Being the most cost-dominant component of the logistics management system, transport plays a vital role in the realization of trade activities across Europe, both on a national and micro level. Various indicators determine the efficiency of the realization of transport activities, amongst which the following hold high importance: quality of transport infrastructure (air, road, maritime, etc.), transport safety and security elements, as well as export/import procedures, etc. This paper aims to perform a comparative analysis among chosen European countries (focusing on CEE countries) concerning the level of transport management preparedness based on the previously mentioned elements. The analysis is performed by using the PROMETHEE II and entropy method. The obtained results should help determine the differences and similarities in transport management preparedness to synchronize better future national policies, strategies, and procedures concerning this issue.

Adequate management of logistics activities is essential for the efficiency of all business activities of a company. Goods home delivery as a concept has gained prominence with the development of internet and e-commerce. During the last decade, owing to the increasingly intense IT development, it has become relevant in the contemporary logistics business. Within this concept, the temporal dimension is important, which is imposed precisely because of the demands of modern lifestyle. The average customer wants more free time,so the goods home delivery option significantly shortens the time required for shopping, leading to preference for online sellers who make delivery as a standard part of their offer. Considering the logistics aspects of the mentioned concept and its implementation on the example of a precise company(Amazon) are the basic subjects of analysis within the paper. The aim of the paper is to indicate that the modern delivery market, in addition to existing major players, is also characterized by the entry of startups, which result from entrepreneurial initiatives. The particularity of engaging entrepreneurs is to develop a new idea embodied in a business philosophy or new technological ideas, whether to solve the existing challenge or to see and take advantage of the new market opportunities.

11. Using respiratory polygraphy in diagnosing obstructive sleep apnea - our experiences

Bacground/Aim. Obstructive sleep apnea (OSA) involves repeated episodes of cessation of breathing that occur due to a decrease in pharyngeal muscle tone. This disorder is more common in men and represents a significant risk factor for serious cardiovascular and cerebrovascular events. The gold standard in the diagnosis of this disorder represents a polysomnography (PSG), which is technically a complex and multidisciplinary method. Respiratory polygraphy (RP) may constitute an adequate replacement for most uncomplicated cases of obstructive sleep apnea. The aim of the study was to examine the efficacy of using respiratory polygraphies in diagnosing of obstructive sleep apnea. Methods. On all the patients with suspected obstructive sleep apnea, RP and a retrospective analysis of the obtained results were performed. Results. By completing our examination, we proved that there was a positive correlation between the results obtained by using RP and the predictors of obstructive sleep apnea such as the Epworth sleepiness scale (EPWORTH) score, neck circumference and body mass index. Conclusion. Respiratory polygraphy represents a cheaper and simpler replacement for PSG, especially with uncomplicated obstructive breathing disorders during sleep.

Business environment at the beginning of the 21st century is characterized by disruptive and frequent changes. These changes lead to restructuring of some traditional industries, such as automotive industry. Great recession, technology innovations and changes in government regulations unfreezed the structure of automotive industry. Consequently, competitors in the industry have to fit their strategies to new reality. Great recession resulted in financial troubles or even bankruptcy of some significant incumbents and created market space for late followers from emerging markets. Digital innovations have created preconditions for emergence of autonomous cars and carsharing and ridesharing service industry. Global pollution puts pressure on national governments to establish higher gas emission standards and ban diesel vehicles in city centers, which encourages development of electric cars. It is hard to predict the course of development of carsharing service industry, autonomous and electric cars due to high risks and uncertainties. Therefore, auto companies focus their efforts on some smaller market segments, form strategic alliances and acquire targets with complementary resources.

