8. 基于弥散张量成像的抑郁症生物学亚型研究.

- Abstract
Background Being complex and highly heterogeneous with regard to the etiology and clinical manifestations of depression, neuroimaging studies make a breakthrough for exploring the biological subtypes of depression, while the current datadriven approach for the identification of subtyping depression using structural magnetic resonance imaging(MRI) data is insufficient.Objective To explore the biological subtypes of depression using diffusion tensor imaging(DTI) and machine learning methods.Methods A total of 127 patients with depression who attended Beijing Anding Hospital from September 2017 to August 2021 and met the Diagnostic and Statistical Manual of Mental Disorders, fourth edition(DSM-IV) diagnostic criteria were included, and another 80 healthy individuals matched for gender and age were recruited through advertisements in surrounding communities during the same period. DTI findings, demographic characteristics and clinical data were collected from all participants. Tract-based spatial statistics(TBSS) and the Johns Hopkins University(JHU) white matter probability maps were used to extract fractional anisotropy(FA) values of white matter tracts. A semi-supervised machine learning technique was used to identify the subtypes, and the FA values for whole brain white matter of patients and controls were compared.Results Patients with depression were classified into two biological subtypes. FA values in multiple tracts including corpus callosum and corona radiata of subtype I patients were smaller than those of healthy controls(P<0. 01, FDR corrected), and FA values in middle cerebellar peduncle, left superior cerebellar peduncle and left cerebral peduncle of subtype II patients were larger than those of healthy controls(P<0. 01, FDR-corrected). Baseline Hamilton Depression Scale-17 item(HAMD-17) score yielded no statistical difference between subtype I and subtype II patients(P>0. 05), while subtype I patients scored lower on HAMD-17 than subtype II patients after 12 weeks of treatment(t=2. 410, P<0. 05).Conclusion Depression patients exhibit two biological subtypes with distinct patterns of white matter damage. [Objective] The present paper aimed to promote the more effective transformation of agricultural scientific and technological a- chievements, it is necessary to build a stable transformation model and a scientific and reasonable profit distribution mechanism. [Method] The paper summarized the typical transformation models through field research on the relevant subjects involved in the transformation of agri- cultural scientific and technological achievements in Sichuan province. Considering the risk-taking, technological innovation and cooperation degree of participants, the paper constructed a benefit distribution model for the transformation of agricultural scientific and technological a- chievements based on the modified Shapley value method, and made a theoretical and empirical analysis on the benefit distribution mecha- nism of each subject in the transformation of agricultural scientific and technological achievements. [Result】(i) Government, research insti- tute, agricultural enterprises, dealers/retailers, village collective economic organizations, cooperatives/family farms/farmers have formed the transformation models of public welfare, operation and poverty alleviation. Resource sharing, complementary advantages, and coordinated development among cooperative entities in different models were conducive to reducing transaction costs, improving the efficiency of profes- sional division, realizing economies of scale and scope, and creating higher overall returns and individual benefits. (ii) Constructing a trans- formation model of agricultural scientific and technological achievements led by agricultural enterprises formed a higher efficiency of scientific and technological resource allocation and promoted the transformation of agricultural scientific and technological achievements. (iii) The re- vised benefit distribution method was more realistic and conducive to the long-term stability of cooperation due to the inclusion of the influ- ence of risk-taking, technological innovation and cooperation degree of participants. 【Conclusion】 By increasing policy support for agricultur-al enterprises leading scientific and technological innovation, enhancing the synergy of cooperative innovation among different subjects, and establishing a scientific and reasonable benefit distribution mechanism, it is conducive to promoting the effective transformation of agricultural scientific and technological achievements. 12. 基于绿地微气候的长沙太平街历史文化街区 热环境数值模拟.

- Abstract
This study used the historical and cultural block of Taiping Street in Changsha as the research object. First the current microclimate environment of the street space was analyzed, then numerical simulations based on ENVI-met software were performed, which compared the microclimate numerical simulation results of three typical greening optimization schemes of the street space in the study area and the benchmark scheme, as well as explained the degree of thermal environment control of the greening optimization scheme. This study is expected to provide a reference for the reconstruction of urban historical blocks as well as optimization and control of the thermal environment. The results showed that: 1) With an hourly change in solar radiation, the thermal environment changed considerably, and the horizontal spatial distribution was characterized by "high temperature and low humidity" and "low temperature and high humidity." Thermal comfort is related to the underlying surface conditions, wind environment, building layout, and other factors. The low temperature and high humidity of the area with relatively concentrated green space along street spaces had a slow-release effect on the thermal environment. The ventilation effect of the area with the dense building layout was poor, and the thermal radiation of the underlying surface was high, affecting the heat dissipation and humidification in the research area. 2) The enclosed space layout of buildings in the study area caused building shadows and poor ventilation to affect the green space, which was not conducive for cooling and humidification. 3) Three types of greening transformation schemes were proposed for the street spaces of three different architectural layouts in the historical block of Taiping Street. The three kinds of street space green space optimization and benchmark schemes (the current street space situation) were simulated using ENVI-met microclimate simulation software. Optimization results showed that the same greening optimization scheme had different regulatory effects on the thermal environment in different building layouts. The three kinds of street spaces significantly reduced the average radiation temperature of the block by increasing the greening area of the block. The optimal regulatory effect of the thermal environment of the tree-shrub+three-dimensional (water) compound green space type was better than that of the tree-shrub and tree-grass scheme in the daytime. Under a certain green space ratio, the area of shade generated by the uniform and decentralized layout of green space in a commercial space covered the largest range of street space. The order of the degree of thermal environment optimization and control was decentralized>parallel> centralized. The residential and square spaces were affected by the architectural layout, and the green space layout should have good ventilation, cooling, and humidification effects; therefore, the maximum degree of thermal environment optimization and control is the centralized layout. 17. Galectin-9阳性肿瘤相关巨噬细胞在肌层浸润性膀胱癌中 的表型、功能及临床治疗意义

