PDF, 要 旨 本研究の目的は、幼稚園、保育所、認定こども園に関する国内の文献を分析し、乳幼児施設における保健活動の現状を検討し今後の課題を見出すことであった。 文献検索方法は、医学中央雑誌を用い、2008 年から幼稚園教育要領および保育所保育指針が改定された 2020 年までの「保健活動 and 幼稚園」および「保健活動 and 保育所」「保健活動 and 認定こども園」をキーワードとして検索し、20 件の文献を分析、検討の対象とした。 幼稚園、保育所ともに養護教諭や看護職者が配置されている園は、配置されていない園と比較して、園児の健康と安全管理について整っているという結果がみられた。さらに、養護教諭や看護職者が配置されている利点としては、怪我や病気の子どもがいる時に専門家が対応してくれるという安心感があった。しかし、乳幼児施設における養護教諭と看護職者の配置率は低く、その背景には財政的余裕がないことや保育士確保を優先するなど、配置の必要性の認識が様々であることが示された。また、保育所においては、専門性が発揮できないことや、保育士などの多職種との関わりに困難を感じていた。保健活動の担い手である養護教諭や看護職者の専門性が保育の現場で理解され、保健活動への意識を高めていけるような働きかけが必要であることが示唆された。

本研究は、T市内保育所・認定こども園の看護職配置の現状と役割、医療的ケアの必要な障害児の受け入れの現状と課題を明らかにすることを目的とした。研究の調査対象者は、T市内にある保育所・認定こども園203施設に所属する所長（園長）・主任保育士・看護職者の内いずれか1名とした。2018年11月～12月の間に、無記名の自記式調査用紙を郵送法にて配布・回収した。本研究は研究者所属大学の人を対象とする研究倫理委員会の承認を得て実施した。 調査用紙116通（回収率57.1％）が回収された。看護職の配置は有が57施設（49.1％）、無は59施設（50.9％）であった。医療的ケアの必要な障害児を受け入れている施設は6施設（5.2％）で、今後受け入れ予定は有が5施設（4.3％）、無は85施設（73.3％）、状況に応じて検討するは21施設（18.1％）であった。受け入れ可能と考える医療的ケア（複数回答）で最も多かったのが、服薬管理26件（47.3％）、次いで気管切開部の管理7件（12.7％）であった。受け入れにあたり必要と考えられる準備（複数回答）は、保護者との話し合い64件（19.8％）、次いで嘱託医・主治医とのカンファレンス59件（18.2％）であった。 保育所・認定こども園の看護職配置は年々増加傾向にある。医療的ケアの必要な障害児の受け入れはまだ少ないが、受け入れを検討する施設が増えてきている。検討課題として、保育所看護職の期待は大きい。さらに、医療的ケア児等コーディネーター養成研究の周知、保育所関係者の受講への整備が今後の課題となる。また、障害児の状態を把握するためには、保護者との話し合いが最も重要であることが示唆された。

本研究では，2016 年公表の中央教育審議会答申に基づいて，現在の幼稚園，保育所，認定こども園におけるカリキュラム・マネジメントの特徴を明らかにした上で，それらにおいて必要とされるカリキュラム・マネジメント研修について考察している。まず，保育者に，所属保育施設の保育の全体的計画において，保育の目標・ねらい・内容の連関性が確保できていないことや国のカリキュラム基準に示される保育の内容のすべてが含まれていることを保証できていないことを認識できる研修が必要である。次に，体験的に，本格的なカリキュラム・マネジメントの具体的実現手順を理解する研修が必要である。さらに，職員をその研修全体を自律的に行うことのできる人材に育成する研修が必要である。

保育者は，自閉スペクトラム症の特性がある子どもとその保護者への支援に当たり，保護者との関係性に困難を感じていることが多い。本研究は，ASD がある子どもの母親の手記等の記載内容を踏まえ，保育の現場における自閉スペクトラム症の特性がある幼児の母親支援のあり方について検討することを目的とした。 母親の心情に関する手記等の記載内容について，基本的には先行研究などと同様であったが，個別性が高いことが確認できた。例えば，専門機関などで「診断等されてショックを受ける」と記載されているのは８事例中７事例であるが，「診断等されてすっきりとした気持ちになる」などと記載されているのは４事例であり，「受け入れる心の準備がない状態で診断を仰ぐのは，親にとってかなり厳しいこと」と記載されている事例があった。母親に子どもの発達特性を受け止めるなどの「心の準備」がないと思われる場合は保育者が専門機関での相談などを勧めることは母親を精神的に追い詰める可能性があることから慎重に検討すべきと考える。保育の現場での「気になる」段階からの支援は，極めて困難な課題であるとの認識のもと，組織的に，可能な範囲で子どもと母親などの状況を把握し，育児に不安や困難を感じていたり，自信を失っていたり，「心の準備」がなかったり，保育者その他周囲の者との関係性で心が傷ついていたり被害的になっていたり，さらに自死を考える可能性があることなど，それぞれの母親の心情を推し量り進めることが重要であると考える。なお，半数以上の事例で子どもが乳幼児期のときの母親の心情として「働きかけへの反応が乏しく不安を感じる」，「親子の絆が感じられず寂しい」，「子どもの行動に不安・負担を感じる」，「コミュニケーションがとれず不安・情けなさを感じる」，「子どもの行動や言うことをきかないことに腹が立つ」，「育て方がわからず不安・育児が困難を極める」，「叱責や体罰を加える」，「発達していることやアタッチメント行動を感じるとうれしい」などと記載されていたことから，この時期の支援は，親子関係の健全な形成を重視して進める必要があると考えた。

