针对苯系物泄露次生突发环境事件的应急响应问题，研究了基于案例推理和处置效果的应急方案生成方法。首先，在计算案例相似度时，为避免历史案例不全产生的情景要素遗失，基于随机森林算法，结合专家知识构建了权重求解方法，采用案例检索方法得到相似案例集；然后，构建了应急处置效果评估指标体系，以全面评价相似案例对应的处置效果；进一步，考虑多专家评价可弥补决策者知识、经验等的不足，引入了自适应群共识调整算法，得到群体评价意见，通过集结相似度与处置效果，得到了方案的综合评价值并生成最优方案；最后，用算例说明了方法的可行性和有效性。

This study is aimed to evaluate the feasibility of replacing alfalfa with tea residue through assessing the rumen fermentation parameters using gas production in vitro. Four goats were used as the donor of rumen fluid. The ANKOM RFS gas production measurement system was used to determine the in vitro gas production, rumen fermentation parameters, and cellulase activity of alfalfa and tea residue. The results showed that: ① alfalfa has a higher gas production than tea residue at 12 h, 24 h, 48 h, potential gas production and a higher slow part of gas production (P<0.05). There are no significant differences in the gas production rate, fast part of gas production between two groups (P>0.05). ② The pH of tea residue was lower compared with alfalfa (P<0.05). Alfalfa had a reduced dry matter degradation level and had an increased NH3-N level relative to tea residue (P<0.05), and the DMO, OMD and ME of alfafa group is significantly higher than the tea residue group (P<0.05). ③ There were no significant differences in total volatile fatty acids, acetate, methane production and acetate/propionate ratio between alfalfa and tea residue (P>0.05). The levels of propionate, isobutyrate, butyrate, isovalerate, and valerate in the alfalfa was higher than tea residue (P<0.05). ④ No significant difference in carboxymethyl cellulase and β-glucosidase cellulase activity occurred for alfalfa and tea residue (P>0.05). However, the xylanase activity of alfalfa was higher than tea residue (P<0.05). It can be speculated that tea residue can be used as feed for goats, but alfalfa has a priority of rumen fermentation parameters over tea residue and thus tea residue can not completely replace alfalfa. Objective To summarize the clinical use of palbociclib and evaluate its efficacy and safety in hormone-receptor (HR)-positive advanced breast cancer patients. Methods We retrospectively analyzed data from 66 HR-positive metastatic breast cancer patients treated with palbociclib and endocrine therapy at the Department of Oncology in the First Affiliated Hospital with Nanjing Medical University between 2018 and 2020. We evaluated the factors affecting the efficacy of palbociclib using Kaplan-Meier method and Log-rank test for survival analysis and Cox regressions for multivariate analysis. Nomogram model was built for predicting prognosis among HR-positive breast cancer patients who received palbociclib. Concordance index (C-index) and calibration curve were used for internal validation to assess the predictive ability and conformity of the model. Results Of the 66 patients treated with palbociclib, 33.3%(22), 42.4%(28) and 24.2%(16) patients were treated without endocrine therapy, first-line endocrine therapy, second-line or above endocrine therapy after recurrence, respectively. 36.4%(24) patients had hepatic metastasis, 16.7% (11) patients were sensitive to previous endocrine therapy, 27.3%(18/66) patients had primary resistance to endocrine therapy, while 56.1% (37) patients had secondary resistance to endocrine therapy. The overall response rate was 14.3% (95% CI: 6.7%, 25.4%) and clinical benefit rate was 58.7% (95% CI: 45.6%, 71.0%). Better clinical outcomes were associated with non-hepatic metastasis (P=0.001), sensitive/secondary resistant to previous endocrine therapy (P=0.004), no or only one line of chemotherapy for metastatic breast cancer (P=0.004), recent pathological confirmation of immunohistochemical analysis (P=0.025). Hepatic metastasis (P=0.005) and primary resistance to endocrine therapy (P=0.016) were the independent risk factors of progression free survival. The C-index of predictive probability for the nomogram constructed from the patient clinical characteristics (whether liver metastasis, whether primary endocrine resistance, lines of chemotherapy after metastasis, lines of endocrine therapy, number of metastatic sites, and time to last immunohistochemistry) to predict the progression-free survival at 6 and 12 months for patients was 69.7% and 72.1%, respectively. The most common adverse events were hematologic toxicities. Conclusions Our report indicates that palbociclib combined with endocrine therapy for HR-positive recurrent metastatic breast cancer is effective and safe; patients with hepatic metastases and primary resistance to endocrine therapy have worse prognoses and are independent risk factors for progression after palbociclib therapy. With the development of emergency and critical care medicine, the patients with prolonged disorders of consciousness (pDOC) after brain injury are increasing year by year. Accurate assessment of patients' level of consciousness has become an important part of diagnosis and treatment. As an event-related potential (ERP) independent