- Abstract
Objective-To explore the phenotype of galectin-9+ tumor-associated macrophages (galectin-9+TAMs) in muscle-invasive bladder cancer (MIBC). To clarify the regulation of galectin-9+TAMs in MIBC microenvironment, and elucidate the mechanism of galectin-9+TAMs inhibiting the effector function of CD8+T cells and its clinical therapeutic significance in MIBC. Methods • Phenotype of galectin-9+TAMs from MIBC peritumor and tumor tissues was detected by flow cytometry. TCGA (The Cancer Genome Atlas) database was used to sort out the cytokines most relevant to LGALS9 and LGALS9 macrophage gene set. Macrophages were stimulated by recombinant human cytokines in vitro, divided into recombinant human macrophage stimulating factor (rhM-CSF) stimulation group, recombinant human interleukin-16 (rhIL-16) stimulation group and recombinant human interferon- y (rhIFN-丫)stimulation group. Expression level of galectin-9 among the three groups was verified by flow cytometry. Expression level of galectin-9 on macrophages was detected by flow cytometry after adding M-CSF neutralizing antibody. Effector functions of TAMs were detected by flow cytometry after treating MIBC single cell suspension with anti-galectin-9 antibody. Galectin-9+TAMs from peritumor and tumor tissues, and human peripheral blood CD8+T cells were sorted and co-cultured in vitro. Effector functions of CD8+T cells were detected by flow cytometry. Tumor tissues cultured in vitro were treated with anti-galectin-9 antibody and programmed cell death protein 1 (PD-1) antibody alone or in combination. Tumor cell apoptosis and effector function of CD8+T cells were detected by flow cytometry. Results • Galectin-9+TAMs exhibited the phenotype with high expression of human leukocyte antigen DR (HLA-DR), CD86, CD206 and programmed cell death-ligand 1 (PD-L1). Increased IL-10 and transforming growth factor- p (TGF-卩)and decreased tumor necrosis factor- a (TNF-a) were secreted by itself. M-CSF, IL-16 and IFN-y showed the significant difference with LGALS9 and LGALS9 macrophage gene set in TCGA database. Galectin-9 on macrophages increased significantly in rhM-CSF stimulated group and its expression decreased by adding neutralizing antibody. Galectin-9 inhibition switched the activation of TAMs from an immunosuppressive phenotype to a more inflammatory state and PD- L1 on its surface significantly decreased. After cultured in vitro, galectin-9+TAMs inhibited the effect of CD8+T cells in a partly galectin-9-dependent manner. Compared with applying PD-1 inhibitor alone, percentage of tumor cell apoptosis, the proliferation of CD8+T cells and its effector molecules were significantly enhanced or increased in both galectin-9 and PD-1 blockade. Conclusion • Galectin-9+TAMs exhibit an immunosuppressive phenotype and function. Tumor-derived M-CSF induced TAMs to express galectin- 9. Galectin-9+TAMs inhibit the function of CD8+T cells to promote the immune escape of MIBC. Galectin-9 and PD-1 blockade can reactivate the function of CD8+T cells more effectively and synergistically. OBJECTIVE: Locking plate is the most commonly used method for the treatment of middle clavicle fracture at present. Ortho-Bridge System is a new type of internal fixation device, which has not been used for a long time. However, many domestic scholars have published many clinical studies to compare the efficacy of the two, but there is no unified opinion. This study compared the efficacy of Ortho-Bridge System and locking plate in the treatment of middle clavicle fracture by meta-analysis. METHODS: A computer-based online search was conducted in PubMed, Medline, The Cochrane Library, MSTC FMRS foreign medical information platform, Ovid Medical Journal Full-text Database, VIP, CNKI, Wanfang digital journal and other platforms to collect the randomized controlled trials published from January 2010 to April 2020 on the treatment of middle clavicular fracture with the combination of Ortho-Bridge System and locking plate. According to the inclusion and exclusion criteria, Jadad scale and MINORS scale were used to evaluate the quality of the included articles. After quality evaluation and data extraction, the operation time, intraoperative blood loss, fracture healing time, complications and 1-year postoperative shoulder joint function score were selected as the evaluation indicators for meta-analysis. The results of the included studies were meta analyzed by RevMan 5.2 software provided by Cochrane Collaboration Network. RESULTS: (1) A total of nine Chinese and foreign articles were included in this study, including three randomized controlled trials and six non randomized controlled trials. A total of 613 patients with middle clavicle fractures were included, including 280 cases in the Ortho-Bridge System group and 333 cases in the locking plate group. (2) Meta-analysis results showed that there was no significant difference in postoperative shoulder function (MD=3.23, 95%CI:0.13- 6.33, P=0.04). However, the Ortho-Bridge System group was better than the locking plate group in operation time (MD=-4.62, 95%CI:-9.08 to -0.16, P=0.04), intraoperative blood loss (MD=-15.29, 95%CI:-15.81 to -14.47, P < 0.000 01), fracture healing time (MD=-2.88, 95%CI:-3.10 to -2.65, P < 0.000 01), and overall complications (OR=0.21, 95%CI:0.09-0.48, P=0.000 2). CONCLUSION: Ortho-Bridge System and locking plate can achieve the expected results in the treatment of mid clavicle fracture. There was no significant difference in shoulder function between the two methods. In terms of operation time, intraoperative blood loss, fracture healing time and overall complications, Ortho-Bridge System has certain advantages over locking plate. However, due to the influence of methodological quality of this study, the results need to be treated with caution, and more large sample and multi center randomized controlled trials are needed to confirm.
