The psychometric properties of the Persian version of the cyberbullying perpetration questionnaire were assessed in this study, which included reliability, validity, confirmatory factorial analysis, and gender differences in youth. This is a correlational research design that adheres to the common principles and techniques of psychometrics. It is an applied survey study that was conducted in 2022 and is based on a test construction methodology. 474 participants were selected from the statistical population, which consisted of young individuals aged 18 to 35 residing in Tehran and Shiraz, using convenience sampling. The cyberbullying Perpetration Questionnaire (CPQ) was translated in four stages. Using Cronbach’s alpha, Pearson correlation coefficient, confirmatory factorial analysis, and independent-samples t-tests at a significance level of 0.05, the collected data were analyzed in SPSS and AMOS software version 26. A one-factor model with fit indexes that were favorable (X2/df= 2.613, GFI= 0.953, IFI= 0.950, CFI= 0.949, PCLOSE= 0.109, RMSEA= 0.058) was supported by the confirmatory factorial analysis. Objective: Astaxanthin (ASX) is a lipid-soluble keto-carotenoid with several biological effects. These effects may benefit polycystic ovarian syndrome (PCOS) patients. Imbalanced apoptosis/anti-apoptosis signaling has been considered the major pathogenesis of PCOS. In a randomized clinical trial, we tested the impact of ASX on the apoptotic pathway in PCOS granulosa cells (GCs). The present study hypothesizes that ASX may improve apoptosis in PCOS patients.Materials and Methods: This trial recruited patients with confirmed PCOS. A total of 58 patients were randomly assigned to take ASX (12 mg) or placebo for 8 weeks. Aspirated follicular fluid (FF) and blood samples were taken from both groups to measure BAX and BCL2 protein expression. Following FF aspiration, GCs from both groups were obtained; Real-Time PCR and Western blotting were used to evaluate the apoptotic pathway’s gene and protein expression levels in GCs.BAXBCL2Results: In GCs analysis, ASX reduced DR5 gene and protein expression after 8 weeks compared to placebo(p0.05) and BAX (p0.05).Conclusion: It seems that ASX consumption among women with PCOS improved serum and FF levels of apoptotic factors and modulated genes and protein expression of the apoptosis pathway in GCs. Nevertheless, further investigations are needed to reveal the potential role of this compound in PCOS treatment.

IntroductionNumerous consensus documents worldwide address luteinizing hormone (LH) supplementation in controlled ovarian stimulation, yet to the best of our knowledge, only one consensus paper has been published in the Arab region. This study presents a Delphi consensus by seven Iranian infertility experts, offering real-world clinical perspectives. The aim was to develop evidence-based opinions on LH's role alongside FSH in various aspects of assisted reproductive technology (ART), including LH levels, monitoring, r-hLH use, and suggested activity.MethodsEmploying the Delphi consensus approach, the Iran consensus unfolded in three steps. In Step 1, eight out of 10 statements gained approval, while two unclear statements were removed. In Step 2, the 20-member extended panel voted on the remaining eight statements.ResultsOnly one (statement 3) lacked consensus (55% agreement), prompting a modification. The revised statement (noted as statement 3′) obtained an 83% agreement.DiscussionThe clinical perspectives included in this consensus complement clinical guidelines and policies that help further improve treatment outcomes, especially for patients with FSH and LH deficiencies.

This work aims to study the anisotropic thermal impacts of employing liquid metal fluids including Na and NaK as coolants in two metallic microchannel heat sinks. The two studied microchannel heat sinks have aspect ratios of 1 and 2 while their hydraulic diameters are 267 µm and 400 μm. In the first stage, the accuracy and reliability of the developed numerical model are verified against benchmark problems and experimental data. In the second stage, the utilized verified and validated numerical model is extended to forced convection of Na and NaK in metallic microchannel heat sinks. The investigated metallic microchannel heat sinks are assumed to be out of stainless steel and Nickel with thermal conductivities of 17 W/mK and 90 W/mK, respectively. Obtained results revealed that NaK provides higher Nusselt numbers than Na regardless of the microchannel aspect ratio and hydraulic diameter. The effect of the microchannel heat sink solid substrate thermal conductivity on the temperature and heat flux distributions over the microchannel walls are also investigated comprehensively. For the investigated steady state Na and NaK flow within metallic microchannel heat sinks, the thermal conductivity of the heat sink solid substrate is observed to have a significant effect on the local temperature and heat flux distributions over microchannel walls.

