application / pdf, 資料, 要旨： 近年、認知症高齢者の徘徊による行方不明が多発している。これに対し牛島地域包括支援センター南寿園では警察署と協働し、地域の協力事業所（者）を対象に「認知症高齢者みまもり模擬訓練」を行った。この模擬訓練は義務化されていないこともあり、開催している地域は少ない。また、マニュアルがないため、地域包括支援センターと管轄警察署が協議しプログラムを作成して行っている状況にあった。 今後、圏域内の地域住民も参加して「認知症高齢者みまもり模擬訓練」を開催することも想定されることから、先行研究や参加した地域の協力事業所（者）のアンケート結果を参考に、マニュアル作成の一助となり得るハンドブック「認知症みまもりサポートブック」を作成し、「認知症高齢者みまもり模擬訓練」の標準化を図ることを試みた。 Abstract： Recently, many older adults with dementia have been reported missing due to wandering. Therefore,the Ushijima Community Comprehensive Support Center, Nanjuen , collaborated with local police and conducteda “Monitoring Training for Dementia Watch of the Elderly” for local cooperating businesses and residents. However, such training sessions were conducted in few areas, partly owing to their non-compulsory nature. Additionally, due to the lack of a standardized manual, the Community Comprehensive Support Center and local police consulted to create different Dementia Watch programs in each area. In the future, local residents will also be expected to participate in the “Monitoring Training for Dementia Watch of the Elderly.” Based on previous studies and the interview results, this “Dementia Watch Support Book Draft” was created as an intermediary step to facilitate the creation of a standardized manual for “Monitoring Training for Dementia Watch of the Elderly” for participating local businesses and residents.

本研究は、地域包括支援センターの専門職を対象に、地域に潜在する認知症が疑われる高齢者の早期発見に向けたネットワーク構築を目指して、情報提供者の特徴を明らかにすることを目的とした。35 都道府県内に設置されている地域包括支援センターの専門職 6,000 人を対象に質問紙による郵送調査を行った。調査内容は、属性、認知症が疑われる高齢者の鑑別診断に向けた受診援助の経験の有無、鑑別診断に向けた受診を行った認知症が疑われる高齢者の情報提供者（ネットワーク構成員）などとした。鑑別診断に向けた受診を行った認知症が疑われる高齢者の情報提供者の回答を用いて潜在クラス分析を行った結果、5 クラスモデルが採択された。そのうち、幅広いネットワークを有していた専門職は全体の約 1 割に留まっており、認知症が疑われる高齢者のアウトリーチのためのネットワーク構築において課題もうかがわれた。今後は手段的サポートにとどまらず、各サポートの機能的にも着目した検討が必要である。, This study investigated the characteristics of networks for outreach to the elderly with suspected dementia among professionals in community general support centers.We performed a questionnaire survey by mailing 6,000 professionals working in community general support centers across 35 prefectures. This questionnaire survey investigated general information, practical experience in consultation assistance for differential diagnosis of the elderly suffering from dementia, and informants (network members) of the elderly with suspected dementia. Further, latent class analysis was employed to classify the informants of the elderly with suspected dementia.The results indicated that five classes model prevailed, and only about 10% of the specialists had a wide network of informal and formal support. Moreover, a problem was identified within the network involved in outreach to the elderly with suspected dementia. Therefore, quantitative research should be aimed at investigating functions and support methods in the future.

超高齢社会の進展に伴い、認知症高齢者の増加が社会的課題の１つになっている。認知症症状の中でも行動・心理症状（BPSD）は認知症高齢者本人の生活の質を低下させることが明らかになっている。非薬物療法の一種である音楽療法は侵襲を伴わず手軽に実施できることから、多くの高齢者施設や病院でも行われている。本研究は、直近13年間の我が国の認知症高齢者への音楽療法の効果に関する研究論文をレビューし、その動向を分析して今後の課題を明らかにすることを目的とした。医学中央雑誌Web版を用いて検索式を「認知症」and「音楽療法」の原著論文としたところ、117件が該当した。65歳以上の認知症高齢者でないもの、音楽療法以外の非薬物療法を併用しているもの、軽度認知障害（MCI）に関する論文は除外し、39件の論文を分析対象とした。各論文を発行年（巻）、タイトル、著者、研究対象、研究目的、研究方法、主な効果に分けた一覧表を作成し全体を可視化した。分析の結果、1事例の研究や集団療法を実施した研究が多いことが明らかになった。また、実施したプログラムでは歌唱が最も多く、受動的より能動的音楽療法の占める割合が多いこと、専門的知識を持った音楽療法士による介入は14件のみであることが判明した。評価指標は主観的な指標も多く、一概に音楽療法の効果によりBPSDが軽減したとは言い難い論文もみられた。今後は一定数の認知症高齢者を対象にした研究の推進や、音楽療法士と看護師、介護福祉士などの専門職との連携が必要である。更に個別的な非薬物療法へ導くフローチャートや、統一した評価ができるようなアセスメントツールの開発が必要であることが示唆された。

