This paper summarizes integrated Traditional Chinese and Western medicine nursing of patient with sacral cyst. Medical staff paid attention to the emotional care of the patient before surgery, closely observed the vital signs of the patient after surgery, and actively prevented the occurrence of complications after the sacral cyst resection, including the prevention of infection of the sacrococcygeal wound. Based on the Traditional Chinese Medicine syndrome differentiation, nursing techniques including auricular acupoint pressing, Chinese herbal medicine hot compress and ointment rubbing therapy were carried out to relieve the pain, improve the wound healing, prevent the constipation and promote the early rehabilitation after surgery. (本文总结1例骶管囊肿患者术后中西医结合的护理。术前注重患者的情志护理, 并做好术后锻炼准备工作; 术后严密观察患者生命体征, 积极预防骶管囊肿切除术后并发症; 基于中医辨证, 采取耳穴贴压、中药热奄、中药膏摩等中医护理技术缓解疼痛、促进伤口愈合、防治腹胀及便秘, 通过中西医结合护理干预, 缓解患者焦虑情绪, 促进早期康复。)

Objective To explore the therapeutic effect and nursing experience of patients with respiratory failure in ICU after prone ventilation. Methods From January 2020 to January 2021, 4 patients with acute respiratory distress syndrome (ARDS) and 1 patient with atelectasis admitted to the Intensive Care Unit were retrospectively analyzed. The effect of prone ventilation was studied, and the nursing experience was summarized. Results After the treatment with prone ventilation, the blood oxygen saturation of 4 patients increased from 85%~93% to 95%~99%, and the sputum was effectively aspirated. CT examination of 1 patient with atelectasis showed significant improvement in lung inflammation. Conclusion The application of prone ventilation in patients with ARDS and atelectasis in ICU can significantly improve the pulmonary ventilation function, promote the effective discharge of pulmonary secretions, increase the functional residual capacity, thus improve the cardiorespiratory fitness and promoting the recovery of patients. (目的 探讨ICU呼吸衰竭患者进行俯卧位通气后的治疗效果及护理经验。方法 回顾性分析2020年1月—2021年1月收治的采用俯卧位通气治疗的急性呼吸窘迫综合征(ARDS)4例和肺不张患者1例的临床资料, 总结护理经验。结果 经俯卧位通气治疗, 本组患者血氧饱和度明显提高, 痰液有效排出, 痰液性质改善, 其中肺不张患者的肺部炎症明显好转。结论 俯卧位通气干预联合专业护理能明显改善患者肺部通气功能, 促进肺部分泌物有效排出, 增加功能残气量, 从而减轻心肺负荷, 有利于疾病恢复。)

This paper summarized the nursing experience of a child with electrolyte disturbance after craniopharyngioma resection. Postoperative children with craniopharyngioma are prone to electrolyte disorders, and children in serious condition may have demyelination and other symptoms, directly affecting the prognosis. Medical staff should take close observation of the vital signs and consciousness, accurately record the fluid intake and output, monitor changes of body temperature, and reduce secondary damage caused by electrolyte disorders. The diet nursing should be carried out in combination with Traditional Chinese Medicine emotional nursing. (本文总结1例儿童颅咽管瘤切除术后患儿出现电解质紊乱的护理经验。颅咽管瘤切除术后患儿容易出现电解质紊乱, 严重者可出现脱髓鞘等症状, 直接影响预后效果。医护人员应严密观察患者生命体征和意识, 准确记录出入量, 监测体温变化, 减少电解质紊乱带来的继发损害; 同时给予饮食护理, 实施中医情志护理。)

This paper observed the effect of Traditional Chinese Medicine (TCM) acupoint application in the treatment of abdominal distension after surgical treatment for intracranial medulloblastoma in an infant. Nursing measures such as routine care, diet nursing and emotional care were summarized. On the basis of routine nursing, the TCM acupoint application was carried out to relieve the abdominal distension. The acupoint application is a safe and easy-to-operate technique of TCM nursing, which could help ease the suffering of children and increase their comfort. (本文总结穴位贴敷治疗1例婴幼儿颅内髓母细胞瘤术后腹胀的护理经验, 主要包括常规护理、饮食护理、情志护理。在常规护理的基础上, 护理人员结合中医特色护理技术穴位贴敷给予干预, 其操作简单, 能有效改善患儿腹胀症状, 在减轻患儿痛苦的同时又能够增加治疗期间的舒适度。)

