Objective To analyze the disease spectrum and age/gender composition ratio of outpatients in the ophthalmology clinics in Funing People’s Hospital of Yunnan Province. Methods All ophthalmology clinics outpatients from January 2023 to June 2023 were retrospectively analyzed. Electronic medical records were fully reviewed. Diagnoses were categorized into ten groups according to the international classification of ocular diseases and trauma (ICD-10). Results A total of 5 008 outpatient cases of ophthalmology clinics were included (2 524 males and 2 484 females). Conjunctival diseases accounted for 26.74%, eyelid, lacrimal tear system and orbital diseases accounted for 22.12%, eye trauma accounted for 17.29%, sclera, cornea, iris and ciliary body diseases accounted for 10.36%, eye muscles, binocular movement, eye accommodation and refractive abnormalities accounted for 8.39%, lens diseases accounted for 6.39%, choroidal and retinal diseases accounted for 3.83%, glaucoma accounted for 2.32%, vitreous and eyeball diseases accounted for 2.32%, optic nerve and visual pathway diseases accounted for 0.24%. Different diseases have different age and gender characteristics. Conclusions The disease spectrum and age/gender composition ratio of outpatients in our hospital’s ophthalmology clinics have regional characteristics with unique features. This study conducted a questionnaire survey to understand outpatients' trust in doctors and influencing factors, among outpatients in a tertiary hospital in Anhui province. The results show that non -first visits, perceived respect for the healthcare process, high trust in the physician group, overall high trust in the system benevolence, and technology dimensions positively affected the total physician trust score. High scores in the healthcare environment also had a positive effect on the total physician trust score and technical dimension scores. Compared to patients who perceived the doctor - patient relationship to be an active - passive model, those who perceived that the relationship should be a directive - cooperative model as well as a co - participatory model patients scored higher on the benevolence dimension. Medical institutions should optimize the medical environment and focus on protecting patients' privacy. At the social level, we should promote group trust in doctors and enhance the sense of access to the health system. Objective To explore the operation effect of the improved full-appointment mode of ambulatory chemotherapy, so as to provide reference for further improving the treatment process. Methods The enhanced full-appointment mode had been implemented in Fudan University Shanghai Cancer Center outpatient information system, which seamlessly integrated daytime chemotherapy assessment with comprehensive information management, formulated precise rules for chemotherapy appointments, and ensured efficient integration of relevant data. A comparative analysis was conducted between the period after optimization (Jan to Sep 2022) and the corresponding period in the previous year (Jan to Sep 2021), considering factors such as patient waiting time, human involvement, patient safety during chemotherapy, as well as nurse and patient satisfaction. Results After optimization, the time spent by patients was reduced from 52.12 (32.73-83.05) to 20.04 (11.87-41.10) minutes,with statistically significant difference (z=-78.144, P<0.001) . Additionally, the time spent by patients before and after optimization was significantly different in the distribution of <30 minutes, 30-60 minutes, and >60 minutes (χ²=5 958.455,P<0.001). Previously, one nurse and 2-3 security personnel were required to schedule appointments in the daytime chemotherapy center, while after optimization, there was no longer a need for nurses to arrange appointment windows and the number of security personnel was reduced to one, thereby optimized human operations. It was observed that the number of cases involving chemotherapy infusion reactions decreased from 59 to 46 following optimization,and the number of patients requiring rescue observation reduced from four to one. Notably, no rescue events occurred during non-day shifts, thus enhanced patient safety during treatment hours outside regular working hours. Furthermore, there was a statistically significant improvement in both nurses’ and patients’ satisfaction levels before and after implementation of these optimizations (P<0.05). Conclusion The modified full appointment mode reduced patient waiting time, optimized human resources utilization, enhanced patient safety during chemotherapy, and improved satisfaction levels among both nurses and patients. Objective To investigate the effect of crisis management in patient complaint management in the pediatric dentistry clinic. Methods Totally 200 cases of pediatric patients who visited the pediatric dentistry clinic between January and December 2021 were included as the control group, and routine nursing management was carried out during the treatment. Another 200 cases of pediatric patients who visited the pediatric dentistry clinic between January and December 2022 were recruited as the study group, and a crisis management model was adopted. The complaint rate, praise rate and satisfaction with nursing service were measured and compared between two groups. Results The complaint rate in the study group was lower than that in the control group (1. 00% vs. 5. 50%, P＜0. 05). The praise rate in the study group was higher than that in the control group (12. 00% vs. 4. 50%, P＜0. 