認知症高齢者グループホーム（以下，GH）は利用者の医療ニーズの高まりをうけて看護師を配置するGHが増加している．しかし看護師の雇用形態は様々であるため，GHによって看護師の実践は異なることが予想されるが，GHの看護師の実践に関する報告は限局的である．そこで看護師の実践状況の横断調査を行う必要があると考え，本研究はGHの看護師計8名にインタビューを実施し質問紙を作成することを目的とした．看護師については用語の定義を行った．「内部看護師」を常勤や非常勤問わずGHに雇用されている看護師とし，「外部看護師」を訪問看護ステーション等に所属しGHとの委託契約により訪問している看護師とした．得られたデータを質的記述的に分析した結果，GH看護師の実践状況の質問項目数は，内部・外部看護師共通の項目が44項目，外部看護師のみ2項目追加され計46項目が抽出された．今後は本質問紙を用いた横断調査によりGH看護師の実践状況を量的にも明らかにし，さらに内部看護師と外部看護師の雇用形態の違いによる実践の違いがあるのかも明らかにすることでGH看護師の実践状況を検討していく．The users of Group Homes (GH) for elderly people with dementia have increasing medical needs, and the number of GH that employ nurses is increasing. Due to the variety of employment patterns of nurses, we feel that it is necessary to conduct a cross-sectional survey of the practical situation of nurses and their practices. The purpose of this study was to interview eight GH nurses and prepare a questionnaire in which “Internal nurses” are defined as nurses employed by GH, regardless of whether they are full-time or part-time, and “external nurses” are defined as nurses who belong to home-visit nursing stations and visit by contract with GH. The results of a qualitatively descriptive analysis of the data obtained from the interviews revealed that 44 items were common to both the internal and the external nurses, and 2 items were added to the external nurses only, for a total of 46 items. In the future, a cross-sectional survey using this questionnaire with a large number of participants will clarify the actual, practical situation of GH nurses, and will also clarify whether there are differences between internal nurses and external nurses in practice due to differences in employment patterns.

認知症高齢者の増加とともに、認知症高齢者の介護問題も日々深刻化している。その中でも特に、自宅で生活している認知症高齢者の介護問題は喫緊の課題である。また、認知症高齢者本人の在宅生活の継続について、本人への支援だけでなく、家族介護者への支援も重要と考えられる。本研究は、認知症高齢者の在宅介護の3事例を取り上げて、認知症高齢者が在宅生活の継続を実現するための課題について明らかにすることを目的とした。その結果: ①心身負担、②家族介護者、特に男性介護者への支援の不足、③社会支援の不足、以上3点が、認知症高齢者の継続的な在宅生活維持に影響を与えることを明らかにした。その結果を踏まえ、今後、認知症高齢者の自宅生活を実現するには、①家族介護者特に、男性介護者への支援の強化、②経済的困難を抱える認知症家族への支援の強化、③社会支援の充実という3項目が重要であることを示唆した。

Short Paper, 本研究の目的は、認知症高齢者を対象とした園芸療法プログラム中の発語量および品詞割合を調査し、園芸活動プログラムをより効果的に活用できる可能性について検討することである。対象者は、認知症高齢者5名とした(年齢範囲: 78～91歳, 男性1名・女性4名)。対象者に、6種類の園芸療法プログラムを実施し、全ての会話を記録した。対象者の会話から単語を抽出し、総単語数と品詞を算出・分類した. 総単語数は、園芸療法プログラムによる有意差が認められた。品詞出現割合も、園芸療法プログラムによる有意差が認められた。形容詞・感動詞の占める割合が多かったのは「押し花クラフト」で、植物を使った創作活動であった。今回の研究結果より、園芸療法プログラム内容によって発語量や品詞割合が異なることが明らかとなった。これは、プログラムに伴う効用や対象者への療法士の関わりが影響していることが理由の一つと考えられた。また、同じプログラムでも、療法士の対象者への関わりによって、対象者の園芸療法介入効果が左れる可能性があることが示唆された。

目的：焦燥性興奮のある認知症高齢者への入眠前のハンドマッサージ効果を検証するために無作為化比較試験を計画した。そこで、本研究はハンドマッサージによる介入の実現可能性と評価について検討するために Pilot Study を行った。 方法：焦燥性興奮のある認知症高齢者9名に対して、2週間で6回の入眠前のハンドマッサージを実施し、介入方法の検討と焦燥性興奮、睡眠状態等について介入前後で比較した。 結果：1名が介入途中で睡眠薬の変更にて脱落した。残りの8名全員は完了し、有害事象は認められなかった。介入後に焦燥性興奮の得点が減少したが、その他の評価指標では一定の見解は得られなかった。 結論：焦燥性興奮のある認知症高齢者へのハンドマッサージの実施について、介入の実現可能性が確認できた。今後は、評価内容や評価方法を検討し、ハンドマッサージの手技を一部修正して、対照群を設けた無作為化比較試験による効果の検証を行う。, 研究報告