of patients' active attention, mismatch negativity (MMN) has a broad application prospect in the field of consciousness disorders. Based on existing research about pDOC and MMN, the classification and diagnosis of pDOC, and generation mechanism, result interpretation, clinical application of MMN. Objective To analyze the status of clinical trials registration in disorders of consciousness (DOC) in recent 5 years and to provide reference and evidence support for clinical research of DOC. Methods The clinical trial data of DOC in recent five years through the ClinicalTrials. gov and Chinese Clinical Trial Registry (ChiCTR) registration platform were collected and analyzed. The methods of bibliometrics and contrast were adopted to analyze the status of clinical trials in consciousness disorder in recent five years in DOC from registr-ation quantities, type, contents, sponsors and other aspects. Results There were 131 records of clinical trial registrations of DOC in the world. 57 were done in China, which rated number one in the world, followed by France, the United States and Belgium, with 20 items, 13 items and 11 items respectively. There were 52 randomized controlled trials of DOC all over the world, 26 were in China, and there were 2 multi-regional clinical trial of DOC, Participating countries include Germany, Belgium, the United States, Ireland and the United Kingdom. The research contents of clinical trials on global DOC in recent five years mainly focus on the diagnosis, prognosis and treatment, while mainly on the prognosis and treatment in China. There were 59 sponsors all over the world, 33 were in China. The top 10 sponsors were mainly research centres, university and medical institutions, such as Liege University of Belgium, Hangzhou Normal University, the First Affiliated Hospital of Zhejiang University, the General Hospital of Massachusetts (MGH) and Hospices Civils de Lyon, etc. Conclusions From the above analysis, it is found that in terms of research methods, DOC clinical trials mainly focus on behavior, including Coma Recovery Scale Revised (CRS-R) and Nociception Coma Scale Revised (NCS-R), electrophysiology, functional imaging, neuromodulation technology and drug therapy, the research content is mainly to improve the diagnostic accuracy, detect consciousness, recover consciousness and evaluate prognosis. Some new directions such as olfaction, music therapy, animal assist therapy, genetics, biological circadian rhythms, metabolic spectrum and intestinal microbiota are also emerging. Domestic research closely follows the international development and provides clues for our research. 目的 探讨健康教育路径表结合云随访平台对食管癌围手术期患者的影响. 方法 选取 2019 年 1-11月在厦门大学附属中山医院接受手术治疗的 90 例食管癌患者,将其随机等分为观察组和对照组,对 照组采用传统健康教育模式对食管癌患者进行护理,观察组采用健康教育路径表结合云随访平台的模式对食 管癌患者进行护理,比较两组患者对健康知识和技能掌握情况, 并发症发生情况、恢复速度(下床活动时间, 肛 门排气时间, 出院时间)和护理服务满意度. 结果 观察组健康知识和技能掌握合格率为 84.44%, 高于对照组 的 62.22%, 差异有统计学意义(P<0.05); 观察组患者并发症(肺部感染, 下肢深静脉血栓)发生率为17.78%, 低于对照组的 40.00%,差异有统计学意义 (P<0.05); 观察组的下床活动时间[3(3,4)d], 肛门排气时间 [3(3, 3)d], 住院时间 [12(10,15) d]均低于对照组[下床活动时间 6(5,6)d, 肛门排气时间 5(4,5)d, 住院时间18(15, 24)d], 差异均有统计学意义 (P<0.05); 观察组的护理服务满意度为95.56%,高于对照组的 75.56%, 差异有 统计学意义 (P<0.05). 结论 健康教育路径表结合云随访平台对食管癌围手术期患者的行为能力有显著提 升,能积极执行健康宣教内容,进而加快恢复速度,减少并发症的发生,并且患者对护理服务满意度较高。 [ABSTRACT FROM AUTHOR]

Objective To investigate the effect and accuracy of task state functional near infrared spectroscopy (f NIRS) in detecting advanced cognitive ability of patients with micro state of consciousness(MCS). Methods Using f NIRS and active motor imagery（ MI） paradigm,5 patients with micro state of consciousness (MCS) and 6 normal controls were tested; Support vector machine（ SVM） and linear discriminant analysis (LDA) were used to classify the responses of the subjects to the “yes” or “no” questions. Results For the basic fact “yes” problem, the response of blood oxygen in the motor area and brain frontal lobe area of the normal control increased significantly, but no significant changes of blood oxygen were detected for the basic fact “no” problem. The relative reliable response of blood oxygen similar to that of normal control was detected in 3 MCS patients in the active Mi paradigm, that is, during the “yes” task, the concentration of oxyhemoglobin (HbO2) in the motor and frontal lobe brain areas increased significantly. During the basic fact “no” task, there was no obvious response to blood oxygen. One of them was able to answer all the questions accurately by this method, and SVM was better than LDA classifier. Conclusions The f NIRS and active MI paradigm can be used to study the brain function of patients with MCS, and can be used as objective indicators to evaluate the level of consciousness and residual consciousness of patients with MCS. In addition, the “yes ” and “no ” problems of motor imagery are classified by machine learning method, which lays a foundation for further communication between patients with consciousness disorders and the outside world through brain computer interface.