Based on the scientific evaluation and acupoint selection by syndrome differentiation, individualized nursing interventions were carried out to relieve the postoperative dysuresia and pain, and improve the recovery of the patient. To implement standardized protective measures and to evaluate the safety and effectiveness of peripherally inserted central venous catheter (PICC) for severely and critically ill patients with COVID-19 in the isolation ward. PICC catheterization was performed to 9 patients who were diagnosed with COVID-19 and treated in the isolation ward. These patients were all given standardized protective measures during the PICC operation. The one-time success rate of catheterization was 100. 00%. There was no postoperative complications such as infection, bleeding, phlebitis, thrombosis, and no nosocomial infection occurred. The implementation of PICC catheterization in severely and critically ill patients in the isolation ward is safe and effective, and standardized protection in catheterization can effectively reduce the risk of cross-infection in hospital. (通过实施规范化防护措施, 评估经外周静脉置入中心静脉导管(PICC)在隔离病房收治的重型和危重型新型冠状病毒肺炎患者中应用的安全性和有效性。回顾隔离病房接受诊治的9例确诊/疑似患者的临床资料, 患者接受PICC置管术, 且术中实施规范化防护措施。9例患者均一次置管成功, 术后未发生感染、出血、静脉炎、血栓等并发症; 无一例置管患者和医务人员发生院内感染。隔离病房收治的重型和危重型新型冠状病毒肺炎患者中实施PICC置管安全、有效, 置管中规范化防护可以有效避免院内交叉感染的发生。)

Key issues and consideration of nursing included: Close observation of vital signs, prevention of cardiac tamponade and intracranial hypertension after interventional surgery, monitoring of kidney function and coagulation function, prevention of arterial puncture bleeding and prevention of venous thrombosis of lower limbs, adopting the treatment of integrated Traditional Chinese and Western medicine to improve the symptoms of constipation and insomnia, improving the postoperative quality of life of patients, comprehensive analysis of patient psychological state after major surgery and implementation of corresponding psychological nursing and specific discharge guidance. (本文总结1例心脏血管经皮冠状动脉腔内成形术(PTCA)合并脑血管介入手术患者的护理措施。护理要点包括: 严密观察生命体征, 介入手术后预防心包填塞与颅内高压, 加强肾脏功能和凝血功能的监测, 预防动脉穿刺处出血和预防下肢静脉血栓形成, 通过给予中西医结合护理改善便秘及失眠的症状, 提高患者术后生活质量, 全面分析患者大手术后心理状态并给予相应的心理护理和有针对性的出院指导。)

This paper reviewed the clinical data of 9 women with chickenpox at third trimester of pregnancy, and summarized related nursing measures. Pregnant women infected with Varicella should be arranged in the isolation ward, and appropriate cleaning and disinfection of contaminated items were implemented in the isolation ward. Based on the illness observation, comprehensive interventions on skin hygiene, hyperthermia, mental status and nutrition were carried out in the prenatal period. Key issues of postpartum care included prevention of postpartum complications, neonatal care, health education before discharge and home-based follow-up. A scientific and adequate nursing is effective to reduce the risk of postpartum complications and ensure the maternal and child safety. (本文回顾性分析9例妊娠晚期合并水痘孕妇的临床资料, 总结护理经验。产前应严格消毒隔离措施, 严密观察患者病情, 积极开展皮肤护理、高热护理、心理护理、饮食护理; 产后加强并发症预防和新生儿护理, 完善出院指导和居家随访。充分做好妊娠晚期合并水痘孕妇的临床护理工作, 可以有效减少母婴并发症的发生, 是确保母婴安全的重要有效措施。)