訪問看護師が認知症高齢者の性をどのように捉えて対応しているかを明らかにする目的で，在宅療養中の認知症高齢者にかかわる訪問看護師18名を対象に個別の半構造化面接を実施した．データを質的帰納的に分析した結果，【高齢者の性に対する捉え方と対応の多様性と国民性】【高齢者の性と性的言動に対する肯定的・受容的認識】【高齢者の性と性的言動に対する非肯定的・非受容的認識】【性的言動への方策】【高齢者が自分に性的言動をした際の対応】【高齢者の性的言動に関する家族への対応と妻のやきもち】【気持ちよい看護をするための構え】【高齢者の性をめぐる組織としての体制づくり】の8カテゴリと，40サブカテゴリが集約された．性の捉え方と対応は多様で年齢や経験によって変化していくという認識，そもそも国民性として高齢者と性を結びつけることに抵抗があるという認識があった．肯定的な認識以外に，性的言動は本人も自覚している，許容範囲を定めて線引きすべきという認識があった．性的言動への方策としては，高齢者を全人的に受け入れる，引きずらないスタンス，看護観の確立等が，特に自身がそれに直面した時の対応としては，受け流す，かわす，拒否する，本人がしてもらいたいケアに絡める等があった．家族には性をタブー視しない，あるいはあえて助言しない姿勢で対応していた．組織の体制づくりとしては，複数スタッフによる訪問看護，スタッフ間での情報共有や連携，カンファレンスや事例検討の企画等があった．一方で，性に関しては触れない，あえて焦点化しないという考え方もあった．

本研究の目的は、認知症グループホームのお茶の時間における認知症高齢者への支援を、介護職員がどのように捉えているのかを明らかにすることである。認知症グループホームの介護職員を対象として、お茶の時間における利用者への支援に対して意識する度合いを問う、独自の質問を記載した質問紙調査票を使用して調査を実施した。全項目に回答している325名の有効回答を分析した。その結果、認知症グループホームの介護職員は、お茶の時間における利用者への支援を、「安心して過ごすこと」、「好みを尊重すること」、「お茶を楽しむ雰囲気づくり」の 視点から捉えていること、およびこの順に関心を向けていることが考察された。, 論文(Article)

The purpose of this study is to clarify the " Snuggle up " that is necessary for care workers to look after elderly people with dementia. An interview took place among 14 care workers, and a figure was compiled using the KJ method. The results show that caregivers suffered from a physical and mental burden. As professionals, however, they faced the contradictory need to maintain high ethical standards while keeping high quality. In such a situation, "unrecognizable behavior" leads to thinking "I don't know," and giving up "because the person is in the state of senile dementia," or "true feeling" comes out.In contrast to "the ideal image," the care worker has "regret, reflection, and hesitation" and fights an inner struggle. Encouraged by "a happy episode," however, the care worker somehow "tries to understand." If he looks at the individualistic aspects of the elderly with dementia, he can recognize that they are the same human beings as care workers. It has been evident that this discovery works as the driving force for care workers. Therefore, " Snuggle up " probably refers to the whole process of trial and error by care workers asthey look after elderly people with dementia.

高齢化の世界的進展に伴い，認知症にまつわる諸問題が国内外で議論されているが，とりわけ我が国では，認知症有病率の高さや膨張する社会保障費抑制の観点からも，その適切な対応が喫緊の課題となっている。こうした中，2019 年6 月に公表された「認知症施策推進大綱」に述べられたように，「通いの場」としての生涯学習関連施設における学習機会について関心が高まっている。本稿は，2016 年度から2019 年度に実施した諸外国における当該テーマに関連した先進的事例の調査結果をもとに，認知症高齢者への学習機会創出の意義について検討したものである。分析の結果，認知症高齢者にとっての学習機会創出は，①当人に意思決定の自由と選択の機会を与え，人間性を維持・発揮するための有効な手段になっていること，②当人の過去と現在につながりを与え，周囲により良い接し方を再考する契機を与えていること，③多くの場合，一般市民との交流の機会となっており，当人に地域社会への帰属意識の醸成を促していること等の意義があることが明らかになった。, Whilst encountering ageing of the population worldwide, it has been becoming imperative to tackle the issues surrounding those living with dementia, as its effects on the whole of society have come to the fore across the globe. Above all, there is an urgent need for Japan to deliberate upon how to resolve the issue of dementia, together with devising an idea in respect of slashing social welfare expenditure, as it is estimated that one in five of those aged 65 or above will have developed dementia by 2025. In this context, a recent political statement in “The Outline of the National Plan for Dementia”, issued by the central government of Japan in 2019, has attached importance to opportunities for lifelong learning in various educational settings as “a place to regularly attend”, which are attracting attention. This paper intends to gauge the meanings of creating learning opportunities for those living with dementia, based on the outcomes of onsite surveys of good examples in some educational settings for lifelong learning with respect to this theme in several countries in North America and Europe, which were conducted by the author from April 2016 to August 2019. The results suggest that the meanings of creating learning opportunities for those with dementia can be found in the following: it has become an efficient means by which the elderly of that kind are able to be offered freedom and room of choice so that they can maintain human dignity and demonstrate their “personhood”; it has given a chance for those with dementia to connect the past with the present, as well as a chance for carers to reconsider their ways of better communication; it promotes social interaction with public citizens, while providing those with dementia with a sense of belonging in the local community.