01). The rate of satisfaction with working attitude, humanistic care, clinic procedure, health education and overall satisfaction rate in the study group were higher than those in the control group(P＜0. 05). Conclusion The crisis management is effective to reduce the rate of patient complaint and improve the nursing quality in the pediatric dentistry clinic. Objective To evaluate the clinical application of paroxetine in outpatients. Methods　 A retrospective analysis was used to collect the prescription information of all outpatients who used paroxetine in the hospital from January 1， 2020 to June 30， 2023. The structural characteristics of patients and information on medication dosage，frequency，and cost were analyzed to evaluate the rationality of prescriptions. Results A total of 28，106 valid prescriptions for paroxetine were included，of which 9 257 were male and 18 849 were female. The use of paroxetine hydrochloride tablets decreased annually while the use of enteric-coated paroxetine hydrochloride sustained-release tablets increased annually. The annual use of paroxetine hydrochloride tablets decreased，while the annual use of enteric-coated paroxetine hydrochloride sustained-release tablets increased. The average cost of the drug per patient unit dose increased each year. Non-compliant prescriptions included off-label use，improper dosage，excessive treatment duration，and off-label use in children and adolescents.Conclusion　 The clinical application of paroxetine is not standardized，and there are unreasonable situations such as overuse. Objective By studying the relationship between outpatient experience and satisfaction, loyalty, and recommendation, find out the current factors affecting the medical experience of outpatient, and improve the overall service quality of the hospital. Methods Using indicators of Outpatient Experience Questionnaire (OPEQ), Patient Experience With Medical care (PEES-50) and other questionnaires, a model of the relationship between patient experience and satisfaction, loyalty, and recommendation was built, and expected satisfaction degree was considered as an intermediary variable to study the interaction between model variables. Results Outpatient experience directly affected their satisfaction and recommendation for hospital services, and indirectly affects satisfaction, loyalty and recommendation through expectation satisfaction. Conclusion The key to improving patient experience lies in improving the quality of hospital services and management, and providing quantitative support for improving the quality of hospital services. (目的 研究门诊患者体验感与满意度、忠诚度、推荐度之间的关系, 找出当前影响门诊患者就诊体验的因素, 提升医院整体的服务质量。方法 便利选取300名门诊就诊患者, 采用门诊患者体验量表(OPEQ)、患者体验医疗服务质量评价表(PEES-50)等问卷指标, 对患者体验感与满意度、忠诚度、推荐度之间的关系构建模型, 并加入中介变量期望满足度, 以确定变量之间的影响作用。结果 门诊患者体验感直接影响其对医院服务的满意度与推荐度, 还通过期望满足度间接影响满意度、忠诚度与推荐度。结论 提高患者的体验感关键在于提高医院的服务与管理质量, 为提升医院服务质量提供量化支撑。)

This paper explores the effects of health literacy and perceived doctor⁃patient empathy on patient compliance and the mediating role of patient trust. It utilized the modified Chinese Patient Health Literacy Scale, the Empathy Scale for Diagnosis and Treatment Relationships, Wake Forest Physician’s Trust Scale and the Patient Compliance Scale to investigate outpatients in a comprehensive tertiary hospital in Anhui province with 720 valid questionnaires. Correlation analysis showed that health literacy, perceived doctor ⁃ patient empathy, and patient trust were all correlated with patient compliance (r=0.230, r=0.419, r=0.380, P<0.01). The results of structural equation modeling showed that patient trust completely mediates between health literacy and patient compliance. Patient trust partially mediates between perceived doctor ⁃ patient empathy and patient compliance. This paper suggests adopting measures to improve patients’health literacy level, paying attention to the improvement of doctors’ empathy ability for diagnosis and treatment, promoting doctor ⁃ patient communication and doctor⁃patient trust, thereby improving patient compliance and maintaining patient health. Objective To understand the current situation of supply and demand of community oral health care services and its influencing factors, and to provide reference for improving the types of community oral health care services and improving the service level. Methods May to October 2017, a questionnaire survey was conducted on patients over 18years old who were able to answer questions independently according to the order of treatment in an oral specialty hospital in Hefei, and the influencing factors of oral health services demand were analyzed by using the Logit model. A total of 550patients were investigated, 500valid questionnaires were obtained, and the effective response rate was 90.9%.Results Among the 500patients, 26.8%（134/500）knew that the dental department was opened in the community;12.0%（60/500）patients had received oral health care services provided by the community; and 71.6%（358/500）had demand for regular oral examinations and consultation services. Age, family monthly income, education level, gender, occupation, knowledge of stomatology, oral diseases and systemic diseases were the influencing factors of oral health demand of community residents(P<0.05).Conclusion The gap between the supply and demand of oral health services is large, so the development of community oral health services should be strengthen; the technical level of community dentists and the accessibility of residents oral health care services should be improved; residents access to information should be expanded; targeted oral services according to different characteristics of people should be provided. 