrous sulfate in sorbitol solution) or placebo for 98 days. Venous blood samples were collected at baseline and on day 99 to assess the serum iron concentration and other markers of iron and haematological status. Findings At day 99, the serum iron concentration was significantly higher in the iron supplementation group than the placebo group (crude difference in means: 2.5 [micro]mol/L; 95% confidence interval: 0.6 to 4.3) and there were significant improvements in other iron and haematological markers. There were 10 serious adverse events (five in each group), 106 non- serious adverse events (54 with iron supplementation; 52 with placebo) and no deaths. There was no marked difference between the groups in maternally reported episodes of diarrhoea, fever, cough, skin infection, eye infection or nasal discharge. Conclusion In exclusively breastfed Gambian infants, iron supplementation from 6 weeks of age was associated with a significant improvement in markers of iron status at around 6 months of age. There was no indication of adverse effects on growth or infections. Objectif Etudier l'effet d'une administration quotidienne de supplements de fer pendant 14 semaines sur la concentration serique en fer et sur d'autres marqueurs relatifs au bilan en fer chez des nourrissons exclusivement allaites au sein en Gambie. Methodes Un essai randomise en double aveugle et controle contre placebo a ete mene en region rurale dans le pays entre le 3 aout 2021 et le 9 mars 2022. Au total, 101 nourrissons sains ages de six a dix semaines et exclusivement allaites au sein ont ete recrutes dans des centres de vaccination et par le biais d'agents de sante communautaires. Les nourrissons ont recu aleatoirement un supplement de fer (7,5 mg/jour de sulfate ferreux dans une solution de sorbitol) ou un placebo durant 98 jours. Des echantillons de sang veineux ont ete recoltes au debut de l'essai et au 99e jour afin de mesurer la concentration serique en fer et d'autres marqueurs relatifs au bilan en fer et au bilan hematologique. Resultats Au 99e jour, la concentration serique en fer etait considerablement plus elevee dans le groupe ayant recu un supplement de fer que dans celui ayant recu un placebo (difference brute moyenne: 2,5 [micro]mol/L; intervalle de confiance de 95%: 0,6 a 4,3), tandis que d'autres marqueurs relatifs au bilan en fer et au bilan hematologique affichaient de nettes ameliorations. Dix effets secondaires graves ont ete signales (cinq dans chaque groupe), 106 effets secondaires sans gravite (54 dans le groupe ayant recu un supplement de fer; 52 dans le groupe ayant recu un placebo) et aucun deces. Aucune difference majeure n'a ete constatee entre les groupes au niveau des episodes de diarrhee, fievre, toux, infection cutanee, infection oculaire ou ecoulement nasal signales par la mere. Conclusion Chez les nourrissons gambiens exclusivement allaites au sein, l'administration de supplements de fer des l'age de six semaines a entraine une nette amelioration des marqueurs relatifs au bilan en fer vers l'age de six mois. Rien n'indique que ces supplements aient eu des effets secondaires sur la croissance ou cause des infections. Objetivo Investigar el efecto de la administracion diaria de suplementos de hierro durante 14 semanas sobre la concentracion serica de hierro y otros marcadores del estado del hierro en lactantes alimentados exclusivamente con leche materna en Gambia. Metodos Entre el 3 de agosto de 2021 y el 9 de marzo de 2022, se llevo a cabo un ensayo con enmascaramiento doble, aleatorizado y controlado con placebo en areas rurales de Gambia. En total, se reclutaron 101 lactantes sanos, alimentados exclusivamente con leche materna y con edades comprendidas entre las 6 y las 10 semanas, en clinicas de vacunacion y a traves de trabajadores sanitarios comunitarios. Los lactantes se asignaron aleatoriamente para recibir suplementos de hierro (7,5 mg/dia como sulfato ferroso en solucion de sorbitol) o placebo durante 98 dias. Se recogieron muestras de sangre venosa al inicio del ensayo y el dia 99 para evaluar la concentracion serica de hierro y otros marcadores del hierro y el estado hematologico. Resultados En el dia 99, la concentracion serica de hierro era significativamente mayor en el grupo de suplementacion con hierro que en el grupo placebo (diferencia bruta de medias: 2,5 [micro]mol/L; intervalo de confianza del 95%: 0,6 a 4,3) y hubo mejoras significativas en otros marcadores hematologicos y de hierro. Abstract Background The World Health Organization recommends universal iron supplementation for children aged 6–23 months in countries where anaemia is seen in over 40% of the population. Conventional ferrous salts have low efficacy due to low oral absorption in children with inflammation. Haem iron is more bioavailable, and its absorption may not be decreased by inflammation. This study aims to compare daily supplementation with haem iron versus ferrous sulphate on haemoglobin concentration and serum ferritin concentration after 12 weeks of supplementation. Methods This will be a two-arm, randomised controlled trial. Gambian children aged 6–12 months with anaemia will be recruited within a predefined geographical area and recruited by trained field workers. Eligible participants will be individually randomised using a 1:1 ratio within permuted blocks to daily supplementation for 12 weeks with either 10.0 mg of elemental iron as haem or ferrous sulphate. Safety outcomes such as diarrhoea and infection-related adverse events will be assessed daily by the clinical team (see Bah et al. Additional file 4_Adverse event eCRF). Linear regression will be used to analyse continuous outcomes, with log transformation to normalise residuals as needed. Binary outcomes will be analysed by binomial regression or logistic regression, Primary analysis will be by modified intention-to-treat (i.e., those randomised and who ingested at least one supplement dose of iron), with multiple imputations to replace missing data. Effect estimates will be adjusted for baseline covariates (C-reactive protein, alpha-1-acid glycoprotein, haemoglobin, ferritin, soluble transferrin receptor). Discussion This study will determine if therapeutic supplementation with haem iron is more efficacious than with conventional ferrous sulphate in enhancing haemoglobin and ferritin concentrations in anaemic children aged 6–12 months. Corneal confocal microscopy (CCM) is emerging as a tool for identifying small fiber neuropathy in both peripheral neuropathies and neurodegenerative disease of the central nervous system (CNS). The value of corneal nerves as biomarkers for efficacy of clinical interventions against small fiber neuropathy and neurodegenerative disease is less clear but may be supported by preclinical studies of investigational agents. We, therefore, used diverse investigational agents to assess concordance of efficacy against corneal nerve loss and peripheral neuropathy in a mouse model of diabetes. Ocular delivery of the peptides ciliary neurotrophic factor (CNTF) or the glucagon-like peptide (GLP) analog exendin-4, both of which prevent diabetic neuropathy when given systemically, restored corneal nerve density within 2 weeks. Similarly, ocular delivery of the muscarinic receptor antagonist cyclopentolate protected corneal nerve density while concurrently reversing indices of systemic peripheral neuropathy. Conversely, systemic delivery of the muscarinic antagonist glycopyrrolate, but not gallamine, prevented multiple indices of systemic peripheral neuropathy and concurrently protected against corneal nerve loss. These data highlight the potential for use of corneal nerve quantification by confocal microscopy as a bridging assay between in vitro and whole animal assays in drug development programs for neuroprotectants and support its use as a biomarker of efficacy against peripheral neuropathy.