application/pdf, 本研究の目的は，認知症高齢者が内面を表出するうえで，日本家屋の施設の生活環境をどのように意味づけているのかを文化的側面から記述することである．研究デザインは，焦点を絞ったエスノグラフィーを用い，香川県内のK施設で，認知症高齢者A氏とA氏に関わる利用者・職員に参加観察を実施した．また，ヘルパーと職員にインタビューを実施した．分析は，生活環境を意味づけている場面について文脈を重視して抽出し，5つの領域に分類，コード化し，カテゴリーを見出し，関連性を記述した．本研究は，香川大学医学部倫理委員会の承認を得て行った．, A氏が内面を表出するうえでの生活環境の意味づけは，【家庭を感じ，安らげる場】【仕事の感覚を，持ち続ける場】【幾度も挑戦し，一喜一憂する場】の3つのカテゴリーと8つのコードで構成されていた．K施設は，利用者が自宅にいるような環境を創り,家族のように関わるといった考えをもつ職員達と日本家屋の物的環境が重なり合うことで【家庭を感じ，安らげる場】となっていた．また職員達は，認知症高齢者の行動には理由があると考え，A氏の言動を妨げずに見守っていた．そのなかで，A氏は【仕事の感覚を，持ち続ける場】や【幾度も挑戦し，一喜一憂する場】と意味づけていた．, A氏は，家庭を感じ安らぎながら，これまでの経験を幾度も再経験し，過去を振り返ることで，過去の後悔に折り合いをつけていた．これらの背景には，過去を何度も想起することができる生活環境とA氏の言動を見守り，家族のように関わる職員の考えがあった．また，A氏自身が，表現していることに寄り添う，聴き手の存在が重要であることが示唆された．, Our purpose was to clarify the significance of the living environment in a nursing facility designed with Japanese-style architecture for demented elderly in helping them express themselves from the cultural aspect. Participant observation was conducted for A, a demented elderly person, and staff who interacted with A at facility K in Kagawa Prefecture. Staff were also interviewed. Scenes reflecting the living environment were extracted focusing on the contexts, coded, and categorized, and then, relevance with A was described. This study was approved by the Ethics Committee of Kagawa University School of Medicine., There were three categories of the meanings of the living environment for A that helped him express his feelings (“a place to feel at home and at ease”; “a place to maintain the feeling of being at work”; “a place to repeatedly take challenges and react to the outcomes”), and 8 codes. The staff’s efforts to create an environment where clients can feel at home and are involved like family members and the physical environment of the Japanese-style facility provided “a place to feel at home and at ease”., The staff observed A’s behaviors without disturbing him since they believed that there are reasons behind the behaviors of demented elderly. It was assumed that A considered this facility as “a place to maintain the feeling of being at work” and “a place to repeatedly take challenges and react to the outcomes”. A dealt with his past regrets by repeatedly re-experiencing and looking back over the past while feeling at ease. This was supported by the living environment that allowed A to repeatedly recall his past, and by the staff who involved A as their family member. The results suggested the importance of people who closely listened to and embraced what A expressed.

