7. COPD 合并Ⅱ型呼吸衰竭治疗中经鼻高流量 湿化氧疗与呼吸功能锻炼联合应用观察.

目的 探讨慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭(RF)治疗中经鼻高流量湿化氧疗(HFNC)联合 呼吸功能锻炼的应用效果。方法 回顾性分析 120 例 COPD 合并Ⅱ型 RF 患者的临床资料, 根据治疗方法不同分为 HFNC 组 67 例和对照组 53 例。两组均接受常规抗炎、抗感染等基础治疗及呼吸功能锻炼, 在此基础上, 对照组采取 无创正压通气(NPPV)治疗, HFNC 组采取 HFNC 治疗, 共治疗 28 d。治疗 28 d 后评价两组临床疗效;治疗前及治疗 28 d 后, 采用 6 min 步行试验(6MWT)评估患者运动功能, 采用 COPD 评估测试(CAT)评价病情严重程度, 采用呼吸 困难评分(mMRC)评价呼吸困难程度;采用肺功能检测仪检测第 1 秒用力呼气容积(FEV1)、用力肺活量(FVC)、 第 1 秒用力呼气容积与用力肺活量的比值(FEV1/FVC), 采用全自动血气分析仪检测氧分压(PaO2)、二氧化碳分压 (PaCO2)、氧合指数(OI); 记录治疗 28 d 内不良反应发生率、气管插管率及 ICU 住院时间。结果 HFNC 组总有效率 为 97. 01%, 高于对照组的 81. 13%(P<0. 05). 治疗后, HFNC 组 6MWT、FEV1、FVC、FEV1/FVC、PaO2 、OI 高于对照 组, CAT 评分、mMRC 评分、PaCO2 低于对照组(P 均<0. 05). HFNC 组不良反应发生率、气管插管率及 ICU 住院时间 均少于对照组(P 均<0. 05). 结论 HFNC 联合呼吸功能锻炼可有效缓解 COPD 合并Ⅱ型 RF 患者呼吸困难症状, 改 善肺功能及动脉血气指标, 增强运动耐力, 且具有较好的安全性。 [ABSTRACT FROM AUTHOR]

- Abstract
According to the independent and controllable advantage of the torque of each wheel for four- wheel independent driving electric off-road vehicle, to improve the traction and off-road ability of off-road vehicle, a coordinated control strategy of driving torque was proposed. The torque of the driving motor was pre-distributed according to the load of each axis. The optimal estimation of road slip rate was realized by the extreme value searching algorithm based on wheel slip rate and road adhesion coefficient. Proportional-integral-differential (PID) control-sliding mode control (SMC) and state-based driving force redistribution method were used to achieve the coordinated distribution of wheel drive torque for restraining the wheelslip under bad road conditions. Through the co-simulation of CarSim/MATLAB and hardware-in-the-loop (HIL) test, the experimental verification of climbing steep slope, single adhesion road surface, split road and continuous unrolling road was carried out. The results show that by the proposed control strategy, the torque of the driving wheel can be distributed according to the actual working conditions, and the slip rate of wheels is reduced to realize the optimization of the driving force of whole vehicle. [Objective] Cassia obtusifolia Kunitz trypsin inhibitor 1 (CoTi1) is a group of functional peptides with 13-trefoil tertiary structure. To explore the effect of site-directed mutagenesis on the inhibitory activity of CoTi1 is of great significance for the study of its structure. [Method] Based on the structural analysis of CoTi1, three residues, Cys44 (between β2 and β3 folds of the top lid) and Tyr 134 and Lys135 (within β9 folds of the bottom β-barrel), were mutated to Ala, respectively. The inhibitory activity of these mutagenesis on bovine trypsin and cabbage worm trypsin-like protease were detected after protein expression. [Result] Compared with wild-type CoTi1, the inhibitory activity of CoTi1C44D on bovine trypsin increased by 41. 00%, while that of CoTi1Y134D and CoTi1Kl35D decreased by 45. 66% and 36. 73%, respectively. The inhibitory activities of CoTi1C44D, CoTi1Y134D and CoTi1Kl3SD on cabbage worm trypsin-like protease decreased by 46. 94%, 49. 00% and 38. 11 %, respectively. [Conclusion] Cys44, Tyrl34 and Lys135 were involved in the formation of l3-trefoil tertiary structure and were important residues for stabilizing the spatial structure and inhibiting the activity of CoTi1, which laid an important theoretical foundation for the structure study of CoTi1. 【Objective】 The effects of nitrogen, phosphorus and potassium formula fertilization on the photosynthetic characteristics, main characters and quality of medicinal materials of Gentiana rigescens were studied, in order to provide a reference for efficient fertilization to G.rigescens.【Method】 One-year-old seedlings of G.rigescens were taken as experimental materials, in a potted fertilizer experiment abiding with ‘3414’ scheme was conducted. By combining ratios of nitrogen, phosphorus, and potassium fertilizers, their growth and photosynthetic physiological indicators were measured under different fertilization treatments (T1-T14), and the optimal ratios of nitrogen, phosphorus, and potassium application were selected through the membership function method. 