糖尿病足溃疡 (DFU) 是一种累及皮肤, 肌肉, 神经末梢和血管的严重并发症. 目前临床上尚无治疗DFU 的特效药. 间充质干细胞具有自我复制, 自我更新及多向分化潜能, 能够借助多种机制调控机体的免疫功能并促溃 疡组织的炎症消退和血管生成. 研究表明, 间充质干细胞外泌体 (MSCs-Exos) 对DFU患者具有潜在的治疗价值, 并 逐渐应用于临床. MSCs-Exos可通过促进凝血, 抑制炎症反应, 促进细胞增殖和血管生成及增加胶原沉积等机制来 促进DFU创面愈合. 外泌体可以与其他药物, 生物材料, 纳米技术等联合应用, 并借助转运, 调控或缓释药物等多 种方式提高药物的治疗效果. 未来还应建立统一规范的纯化分离技术指导规范, 获得临床应用许可, 设计大样本, 多中心的临床研究验证外泌体在DFU治疗中的安全性和有效性. Ischemic hepatitis is increasingly common in the intensive care unit and often has a poor prognosis. Due to the complex pathogenesis of ischemic hepatitis and a lack of specific clinical manifestations, auxiliary examination methods are limited and there are still no unified diagnostic criteria and treatment methods. Early identification and treatment of ischemic hepatitis is of particular importance. This article reviews the epidemiology, pathogenesis, clinical manifestations, auxiliary examination, diagnosis, and treatment of ischemic hepatitis and related research advances, in order to improve the awareness of this disease among clinicians and promote the timely diagnosis and treatment of this disease. Objective To investigate the levels of hepatitis B surface antigen(HBsAg) and hepatitis B surface antibody(HBsAb) on children in Rongchang district of Chongqing,providing the basis for strengthening hepatitis B vaccine inoculation in preschool children and reference for formulating feasible measures to eliminate hepatitis B in this area.Methods Statistics of hepatitis B virus serological markers test results of children who visited Rongchang District Maternal and Child Health Care Hospital from January 1 to August 31,2019 were performed,calculated the positive rate of HBsAg,HBsAb.Results The study included 1 949 children.The positive rate of HBsAg and the positive rate of single HBsAb were 0.3% (5/1 949) and 71.0% (1 383/1 949),respectively. The highest positive rate of HBsAb was 90.9% in 1-2 years old children,78.3% in >2-3 years old children,and about 50% in over 4 years old children.Conclusion The overall positivity rate of HBsAb is high among healthy children in Rongchang district of Chongqing.The antibody level of children over 4 years old is low.We should strengthen the monitoring of HBsAb level in children of this age group,and strengthen immunity in time for high-risk children with negative antibody,so as to reduce the risk of hepatitis B virus infection. 医学是认识人类生命活动规律的科学，即医学是自然科学的一个分支，又是人文科学。其对象为疾病，同时人是疾病的载体。在诊治疾病的同时，医生与患者间的医患沟通模式也随医学模式而变化。该文回顾了历史上的医学模式，对目前医学模式下医患沟通进行了反思，并在儿科系医学生见习过程中学习认识疾病的同时展开医患沟通教育实践，取得了良好效果。 [ABSTRACT FROM AUTHOR]

The climate change in the upper-middle reaches of Huaihe River Basin was projected in scenarios of 1.5 °C and 2.0 °C global warming using five General Circulation Models (GCMs) of the Coupled Model Intercomparison Project Phase 5 (CMIP5) and three Representative Concentration Pathways (RCPs). Then the hydrological responses to climate change were simulated using hydrological model SWAT (Soil Water Assessment Tool), and the uncertainties of the results were estimated and compared quantitatively. This study draws following conclusions: 1) The simulation results of SWAT model are in agreement with discharge observations very well in both calibration period and validation period, so it is feasible to apply SWAT model in estimating the impacts of climate change. 2) The annual average temperature in the upper-middle reaches of Huaihe River Basin will rise by 1.1 °C and 1.7 °C compared to the baseline (1986-2005). The annual precipitation will increase by 4% and 7% in scenarios of 1.5 °C and 2.0 °C global warming respectively. The ensemble mean (MME) annual runoff simulated based on SWAT will increase by 5% and 8% in scenarios of 1.5 °C and 2.0 °C global warming respectively. 3) The projected monthly runoff distribution will not change, and runoff will be concentrated in midsummer and early autumn (from June to September). Global warming will increase monthly high runoff dramatically, especially 2.0 °C global warming. As a result, the risk of flooding would increase obviously in the future, especially by 2.0 °C global warming. 4) There are big uncertainties in projected precipitation and runoff, and the uncertainty comes mainly from GCMs structure, especially in the scenario of 2.0 °C global warming. Objective To study the association between human cytomegalovirus(HCMV)active infection [HCMV(+)and unexplained hepatitis in infants. Methods A total of 236 children with unexplained liver function impairment(unexplained hepatitis group)and 116 children with normal liver function of breast milk jaundice(control group)were selected. The general condition and clinical manifestations of the two groups of children were recorded, and the levels of HCMV-DNA, HCMV-IgM, ALT, AST and TB were measured. And it was retrospectively analyzed. Results The positive rates of HCMV-IgM in the unexplained hepatitis group and the control group were 74.2% and 19.7%, respectively, and the difference was statistically significant(P<0.05). HCMV(+)group and HCMV(-)group were mainly caused by jaundice, liver enlargement, fever, diarrhea, etc., and HCMV(-)group was more likely to have hepatic enlargement, the difference was statistically significant(P<0.05). There were no significant differences in ALT, AST and TB levels between HCMV(+)and HCMV(-)groups(P>0.05). The ALT levels in HCMV(+)group and HCMV(-)group were equal or less than 2 times, >2-5 times, >5-10 times, and the ratios were more than 10 times. The differences were statistically significant(P<0.05). Conclusion Unexplained hepatitis in infants has a significant correlation with HCMV infection. The main clinical manifestations of jaundice, liver enlargement, fever, diarrhea, etc. , but the severity of liver injury is not associated with HCMV active infection. When the ALT value is>5-10 times normal, it is more likely to be infected with HCMV, and it is more suitable for anti-HCMV treatment in advance. [ABSTRACT FROM AUTHOR]

目的系统评价髓核摘除结合棘突间分离系统动态固定治疗腰椎间盘突出症（LDH)的临床疗效。方法 计算机检索建库至 2016 年 7 月 Cochrane Library、PubMed、Medline、Academic Search Premier、中国生物医学文献数 据库、中国知网、维普和万方数据库文献，同时辅助手工检索，收集采用髓核摘除结合棘突间分离系统动态固定与 单纯髓核摘除治疗LDH的临床对照研究（分别为观察组和对照组）。采用Revman5.3 软件对相关数据进行Meta 分析。结果纳人11篇文献，共675例LDH患者，其中观察组312例、对照组363例。Meta分析结果显示，两组手 术时间、切口长度、术中出血量比较差异均无统计学意义（P均>0.05);观察组术后1年疼痛视觉模拟评分、Oswestry 功能障碍指数问卷表评分均低于对照组 ，术后 1 年及 2 年 J0A 评分、术后优良率、术后责任节段椎间高度均高 于对照组(P均<0.01)。结论髓核摘除结合棘突间分离系统动态固定治疗LDH疗效好，患者术后责任节段椎 间高度维持好、腰背痛残留少。 [ABSTRACT FROM AUTHOR]