As the support for older people with dementia to continue to live at home, “support to bring out the strength of older people with dementia and their family” was most frequently reported, and “support to stabilize the mind and body” and “support to promote informal support from their neighbors and friends” were also reported. The assessment and promotion of self-care ability of older people with dementia and their family, and the improvement of the environment to promote use of informal services were also observed. In addition, “responding at an appropriate time” and “connecting between the relevant professions” were reported as types of support provided by multi-professional collaboration. 【目的】認知症高齢者の食事性低血圧(PPH)と自律神経活動の関連と,PPHの発現頻度,自律神経活動との関連を明らかにする.【方法】歩行可能な入院中の認知症高齢者を対象に,食前と食後の血圧,脈拍,指尖脈波を用いた自律神経活動測定を,座位にて5日間実施した,【結果】対象者は男性11人,女性20人,合計31人で,平均年齢85.42±6.36歳であった.病名別ではアルツハイマー型認知症(AD)23人,レビー小体型認知症(DLB)6人,ウェルニッケ･コルサコフ症候群1人,脳血管性認知症1人であった.対象者の61%にPPHの発現があった.ADの70%にPPHの発現があった.PPH発現時のLn(LF/HF)は食前0.93±0.24,食後0.81±0.17(p＜0.05)で有意差があった.PPHの発現なし時のLn(LF/HF)の平均値は食前0.93±0.28,食後0.92±0.26であり,有意差はなかった.【考察】PPH発現のケースでは食前･後のLn(LF/HF)に差があり,PPH発現なしではLn(LF/HF)に差がないことから,PPHの発現ケースには心臓交感神経活動が食前と比して食後に有意に減弱しており,交感神経活動にPPHが影響していることが示唆された., Purpose : This study aims to clarify the correlation between postprandial hypotension (PPH) and autonomicnervous activity in elderly people with dementia.Method : We examined the autonomic nervous activities of ambulatory patients by measuring their blood pressure, pulse and finger plethysmogram before and after meals for five days.Results : There were 11 male and 20 female subjects with a median age of 85.42 ± 6.36. We had 23 patientswith AD, six with DLB, one with Wernicke-Korsakoff, and one with cerebrovascular dementia. 61 percent of the subjects showed symptoms of PPH. 70 percent of the patients with AD showed symptoms of PPH. 本研究の目的は，学生が実施している認知症高齢者への「聞き書き」の学びを明らかにすることである。A大学老年看護学実習において学生が行った認知症高齢者に対する「聞き書き」のリフレクションレポートを質的帰納的に分析した。その結果，【語り手の個人史や価値観の理解が深まる】【聞き書きは個別性のあるケアを創り出す】【語り手の豊かな感情を呼び起こす】【語ることで人生を振りかえり，過去の記憶や思い出に親しむ】【語りを引き出すための事前準備の重要性】【語りを傾聴し，ありのままを受けとめる姿勢の重要性】【記憶の想起を助ける工夫の重要性】【聞き書き中は語り手に合わせた状況判断が求められる】の8つのカテゴリが抽出された。学生は「聞き書き」を通して，認知症高齢者がもつコミュニケーション障害を考え，学生自らが相手に応じて瞬時に状況判断を行って工夫し，認知症高齢者との関係づくりに取り組めていたと考える。また学生は，「聞き書き」を通して，認知症高齢者に対する態度形成とQOLを高めるケア提供の機会を得ていたと考える。これらのことから，「聞き書き」は，学生が認知症高齢者との対話を通して，老年看護学教育において重要な認知症高齢者の理解が深まる機会になることが示唆された。教員は，今後も学生個々が「聞き書き」という相互行為を通して体験した内容，感情，思考などを言語化したものから，学生が認知症高齢者理解を深めていく過程を確認していくことが重要である。

韓国において、誰が責任無能力者の法定監督義務者になるべきかという問題につき、通説は、後見審判を受けた場合には後見人が法定監督義務者に当たると解する。後見人のいない責任無能力者については、慣習法または条理に基づいて、法定監督義務者を決定すべきであるという。精神健康福祉法上の保護義務者は、後見人と扶養義務者の順で就任し、精神疾患者の他害防止義務を負っている。禁治産宣告を受けていない心神喪失者の不法行為につき、条理上、妻、父母、戸主の順で法定監督義務者になるとの判例がある。一方、社会の実情に目を向けると、認知症高齢者、重症精神疾患者、知的障害者等が増えている現実がある。ある調査によると、親族が後見人として選任される比率が87.3％、親族の中では、子女が後見人として選任される頻度が最も多く、続いて配偶者、父母、兄弟姉妹等の順である。認知症高齢者が増える中、子女が後見人になることが最も多いことから、法定監督義務者としての子女がいかに高齢者の監督ができるのか、また監督すべきであるのか課題が少なくない。また、痴呆患者をケアする家族の負担緩和に向けた国家や地方自治団体に対する努力義務の挿入、後見人は法定監督義務者に当たらないとする見解、親権者であるから当然に法定監督義務者になるわけではないとの見解の登場など、変化の兆しもないわけではない。, Regarding the question of who should become the legal supervisor of the non-responsible person in South Korea, general opinion is that the guardian is the legal supervisor if he receives a guardianship referee. For those who have no guardian, the legal supervisor must be determined based on customary law or reason. Obligors for protection under the Mental Health and Welfare Law are determined by guardians and dependents in order and are obligated to prevent harm to mentally ill persons. It is the jurisprudence who should become for the legal supervisor of the non-responsible person who have not been declared banned from sanctions for legal conduct in order of the wife, parents, and the landlord.On the other hand, when looking at the actual situation of society, there is a reality that elderly people with dementia, people with severe mental illness, people with intellectual disabilities, etc. are increasing. According to one survey, the proportion of relatives elected as guardians was 87.3%, and among relatives, children were most often elected asguardians, followed by spouses, parents, siblings, and so on. As the number of elderly people with dementia increases, the number of children is often the guardian, so there are many issues about how children as legally mandated supervisors can and should supervise the elderly. In addition, the government and local self-governmental organizations are required to make efforts to alleviate the burden on families who care for patients with dementia. There are signs of change, and change may occur.

