BACKGROUND: The method of injecting adeno-associated virus subpial can limit the virus to a small area centered on the injection point,with less systemic toxicity and side effects, and save the amount of virus. However, there are no reports on the application of this method in spinal cord injury rats, and the transfection of astrocytes and neurons at the injured site is still unclear.OBJECTIVE: To investigate the transfection effect of subpial injection of adeno-associated virus 2 on astrocytes and neurons in the spinal cord injury site and the technical points of the surgical procedure.METHODS: The study was approved by the Experimental Animal Ethics Committee of Jilin University. Twenty-four Wistar rats were randomly divided into two groups: adeno-associated virus group and model group. Rats in both groups were used to establish the spinal cord injury model using the clamp method. The adeno-associated virus group received 60 μL of virus solution by subpial injection, and the model group received an equal amount of 5% dextran solution. At postoperative 7, 14, and 21 days, spinal cord specimens were taken for immunofluorescence staining to observe the distribution of astrocytes and the transfection of adeno-associated virus 2.RESULTS AND CONCLUSION:(1) After subpial injection of the virus, the virus solution evenly distributed in the range of about 1 cm above and below the injured site.(2) At 7 and 14 days after surgery, only a small amount of green fluorescent protein was expressed in the spinal cord injury area. Until day 21, a large number of reactive astrocytes were observed around the spinal cord injury cavity. The strongest expression was in the periphery of the spinal cord cavity, with obvious glial boundary membrane. It was also found that adeno-associated virus 2 could stably transfect astrocytes and neurons, in which astrocytes were concentrated around the periphery of the spinal cord cavity,while neurons were scattered within the scope of virus transfection. Only trace adeno-associated virus 2 transfection was found in normal tissues outside the scope of injection.(3) The method has the advantages of confining the virus to the injured site, targeting astrocyte and nerve cell transfection, low viral dosage, low systemic toxicity and side effects, and low cost. Objective: To observe the changes and the relatively stable time of corneal curvature after phacoemulsification(PHACO) and IOL implantation on age-related cataract patients with transparent corneal incision. Methods: A total of 200 patients(216 eyes) with 3.0 mm transparent corneal incision cataract phacoemulsification and IOL implantation were collected in the hospital of the First Affiliated Hospital of Harbin Medical University(Wu Liande Memorial Hospital)from June 2016 to August 2016. Among them, 88 males, 128 females, mean age 71.2 years old. Performed relative examination before the operation, and check the corneal curvature,vision, intraocular pressure of preoperative, first day after operation, first week after operation, first month after operation, and three months after operation, and perform the corresponding statistical analysis. Results: After surgery, the recover of vision which better then0.5 at different time: the first day was 147 eyes(68.05 %), one week was 175 eyes(81.02 %), one month was 193 eyes(89.35 %) and the three month was 197 eyes(91.20 %). The preoperative corneal curvature was 43.94±1.35, postoperative the first day and the first week corneal curvature respectively of 44.98±1.06, 44.45±1.18, compared with the preoperative have significant difference(p < 0.05), one month and three months after the corneal curvature were 44.13±1.27, 44.02±1.24, compared with the preoperative was no statistically significant difference(p < 0.05); Intraoperative astigmatism reached its maximum on the first day after surgery, then gradually decreased,there was a significant difference between one month, three months and one day after surgery(p < 0.05). There was no significant difference between three months and one month after surgery(p < 0.05). Intraoperative astigmatism decreased gradually and stabilized in a month. Conclusion: Patients with 3.0 mm transparent corneal incision cataract phacoemulsification and IOL implantation was stable in one month after operation, and recovery to preoperative status. Objective To investigate clinical efficacy of transforaminal approach debridement with fusion, thoracolumbar single segment of Brucella spondylitis pedicle screw fixation (TLIF surgery). Methods We analyzed retrospectively the clinical data of 28 patients with Brucella spondylitis treated in our department between January 2009 and January 2014 with TLIF surgery (Group A) and internal fixation with a simple posterior anterior interbody disease debridement, autogenous bone graft (Group B). The two groups were compared in operation time, blood loss, postoperative ambulation time, hospitalization days, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), American Spinal Injury Association (ASIA) classification, visual analogue scale (VAS), Oswestry Disability Index (ODI), Cobb angle of vertebral bone graft healing, and complications. Results All the patients were followed up for an average of 20.2 months (18 to 27 months). They were all cured. Compared with those in Group B, patients in Group A had shorter operation time (164.60±59.19)min, significantly reduced blood loss (346.00±108.90)mL and complications (1 case); significantly shorter postoperative ambulation time (3.36±1.11 days), hospitalization days (17.36±4.19) days and duration (13.16±3.94) months (P<0.05). The two groups did not significantly differ in VAS scores, ODI, ESR CRP, or Cobb angle (P>0.05). Conclusion On the basis of norms of anti-drug treatment for brucellosis, TLIF surgery on Brucella spondylitis has the advantages including less trauma, shorter operation time, easier operation, less bleeding, earlier postoperative ambulation, and lower complication rate.