Background: Patient satisfaction is considered as one of the most important elements of the evaluation of healthcare centers. In this research, the level of satisfaction of infertile patients was evaluated. Methods: This study is a cross-sectional descriptive study. The research population is infertile patients who referred to the infertility clinic (at least 4 visits) and hospitalized in four hospitals affiliated with the Tehran University of Medical Sciences. The satisfaction questionnaire was valid researcher-made which included 56 questions in 9 areas, and were collected from May 2021 to September 2022. In order to comply with the ethical principles in research, hospitals were named 1-4.The results were analyzed using the chi-square test or Fisher's exact test for qualitative variables and analysis of variance for quantitative variables in SPSS software, version 24 (SPSS Inc., Chicago, IL, US). Results Three hundred–four infertile women in four hospitals were studied. The mean age of the participants was 33.8 years, with a standard deviation of 5.8 years. The average satisfaction with admission and discharge services, medical services, nutrition, accounting, and insurance services in the number-1 Hospital had the highest score, and the average satisfaction with nursing or midwifery services, diagnostic and therapeutic services (radiology/laboratory), hospital services, charter patient rights, management services, paid expenses in Hospital 4 received the most points. The mean score of satisfaction was the highest among those who visited the infertility clinic of Hospital 3 (161.54±21.41). Average satisfaction with admission and discharge services, medical services, nutrition, accounting, and insurance services in Hospital No. 1 had the highest score, and average satisfaction with nursing or midwifery services, medical diagnostic services (radiology/laboratory), hospital services, respecting the patient's rights, management services, paid expenses in hospital number 4 got the most points. Introduction: Breast cancer is the leading cause of malignancy related deaths in women worldwide. In 20%-30% of breast cancers, HER2 gene amplification or overexpression is observed. Determination of HER2 amplification is performed mainly by IHC technique to measure its protein expression on the cell surface and by FISH to check gene amplification. This study was aimed to compare the sensitivity and specificity of IHC in comparison to the FISH method for detection of HER2 gene amplification. Abstract Genetic defects in the development, maturation, and/or function of the immune cells can lead to Inborn errors of immunity (IEI) which may predispose patients to malignancies. The overall risk for cancer in children with IEI ranges from 4 to 25% and the type of malignancy is highly dependent on the specific mutant gene underlying IEI. We investigated 3056 IEI patients registered in the Iranian national registry between the years 1999 and 2020 in this retrospective cohort study. The frequency of malignancy and its association with the type of IEI in these patients were evaluated. A total of 82 IEI patients with malignancy were enrolled in this study. Among them, predominantly lymphoma was the most common type of malignancy (67.1%), followed by leukemia (11%), and cancers of the head and neck (7.3%). Among identified lymphoma cancers, non-Hodgkin’s lymphomas were the most frequent type (43.9%) followed by different subtypes of Hodgkin’s lymphoma (23.2%). Solid tumors (18.3%) appeared to be very heterogeneous by type and localization. The correlation between the type of malignancy and survival status and the association between the type of malignancy and IEI entities were unremarkable. The awareness of the association between the presence of IEI and cancer highlights the importance of a synergistic effort by oncologists and immunologists in the early diagnosis of malignancy and personalized therapeutic strategies in IEI patients.

Background: The purpose of this prospective study was to compare the ovarian response and pregnancy outcomesin the infertile women with endometrioma undergoing assisted reproductive technologies (ART) in twogroups, who were underwent laparoscopic cystectomy and received gonadotropin releasing hormone-agonist(GnRH-agonist) and who only received GnRH-agonist without any surgery.Materials and Methods: In this prospective clinical trial study, 79 infertile women with asymptomatic endometriomascyst (2-6 cm) were enrolled and randomly assigned to two groups. First group underwent laparoscopiccystectomy and received GnRH-agonist. Second group only received GnRH-agonist without any surgery. Followingovulation induction, all patients underwent intracytoplasmic sperm injection (ICSI). Different parameterssuch as the number of retrieved oocytes and embryos; were made our outcomes that analyzed using SPSS.Results: The pregnancy rate, chemical and clinical, and live birth rate were higher in the combined group,although these differences were not statistically significant (48.48% vs. 30.8%, P=0.12, 36.36% vs. 25.6%,P=0.32, 36.36% vs. 23.1%, P=0.29). The number of injections, antral follicles, retrieved oocytes, mature oocytes,total embryos, transferred embryos and duration of stimulation were similar in two groups.Conclusion: Laparoscopic cystectomy followed by receiving GnRH-agonist improves pregnancy outcomes in endometriomaprior to treatment with ART (registration number: IRCT201106116689N2).