13. Overall survival of patients with non-small cell lung cancer after surgery treatment

Background/Aim. Lung cancer is one of the most common malignant tumors. About 80% of all lung cancers are non-small cell lung cancer (NSCLC). According to histopathological characteristics, the most common types of NSCLC are squamous cell carcinoma and adenocarcinoma. The aim of this study was to evaluate the overall survival rate in the NSCLC patients initially received surgery according to its histopathological type and T – primary tumor, N –regional lymph nodes, M – distant metastasis (TNM) stages which were treated with surgical treatment, and after that, according to the TNM stage, chemotherapy protocols and/or radiation therapy. Methods. This retrospective case series study included all patients with NSCLC admitted to the Military Medical Academy in Belgrade in the period 2010–2015. A total number of selected patients was 85 (27 females and 58 males). Results. Out of 41 patients with squamous cell carcinoma, 19.5% deceased. On the other hand, in the group of patients with adenocarcinoma, 43.2% out of 44 patients deceased. The average cumulative survival was statistically significantly lower in the adenocarcinoma patients in comparison to the patients with squamous cell carcinoma (1,605.2 vs.1,304.8 days; p = 0.005). On the other hand, the average cumulative survival was statistically significantly lower in our patients in the recurrence group with adenocarcinoma in comparison to the recurrence group with squamous cell carcinoma (1,212.8 vs. 1,835.5 days; p = 0.032). Conclusion. Adenocarcinoma is more aggressive cancer in comparing to squamous cell carcinoma with lower overall survival in comparing to squamous cell carcinoma. Additional studies are needed to identify risk factors for recurrence after surgery, and to additionally explain role of tumor markers and molecular biological techniques in the progression of this kind of cancer.

16. Specific bronchial hyperreactivity and hypersensitivity in patients with allergic asthma

Background/Aim. Bronchial asthma is a disease that is characterized by the variability of the clinical picture, physical and functional status and the existence of bronchial hypersensitivity and hyperreactivity with varying degrees. Bronchial responsiveness and sensitivity are tested in patients with clinically suspected existence of asthma and normal spirometry test. The aim of the study was to analyze the patients with atopic asthma and study test results of skin sensitization to inhaled allergens, nonspecific bronchial hyperreactivity and specific hyperreactivity estimated by bronchial provocation tests with inhalant allergens. Methods. The prospective study at the Pulmonology Clinic of the Military Medical Academy in Belgrade Serbia, during 2014, included 70 male subjects aged 18–30 years, who had perennial asthma symptoms. All subjects were nonsmokers, with normal spirometry findings, with normal radiological chest findings and with no symptoms of respiratory infection over the past two months. All respondents were tested with skin prick tests with inhalant allergens and nonspecific bronchial provocation test with histamine. On the basis of histamine test, subjects were divided into two groups: the group I, in which there was a slight degree of hypersensitivity [provocation concentration of histamine causing a 20% fall in forced expiratory volume – PC20 = 6.09 ± 1.1 mg/mL], and the group II with negative histamine test (PC20 = 14.58 ± 6.34 mg/mL). Specific bronchial provocation test was performed in all patients, and the selection of the allergens was carried out based on the results of testing of skin hypersensitivity. Results. Results of skin sensitization show the highest incidence of mites Dermatophagoides pterronissinus (83.3% group I and 85.0% group II) followed by grass pollen (53.3% group I and 52.0% group II), and house dust (33.3% group I and 50.0% group II). There were no statistically significant differences in allergens between groups (p > 0.05). In both groups, spirometry findings were within normal values [forced vital capacity – FVC and forced expiratory volume 1 – FEV1 > 80% predictive value], but statistically significant difference was found in FEV1 between groups (p < 0.05). Specific bronchial provocation tests with solutions of inhaled allergens in both groups caused a significant decline in FEV1 (≥ 20%) in all patients individually. No statistically significant differences were found neither between groups, nor between individual allergens (average decline in FEV1: Group I 32.9 ± 2.4% and group II 31.5 ± 2.2%). Conclusion. There is no relationship between the degree of specific and non-specific bronchial hyperreactivity in patients with allergic asthma.

Radical changes of business environment have been caused by growing globalization at the beginning of the 21st century. Consequently, a strong network of trade and investment relationships between national economies has been established. Although it entails many risks, global business environment opens up great opportunities, as well. In order to seize these opportunities, many companies from emerging markets internationalize their business operations, and thus challenge once untouchable leaders from developed countries. Traditional theories on multinational companies cannot fully explain their motives for going abroad. Recently, the automotive industry has become a new field where the late followers from emerging markets try to challenge the leaders. This paper includes the following three case studies: Hyundai - Kia, Geely - Volvo, and Tata Motors - Jaguar Land Rover. These case studies show that although the late followers in the automotive industry use different internationalization strategies, they all perceive internationalization as a means of obtaining the lacking competencies and building up a positive reputation.

