看護学部2年生及び学士3年生を対象とした実習の一環として，実習前自己学習にて，病棟実習を想定したシミュレーション演習を行った。シミュレーション演習内容は，事前に学部2年生に希望調査を行い，また上級生のラーニングアシスタント作成による病棟で遭遇する事例集を活用した。自己学習終了時のアンケート結果より，参加者全員より「参加してよかった」という回答が得られた。また，演習内容に関しても，実習に向けた準備性が高まったとの回答が99％であり，「復習できた」，「新たな学びが得られた」といった評価が得られた。ウィズコロナ時代に向け，看護学生の看護実践能力育成に，更にシミュレーション演習が必要とされることから，今回の結果を踏まえ臨床実習をより想定できるよう内容を改善する必要があると考える。, A simulation practice was conducted assuming hospital ward practice in self-study before practical training for second-year nursing students and third-year undergraduate students. A survey of second-year undergraduate students regarding the content of the simulation exercise was conducted in advance, and a collection of cases encountered in the hospital ward was created employing a learning assistant. Results of the questionnaire at the end of the self-study showed that all participants were pleased to participate in the program. Regarding the contents of the exercises, 99% of the respondents answered that they were more prepared for the practical training, and the participants responded that they were able to review the contents and that they could learn something new. During the COVID-19 era, when simulation exercises are required to develop the practical skills of nursing students, we appraise that it is necessary to improve the contents so that clinical training can be envisioned more based on the results of this study.

看護専門科目（必修）としての「援助的人間関係論」の授業に，SP（模擬患者）参加型演習を取り入れて行った．本科目を履修した学生のうち調査に同意の得られた34名（42％）と，授業に参加したSP 7名（100％）を対象に質問紙による調査を行ったところ，本科目全体について学生は，29名の学生（85％）が「とても」あるいは「まあまあ有意義であった」と感じていることがわかった．また，SP 参加型演習に対する取り組みに関しての学生の自己評価も，34名中33名の学生が，「関心を持って取り組むことができた」「まあまあ関心を持って取り組むことができた」と自己評価しており，態度の変容を期待させた．授業運営に関しては，①多数の学生が見ているところで演じることによる緊張の低減，②より多くの学生がSPとのセッションを体験できるような授業運営の工夫，③学習を促進するためのよりリアルな設定，④シナリオの工夫，⑤コミュニケーションスキル練習の強化，⑥ SP との関係づくりの充実，⑦コミュニケーションのプロセスが学習しやすいレポートの書式の工夫，⑧ SP と学生のセッションにおけるファシリテータとしての役割を含む教員の教育能力の改善，これらの課題が明らかになった．

成人看護学における看護実践能力の育成にむけて、急性期実習前の学内演習プログラムを新たに作成し、その有効性を明らかにすることを目的として、研究への協力が得られた３年生45名を対象に演習プログラムの実施および調査票を用いての評価を行った。 学内演習プログラムの実施時期や時間、演習項目、演習方法に対する評価は概ね高かった。周手術期の演習項目の到達度評価では、術直後の覚醒状態、循環状態、痛みのアセスメントについては比較的自己評価が高かった。一方、各種ライン挿入中の体位変換の実施と酸素療法の実施については自己評価が低い傾向がみられた。重症患者の演習項目の到達度評価では、酸素飽和度の測定、意識レベルのアセスメント、認知領域の学習目標の到達度評価では、人工呼吸器管理は一部の項目を除いて比較的自己評価が高かった。一方到達度評価が低かったものは、モデル人形への口腔内・鼻腔内吸引の実施、モデル人形への点滴静脈内注射の実施であった。 演習の学習目標の到達度評価が低かった項目には、実習中に経験することが難しかったり、高度な学習内容を含んでいたり、習得にはトレーニンを重ねる必要がある項目が含まれており、学生にとってイメージしやすいような工夫やトレーニングのための機会を増やすことの必要性が示唆された。

成人看護学における看護技術教育の方法・内容と成果を明らかにし、本学成人看護学における看護技術教育の内容・方法への導入を検討することを目的に、研究論文のレビューと分析を行った。キーワードを「技術教育」、「演習」、「学内実習]、「成人看護」とし、53文献を検討した。その結果、1)演習内容は、看護過程を用いた演習13件であり、その多くは実技演習を組み合わせている。実技のみを行う演習では、看護過程併用に比べて実施する実技項目が多かった。2)学生による自己評価のみで演習の目標を評価している文献が多く、測定用具を用いているものもあったが、総数は少ないことが明らかとなった。看護技術教育については、本看護学部全体で卒業時の到達内容を検討する必要があり、看護過程を併用した実技演習と紙上事例での実技演習とを、実習環境等を考慮して適応させる必要がある。看護技術教育の方法の妥当性を評価するには、学生と教員がともに技術の評価を行う必要があると考えられた。