Abstract Background COVID-19 has affected the training programs and the clinical schedules of surgical wards in many countries, including Iran. Also, the continuous involvement with COVID-19 patients has caused stress in health care workers; among them, residents are on the frontlines of care delivery. Therefore, we designed a study to assess the mental effects of these circumstances, and the effects on General Surgery and Obstetrics & Gynecology residency training in the busiest surgical departments of our university. Methods Participants of this cross-sectional study were residents of General Surgery and Obstetrics & Gynecology of Tehran University of Medical Sciences, and the conventional sampling method was used. We used a questionnaire consisting of 47 questions (mostly using multiple choice questions and answers on the Likert scale) about personal, familial, and demographic characteristics; training activities, and mental effects of COVID-19. Results The response rate was 63.5%. (127 filled questionnaires). Around 96% of the residents had emotional problems, 85.9% were highly stressed about contracting COVID-19, 81.3% were worried about transferring it to their families; and 78% believed that their residency training had been impaired. Conclusion Overall, our study shows the negative impact of COVID-19 on mental health and the training of residents. We propose that appropriate emotional support and suitable planning for compensation of training deficits is provided for residents.

Abstract Background Thyroid cancers are histologically classified into three types; differentiated thyroid carcinoma (DTC), medullary thyroid carcinoma (MTC), and anaplastic thyroid carcinoma (ATC). Among the several therapeutic strategies for treatment and management of thyroid cancer, surgical resection in combination with radioactive iodine therapy (RAI) is indicated for moderate to high‐risk differentiated thyroid cancer (DTC) patients‐ according to current guidelines. However, external radiation therapy (EBRT) can be a viable alternative treatment option for these patients and scarce evidence is available regarding the efficacy and effectiveness of EBRT on thyroid cancer. Aim This study aims at evaluating the role of EBRT in the management of thyroid carcinomas. Methods and Results In this retrospective cohort study, the records of 59 patients with thyroid cancer were accessed who were treated by EBRT from 2008 to 2016. The indications for EBRT included unresectable primary (definitive) or loco‐regional recurrences (salvage) not suitable for RAI, palliation for local disease or metastatic foci (palliative), and the adjuvant treatment for suspected residual disease following resection. Progression‐free survival (PFS) and overall survival (OS) were calculated for different types of cancer. PFS was measured from the start of EBRT to the last uneventful follow‐up, recurrence, or death. Kaplan–Meier model was used for the survival analysis. Fifty‐nine patients were evaluated. The histopathology of the tumors was differentiated and poorly‐differentiated, medullary and anaplastic thyroid carcinomas in 22 and 6, 15 and 16 patients, respectively. Twenty‐seven patients received external beam radiotherapy (EBRT) as adjuvant therapy and 18 of the cases as palliative therapy while the remaining received salvage or definitive primary EBRT. The stage of patients' cancer was as follows: stage II in 3 and III in 1, IVA in 18 and IVB in 18 and IVC in 19. Stage‐based median overall survival was 26 months for IVA, 44 for IVB, and 29 for IVC. The median PFS was 18, 22 and 21 months for stages IVA, IVB and IVC, respectively. Conclusion Based on our findings, EBRT may still play a role in the management of patients with thyroid carcinoma and should be considered in the armamentarium against thyroid cancers.