Introduction. Pseudomesotheliomatous lung carcinoma is a special, rare entity characterized by large pleural growth and minor invasion of lung tissue. Clinically, radiologically, macroscopically and even histologically this tumor can be misdiagnosed as malignant pleural carcinoma. Case report. We represent a 64-year-old male patient, former smoker. Due to difficulties in the form of dry cough, feeling of dis-comfort and pain in the right hemithorax, fatigue, heavy breathing, sweating, fever up to 39.6°C the patient was treated as with combined antibiotic therapy (macrolides, cephalosporins and penicillin), but without improving of his condition. Chest radiography showed a shadow of pleural effusion by the height of the front end of the third right rib. Chest MSCT showed the extremely thickened pleura apically and to the posterior along the upper right lobe in addition to existence of massive pleural effusion. Subpleural condensation of parenchyma ranging about 30 mm was described in the upper right lobe. Cytological analysis of the pleural effusion showed the presence of malignant cells impossible to differentiate whether they were metastasis of adenocarcinoma or malignant pleural mesothelioma. By histochemical and immunohistohemical analyses of a pleural sample, pseudomesotheliomataus lung adenocarcinoma was diagnosed. Conclusion. Pseudomesotheliomataus carcinoma of the lungs can be a diagnostic problem. Its diagnosis is based on recognition of histopathological characteristics which enable its discernment from the epithelial variant of malignant pleural mesothelioma.

19. Hereditary hemorrhagic telangiectasia with bilateral pulmonary vascular malformations: A case report

Introduction. Hereditary hemorrhagic telangiectasia (HHT) also known as Osler-Weber-Rendu syndrome is an autosomal dominant disease that occurs due to vascular dysplasia associated with the disorder in the signaling pathway of transforming growth factor β (TGF-β). The clinical consequence is a disorder of blood vessels in multiple organ systems with the existence of telangiectasia which causes dilation of capillaries and veins, are present from birth and are localized on the skin and mucosa of the mouth, respiratory, gastrointestinal and urinary tract. They can make a rupture with consequent serious bleeding that can end up with fatal outcome. Since there is a disruption of blood vessels of more than one organic system, the diagnosis is very complex and requires a multidisciplinary approach. Case report. We reported a 40-year-old female patient with a long-time evolution of problems, who was diagnosed and treated at the Clinic for Lung Diseases of the Military Medical Academy in Belgrade, Serbia, because of bilaterally pulmonary arteriovenous malformations associated with HHT. Embolization was performed in two acts, followed with normalization of clinical, radiological and functional findings with the cessation of hemoptysis, effort intolerance with a significant improvement of the quality of life. Conclusion. HHT is a rare dominant inherited multisystem disease that requires multidisciplinary approach to diagnosis and treatment. Embolization is the method of choice in the treatment of arteriovenous malformations with minor adverse effects and very satisfying therapeutic effect.

With the increase of natural and man-made disasters worldwide such as earthquakes, tsunamis, pandemics, wars etc. the role of adequate management of humanitarian logistics is becoming more and more prominent on the disaster relief agenda. One of the main aims of the humanitarian logistics operations is to eliminate the negative effects of the disaster and to do so numerous actors must be involved in the process (governments, NGOs, military, civil societies and humanitarian organizations). The implementation of disaster relief actions and coordination of various and often conflicting goals of the mentioned actors is facilitated by the usage of smart, digital solutions which are becoming an irreplaceable tool for the realization of humanitarian logistics activities. The analysis within this paper considers the usage, role and benefits of chosen digital solutions for the facilitation of humanitarian logistics operations. The results of the analysis based on the secondary data sources show threefold conclusions regarding the adequate implementation of the analyzed digital solutions in aiding the humanitarian logistics operations. First of all, the ability of humanitarian logistics to respond in an efficient manner to natural disasters is influenced by the adequate selection of an appropriate digital solution and by the level of individual training to use the precise solution. Second, the community facing a disaster can use the full potential of chosen digital solutions only if it has the capabilities to support its demands and the capacities for this type of interaction. Finally, the usage of adequate digital solutions in disaster stricken areas definitively shows positive results in practice when it comes to annihilating fatal outcomes and harsh situations for the environment. Pored ključne primene u realizaciji komercijalnih aktivnosti velikog broja privrednih subjekata, upravljanje logističkim procesom ima i važnu ulogu podrške u operacijama humanitarnog karaktera. Date operacije se odnose na neutralisanje negativnih efekata po različite društvene zajednice širom sveta izazvanih prirodnim katastrofama (požari, zemljotresi, epidemije i sl.) ili onim ljudskog porekla (ratovi, političke nestabilnosti i sl.). Takve okolnosti iznedrile su specifičan logistički proces tzv. upravljanje humanitarnom logistikom. Predmet analize u ovom radu jeste identifikacija i razmatranje osobenosti ključnih faza, tj. aktivnosti procesa humanitarne logistike. Cilj rada je dvojak. U prvom redu, želja autora rada je da dodatno promovišu relativno nov koncept tzv. humantirane logistike u regionalnim naučnim i poslovnim krugovima. Pored toga, drugi cilj je da se dobro upozna svaka od faza humanitarnog logističkog procesa, kako bi se svakom od njih što bolje ovladalo i upravljalo u praksi.

