目的 观察新生儿坏死性小肠结肠炎（NEC）血清胰高血糖素样肽 2（GLP-2）、三叶因子 3（ITF3）水平变 化，并分析其对NEC疾病严重程度的早期诊断价值。方法 选择NEC新生儿140例（NEC组），非NEC新生儿40例 （对照组），NEC组根据修正Bell-NEC诊断标准分为轻度NEC 108例（Ⅰ期）、重度NEC 32例（Ⅱ+Ⅲ期）。采用ELISA 法检测两组血清 GLP-2、ITF3水平，多因素 Logistic 回归分析影响 NEC 病情严重程度的危险因素，绘制血清 GLP-2、 ITF3 诊断重度 NEC 的受试者工作特征曲线（ROC 曲线），并分析其诊断效能。结果 NEC 组、对照组新生儿血清 GLP-2 水平分别为（1. 51 ± 0. 34）、（1. 93 ± 0. 45）ng/mL，血清 ITF3 水平分别为（7. 42 ± 1. 73）、（10. 79 ± 1. 40）ng/ mL，NEC 组新生儿血清 GLP-2、ITF3 水平均低于对照组（P 均<0. 01）。轻度、重度 NEC 患儿血清 GLP-2 水平分别为 （1. 82 ± 0. 39）、（0. 67 ± 0. 21）ng/mL，血清ITF3水平分别为（13. 11 ± 3. 35）、（8. 28 ± 2. 67）ng/mL，重度NEC患儿血 清 GLP-2、ITF3水平均低于轻度 NEC患儿（P均<0. 01）。血清 GLP-2、ITF3水平降低是影响 NEC患儿病情严重程度 的独立危险因素（P均<0. 05）。血清 GLP-2、ITF3单独诊断重度 NEC的 AUC分别为 0. 779（95% CI：0. 629~0. 929）、 0. 866（95% CI：0. 779~0. 952），血清 GLP-2、ITF3 联合诊断重度 NEC 的 AUC、灵敏度及特异度分别为 0. 919（95% CI：0. 831~0. 994）、90. 8%、92. 3%。结论 NEC新生儿血清 GLP-2、ITF3水平均降低，两者是影响 NEC病情严重程 度的独立因素，其联合检测有利于重度NEC的早期诊断。 [ABSTRACT FROM AUTHOR]