目的 在控制门诊患者的人口学和临床特征后, 比较不同类型公立医疗卫生机构的门诊患者满意度, 为公 立医疗卫生机构和卫生行政部门开展绩效考核提供参考信息。方法 对上海市107 所公立医疗卫生机构开展门诊 患者满意度问卷调查, 通过多层混合线性模型分析和Snijders & Bosker 法比较不同类型公立医疗卫生机构的门诊 患者满意度。结果 在控制医疗卫生机构的随机效应以及其他固定效应后, 公立三级综合医院和三级专科医院的 门诊患者满意度相对较低, 且高住院病例组合指数组医院的门诊患者总体满意度和对医生服务满意度有走低趋 势。此外, 公立医疗卫生机构类型可解释69.13% 的机构间门诊患者总体满意度差异。结论 诸多因素可能导致公 立三级医院门诊患者总体满意度相对较低, 医疗卫生机构应聚焦于可控因素以提升门诊患者满意度. Substantial research supports bidirectional links between intimate relationship discord and individual psychopathology, including depressive symptoms. However, few studies have utilized daily diary methods to capture the micro‐level processes underlying the association between couple discord and depression, particularly among populations that are at elevated risk for both interpersonal and individual dysfunction. To address this gap, we examined whether daily changes in satisfaction with relationship functioning were associated with daily changes in negative affect and positive affect over the span of 2 weeks among mental health outpatients of low socioeconomic status. Participants were 53 low‐income outpatients from community mental health clinics who completed a semi‐structured interview about the quality of their intimate relationships followed by 14 daily reports of positive and negative mood and satisfaction with relationship functioning across several domains. Growth curve analytic techniques revealed the hypothesized bidirectional relations. Decline in satisfaction with relationship functioning predicted escalation in negative affect and deterioration in positive affect over 2 weeks, and deterioration of mood predicted declining satisfaction with relationship functioning. This study extends existing knowledge about couple dysfunction and individual psychopathology by highlighting the immediate nature of this dynamic process as it unfolds over time. Objective To explore the practice of high-quality nursing service defined by help in improving outpatient service. Methods The patients in the outpatient department of Yunnan Third People's Hospital from January to November 2018 were taken as the experimental group, and the high-quality outpatient nursing service defined by help was implemented. The patients in the outpatient department from January to November 2017 were taken as the control group, and the routine outpatient nursing service was implemented. The implementation effects of the two groups were compared. Results There were significant differences between the experimental group and the control group in terms of patient registration, payment, waiting time for examination results, outpatient refund rates, outpatient complaint rates and patient satisfaction (<0.05). Conclusion The implementation of "help as definition" outpatient quality nursing service can effectively shorten outpatient registration and reduce the payment. The waiting time of paying and examination results can be reduced, and the rates of outpatient refund, registration exchange, and complaints from outpatients could be reduced, therefore the satisfaction of outpatients could be improved. OBJECTIVE: To investigate the medication structure and rationality in outpatients with neuropathic pain (NP) in a three-A level hospital (hereinafter referred to as “this hospital”). METHODS: Related data of outpatients with NP in this hospital in 2017 was extracted from Hospital Information System, and Excel 2013 was adopted to conduct statistical analysis. RESULTS: A total of 237 outpatients (342 case-times, 342 prescriptions) with NP in this hospital in 2017, mostly were female (159 patients, 67.09%); 65 patients were in subsequent visits, with the rate of 27.43%. The total DDDs was 516 times, mostly were oral chemotherapeutic drugs (445 times, 86.24%); the per capita drug cost was 135.25 yuan; the average categories of drug was 1.51 in one prescription, there were 230 prescriptions (67.25%) applied with only 1 drug; the top 3 clinical diagnoses ranked in case number were respectively prosopalgia, neuralgia and postherpetic neuralgia; the first drug ranked in DDDs was Mecobalamine capsule, with per capita cost of 67.04 yuan, mainly for the treatment of prosopalgia and other kinds of neuralgia, which was not within the recommended drug variety by most guidelines; the second drug ranked in DDDs was Gabapentin capsule, with per capita cost of 109.93 yuan, which was in the first-line for treatment of NP. CONCLUSIONS: In 2017, the treatment of outpatients with NP was mainly oral chemotherapeutic drugs, the DDDs of Mecobalamine capsules ranked the first, while its rationality needs further study. Objective To understand outpatient's attention and comprehension to package insert in the grassroots hospital. Methods 608 outpatients were randomly selected to answer the questionnaires designed with relevant data. Results Patients paid the highest attention to effective period indication adverse reactions and dosage in package insert with rate above 80%. The comprehension rate was low. Only three items (indication, dosage and expiration date) were above 30%. Patient's capability to understand package insert is related to their educational level and the area they live (urban area verse rural area). Conclusion Patient's attention to package insert is high, but comprehension level is low. We recommend that package insert should be written in the way of easy to understand for patients. Pharmacists should play an importent role in teaching patients rational drug use to ensure medication safety. [ABSTRACT FROM AUTHOR]