Objective: Sulforaphane (SFN) is a natural free radical scavenger that can reduce oxidative stress (OS) through mediating nuclear factor (erythroid-derived 2)-like 2 (NF-E2-related factor 2 or NRF2)/antioxidant response element (ARE) signaling pathway and the downstream antioxidant enzymes. Here, we intended to study the role of SFN in OSinduced human granulosa cells (GCs) by investigating the intracellular levels of reactive oxygen species (ROS), cell death, and NRF2-ARE pathway. Materials and Methods: This experimental study was conducted on GCs of 12 healthy women who had normal menstrual cycles with no history of polycystic ovary syndrome (PCOS), endometriosis, menstrual disorders, hyperprolactinemia, or hormonal therapy. After isolation of GCs, the MTT assay was performed to explore GCs viability after treatment with SFN in the presence or absence of H2O2. Flow cytometry was utilized to determine the intracellular ROS production and the apoptosis rate. Evaluation of the mRNA and protein expression levels of NRF2 and phase II enzymes including superoxide dismutase (SOD) and catalase (CAT) was performed by quantitative real-time polymerase chain reaction (PCR) and western blotting. ABSTRACT Objective: To evaluate the effect of metabolic syndrome (MetS) diagnosis on oocyte quality and pregnancy outcomes in infertile women with polycystic ovary syndrome (PCOS) who undergoing antagonist-controlled ovarian stimulation (COS) and in vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI) cycles. Subject and methods: This prospective cohort study was conducted from November 2019 to November 2020 across two university-affiliated infertility centers in Iran. The PCOS diagnosis was defined according to the Rotterdam criteria. The patients prior to IVF/ICSI cycles were evaluated for MetS diagnosis. MetS was detected according to the National Cholesterol Education Program/Adult Treatment Panel III with the presence of at least three or more of the specific clinical criteria. The cycle outcomes were compared between MetS and non-MetS groups. Results: Overall, 68 eligible infertile PCOS patients with MetS diagnosis and 126 without MetS participated. The MetS diagnosis was associated with the increased requirement of gonadotropins and the COS duration significantly (P = 0.001). Although the total numbers of retrieved and MII oocytes, obtained and top-quality embryos as well as clinical pregnancy and live birth rates in the MetS group were lower than those of in the non-MetS group, the differences were not statistically significant (P > 0.05). In follow-up of the obstetrics complications, the rate of preeclampsia was significantly higher in patients with MetS (P = 0.02). Conclusion: MetS diagnosis in PCOS patients was associated with non-significant poor COS and pregnancy outcome. Further studies with larger sample sizes are recommended to clarify the risk of MetS in patients undergoing ART cycles.

Background: Various studies have confirmed the effectiveness of compassion-focused therapy and reality therapy on improving mental health indices among clients with different problems, but no study has compared the effectiveness of compassion-focused therapy and reality therapy on alexithymia among male prisoner clients. Aims: The present research was conducted by the aim of comparing the effectiveness of compassion-focused therapy and reality therapy on alexithymia among male prisoner client. Method: The current research was a semi-experimental study which was conducted through a pretest-posttest with control group design. The research population included all the male prisoner clients in Aligoodarz city, Iran, in 2019 who were totally 209 individuals. The sample included 45 prisoners who were selected by purposeful sampling method and were assigned into three groups including two experiment groups and one control group (15 subjects per group) ny random assignment method. The research tool was Toronto Alexithymia Scale of Bagby, Parker and Taylor (1994). The first experiment group received the compassion-focused therapy (Gilbert, 2010; 2014) and the second experiment group received reality therapy (Wubbolding, 2013) both in eight 90-minute sessions. The control group received no intervention until the end of the research. The univariate analysis of covariance and the Bonferroni ad hoc test were used to analyze the data. Results: Results suggested that both therapeutic interventions were effective on improving alexithymia in the experiment groups (p< 0/01). Moreover, there was no significant difference between compassion-focused therapy and reality therapy and both had similar effectiveness (p> 0/05). Conclusions: The results of the present research provide an appropriate empirical support for compassion-focused therapy and reality therapy. Psychologists and counselors working in prisons are recommended to use these two approaches for enhancing mental health among prisoners.

ObjectiveThis was a non-blinded randomized controlled study to evaluate whether endometrial irrigation via office hysteroscopy during the early follicular phase would lead to a higher level of live birth rates compared to no irrigation in the fresh embryo transfer cycle.MethodThe study was conducted in Tehran university of medical sciences from June 2015 to June 2016. women under the age of 40 with primary infertility without history of previous IVF/ICSI or hysteroscopic examination, were included. Controlled ovarian hyperstimulation was done. Hysteroscopy was performed in the early mid-follicular phase of a stimulation cycle (day 5-7) with a vaginoscopy approach and saline irrigation in hysteroscopy group. Embryo-transfer was done in the same cycle.Results228 patients completed their participation in the study. In the fresh cycle, clinical pregnancy rate was 46% in the hysteroscopy group and 40.43% in the control group. (p-value= 0.326, RR= 1.16 [95%CI: 0.862 to 1.56]). Live birth rate was 41.28% in the hysteroscopic group and 31.93% in the control group (p-value=0.143, RR= 1.293 [95%CI: 0.916 to 1.825]). For those patients having surplus cryopreserved embryos, after 2 months, a second embryo transfer was performed. The cumulative LBR was 44.05% in the hysteroscopic group and 32.25% in the control group (p-value=0.029, RR= 1.368 [95%CI: 1.031 to 1.815], RD= 11.9% [95%CI: 1.2% to 22.3%] and NNT= 8 [95%CI: 4 to 85]).ConclusionThe current study clearly demonstrated a significantly higher cumulative live birth rate in the intervention group.Clinical Trial Registration[https://www.irct.ir/trial/19586], identifier IRCT2016011022795N2.