BackgroundIn low-income countries, point-of-care photometers are used in the screening and management of anaemia in individuals, but also in the assessment of population iron status when evaluating efficacy of intervention studies or public health interventions.AimsWe aimed to evaluate the accuracy of a commonly used photometer, HemoCue-301, in determining haemoglobin concentration among anaemic children aged 6-12 months in a field setting in rural Africa.MethodsThis report concerns a secondary analysis of data from Gambian infants being screened for an ongoing randomized controlled trial. In those found to be anaemic by HemoCue-301, haemoglobin concentration was measured by Sysmex XN-1500, an automated haematology analyser that was used as a reference. Passing-Bablok regression analysis was used to estimate the regression constant (systematic deviation between two measurement methods that remain consistent across the range of measurements) and proportional bias (systematic deviation between two measurement methods that change in magnitude relative to the value being measured).ResultsAnalysis was based on 227 participants. There was strong evidence of absolute bias among moderately anaemic participants (haemoglobin concentration at 8.0g/dL) (absolute bias: 1.12g/dL; 95% CI: 0.91 to 1.37g/dL; proportional bias: 14.0%; 95% CI: 11.4% to 17.1%) in haemoglobin concentrations measured by HemoCue-301 compared to those measured by Sysmex XN-Series1500. Bias was marginal at haemoglobin concentration of 11.0g/dL (absolute bias: -0.08g/dL; 95% CI: -0.18 to 0.07g/dL; proportional bias: -7.3%; 95% CI: -6.5% to 0.6%).ConclusionHaemoglobin measurements by HemoCue-301 seem substantially biased in participants with haemoglobin less than 8.0g/dL.

Abstract Introduction A recent analysis of Gambian infants showed that plasma iron concentrations decline rapidly from birth to levels below the 95% reference range within 5 months of age. To address this issue, a clinical trial was initiated to assess the efficacy of early iron supplementation in breastfed infants under 6 months of age (‘Iron Babies’; NCT04751994). To understand if such an intervention is feasible for scaleup, the acceptability, through the lens of local stakeholders and mothers (as the primary caregiver), must be considered. Methods An embedded qualitative study, therefore, explored acceptability through focus group discussions with local stakeholders and interviews with mothers. Four focus group discussions with local stakeholders (n = 19) and individual interviews (n = 14) with mothers whose infants were enrolled in the clinical trial were conducted. Qualitative data were analysed with respect to an implementation acceptability framework using Template Analysis. Results From the focus group discussions, there was a general sense of enthusiasm for the intervention, but also a concern due to confusion around nutritional messaging. The intervention was acceptable to mothers with limited mention of side effects and enthusiasm for participating in clinical trials on iron supplementation. However, when looking at scaleup, there were mixed opinions on the practicality of daily supplementation, as well as concerns around costs. In addition, it was suggested that proper dissemination of information and inclusion of all family members in decision‐making, especially fathers, is key to acceptability. Conclusion There was no indication that stakeholders or mothers were resistant to iron supplementation of breastfed infants.