Disaster prevention awareness in Early Childhood Education and Care (ECEC) is an urgent issue to improve. In this context, the authors have been exploring and practicing disaster-prevention childcare for infants and toddlers young children (0-6years) that is experiential, encourages children's spontaneous activities, and allows teachers to work on disaster prevention themselves, sometimes including their parents. In this report, we summarize the results of a questionnaire survey for 10 years of parents and teachers that we have conducted through these disaster prevention childcare practices.Based on the results,we decided to create a roadmap for “qualitative improvement” for future disaster prevention ECEC practices.

At 7：58 a.m.on June 18, 2018,a large earthquake struck the northern part of Osaka Prefecture, causing damage in the cities of Osaka,Takatsuki and Ibaraki.It is important to record the impact of the earthquake on human activities as well as various human casualties due to its spatiotemporal characteristics, as it occurred in a “densely populated urban area”and “during the time of going to school and going to work”. In particular,we decided to record the situation after the time of the earthquake at the nursery school, which is located in the area that was the center of the damage and has been in contact with the school for a long time, for use in future disaster prevention.

本稿は、2019年度北海道大学教育学部専門科目「教育基礎論調査実習Ⅰ・Ⅱ」において、実施された調査研究の報告書である。本年度は、北海道上士幌町を対象に、町の教育行政および学校経営にかかる調査を実施した。本調査では、上士幌町の取り組みが、「地方創生の一環として行われる子育て支援」、「地域と結びついた一貫教育」などの将来の日本全体における教育を考える手掛かりになると考え、上士幌町を調査対象とすることとした。その中でも、上士幌町の子育て支援政策とコミュニティ・スクール（CS）に焦点を当てることとした。

This study examines the implementation of curriculum organization in a newly established center for early childhood education and care (certified child daycare center) in municipalities, conducted through collaboration between the municipal administration and the daycare center. Using the circulating workshop method, a training support system was constructed, and this study clarifies the information and issues found as a result of curriculum organization. The purpose was to produce an organized curriculum and tasks from the circulating workshop, based on evaluations by the participants. As a result, the examination of the specific curriculum organization and shared processes was performed well and exhibited the collaboration between municipal administrations and centers. It was concluded that circulating workshops help improve the quality of childcare.

2017（平成29）年の改定では、保育所保育指針において、「保育の計画」という文言はそのままで、「保育課程」という文言はなくなり、「保育課程」は「全体的な計画」と改められた。それと同時に、幼稚園教育要領の「教育課程」のところに「全体的な計画」という文言が新たに付け加わることとなった。そして保育所保育指針、幼稚園教育要領、幼保連携型認定こども園教育・保育要領のいずれにおいても「全体的な計画」という共通した文言が存在することになったのである。また、2017年の幼稚園教育要領、幼保連携型認定こども園教育・保育要領において、教育（保育）活動の質の向上を図っていく「カリキュラム・マネジメント」に努めるように書かれていることも重要な内容である。保育所保育指針の総則「３保育の計画及び評価」から、保育士が目指す子どもの姿に向けて、日々の保育を評価及び反省し、環境を再構成して実践するという循環的行動を自発的に行うことを読み取ることができる。しかし、「カリキュラム・マネジメント」に対応するようなキーワードはない。今後は、就学前の保育・教育を行う施設の保育者の共通理解と意識の向上を目指すために、更に、文言の統一化を図る必要があるのではないかとも考える。また、5歳児修了時までに育ってほしい姿がすべてに明記され、アプローチカリキュラムが必要とされている昨今、せめて年長児のクラスにおける就学前の1年間は、保育所と幼稚園が共に「教育課程」を検討して行くことが必要となってくるのではないだろうか。

子ども・子育て支援新制度になり、認定こども園の普及が進み始めた。本稿では、この子ども・子育て支援新制度という大きな制度改革の中で、比較的小規模な民間組織である認定こども園の経営とそれを設置する法人の会計問題の関係を明らかにしようと試みた。

本研究の目的は，保育者が何を根拠に生活困難を抱える子どもや家庭を判断するのかを明らかにすることである。保育者120 名に子どもの様子，保護者の様子，その他の3 つの視点から自由記述による回答を求めた。回答項目を1 つの意味のまとまりごとに分け，意味内容の類似性に基づき分類した。結果，子どもの様子は「不衛生である」などの11項目に，保護者の様子は「身だしなみの変化」などの14 項目にまとめられた。児童虐待研究と比較すると，子どもの様子はほぼ共通し，違いは性的虐待の子どもの様子を示すものが抽出されなかった点である。保護者の様子もほぼ共通していたが，身だしなみの変化が児童虐待研究では言及されていなかった。今後の課題は，児童虐待チェックリストと比較しその相違を明確にする点，量的データの収集・分析から実証的に検討する点，抽出された項目をもとにチェックリストを作成し，その有効性を検証する点である。

Before the declining birthrate and aging society, it is necessary to establish a system with a realistic viewpoint of a sustainable social welfare system and financial resources. It is necessary to design, the aim of the new system aiming at liberalization of price and liberalization of entry is probably the main point.