SPを導入したコミュニケーション演習を演習方法の改善を試みながら平成16年度〜平成18年度の1年次生を対象に実施した。本研究では、学生に対する3年間の演習の企画・運営とSP評価に関する調査および平成17年度と平成18年度におけるコミュニケーション技術の習得度に関する質問紙調査を行い、今後の課題について検討した。その結果、演習の企画・運営に対する評価では、病室環境をいかに設定するか、シナリオの工夫の必要があると考えられた。コミュニケーション技術の到達度では、非言語的コミュニケーションの活用や沈黙の意味を理解し的確に対応することが低得点であった。看護を行うための問題解決や看護援助に関しては、授業や演習、実習などの進行に伴う段階的な評価基準作成の必要性が示唆された。, Improving a Nursing Communication Training Course and Its Effect on Learning Using Simulated Patients A nursing communication training course using simulated patients (SP) was conducted for first-year students from 2004 to 2006. The purpose was to evaluate the effectiveness of the sessions held during a regular class, and feedback from the students was used in making improvements to the course. Information was gathered from student evaluations and assessing their levels of attainment in communication skills was also examined. As a result, the students' evaluations suggested that improvements in the practice environment and intelligibility of the scenario were necessary. Student ability in decoding nonverbal communication, including silence, was also difficult. The results suggested that it is necessary to develop phased criteria to evaluate problem solving and nursing interventions as students advance in learning through classwork and clinical practice.

本研究は、看護学生がコミュニケーション技術を学習するのに効果的な演習を企画するための授業評価を行い、授業の改善に資することが目的である。演習にはSimulated Patient (模擬患者:以下SP) を導入し、看護学科の1年次生を対象に2005年度から学生全員がセッションで看護学生役を体験できるようにプログラムし、演習終了後に調査を実施した。その結果、演習の企画・運営とSPに対する評価は、物理的環境とシナリオのイメージのしやすさを除いて2004年度と同様にどの項目も高値を示した。物理的環境は騒音対策が未解決で、シナリオのイメージのしやすさでは2004年度の場面設定に関する問題は解決したが、新たな問題として学生のレディネスに沿って詳細なシナリオの検討が課題となった。さらにファシリテータの役割が有効に機能しているか不明であること、また学習の特徴として看護学生役である私とSP、看護学生役と観察役である私という二人称の関係からの学び、の傾向が明確になった。, The purpose of the study was to identify the challenges for building improved plans to teach communication skills to first-year nursing students. The teaching program of 2005 was designed based on the implementation and evaluation in 2004, so that every student could play the role of nurse in student-simulated patient situations. Scenarios used were also visualized by adding some details. The students completed a questionnaire on program management, the use of a simulated patient role-playing and the understandability of the scenarios. As a result, scores were high in planning and management of the program and use of simulated patients, as in 2004, but not in the physical environment or the quality of the scenarios. It was impossible to determine whether the facilitators played a successful role during simulation. It seemed that the group representatives had failed to share their learning with their group members. The findings suggest that the physical environment needs more noise control, and that the scenarios should be more appropriate for students.

手洗い法は院内感染予防において、もっとも基本的であり、看護教育に不可欠である。本学カリキュラムの場合、基礎看護学の中で一過性菌の除去を目的とした「衛生的手洗い法」と、成人保健看護領域の周手術期看護で、高水準な手洗い法として「手術時手洗い」の演習を行っている。成人保健看護領域の「手術時手洗い」演習において、本学３年生82名は減菌効果を確認するためにぺたんチェックスタンプ法を用いて実験を行った。その結果をもとに、「手術時手洗い」の演習方法と結果の評価について検討した。また学生の演習後のレポートからぺたんチェックスタンプ法を用いた学習の波及効果を捉える試みをした。学生82名全員の「手術時手洗い」前後の細菌数を局所的・半定量的に比較した結果は、右手５本の指で明らかに減菌しており、手洗い直後の減菌効果を評価できた。また「手術時手洗い」後における残菌部位は、「衛生的手洗い法」での洗い残しの多い部位と一致していた。演習記録内容は、手指付着菌に関する表現、洗い方・技術に関する表現、手指の状態に関する表現、患者に与える影響に関する表現、「手術時手洗い」の効果および消毒効果に関する表現等をキーワードから捉えることができた。現在の臨床現場は、「手術時手洗い」に関する様々な方法や学説が混在し、一定の手洗い方式が用いられているわけではない。当学内では「手術時手洗い」を学ばせるためにぺたんチェックスタンプ法による評価を取入れているが、手指付着菌を確認し、減菌する方法を理解するために有効であるといえる。また医療現場の現状を踏まえてレベルの違う手洗い法を学習することは、科学的な裏付けをもって、感染防止の基本原則を学ぶために有用であると考える。To learn the hands washing technique for nurse student is important to prevent the spread of infections, and educational approach is also important to have the students understand it well. To evaluate the educational approach of the surgical hand scrub in our college curriculum, we used the market products "Petan check stamp method" by comparing the bacterial corny counts on fingers before and after the surgical hand scrub. Eighty-two students were stamped their fingers on Petan check before and after hands washing, and the stamp was incubated for overnight. Student counted microbiological colony on the stamp, and reported it. After washing hands, a number of colonies on the right hand fingers of them were reduced remarkably, and they could estimate clearly the effect of the surgical hand scrub. The wash less parts by surgical hand scrub was same position in the hygienic hand washing. The student could understand 5 classified of accomplish level in this educational approach. These results indicate that Petan check stamp method is useful tool to educate the hand washing technique for the student.