Abstract Human neonates elicit a profound hypoferremia which may protect against bacterial sepsis. We examined the transience of this hypoferremia by measuring iron and its chaperone proteins, inflammatory and haematological parameters over the first post-partum week. We prospectively studied term, normal weight Gambian newborns. Umbilical cord vein and artery, and serial venous blood samples up to day 7 were collected. Hepcidin, serum iron, transferrin, transferrin saturation, haptoglobin, c-reactive protein, α1-acid-glycoprotein, soluble transferrin receptor, ferritin, unbound iron-binding capacity and full blood count were assayed. In 278 neonates we confirmed the profound early postnatal decrease in serum iron (22.7 ± 7.0 µmol/L at birth to 7.3 ± 4.6 µmol/L during the first 6–24 h after birth) and transferrin saturation (50.2 ± 16.7% to 14.4 ± 6.1%). Both variables increased steadily to reach 16.5 ± 3.9 µmol/L and 36.6 ± 9.2% at day 7. Hepcidin increased rapidly during the first 24 h of life (19.4 ± 14.4 ng/ml to 38.9 ± 23.9 ng/ml) and then dipped (32.7 ± 18.4 ng/ml) before rising again at one week after birth (45.2 ± 19.1 ng/ml). Inflammatory markers increased during the first week of life. The acute postnatal hypoferremia in human neonates on the first day of life is highly reproducible but transient. The rise in serum iron during the first week of life occurs despite very high hepcidin levels indicating partial hepcidin resistance. Trial Registration: clinicaltrials.gov (NCT03353051). Abstract The COVID-19 pandemic has affected all domains of human life, including the economic and social fabric of societies. One of the central strategies for managing public health throughout the pandemic has been through persuasive messaging and collective behaviour change. To help scholars better understand the social and moral psychology behind public health behaviour, we present a dataset comprising of 51,404 individuals from 69 countries. This dataset was collected for the International Collaboration on Social & Moral Psychology of COVID-19 project (ICSMP COVID-19). This social science survey invited participants around the world to complete a series of moral and psychological measures and public health attitudes about COVID-19 during an early phase of the COVID-19 pandemic (between April and June 2020). The survey included seven broad categories of questions: COVID-19 beliefs and compliance behaviours; identity and social attitudes; ideology; health and well-being; moral beliefs and motivation; personality traits; and demographic variables. We report both raw and cleaned data, along with all survey materials, data visualisations, and psychometric evaluations of key variables.

Crucial transitions in cancer-including tumor initiation, local expansion, metastasis, and therapeutic resistance-involve complex interactions between cells within the dynamic tumor ecosystem. Transformative single-cell genomics technologies and spatial multiplex in situ methods now provide an opportunity to interrogate this complexity at unprecedented resolution. The Human Tumor Atlas Network (HTAN), part of the National Cancer Institute (NCI) Cancer Moonshot Initiative, will establish a clinical, experimental, computational, and organizational framework to generate informative and accessible three-dimensional atlases of cancer transitions for a diverse set of tumor types. This effort complements both ongoing efforts to map healthy organs and previous large-scale cancer genomics approaches focused on bulk sequencing at a single point in time. Generating single-cell, multiparametric, longitudinal atlases and integrating them with clinical outcomes should help identify novel predictive biomarkers and features as well as therapeutically relevant cell types, cell states, and cellular interactions across transitions. The resulting tumor atlases should have a profound impact on our understanding of cancer biology and have the potential to improve cancer detection, prevention, and therapeutic discovery for better precision-medicine treatments of cancer patients and those at risk for cancer.

