これまでの研究や政策立案では、薬物依存の課題を抱える刑事施設等出所者を対象とした地域生活定着支援センターにおける支援の現状と課題については明らかにされていない。そこで、本稿では、地域生活定着支援センター職員に対するインタビュー調査をまとめることによって、薬物依存の課題を抱える刑事施設等出所者の地域支援に関する課題を明らかにする。インタビュー調査の結果、地域生活定着支援センターにおいて、薬物依存の課題を抱える刑事施設等出所者の支援においては、その対象者をいかに「生活者」として把握していけるかが鍵を握ることが明らかになった。実践的な課題として、地域生活定着支援センターと精神保健福祉センターの連携が、学術的な課題として、地域生活定着支援センターを利用し、社会福祉サービスの活用によって地域生活を営んでいる当事者の事例の縦断的な検討が明らかになった。

本稿の目的と関心は，全国的にも高い評価を受ける北海道帯広・十勝圏域の精神保健医療福祉実践を概観するとともに，その歴史的事項の整理を行いながらわが国の精神保健福祉士のあり様について試論を述べることにある。この実践を検討する際には，諸家が「オープンシステム」を評価していることに注目するべきである。それは，「どこの病院の患者であろうと利用できる。誰が創ろうと地域に創られた資源は，医療機関も同様に住民が自由に活用できることが当たり前であり，社会資源は社会の共有資源であることを基本として相互に利用しあうこととなった」というものである。諸家の記録を検討すると，これらの実践では，当事者が置かれている環境へのきわめて素朴な疑問から関係者が様々な連携・協働の下にシステムを構築してきたことが明らかであった。この点の重要性を精神保健福祉士は再認識すべきであると考えられた。, This study has the objective and interest of outlining mental health welfare practices in Obihiro and Tokachi districts of Hokkaido, which are highly esteemed nationwide, with presentation of the author’s opinions of how psychiatric welfare workers should fulfill their duties by summarizing related historical information. In examining this practice, it is noteworthy that many experts appreciate the “open system.” According to the system, “Any patient of any hospital can use it. No matter who creates it, the resource created at a community, including healthcare facilities, should be used naturally and freely by residents. Therefore, social resources have been used communally on the premise that they should be shared as social resources.” Examination of records by the experts revealed that the people involved have created a system with various cooperation and collaboration from simple problem consciousness they have of the environment in which patients are placed. The author infers that psychiatric social workers should recognize the importance of this point.

The purpose of this study was to explore and make it clear the current trends and issues of home-basedrehabilitation（HBR）that the rehabilitation therapists practicing in the field. Investigators carried out semi-structuredinterviews targeting the four reha-therapists who provided HBR services．Interview of about 30 minutes per person wasperformed and created the verbatim records．Two researchers examined categories and their relevance by conducting thecoding through discussion based on the verbatim records．As a result, ① users are passive, and have needs such as to bereleased from the body functional training and pain．This makes a contrast against the user image which reha-therapists haveexpected．And this is causing a gap between the support and needs．② Therapists try to get an understanding of the user，but it is difficult．③ Awareness of users are born from their experience at hospital and lack of well-recognized method by caremanager．④ Reha-therapists are lobbying for care manager to let users understand the service．But it is not really workingand still is an issue．⑤ Understanding for HBR is different by care manager, and it has become clear that there remains aproblem in the well-known method to the users．In the background of this structure, the stage of rehabilitation is converted tolife model support model from a medical model is due to transition from hospital to home, it is suggested that it is affected．

医師不足・偏在への対応，病院勤務医の業務内容の負担増の影響を受けて，看護への期待が医療面への拡大に向かっている。重ねて，その影響を受け，国立病院機構病院に勤務する療養介助員の業務内容が，看護の領域にシフトしている。 本研究におけるアンケート調査の結果を見ると，現在の仕事内容が介護の専門性だと思い働いている療養介助員は，わずか18％であった。さらに，保有資格の差異により療養介助員間でも不満の声を聞いた。 病院では，協働するケアチームのメンバー（専門家）が，同じ場所にいて集まりやすく，患者の状態の改善や悪化防止がトータルな情報下で理想的にサポートできる。しかし，それぞれの専門分野の充実への適正評価は低い。 患者に良質な医療（技術）・（生活)介護を継続的に提供していくためには，医学モデル注１から生活モデル注２への転換が早急に望まれる。さらに，介護学では，日々展開される患者の日常生活の現実を重視することが実行されなければ患者の主体性は尊重できないという考え方である。すなわち，この場面において専門性を発揮できる職の領域が「介護」である。病院という場所だから介護の専門性が発揮できないのではなく，発揮できる最も近い場所に療養介助員たちはいるのである。