- Abstract
Objective To explore the changes of serum pregranulin(PGRN) and serum amyloid A(SAA) levels in children with severe pneumonia and their relationships with the illness and prognosis of children. Methods Totally 204 children with pneumonia were selected as the research objects, and were divided into the severe pneumonia group(n=93) and ordinary pneumonia group(n=111) according to the condition. Eighty healthy children in the same period were selected as the control group. The serum levels of PGRN, SAA, C-reactive protein(CRP) and procalcitonin(PCT) in each group were compared. According to the prognosis, children in the severe pneumonia group were divided into the death group(n=10) and the survival group(n=83), and the above indicators were compared between the two groups. Pearson correlation analysis was used to analyze the correlation between serum PGRN, SAA levels and pediatric critical illness score(PCIS) in children with severe pneumonia. Multivariate Logistic regression was used to analyze the risk factors of death in children with severe pneumonia. The ROC curve was used to analyze the levels of serum PGRN and SAA in predicting the prognosis of children with severe pneumonia. Results The serum levels of PGRN, SAA, CRP and PCT in the severe pneumonia group and common pneumonia group were higher than those in the control group(all P<0. 05). The serum levels of PGRN, SAA, CRP and PCT in the severe pneumonia group were higher than those in the common pneumonia group(all P<0. 05). The levels of serum PGRN, SAA, CRP and PCT in the death group were higher than those in the survival group(all P<0. 05), and PCIS was lower than that in the survival group(P<0. 05). The results of Pearson correlation analysis showed that serum levels of PGRN and SAA were negatively correlated with PCIS in children with severe pneumonia(r=-0. 412, -0. 492, all P<0. 01). Multivariate logistic regression analysis showed that serum PGRN(OR=1. 120, 95% CI: 1. 022-1. 228) and serum SAA(OR= 1. 212, 95% CI: 1. 056-1. 389) were independent risk factors of death for children with severe pneumonia(all P<0. 05). ROC curve showed that serum levels of PGRN and SAA had a good predictive value for the death of children with severe pneumonia(P<0. 05). The area under the curve(AUC) was 0. 831 and 0. 801, the best critical values were 41. 750 µg/L and 23. 255 mg/L, the sensitivities were 84. 3% and 81. 5%, and the specificity were 80. 0% and 60. 0%, respectively. Conclusion The serum levels of PGRN and SAA in children with severe pneumonia are significantly increased, which are closely related to the severity of the disease, and are independent risk factors of death in children with severe pneumonia. Garlic cultivation is highly demanding for a single seed to maintain upright-directional sowing with the roots down ward vertically. However, current adjustment devices for the direction of garlic cloves cannot be suitable for hybrid garlic varieties. In this study, an intelligent adjustment device was designed for the direction of garlic clove using edge computing. The device consisted of a feeding channel of garlic clove, a camera, a camera bracket, clove direction adjustment mechanism, turning servo and brackets. The adjustment mechanism of clove direction was composed of flip frame, reset spring, hopper opening and closing servo, Y-type hopper I and II. A dual convolution neural network (CNN) structure was adopted in the control system, where a custom deep learning CNN was for garlic monitoring in real time, and a ResNet-18 network was for the determination of garlic clove orientation. In monitoring, the garlic clove was distinguished from the background of images, thereby determining whether the clove was fed to the adjustment. Areal-time detection of orientation was to keep the pointy end of garlic clove facing upward, while the blunt end down into the soil. A suitable control strategy was provided to promptly adjust the direction of garlic clove. Higher identification accuracy and real-time performance were achieved in two different networks for separate detection and orientation of garlic clove. The specific procedure of orientation adjustment was as follows. An image processing was performed to determine whether the garlic clove entered the Y-shaped hopper from the feeding channel. Once the garlic clove was detected to be in the hopper, an image was real-time captured by the camera. The captured image was processed immediately through the deep learning network of detection and orientation. When the scales (blunt end)of garlic cloves were facing upward, the opening and closing servos of a hopper rotated at a certain angle to open the lower end of Y-type hopper I and II. As such, the garlic clove fell directly into the inserting with the scales facing upward. If the scales of garlic cloves were facing downward, the turning servos and adjusting mechanism individually rotated 180°, to accurately tailor the orientation of scales when the garlic clove was sliding down the guide slot of Y-type hopper I. Both theoretical and empirical data demonstrated that the structural parameters of the adjusting mechanism greatly dominated the success rate of the adjusted scale bud. A discrete element method (DEM) was performed on a