北海道文教大学人間科学部看護学科老年看護学領域は，2018 年度に恵庭市地域と連携をとり，学生の認知症高齢者の理解を深めるために，認知症高齢者とかかわることができる企画を考案し実施した．本資料では「RUN 伴in 北海道文教大学」「認知症カフェin オープンキャンパス」「看護師国家試験応援出張認知症カフェ」の実践報告をする．結果，学生は実際に認知症高齢者とかかわり共に過ごすことで，認知症高齢者をイメージすることができた．また，認知症高齢者にも笑顔がみられ，脳への刺激となったといえる．今後は，グループホーム（以下：GH）に出向いて認知症高齢者の方とコミュニケーションをとり，どのように生活をしているのか学習する実習を考えていきたい．さらに，学生プロジェクトを立ち上げ，学生が地域に出ていき認知症の理解を深めることができるような，企画にも発展させたいと考える．

本研究の目的は，「認知症高齢者のその人らしさを尊重したケア」に関する認識と実践の実態とその影響要因を明らかにすることである．全国329か所のグループホームのうち研究同意の得られた32施設のケア提供者250名を対象に先行研究で得られたその人らしさを尊重したケアを示す14項目についての認識と実施に関する質問紙調査を行った．その結果，「その人らしさを尊重したケア」14項目とも十分認識されていたが，実施は有意に低かった．「その人らしさを尊重したケア」14項目に影響する要因は，僅かではあるが，認知症ケア経験年数が14項目中4 項目に影響し，研修回数が1 項目に影響していた． This study aimed to examine the actual state of awareness regarding care that respects individuality for elderly persons with dementia, as well as its implementation and influencing factors. The subjects were 250 care providers from 32 facilities who were recruited from the 329 group homes across Japan and consented to participate in this study. The subjects completed questionnaire surveys with 14 items that, according to a previous study, indicate care that respects individuality. The results revealed that while there is ample awareness for all 14 survey items regarding care that respects individuality, its implementation was significantly low. It appears to be that years of experience with dementia care influences four of 14 survey items and numbers of training times influences one of 14 survey items.

本研究の目的は「認知症高齢者の気になる行動」を中心に、生活者としての認知症高齢者と家族の抱える問題について分析し、認知症高齢者と介護者の生活支援の方法を検討することである。インタビューから得られたデータを抜粋し、KJ法により分類した結果、認知症高齢者の気になる行動は「もの忘れ」「怒りっぽい」「不安になる」「徘徊・うろうろする」「痛みがある・病気・身体の不調」「状況がわからない」「排泄の問題」「問題行動」といった8つのカテゴリーに分類された。生活上の最も困難な事象は認知症高齢者の問題行動であると考えられる。「徘徊・うろうろする」「認知症高齢者の不安から生じるさまざまな気になる行動」などは、いわゆるノーマルなものではなく、それなりの生活問題として重要であるが、最も困難な事象は弄便（ろうべん）であると考えられた。これらの問題行動は認知症という疾患が原因となるものであり、疾患を理解することで介護者の精神的な負担は軽減することが先行研究や本研究からも推察される。介護者は認知症高齢者の奇異な行動にも意味があることの理解をすることが、介護者の精神的な介護負担軽減に有用であると考えられた。これらの結果より、介護者への認知症高齢者の疾患の理解を促す支援が重要であることが示唆された。

The “program on the creation of institutional environments supportive of elderly people with dementia” was used to compile 266 examples of environment creation implemented at traditional special nursing homes for the elderly, and to analyze service outcomes based on environment creation objectives, implementation methods, employed environmental elements, and Professional Environmental Assessment Protocol (PEAP Japanese version 3). Environment creation objectives were categorized into 47 sub-items. The environment creation initiative was a combination of physical support and care support in approximately 60% of examples, and small-scale environmental elements were often employed, such as accessories, furniture and welfare equipment. As a service outcome, a high rate of improvement of 70%–80% was achieved in eight dimensions of the PEAP V3. A database of practical examples of environment creation demonstrating initiatives and service outcomes of environment creation according to each environment creation objective was subsequently proposed., 要旨 :「 認知症高齢者に配慮した施設環境づくり支援プログラム」を用いて、従来型特別養護老人ホームで実施された環境づくり266 件を取り上げ、環境づくりの目的・実施手法・用いた環境要素・認知症高齢者環境支援指針（以下PEAP 日本版３）に基づくサービス成果について分析を行った。環境づくりの目的は、47 の小項目に分類された。環境づくりの実施内容は、約6 割で物理的対応とケア的対応がセットで実施され、小物や家具・福祉用具など小規模な環境要素が多く用いられた。サービス成果として、PEAP 日本版3 の8 つの次元において、70 ～ 80％の高い向上率が得られた。個々の環境づくりの目的別に、環境づくりの実施内容とサービス成果を示した環境づくり実践事例データ集を提案した。

