BACKGROUND: Topical application of tranexamic acid can reduce the drainage after total hip arthroplasty for degenerative diseases. However, there are few reports on the application of tranexamic acid in primary total hip arthroplasty for simple elderly femoral neck fracture. OBJECTIVE: To analyze the safety and effectiveness of topical application of tranexamic acid to reduce drainage of elderly femoral neck fractures after total hip arthroplasty. METHODS: Forty-five elderly femoral neck fractures patients undergoing total hip arthroplasty in the Zhengzhou People’s Hospital from January to December 2019 were enrolled in this study, including 19 males and 26 females at the age of 59-82 years old. Twenty-one patients in the observation group were injected with tranexamic acid retrogradely through the drainage tube after total hip arthroplasty. Twenty-four patients in the control group were injected with normal saline retrogradely through the drainage tube after total hip arthroplasty. The postoperative drainage volume and adverse events were recorded; the wound healing was closely observed; and the coagulation function was checked. This study was approved by the Medical Ethics Committee of Zhengzhou People’s Hospital. RESULTS AND CONCLUSION: (1) All patients’ incisions healed grade A, and no myocardial infarction, cerebral infarction, pulmonary embolism, or deep vein thrombosis of the lower extremities occurred. (2) At postoperative 3 days, total drainage and hemoglobin loss were less in the observation group than those in the control group (P < 0.05). (3) Activated partial prothrombin time, prothrombin time, thrombin time, and fibrinogen were not significantly different between the two groups after surgery (P > 0.05). (4) Results suggested that topical application of tranexamic acid in total hip arthroplasty for elderly femoral neck fractures is safe and effective. 目的 探讨后路复位内固定术在治疗胸腰椎骨折过程中经椎弓根伤椎植骨置钉手术的有效性和安全 性。方法收集2013年1月至2015年1月该院收治的胸腰椎单节段骨折患者91例，其中，经椎弓根伤椎植骨置钉手 术患者42例（试验组），经常规跨伤椎后路复位内固定手术患者49例（对照组）。记录两组患者的手术并发症、腰痛程度、 术中出血量及手术总时间，比较术前与术后伤椎与相邻椎骨的Cobb角、伤椎椎体前缘高度、伤椎椎管直径和计算椎管侵 占率。结果试验组手术时间较对照组延长，差异有统计学意义（P<0.05 )。试验组出血量与对照组比较，差异无统计学意 义（P>0.05 )。试验组患者术后与末次随访时 Cobb 角和椎体前高压缩比与对照组对应时间点比较，差异有统计学意义（P< 0.05);而两组患者椎管侵占率比较，差异无统计学意义（P>0.05)。两组术后Cobb角及椎体前高压缩比与末次随访时比 较，差异均无统计学意义（P>0.05)。结论经伤椎椎弓根植骨置钉能较好地矫正后凸畸形、重建椎体高度、维持矫正效果 和减少内固定的松动断裂等并发症，与常规跨伤椎椎弓根螺钉的治疗效果比较更为有效、安全。 [ABSTRACT FROM AUTHOR]

目的對比分析經前路與后路內固定術治療胸腰段脊柱骨折的臨床療效。方法將2012年2月至2014年2月該科收治的80例胸腰段脊柱骨折患者分為兩組,各40例。前路組行經前路內固定手術,后路組采取經后路內固定手術方案,比較兩組臨床療效及并發癥發生情況。結果前路組術后后凸Cobb角、椎體高度、運動功能評分改善效果及并發癥發生情況均優于后路組,差異有統計學意義(P<0.05);兩組術后觸覺功能評分及總有效率比較,差異無統計學意義(P>0.05)。結論經前路內固定手術行胸腰椎脊柱骨折效果好,有助于更好地改善神經功能,促進預后,可作為優選手術方式。 [ABSTRACT FROM AUTHOR]