Abstract Background Polymorphisms in ATP2B4 coding for PMCA4b, the primary regulator of erythrocyte calcium concentration, have been shown by GWAS and cross-sectional studies to protect against severe malaria but the mechanism remains unknown. Methods Using a recall-by-genotype design, we investigated the impact of a common haplotype variant in ATP2B4 using in vitro assays that model erythrocyte stage malaria pathogenesis. Ninety-six donors representing homozygotes (carriers of the minor alleles, T/T (variant), heterozygote T/C and wildtype C/C (ancestral)) carriers of the tagging SNP rs1541252 were selected from a cohort of over 12,000 participants in the Keneba Biobank. This study focuses on the meaning and acquisition of the word thesaurus in monastic early medieval sources. The term essentially refers to objects characterized by strong symbolic, economic, cultural and sumptuary value. In the monastic Rules the word does not appear because the forms of accumulation and conservation of wealth were extraneous to the vow of poverty disciplined and emphasized by the drafters. With the growth of the monasteries, the promise of royal protection, the strengthening of ties with the power elites and the establishment of the institution of exemption, the treasures grow in importance, underlining the prestige acquired by the single monastery. The objects that compose it become a non-secondary element of what we can define the material memory of the monastic community, providing information on patrons, on the workshops active in the monastery, on the exchanges established with monastic foundations and with local communities, on donations from power groups active in the territory and finally on the subtraction and dispersion suffered.

Abstract Widespread, comprehensive sequencing of patient tumors has facilitated the usage of precision medicine (PM) drugs to target specific genomic alterations. Therapeutic clinical trials are necessary to test new PM drugs to advance precision medicine, however, the abundance of patient sequencing data coupled with complex clinical trial eligibility has made it challenging to match patients to PM trials. To facilitate enrollment onto PM trials, we developed MatchMiner, an open-source platform to computationally match genomically profiled cancer patients to PM trials. Here, we describe MatchMiner’s capabilities, outline its deployment at Dana-Farber Cancer Institute (DFCI), and characterize its impact on PM trial enrollment. MatchMiner’s primary goals are to facilitate PM trial options for all patients and accelerate trial enrollment onto PM trials. MatchMiner can help clinicians find trial options for an individual patient or provide trial teams with candidate patients matching their trial’s eligibility criteria. From March 2016 through March 2021, we curated 354 PM trials containing a broad range of genomic and clinical eligibility criteria and MatchMiner facilitated 166 trial consents (MatchMiner consents, MMC) for 159 patients. To quantify MatchMiner’s impact on trial consent, we measured time from genomic sequencing report date to trial consent date for the 166 MMC compared to trial consents not facilitated by MatchMiner (non-MMC). We found MMC consented to trials 55 days (22%) earlier than non-MMC. MatchMiner has enabled our clinicians to match patients to PM trials and accelerated the trial enrollment process.

Abstract Background Cancer cachexia (CC) is a severe complication during the last stages of the disease, which is characterized by the substantial loss of muscle and fat mass. Currently, there is no effective treatment of CC. Erythropoietin plays tissue‐protective role in different tissues. Based on the structure of erythropoietin, small non‐erythropoietic peptides were synthesized, which activate tissue‐protective signalling pathways. Methods Here, we investigated the influence of the tissue‐protective peptide ARA 284 on CC in rats using the Yoshida hepatoma model. IntroductionSevere acute respiratory syndrome coronavirus-2 (SARS-CoV-2) transmission frequently occurs within households, yet few studies describe which household contacts and household units are most likely to engage in transmission-interrupting behaviors.MethodsWe analyzed a COVID-19 prospective household transmission cohort in North Carolina (April to October 2020) to quantify changes in physical distancing behaviors among household contacts over 14 days. We evaluated which household contacts were most likely to ever mask at home and to ever share a bedroom with the index case between days 7–14.ResultsIn the presence of a household COVID-19 infection, 24% of household contacts reported ever masking at home during the week before study entry. Masking in the home between days 7–14 was reported by 26% of household contacts and was more likely for participants who observed their household index case wearing a mask. Participants of color and participants in high-density households were more likely to mask at home. After adjusting for race/ethnicity, living density was not as clearly associated with masking. Symptomatic household contacts were more likely to share a bedroom with the index case. Working individuals and those with comorbidities avoided sharing a bedroom with the index case.DiscussionIn-home masking during household exposure to COVID-19 was infrequent in 2020. In light of the ongoing transmission of SARS-CoV-2, these findings underscore a need for health campaigns to increase the feasibility and social desirability of in-home masking among exposed household members. Joint messaging on social responsibility and prevention of breakthrough infections, reinfections, and long COVID-19 may help motivate transmission-interruption behaviors.

Understanding collective behaviour is an important aspect of managing the pandemic response. Here the authors show in a large global study that participants that reported identifying more strongly with their nation reported greater engagement in public health behaviours and support for public health policies in the context of the pandemic.

Background: In many countries, non-pharmaceutical interventions to limit severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) transmission resulted in significant reductions in other respiratory viruses. However, similar data from Africa are limited. We explored the extent to which viruses such as influenza and rhinovirus co-circulated with SARS-CoV-2 in The Gambia during the COVID-19 pandemic. Methods: Between April 2020 and March 2022, respiratory viruses were detected using RT-PCR in nasopharyngeal swabs from 1397 participants with influenza-like illness.