認知症が進行していく過程では，誤嚥性肺炎，窒息，転倒による骨折が起こりやすく，身体疾患の治療目的で急性期病棟へ入院する認知症高齢者が増加している．そこで今回，急性期病棟における看護師の認知症高齢者に関する認識や看護実践の現状を明らかにすることを目的に文献検討を行った．文献検討の結果，急性期病棟で働く看護師は，認知症高齢者に対し苦手意識やいらだちといった否定的な認識をもっていたが，認知症高齢者の反応を引き出すといった成功体験や，認知症や認知症看護について学習することによって，認識を肯定的なものへと変化させていた．また，急性期病棟で働く看護師は， BPSD といった認知症特有の症状や認知症高齢者に抑制をせざるを得ない状況，認知症高齢者とゆっくり関わることができない看護体制に対し，困難を感じていた．その一方で，看護師は医師や介護職などの多職種と協力し，関わり方の工夫を行いながら認知症高齢者を尊重した対応をしていた．しかし，急性期病棟に入院する認知症高齢者本人に焦点を当てた研究はほとんどなく，認知症高齢者自身の視点で，看護に対する反応や思いについて十分に検討されていないことが明らかになった．

認知症高齢者の家族介護者が抱く介護負担感に関する簡便な支援の効果を明らかにした.研究対象者は,研究協力施設の2施設を利用している認知症高齢者の主介護者34人とした.介入として,短縮版Zarit介護負担尺度日本語版(J-ZBI_8)に基づいた簡便な支援を介護支援専門員が行った.対象者には,支援1か月前,支援直後,支援1か月後に聞き取り調査か自記式質問紙調査を行った.介護者の概要は,女性が28人と多くを占めていた.支援前後では,介護負担感(ρ=0.992),肯定的認識(ρ=0.735),介護継続意思(ρ=0.152)で有意差は認められなかった.しかし,介護負担感がハイリスク群の対象者では,介護継続意思の変化量が非ハイリスク群と比べて有意に向上していた(ρ=0.045).本研究の結果から,認知症高齢者の介護者の介護負担感の軽減には,主観的な介護負担感を把握し,それに基づく支援をさらに充実させることで効果を高める必要性が示唆された., The present study aimed to verify the efficacy of using an onsite prompt support for caregiver burden for caregivers of persons with dementia in caregiving appraisal. Thirty-four primary caregivers of persons with dementia staying at two care facilities consented to participate in this study. Long-term care support specialists provided support based on care burden standards. The measures were administered as selfreport questionnaires or as interviews before the support was conducted, during the support, immediately after, and one month after to evaluate changes in caregiver appraisal. Participants were predominately women (n = 28) . There was no significant difference in caregiver burden (p = 0.992), positive appraisal (p = 0.735), or caregiving continuation (p = 0.152) before and after the support. However, among participants who had a high risk of caregiver burden, continuation of caregiving improved significantly over the period of support; this was not found for participants without a high risk. According to the study results, caregiver burden among caregivers of persons with dementia could be reduced by improving their understandingof the subjective feelings of caregiver burden., 研究報告