【Result】 Significant differences in plant traits and gentiopicroside content were observed among different fertilizer treatments (P < 0.05, the same as below). The T3 treatment (N1P2K2) had the highest net photosynthetic rate Pn), root length, root thickness, root weight, and gentiopicroside content, that respectively were 10.88 μmol/(m²·s), 27.44 cm, 1.89 mm, 1.05 g, and 7.93%.These values were 1.87, 2.07, 1.54, 3.09 and 1.17 times higher than those of the T1 treatment (CK) (N0P0K0), respectively. The comprehensive evaluation of the main characteristics using the membership function method showed that the performance of the T3 treatment was the best of all treatments. The effect size of nitrogen, phosphorus, and potassium on the Pn, stomatal conductance (Gs) of G.rigescens ranked in the order of K > N > P, and on the intercellular concentration of CO2(Ci) in the order of K > P> N, and on transpiration rate (Tr) in the order of N > P > K. The effects on plant height, stem thickness, stem weight, leaf weight, root length, and root dry weight of G.rigescens ranked in the order of N > K > P, and on leaf length, leaf width, and root thickness in the order of P>N>K. The effects on branching number and gentiopicroside content ranked in the order of K>N>P, and on the number of leaves in the order of K > P > N. 【Conclusion】 All fertilizer treatments beneficial for improving the photosynthetic parameters, agronomic traits, yield, and effective ingredient content of G.rigescens. Based on the results of the membership function analysis, the T3 treatmenth (N1P2K2, N 1.5 g/m², P2O5 3.g/m², K2O 3 g/m²) ould be considered as the optimal formula for potted G.rigescens. BACKGROUND: With the development of high-throughput technology, non-coding RNAs have gradually attracted attention and are widely involved in the course of various respiratory diseases. The progression of idiopathic pulmonary fibrosis is accompanied by stem cell abnormality and recent studies have shown that non-coding RNAs are associated with stem cell abnormality, thus affecting the progression of pulmonary fibrosis. Gene therapy that targets non-coding RNAs hints at the possibility of blocking the fibrosis process. OBJECTIVE: To summarize the research progress of non-coding RNAs in stem cell dysfunction and treatment of idiopathic pulmonary fibrosis. METHODS: English search terms included “IPF, stem cells, miRNA, lncRNA, circRNA, treatment” and Chinese search terms contained “idiopathic pulmonary fibrosis, stem cells, miRNA, lncRNA, circRNA, treatment”. Databases including PubMed, Wiley InterScience, CNKI, and Wanfang were used for retrieval, covering articles in the past decade. Screening was performed based on the inclusion and exclusion criteria. Finally, 82 articles were included for analysis. RESULTS AND CONCLUSION: (1) The recent studies on the correlation between non-coding RNAs and the mechanisms of stem cell abnormality in idiopathic pulmonary fibrosis were summarized, revealing that miRNAs, lncRNAs, and circRNAs jointly regulate the abnormal activation and differentiation of stem cells, and miRNAs are often the central link of the mechanisms of lncRNAs and circRNAs. (2) Existing trials have confirmed the safety of transplantation therapy to a certain extent, but in terms of efficacy, the diffusion function of patients has only been slightly improved, but the degree of fibrosis has not been significantly alleviated. Most importantly, the trials were non-randomized and lack of placebo controls, so more placebo-controlled randomized trials are needed. (3) The most important advantage of non-coding RNA-targeted gene therapy is that it has the characteristics of multi-gene regulation, which can intervene stem cell abnormalities from the upstream pathways. Based on the breakthroughs made in vivo and in vitro research, non-coding RNAs have been confirmed to be related to the targets of several drugs for idiopathic pulmonary fibrosis. (4) However, the disadvantages of gene therapy include that it still can only produce short-term effects, and it also faces problems such as effective dose, drug stability, and delivery in the human body. These problems need to be further resolved by future research. (5) In conclusion, although non-coding RNA-targeted