本稿は、地域を基盤としたソーシャルワーク実践展開に向けて、ソーシャルワーク理論モデル・アプローチを活用する際の課題について探ることを目的とする。2006～2010年度の専門職大学院の授業で受講生がまとめた131事例を取り上げる。事例の考察に用いられた主なアプローチは、役割理論36、コミュニケーション理論30、課題中心アプローチ29、自我心理学26、危機介入アプローチ7、他である。事例の支援計画の内容を分析した結果、29事例の支援標的の設定が従来のアプローチの活用と異なる、14事例が治療モデルを志向していると捉えられた。これには実践者のおかれている状況が関連しており、従来のソーシャルワーク理論モデル・アプローチはその点を考慮して示していないという問題がある。地域を基盤としたソーシャルワーク実践展開においては、連携協働の中で求められる機能、予防的視点を考慮することが課題である。, This article reports on some challenges of the applicability about personal support theories of social work in the community based practice. We investigated 131 case reports that students presented in the class of the Professional Graduate School in 2006~2010. The personal support theories used for case study are Role theory 36, Communication theory 30, Task-centered approach 29, Ego psychology 26, Crisis intervention approach 7, and so on. As a result of having analyzed the contents of their support plans, the support targets of 29 case reports disagree with the conventional approach and 14 case reports base on medical model. The practitioners’ situation is related to this result, there is the issue as the conventional personal support theories do not regard to the practice situation. There are the challenges to use the personal support theories as regards to their collaboration and a preventive perspective in the community based practice.

背景 戦後の核家族化を契機に家庭や地域での子育てが大きく変容し、子育てや子育て支援に様々な問題が生じている。深刻化する一方の少子化や、増加の一途をたどる児童虐待をみると、既存の子育て支援方策は有効といえない状況にある。これまでの子育て支援策は出産後の子育てに重点をおいているため、十分な効果を見ないのが現状であり、新たな視点からの支援策が求められている。目的・方法 ここでは子育てを妊娠時から始まる妊娠期、出産、産後の子育てを産み育てという一連の営みであると捉え、先行研究の分析から新たな子育て支援システムの構築基盤を考察する。結果・考察 地域社会や家族が変容した現状や、高齢者介護保険の効果などを踏まえると、産み育てる母親の支援は家庭内では限界があり、家族を越えた支援ネットワークの構築が必要であり、それを可能にするには助産師やドゥーラなどの存在が重要とされる。結論 産み育て支援には、母親の生活環境全般を視野に入れて、家庭内外の支援者を包含した、妊娠期から出産後の育児まで含んだ総合的、かつ連続的な対応、すなわち「生活モデル」の導入が必要である。

本稿は，介護保険制度の施行を契機として導入された介護支援専門員（通称ケアマネジャー）の専門技法であるケアマネジメントに関して考察することを目的としている。新たな専門職は，その技法が十分に習熟しないまま介護現場に投入されたことで，介護保険制度と利用者ニーズとの狭間で動揺している様相がある。介護保険版ケアマネジメントであるという批判もあるが，しかし，ケアマネジメントが日本に本格的に導入され，一つの専門職が誕生した意義と課題は看過してはならない。現状を分析しつつ課題を整理することで，ケアマネジメントの意義を再確認し，これを介護支援専門員の技法として確立していくための視点を考察する。

ケアマネジメント技法に関して、我が国は介護保険制度導入にあたり、欧米のケアマネジメント技法を応用しつつ、展開している。障害者福祉制度も支援費制度に移行するなかで、ケアマネジメント技法の導入が行われたが、障害者ケアマネジメントにおいてはエンパワメント等の特徴を踏まえて展開しようとしている。そのような障害者ケアマネジメントにおける特徴について各論を分析し、そこにおける理論的追究を試みたものである。