【目的】高齢者施設の入居者の多くは、重度な要介護状態であり、身体機能の低下および低栄養状態などの対応に苦慮している。われわれは、横断研究により、高齢者施設入居者ではMMSE が10点以下になると骨格筋量が減少するリスク要因であることを明らかにした。そこで、今回は高齢者施設入居者を対象に1 年間の追跡調査を行い、認知症レベルと骨格筋量の変化について縦断的に検討した。【方法】特別養護老人ホームの入居者127名（男性25名、女性102名、平均年齢84.6±6.9歳）を対象とした。認知症レベルの評価は認知機能検査（MMSE）を用い、体組成はInBody S-10を用いて四肢筋肉量を測定し、骨格筋指数（SMI）を算出した。栄養状態の評価は簡易栄養状態評価表（MNA-SF）を用いて調査を行った。1 年経過後に同様の調査を行い、比較検討した。【結果】エネルギー摂取量、たんぱく質摂取量は有意に減少した。SMI は有意に低下したが、体脂肪率は有意に増加し、その結果、BMI に明らかな変化は認められなかった。SMI が1 年間に10％以上減少するリスクとして、性別、調査開始時の年齢、たんぱく質摂取量およびSMI で調整したロジステック回帰分析を行った結果、調査開始時のMMSE が10点以下であることのオッズ比は2.76（95％CI：1.07－7.12, p=0.036）であった。以上のことから、MMSE が10点以下であることは、SMI が一年間で10％以上減少するリスク要因と考えられた。, Many residents of elderly facilities are in serious need of long-term care. Caregivers are struggling with the declining physical function and malnutrition of residents. Our previous cross-sectional study of elderly facility residents revealed that a Mini Mental State Examination (MMSE) score of ≦10 is a risk factor for skeletal muscle mass reduction. Therefore, in this study, we conducted a one-year follow-up survey of elderly residents and examined longitudinal changes in dementia level and skeletal muscle mass. A total of 127 people (25 males, 102 females; average age 84.6 ± 6.9 years old) residing in a special nursing home for the elderly were investigated. The MMSE was used to evaluate dementia level. Skeletal muscle mass was measured using InBody S-10 for body composition, and skeletal muscle index (SMI) was calculated. The Mini Nutritional Assessment-Short Form was used to evaluate nutritional status. The same survey was conducted one year later, and the results were compared. Energy intake and protein intake decreased significantly after one year. SMI significantly decreased and body fat percentage significantly increased, resulting in no significant change in BMI. Logistic regression analysis adjusted for sex, SMI at baseline, age, and protein intake revealed that for subjects with a baseline MMSE score of ≦10, the odds ratio for a 10% decrease in SMI in one year was 2.76 (95% CI: 1.07–7.12, p=0.036). These results suggest that an MMSE score of ≦10 is a risk factor for SMI to decline by 10% in one year.

本研究は、対象理解に焦点をあてた認知症高齢者の在宅療養継続を目指した訪問看護師の支援について明らかにすることを目的とした。対象は訪問看護師３名で、半構成的面接法を用いてインタビューした内容を質的記述的に分析した。その結果、訪問看護師は、【培ってきた人生に敬意を払い療養者の尊厳を守る】という信念に立ち、【言葉に頼らず全身を使って気遣いを表現し焦らず徐々に心を通わせ】、【観察とケアチームとの連携により療養者が自ら表現できない状態をアセスメント】して療養者の世界に身を置いていた。その上で、【療養者の状態を経時的に捉え起こりうるリスクをアセスメントし予防】し、【療養者が言葉にできない症状を感知し適切な治療につなげ】、【療養者のできる力を最大限に引き出し生活に取り入れ】回復を助ける支援を行っており、カテゴリーの１つ１つが連動して実施されることが、認知症高齢者への支援に重要であることが示唆された。

わが国における認知症高齢者の増加，認知症施策推進総合戦略（新オレンジプラン）の策定などに伴い，これまで以上に認知症に対する関心が高まっている．しかし，認知症という言葉は身近になっているにもかかわらず，社会における認知症スティグマが軽減されているとは言い難い現状がある．このような背景から，認知症スティグマに着目した研究が増加しつつある．一方，認知症高齢者と高齢者ケア専門職，双方の認知症スティグマに着目した研究は少ない．そこで，本研究では文献研究の手法を用いて先行研究を踏まえ，本研究における認知症スティグマを定義するための理路を提示した．また，認知症のアセスメントとケアの現状と課題についても言及し，エビデンスとナラティヴに関する先行研究からの知見を援用した．以上の結果を統合し，実践への提言として，オノマトペ（擬音語・擬態語）を認知症ケアとアセスメントに使用することの可能性をまとめた．, Interest in dementia has been increasing recently due to the growing number of elderlypeople with dementia in Japan, and the formulation of the Japanese national dementia strategy（New Orange Plan）. Despite the situation, the stigma around dementia has not been reducedin society. While such background has increased the number of studies focusing on the stigmaaround dementia, there have only been a few studies focusing on the stigma of dementia fromthe perspectives of both the elderly with dementia and elderly care professionals. The aimof this study is to provide a rationale for defining the stigma of dementia based on review.Additionally, I mention the situation and challenges for dementia care and its assessment,and employ findings from prior studies on evidence and narrative. I integrate the results ofthe study and compiled the possibilities to use onomatopoeia （echoic and mimetic words） fordementia care and its assessment as a recommendation for dementia care practice.

In this study, we conducted a questionnaire survey targeting 576 facility staff in charge of hospitalsfocusing on admission cooperation between facilities regarding the life history of the elderly withdementia. We concluded that there was no cooperation between facilities for information only limited tolife history due mainly to “Not enough time.”, “Private contents.”, “Insufficient amount/contents ofinformation.”, and “inadequacy of the recording paper.”. As a result of these problems, the followingitems should be added to the assessment sheet: (1) preparation of the assessment sheet, (2) guidancemanual to enhance cooperation between facilities, and (3) a“ self-record notebook” written by an elderlyperson himself/herself.