commercial software EDEM to simulate the working effect of the adjusting mechanism. An orthogonal test was also utilized to optimize the key parameters of adjusting mechanism. An optimal combination of parameters was obtained, where the inclination angle of the garlic clove was 80°in the feeding channel, the radius of the Y-shaped hopper was 12.49 mm, and the height difference of the hopper was 20mm.Finallya bench test was carried out to verify the direction adjustment of garlic cloves. In scale bud, the success rate of identification was 97.25%, and the success rate of adjustment was 96.25%, where the success rate of adjustment was slightly lower than that of recognition. The reason was that the correctly identified garlic turned over unexpectedly when falling, due to the irregular center of gravity in a single seed. The mean inference time of the model was 0.045 s, indicating a small proportion of adjustment time for the scale bud. The average adjustment time was 0.785 s, where the mean value was 0.59 s when the garlic cloves were facing up and 0.98 s when facing down. There was a relatively large difference in the adjustment time when the garlic buds were faced up and down. This difference came into being because there was inconsistent movement stroke of the adjustment mechanism in two cases, particularly where the rotation speed of the drive motor was the same when the scale buds were facing down. - Abstract
Potato is an important food crop with planting area been increased annually. Potato should be cut according to agronomic requirements before planting to make the potato sprout early and to increase the yield and to save the seed potato. The bud position and the weight of the seed potato cutting have an important influence on late growth of potato. Accurate identification of the bud eye is the premise of automatic cutting of seed potatoes. In addition, bud eyes are easy to be confused with mechanical damages such as growth spots, soil and skin breakage. In order to reduce the false recognition rate and improve the success rate of bud eyes recognition, a method for identifying potato buds based on three-dimensional geometric features of color saturation are proposed. By comparing the color space of potato, it is found that the color saturation component S in the HSI color space is the most obvious. In addition, it is less affected by light conditions, avoiding low recognition accuracy caused by interference of brightness factors. In this paper, four eigenvectors based on their longitudinal section curves and their first derivatives were proposed by analyzing the S component in the three-dimensional geometric space. The four-feature comprehensive judgment criterion was used to identify the bud eye longitudinally, and then the bud eye vertical recognition result was screened again according to the lateral characteristics of the bud eye. The modified method can greatly reduce the bud eye false recognition rate. The specific potato bud eyes recognition algorithm was as follows: 1) Color saturation S component was obtained by converting the acquired image from RGB color space to HSI color space. 2) The S component curve was obtained by longitudinally column-by-column interception of the three-dimensional map of the S component. Then derivative curve was obtained by deriving the S component curve. 3) First, all the valley positions of the first derivative curve of the S component were obtained, and the four bud eye feature parameters corresponding to the position were obtained. Then, the bud eye was identified according to the determination rule. If the condition was satisfied, the corresponding row and column position was set to 0, otherwise it was set to 1. The bud eye binarization matrix was obtained by performing bud eye feature determination on all longitudinal columns of the S component three-dimensional map.4) According to the lateral continuity of the bud eye and the arc-like structure of the lower edge of the bud, the morphological processing of the bud eye binarization matrix was carried out, and the false bud eyes were further removed to complete the bud eyes recognition. The influence of defects such as broken skin, growth spots and mechanical damage on the accuracy of bud-eye recognition was also analyzed in this paper. The data indicated that mechanical damage, growth spots, etc. was not confused with bud eyes and caused misjudgment of the buds. The algorithm can effectively prevent the influence of these interference factors.The experimental results showed that the recognition rate of bud eyes was 91.48%, of which, the recognition rate of non-germinated buds, germinated buds and the false recognition rate of bud eyes was 92.21%, 89.00% and 4.32%, respectively. The probability of seed potato cutting without buds due to false recognition of bud eyes was less than 1.01%. This can effectively prevent the seedling shortage caused by potato cutting without buds, resulting in reduced production. The average time taken to identify a single image is 2.68 s. The results indicated that the method can provide reference for the bud eyes recognition of seed potato automatic cutting machine due to its low false recognition rate of bud eye, strong anti-interference ability and high stability.
