目的探讨髋关节置换术前后股骨假体周围骨密度(BMD)变化对患者近期疗效的评估作用。方法选择采用非骨水泥生物型股骨假体行髋关节置换术患者121例,术前Harris髋关节功能评分47~62(53.48±6.82)分。根据术后1年Harris髋关节功能评分评价疗效(≥90分为优,80~89分为良,70~79分为中,<70分为差),分为疗效优良组84例(≥80分)、疗效中差组37例(<80分)。采用双能X线骨密度仪检测两组术前及术后1年股骨假体周围各Gruen-Amstutz分区(R1~R7)的BMD,术后BMD升高为骨量升高,降低为骨量降低,计算骨量变化率。结果两组手术前后R2~R6 BMD比较均无统计学差异(P均>0.05);疗效优良组手术前后R1 BMD比较无统计学差异,疗效中差组术后R1 BMD明显高于术前及疗效优良组术后(P均<0.05);与术前比较,两组术后R7 BMD均降低,但疗效中差组组降低明显(P均<0.05)。疗效优良组R1、R5、R6骨量升高率及R2、R3、R4、R7骨量降低率均低于疗效中差组(P均<0.05)。结论髋关节置换术前后股骨假体周围BMD变化越小,患者的近期疗效越好。 [ABSTRACT FROM AUTHOR]