Polycystic ovarian syndrome (PCOS) is related to pro‐apoptotic and pro‐inflammatory conditions generated by Endoplasmic reticulum (ER) stress. This study aimed to determine the effect of Astaxanthin (ASX), as carotenoid with potent antioxidant and anti‐inflammatory properties, on serum inflammatory markers, apoptotic factors and ER stress‐apoptotic genes in peripheral blood mononuclear cells (PBMCs) of women with PCOS. This randomized, double‐blind clinical trial included 56 PCOS patients aged 18–40. For 8 weeks, subjects were randomly assigned to one of two groups: either 12 mg ASX (n = 28) or placebo (n = 28). Real‐time PCR was used to quantify gene expression associated with ER stress‐apoptosis in PCOS women's PBMCs. The levels of TNF‐α, IL18, IL6 and CRP were determined by obtaining blood samples from all patients before and after the intervention using Enzyme‐linked immunosorbent assay (ELISA). Also, the levels of active caspase‐3 and caspase‐8 were detected in the PBMC by ELISA kit. Furthermore, we evaluated the efficacy of ASX on disease symptoms. Following the 8‐week intervention, ASX supplementation was able to reduce the expression of GRP78 (p = 0.051), CHOP (p = 0.008), XBP1 (p = 0.002), ATF4 (0.038), ATF6 (0.157) and DR5 (0.016) when compared to the placebo. However, this decrease was not statistically significant for ATF6 (p = 0.067) and marginally significant for GRP78 (p = 0.051). The levels of TNF‐α (p = 0.009), IL‐18 (p = 0.003), IL‐6 (p = 0.013) and active caspase‐3 (p = 0.012) were also statistically significant lower in the therapy group. However, there was no significant difference in CRP (p = 0.177) and caspase‐8 (p = 0.491) levels between the treatment and control groups. In our study, ASX had no significant positive effect on BMI, hirsutism, hair loss and regularity of the menstrual cycle. It appears that ASX may benefit PCOS by changing the ER stress‐apoptotic pathway and reducing serum inflammatory markers; however, additional research is required to determine this compound's potential relevance. Aim and Background: One of the most important specifications of marital relationships in cycles and communicational patterns, which emerge among them anf may be fixing by repetition. Aim of this research was to comparison the effectiveness of Emotionally focused approach, Gottman’s relationship Enrichment on woman’s Communication patterns. Methods and Materials: This research was a quasi-experimental field research with pre-posttest and control group with three-months follow up. The statistical population of the study consisted of women in Khomein city who referred to Faraz Counseling Center in 2017 due to communication problems with their spouses. 36 of them were selected by available sampling method and randomly assigned to two experimental and one control group. To collect the data, Communication Patterns Questionnaire (Christensen & Salvey, 1984) was used. To analyze the data, Ancova and Bonferroni analysis were used. Findings: The findings showed that there was a significant difference in the communication patterns between the test and control groups in the post-test and follow up stages, so that the emotionally focused approach and enrichment of the relationship were able to significantly decrease the communication patterns Avoidance, Demand Withdraw and enhancement of mutual constructive communication model in both post-test and follow-up phases. Also, the results of the Bonferroni test showed that the difference between the group of emotionally focused approach and the Gottman enrichment group in women's communication patterns was meaningful; hence, the Gottman enrichment approach was more effective than the emotionally focused approach on communication patterns. Conclusions: Therefore, it is possible to use both approaches, especially the Gottman's enrichment approach to improve couples' communication patterns and ultimately increase the quality of marital life.

Aim and Background: Systematic Motivational Counseling (SMC) and Acceptance and Commitment Therapy (ACT), with emphasis on motivational structure modification, and the acceptance of life’s problems, can improve conflict resolution styles of incompatible marital women. The purpose of this study was to compare the effectiveness of SMC and ACT on conflict resolution styles of incompatible marital women. Methods and Materials: The method of this study was semi-experimental method with pre-test and post-test design and three-month follow-up. The statistical population of the study consisted of all incompatible women who referred to counseling centers in Arak city in 2018. Therefore, 36 participants were selected and randomly assigned to two test groups and one control. Data were collected in the pre-test, post-test and follow-up stages with Rahim Organizational Conflict Inventory-II (ROCI-II). The experimental group members participated separately in 10 SMC sessions and 12 ACT sessions after running the pre-test. For data analysis, analysis of variance with repeated measures test were used.
