目的：观察体表心电图T波峰－末（TpTe）间期及TpTe间期与QT间期的比值（TpTe／QT）对Brugada综合征（BrS）患者恶性室性心律失常的预测效能。方法收集22例BrS患者的临床资料，按是否并发恶性室性心律失常（晕厥、临床记录多形室速／心室颤动、电生理诱发多形室速／心室颤动）分为恶性室性心律失常组8例与无恶性室性心律失常组14例，比较两组PQ、QRS、QT、TpTe间期和TpTe／QT。绘制TpTe间期、TpTe／QT预测BrS患者恶性室性心律失常的受试者工作曲线（ROC），计算曲线下面积、截断点及据此预测恶性室性心律失常的灵敏度和特异度。结果恶性室性心律失常组、无恶性室性心律失常组V2导联TpTe间期分别为（108．75±12．46）、（84．29±15．55）ms，TpTe／QT分别为0．31、0．22，两组相比，P均＜0．05；两组V2导联PQ、QRS、QT、RR间期相比P＞0．05；两组其他导联PQ、QRS、QT、RR、TpTe 间期和 TpTe／QT相比 P＞0．05。ROC 曲线 TpTe 间期曲线下面积为0．893， TpTe／QT曲线下面积为0．969。取 TpTe 间期≥95 ms 预测 BrS 患者恶性室性心律失常的灵敏度0．875，特异度0．786；取TpTe／QT≥0．257预测BrS患者恶性室性心律失常的灵敏度0．875，特异度0．929。结论体表心电图TpTe间期、TpTe／QT预测BrS患者恶性室性心律失常效能较好。体表心电图V2导联TpTe值≥95 ms或TpTe／QT≥0．257提示BrS患者恶性室性心律失常。 [ABSTRACT FROM AUTHOR]