23. Primary pulmonary alveolar proteinosis

Introduction. Pulmonary alveolar proteinosis is an uncommon disease characterized by the accumulation of surfactant proteins and phospholipids within the alveolar spaces. Acquired disease can be idiopathic (primary) and secondary. The prevalence of acquired pulmonary alveolar proteinosis is about 0.37 per 100,000 persons. Common symptoms are dyspnea and cough. Chest X-ray shows bilateral perihilar infiltrates. Open-lung biopsy is the gold standard for the diagnosis. Treatment includes whole-lung lavage, application of granulocyte-macrophage colonystimulating factor and lung transplantation. Case report. We reported a 51 year-old man with primary form of the disease. It was the second case of this extremely rare disease in the past 30 years in our clinic. The symptoms were longlasting dry cough, fever and physical deterioration. Chest Xray revealed bilateral pulmonary infiltrates; computed tomography showed patchy ground-glass opacification with interlobular thickening. The diagnosis was established by open lung biopsy. Additional tests were performed to exclude secondary form of the disease. Conclusion. We presented a rare clinical entity with typical clinical features and clinical and radiological course of the disease, in order to improve differential diagnostic approach to patients with bilateral lung infiltrations. In patients with pulmonary alveolar proteinosis timely diagnosis and adequate treatment can improve a prognosis.

24. Symptoms, physical findings and bronchial hypersensitivity in patients with bronchial asthma and normal spirometry

Background/Aim. The diagnosis of bronchial asthma, a chronic inflammatory disease of the respiratory tract, is made on the basis of anamnesis, pathologic auscultatory findings of the lungs, lung function disturbances, skin tests, as well as the basic indices of immunologic condition in bronchial trunk. The aim of the study was to find out correlation of objective indices of the disease and than relation with the symptoms in the patients with bronchial asthma. Methods. The study included 60 young male non smokers with long lasting symptoms of bronchial asthma including shortness of breath, wheezing, hard breathing, nonproductive or productive cough, weakness and night hard breathing. There were no symptoms of respiratory infection over the past two months and lung radiography and spirometry were normal. Based on the results of nonspecific bronchoprovocative test two groups of the patients were formed, group I (n = 30) with positive histamine test (average value of the inhaled histamine concentration with FEV1 drop by 20% in regard with the initial value (PC20) = 2.99 ± 0.51 mg/ml of histamine) and group II (n = 30) with negative histamine test (PC20(a) = 14.58 ± 6.34 mg/ml of histamine). Results. The obtained spirometry results revealed a statistically significant difference in values of FEV1 between groups: I group - FEV1 = 93.2%; II group - FEV1 = 101.8%; (p < 0.05, Wilcoxon test), although all the FEV1 values were normal. Regarding the presence of the most common symptoms there was not statistically significant difference between the groups (p > 0. 05, chisquare test). Pathologic auscultatory lung findings were found in 73.4% of the patients in the group I and 27.5% of the patients in the group II. There was statistically significant difference (p < 0.05, chi-squared test). A positive correlation between the degree of hypersensitivity and lung physical findings was confirmed (p < 0.05 Spearman's rho), but there was no correlation with FEV1 values. Conclusion. There is a correlation with lung pathologic physical findings, lower values of FEV1 (in a range of normal values) and the degree of nonspecific bronchial sensitivity as objective indices of activity of bronchial asthma. There is no correlation of these parameters with patient's symptoms as subjective indices of bronchial asthma.

