高次脳機能障害は一般的には回復が不良で、社会復帰を果たすのは困難な例が多い。 今回我々は、突然の心室細動発作により心肺停止となり、蘇生後の低酸素脳症により多彩な高次脳機能障害をきたした一例を経験した。洛和会音羽病院での急性期治療の後、当院回復期リハビリテーション病棟へ転院し高次脳機能訓練を中心としたリハビリテーションを開始した。180日入院し、退院後は外来リハビリテーションにて週1回の高次脳機能訓練を継続し、自宅でも家族と共に家事動作や調理動作などの訓練を継続した。発症1年2カ月後には調理師として社会復帰を果たし、2年後には自動車運転も可能となった。入院期間のみならず、退院後も切れ目のないリハビリテーションを長期間継続したことにより、著しい高次脳機能の回復につながったと考えられる。

治療と仕事の両立支援を進めていくうえで，脳卒中患者やその家族，医療機関，事業所それぞれの立場で悩みは異なり，就労には多くの課題がある．両立支援を行う医療機関や事業所に対しては，厚生労働省がガイドラインを公表しているが，脳卒中患者やその家族に対する指針等はなく，脳卒中を発症後，どのように治療と仕事を進めていけば良いか悩む患者が多い．そのため，既に公表されているがん患者を対象とした「仕事とがん治療の両立お役立ちノート」のコンセプトを元に，「脳卒中の治療と仕事の両立お役立ちノート（以下，お役立ちノート）」を作成し公表した．脳卒中の概要，脳卒中を発症直後の時期から復職後の経過に沿った内容で，「誰」と「いつ」「どこで」「何をして」「どのように」関わっていけばよいか，それぞれの病期に合わせて確認できるようにしている．お役立ちノートは，患者や家族また就労支援に関わる人々に働きかけ，脳卒中患者の社会参加を促すなど行動変容につながる情報ツールとなり，就労者が増加し復職率向上に寄与するものと考えられる．When stroke patients consider a return to work, several difficulties are encountered regarding the promotion of support for both their health and employment due to the different perspectives of the patients, their families, and medical institutions. The Japanese Ministry of Health, Labour and Welfare has published a guideline for medical institutions and offices, but not for stroke patients or their families. As a result, patients and their families do not know how to process both medical treatments and their work after the occurrence of a stroke. We have therefore published the “Guidebook for Stroke Patients to Promote Health and Employment Support” based on the concept of the previously published “Guidebook for Cancer Patients for Promoting Health and Employment Support”. This publication offers an overview of stroke and the flow from stroke onset to return to work. Patients can confirm how they should be handled at different phases of stroke. A stroke affects the patient, their family, and the individuals providing support, so this publication offers an information tool to facilitate the participation of stroke patients in society. The guide can thus be expected to contribute to an increased rate of return to work by stroke patients.

In depression, the health condition of family members (particularly spouses) may influence the reinstatement process of employees on temporary leave. The length of leave from work may reflect the quality of the process leading to the stage of reinstatement, and a short period to reinstatement may increase the possibility of a smooth reinstatement. In this study we investigated the relationship between spouse factors and length of leave period. The subjects were 63 permanent employees on temporary leave for depression and their spouses. We investigated demographic characteristics, spouse factors, individual factors, household factors, and workplace factors, and extracted the factors related to the length of leave period through hierarchical multiple regression analysis. Results revealed that the Frenchay Activities Index (FAI), a spouse factor, was consistently and significantly related to the length of leave (β = −0.37, P < 0.01). The higher the spouse FAI, the shorter the leave period of employees on temporary leave. In particular, the spouses’ leisure activities were related to the leave period, whereas the effects of housework and work were unclear. The results suggest that the assessment and intervention of the spouses’ activities of daily living are important in reinstatement support.

高次脳機能障害者の患者数は明らかにされていないが、推計患者数は算出されている。しかしながら、推計患者数に相違があり、未だ明確な数値は表されていない。医療分野では、高次脳機能障害者は行動障害、記憶障害などの症状があり、精神疾患を併発することがある。うつ病等精神疾患を併発した患者は社会復帰が困難であることがわかっており、支援方法の1つとして精神保健福祉援助技術の必要性が検討され、社会生活技能訓練（以下、SST）などによる援助技術が有効と考えられる。今回の研究では高次脳機能障害者患者数の推計を調査し、患者数から経済学的分析を試み、高次脳機能障害者の経済損失額を推計した。本研究では、経済損失額を推計することで高次脳機能障害者への支援を検討し、精神疾患を併発した患者に対する精神保健福祉援助技術の必要性が示唆された。

The conditions for stroke rehabilitation such as individual therapeutic procedure and medical treatment system in Japan have drastically changed over the past decade: increasing incidence of ischemic stroke, the use of intravenous recombinant tissue plasminogen activator, hospital specialization, introduction of convalescent rehabilitation wards, and public long-term care insurance. However, it is not known whether these changes have influenced the time course of return to work (RTW) after stroke. In this study we compared the time course of RTW after stroke in Japan that was reported in two cohort studies performed 20 years apart. The cumulative rate of RTW after first stroke was similar in the two studies, even though they were separated by an interval of two decades. This shows that advances in stroke rehabilitation have not impacted RTW, and we suggest that the social security system, particularly sickness benefit, has a strong influence on RTW.わが国の脳卒中リハビリテーションの状況は，近年，個別治療手技および診療システムの劇的な変化がみられた．すなわち，虚血性脳卒中発症率の増加，静脈注射用組織プラスミノーゲンアクチベーターの使用，病院の機能分化，回復期リハビリテーション病棟の導入，介護保険制度である．しかしながら，これらの変化が脳卒中後の職場復帰（復職）の経過に影響を与えているのかどうかは不明である．本研究では，20年以上隔てられて実施された2つのコホート研究－脳卒中後の復職の経過分析－を比較した．両研究は20年以上離れているが，初発脳卒中患者の累積復職率は両研究でほぼ同様であった．この結果は，脳卒中リハビリテーションの進歩が復職に大きな影響を与えず，むしろ傷病手当金などの社会保障システムが復職に大きな影響を及ぼすことを示唆している．

A総合病院において勤務する看護職員で，２００１年度から２００４年度に産前・産後休暇や育児休業をとり，２００６年１月の時点で職場復帰している看護師１５名が職場復帰時に経験した問題点から，復帰をスムーズに行うために希望する研修について検討した． 看護職員は職場復帰時，休業中に変化のあった病院のコンピューターシステムや，新たに配属された部署における疾患や治療などについて研修を希望していた．そのため長期休業者を抱える施設では，看護職員に対して職場復帰時に看護管理室が中心となる研修と，部署における研修を計画実施し，スムーズに職場復帰できる支援が必要である．, The investigation determined what on-the-job training they desired vis-à-vis the problems they experienced upon their return in order to aid in their smooth return to the workplace. The respondent to this questionnaire survey were fifteen general hospital nurses. They were granted maternity and child care leave between April １, ２００１ and March ３１, ２００５ and returned to their workplace in January, ２００６. Those nurses desired training relating to changes made to computer systems during their leaves, information and on ailments and the associated treatments relevant to their newly assigned posts. Therefore, hospitals with such personnel may find it advantageous to have the nurses’ administration office conduct training, and plan and conduct such training specific to each post for their nursing staff when their granted leave is ended and they wish to return their former workplace in order to enable the smooth return of such staff.