therapy is difficult, it can be regarded as a novel way to improve the abnormality of stem cells in idiopathic pulmonary fibrosis, and combining this therapy with stem cell transplantation may achieve better efficacy. The wind damage of thin⁃film greenhouse buildings is serious due to the frequent strong wind or typhoon in tropical islands. However, there is little research on the wind⁃resistant design of thin⁃film greenhouse buildings in islands in China. It is of great theoretical and engineering significance to research near⁃surface wind field and the wind effect of thin⁃film greenhouse buildings. This paper states the background and significance of wind⁃resistant research on thin⁃film greenhouse buildings in tropical islands, introduces the current research status of near⁃surface wind field characteristics, and summarizes the wind resistance of agricultural greenhouse buildings and membrane structures from different hands of field measurement, wind tunnel test, and numerical simulation. Based on the deficiencies of current research, according to distributed optical fiber sensing technology, field measurement, wind tunnel aeroelastic test, fluid⁃structure interaction numerical simulation and theoretical analysis, etc., it is proposed to develop the research on wind effect, wind⁃induced damage mechanism, wind⁃resistant measures and design theory of thin⁃film greenhouse buildings in tropical island [ABSTRACT FROM AUTHOR]

OBJECTIVE: To probe into the effects of Qingre Huatan decoction combined with Sangbaipi decoction on pulmonary ventilation function and immune function in patients with chronic obstructive pulmonary disease (COPD) of phlegm-heat stagnation type. METHODS: A total of 92 patients with COPD of phlegm-heat stagnation type admitted into our hospital from Nov. 2019 to Nov. 2020 were prospectively selected to be divided into the control group and the observation group via the random number table, with 46 cases in each group. Both groups were given western medicine treatment. On this basis, the control group was treated with Sangbaipi decoction, while the observation group received Sangbaipi decoction combined with Qingre Huatan decoction. After treatment of 40 d, the clinical symptoms and pulmonary function were compared between two groups. The levels of T lymphocyte subsets, tumor necrosis factor-α (TNF-α), transforming growth factor-β1 (TGF-β1) and C-reactive protein (CRP) were measured, and the clinical efficacy was observed. RESULTS: After treatment, the traditional Chinese medicine syndrome scores of chest tightness, cough and expectoration symptoms decreased significantly in two groups; compared with the control group, the traditional Chinese medicine syndrome scores of chest tightness, cough and sputum in the observation group were significantly lower, the differences were statistically significant (P<0. 05). After treatment, the maximum expiratory flow (MEF), forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) levels of two groups increased significantly; compared with the control group, the levels of MEF, FVC and FEV1 in the observation group were significantly higher, the differences were statistically significant (P<0. 05). After treatment, the serum levels of CD4+ and CD4+ / CD8+ increased, while the levels of CD8+, TNF-α, TGF-β1 and CRP decreased in two groups; compared with the control group, the serum levels of CD4+ and CD4+ / CD8+ in the observation group were higher, and the levels of CD8+, TNF-α, TGF-β1 and CRP were lower, with statistically significant difference ( P < 0. 05). Compared with the control group (82. 61%, 38/46), the total effective rate of the observation group was significantly higher (97. 83%, 45/46), and the difference was statistically significant (P<0. 05). CONCLUSIONS: The clinical efficacy of Qingre Huatan decoction combined with Sangbaipi decoction on the basis of western medicine in the treatment of COPD of phlegm-heat stagnation type is significant, which can reduce the traditional Chinese medicine syndrome score of patients, effectively inhibit the spread of inflammatory reaction, improve the immune function, and contribute to the recovery of pulmonary ventilation function of patients.