Huge changes in marketing strategies and, more generally - in business philosophies are not so often. During the last 20 years global and/or national companies have already defined their general approaches of marketing instruments implementation. Therefore, when a contemporary company changes something in its marketing approach, it usually only refers to 'subtle adjustment' of the already determined strategy. Another reason for changes in a particular segment of marketing is a crisis of some kind that forces firms to implement innovations, especially regarding the elements such as costs - price - or service quality. A global enterprise segment consisting of low tariff airline companies is here identified as the one that has dramatically changed its marketing approach within the last 20 years, which continually resulted in its improved market position and business results. Changes in marketing strategies of low-cost companies have truly been revolutionary and are completely the consequence of recognizing the real needs of clients who use services of air transport. The success of low-cost airline companies is forcing the traditional, classic, air carriers to adapt their marketing instruments to this new model of business management. This paper also identifies one very significant assumption of the low-cost carriers phenomenon and their marketing approach - the liberalization of the global air transport market.

Supply chain management represents new business philosophy and includes strategically positioned and much wider scope of activity in comparison with its "older brother" - management of logistics. Philosophy of the concept of supply chain is directed to more coordination of key business functions of every link in distribution chain in the process of organization of the flow of both goods and information, while logistic managing instruments are focused on internal optimum of flows of goods and information within one company. Applying the concept of integrated supply chain among several companies makes the importance of operative logistics activity even greater on the level of one company, thus advancing processes of optimum and coordination within and between different companies and confirms the importance of logistics performances for the company’s profitability. Besides the fact that the borders between companies are being deleted, this concept of supply chain in one distribution channel influences increasing of importance of functional, i.e. traditional business managing approaches but instead it points out the importance of process managing approaches. Although the author is aware that "there is nothing harder, more dangerous and with uncertain success, but to find a way for introducing some novelties (Machiavelli), it would be even his additional stimulation for trying to bring closer the concept and goals of supply chain implementation that are identified in key, relevant, modern, theoretical and consulting approaches in order to achieve better understanding of the subject and faster implementation of the concept of supply chain management by domestic companies.

29. Potential role of cholesterol in distinguishing malignant from benign pleural effusion

Cholesterol and carcinoembryonic antigen (CEA) levels in pleural effusion and sera, were measured in 199 patients with pleural effusions of various origins. Malignant cause was found in 93, and nonmalignant in 106 patients. Mean cholesterol level in sera of patient with malignant disease was 5.0 ± 0.93mmol/L, and in nonmalignant group 4.34 ± 1.32 mmol/L. The difference was not statistically significant. Mean cholesterol level in nonmalignant pleural effusions was higher thAn those in malignant (2.51 ± 1.23 mmol/L; and 2.28 ± 1.06 mmol/L), but the difference was also not significant. Average pleural fluid/serum cholesterol ratio (HolI/S) in nonmalignant group was 0.61 ± 0.32 and in malignant group 0.46 ± 0.22. The difference between those mean values was significant. Higher ratio, at the cut off value of 0.5 was found in 79/106 and in 25/93 malignant patients. Calculated sensitivity was 75%, specificity 73%, positive predictive value 76%, negative predictive value 65% and accuracy 69%. In patients with chronic respiratory diseases that last since the early childhood, primary ciliary dyskinesia (PCD) needs to be considered. Four patients reviewed in this paper were with typical disease history and clinical picture, as well as clear ciliary axonema damage. Complete examination was performed in all the patients, including bronchoscopy with bronchography, and the examination of the biopsy samples of respiratory airways’ mucous membrane, obtained by transmission electron microscope (TEM). In two of the patients spermatozoa were also examined by TEM. Large anatomic deffects of airways were found in all the patients, but pulmonary function was normal (except in one case), representing one of PCD’s significant characteristics. First two cases fulfilled the criteria for Kartagener’s syndrome, which was initially sufficient for the diagnosis of PCD.

That is followed by the structure of the "milk flow" in Serbia, with basic performances of the market and analysis of most important participants in the market. Military logistics as a specific area of logistics management makes the difference between a wellsupplied and self-sustainable army which due to an efficiently created supply chain, can perform it's military actions more efficiently, and an army that, due to insufficient commitment to military logistics activities creates an inefficient army that is cut off from food, water, and other basic resources without which the army can't function. Military logistics implies a complex system, formally separate, but essentially very close to business logistics. The aim of this review paper is to point out the features that distinguish military logistics management from business logistics management, but also to point out the complexities and limitations that military logistics brings with it. The methodology used for pointing out the abovementioned goals will be literature review and comparative analysis. The key determinants of the difference will be presented through the organization of the logistics supply chain, the subjects, participants and goals it involves, but also the risk, the corresponding law, logistics process management approach, innovations, organization systems, and the basic methods of transportation that military logistics brings with it. The given specifics need to be analyzed and compared with the key features of business logistics, where through the examples from practice will be pointed out the points of contact, but also the key differences between these two areas of logistics. The results of the analysis show that specifics of military logistics make this branch of logistics special, formally separated, but essentially very close to the area of business logistics. Složena i dinamična poslovna filozofija upravljanja lancem snabdevanja predmet je preokupacije stručne i naučne javnosti od momenta njenog koncipiranja, tri decenije unazad. Dokaz plodotvornosti ove filozofije je mnoštvo poslovnih koncepata koji su razvijeni pod njenim okriljem, a zatim i uspešno primenjeni u praksi. Jedan od najaktuelnijih koncepata te vrste jeste koncept zelenog lanca snabdevanja. Bazirajući koncept zelenog lanca snabdevanja jednako na ekološkim, socijalnim i ekonomskim principima, usmerenost ka ostvarivanju konkurentnosti ovakvog lanca omogućena je putem aktivnosti njegove ključne dimenzije- povratne logistike. Utvrđivanje međuzavisnosti koncepta zelenog lanca snabdevanja i povratne logistike, kao njegove ključne dimenzije i faktora koji utiču na potencijal te veze, polazište je svih istraživanja u disertaciji. Celokupna disertacija podeljena je u tri glavne celine. U okviru prve celine tretiraju se pitanja vezana za promenu paradigme u lancu snabdevanja i prelazak sa tradicionalnog na zeleni lanac- glavni koncepti koji nastaju kao posledica te promene, globalni regulatorni okvir za odabrane zemlje koji se odnosi na ovo pitanje, osnovne dimenzije zelenog lanca snabdevanja i način ostvarivanja njegove konkurentnosti. Druga celina posvećena je analizi aktivnosti povratne logistike- njenom profilisanju, utvrđivanju povratnog logističkog procesa i njegovih entiteta, kao i razmatranju modela ključnih faktora primene povratne logistike. Konačno, poslednja celina vezana je za empirijsku proveru da li su globalno afirmisani modeli ključnih faktora primene povratne logistike adaptibilni uslovima srpske poslovne prakse, te od čega zavisi uticaj povratne logistike na konkurentnost zelenog lanca snabdevanja u srpskoj poslovnoj praksi i da li je taj uticaj uvek jednoobrazan., A complex and dynamic business philosophy of supply chain management is the subject of preoccupation of the scientific and expert public since the moment of it's formation, three decades ago. The proof of the fruitfulness of this philosophy is a great number of business concepts which have been developed under it's wing, and then successfully implemented in practice. One of the newest concepts of that kind is the concept of green supply chain. Establishing the concept of green supply chain equally on ecologic, social and economic principles, the focus of achieving competitiveness of this chain is enabled by the activities of it's key dimension- reverse logistics. Determining the relationship between the concept of green supply chain and reverse logistics, as it's key dimension, as well as factors which influence the potential of this relationship, is the starting point of all research within the dissertation. The entire dissertation is divided into three main parts. Within the first part, issues connected with the supply chain paradigm change and the transfer from the traditional onto the green supply chain are discussed: main concepts derived as the consequence of the change, global regulatory framework for selected countries which refers to this issue, basic dimensions of the green supply chain, as well as the way of achieving it's competitiveness. The second part is dedicated to the analysis of reverse logistics activities- it's profiling, determining the reverse logistics process and it's entities, as well as the revision of reverse logistics key factors of implementation models. The paper is divided into three segments: the definition of artificial intelligence concept, its usage in international logistics management and the analysis of practical options of its implementation. The expected research contribution is to point out the importance and full capacities of using one such Logistics 4.0 concept as artificial intelligence in improving the management of international logistics operations.
