5. 肝爽颗粒联合恩替卡韦对乙型肝炎肝硬化患者 门静脉血栓发生的影响 .

- Abstract
Objective To investigate the effect of Ganshuang granule combined with entecavir on portal vein thrombosis(PVT) in patients with hepatitis B cirrhosis. Methods A total of 356 patients with hepatitis B cirrhosis who attended and were hospitalized in The Third People’s Hospital of Kunming from January 1, 2018 to December 31, 2020 were enrolled and randomly divided into combination group with 191 patients and control group with 165 patients. The patients in the combination group received Ganshuang granule combined with entecavir, and those in the control group received entecavir alone. The course of treatment was at least 24 weeks. The t-test or the Mann-Whitney U test was used for comparison of continuous data between two groups, and the chi-square test or the Fisher’s exact test was used for comparison of categorical data between two groups. The Kaplan-Meier method was used to analyze the cumulative incidence rate of PVT in both groups, and the log-rank test was used for comparison between two groups. Univariate and multivariate Cox proportional-hazards regression model analyses were used to investigate the influencing factors for PVT in patients with hepatitis B cirrhosis. Results The 191 patients in the combination group were followed up for 296.25 person-years in total, with a mean follow-up time of 1.55±0.65 years, and there were 8 patients with PVT, with an incidence rate of 4.19% and an incidence density of 1.41 per ten-thousand person-years. The 165 patients in the control group were followed up for 253.25 person-years in total, with a mean follow-up time of 1.53±0.67 years, and there were 20 patients with PVT, with an incidence rate of 12.12% and an incidence density of 4.79 per ten-thousand person-years. There was a significant difference in the incidence rate of PVT between the two groups(χ~²=7.687, P=0.006). The cumulative incidence rate of PVT plotted by the Kaplan-Meier method showed that the combination group had a significantly lower cumulative incidence rate of PVT than the control group(χ~²=7.226, P=0.007), with a relative risk of 3.155(95% confidence interval [CI]: 1.351-7.370). The univariate Cox analysis showed that hypertension, alanine aminotransferase, aspartate aminotransferase(AST), albumin(Alb), cholinesterase, estimated glomerular filtration rate, alpha-fetoprotein, D-dimer(D-D), Child-Pugh class, and Ganshuang granule combined with entecavir were influencing factors for PVT(all P<0.05); the multivariate Cox analysis showed that AST(hazard ratio [HR]=1.002, 95% CI: 1.000-1.004, P=0.025), and D-D(HR=1.907, 95%CI: 1.554-2.338, P<0.001) were independent risk factors for PVT in patients with hepatitis B cirrhosis, while Alb(HR=0.844, 95%CI: 0.755-0.944, P=0.003) and Ganshuang granule combined with entecavir(HR=0.350, 95%CI: 0.144-0.851, P=0.021) were independent protective factors against PVT in patients with hepatitis B cirrhosis. Conclusion Ganshuang granule combined with entecavir can significantly reduce the incidence rate of PVT in patients with hepatitis B cirrhosis, thereby exerting a certain preventive effect against PVT. 7. 富马酸替诺福韦酯联合肝爽颗粒对乙型肝炎肝硬化门 静脉系统血流动力学的影响.

- Abstract
Objective To investigate the effect of tenofovirdipivoxil combined with Ganshuang granules on hemodynamics of portal venous system in hepatitis B liver cirrhosis. Methods 180 patients with hepatitis B cirrhosis were randomly divided into three groups: 60 cases in tenofovir disoproxil fumarate combined with Ganshuang granule group (group A), 60 cases in tenofovir disoproxil fumarate combined with Compound Biejia Ruangan Tablet group (group B) and 60 cases in tenofovir disoproxil fumarate alone group (Group C), treatment 6 Months. Before and after treatment, we collected the gender, age, blood biochemical indexes, liver stiffness, hemodynamic indexes of portal vein system, portal vein diameter (DPV), portal vein mean blood flow velocity (VPV), splenic vein diameter (Dsv), spleen Mean venous blood flow velocity (Vsv), portal vein blood flow (Qpv), splenic vein blood flow (Qsv), etc. Results: The blood biochemical indexes, hemodynamic indexes and liver hardness of the three groups were significantly improved after treatment. After treatment, the recovery of blood biochemical indexes, liver hardness and hemodynamic indexes in group A and B were better than those in group C (P < 0.05); After treatment, compared with group B, ALT and AST decreased, liver hardness decreased, and some portal hemodynamic indexes (DPV, VPV, qpv) recovered better in group A than in group B. Conclusions Tenofovir disoproxil fumarate combined with Ganshuang granule could better improve the liver function, fibrosis and hemodynamic indexes of portal vein system in hepatitis B cirrhosis. The curative effect is significantly better than that of tenofovir disoproxil fumarate alone or combined with Compound Biejia Ruangan Tablet. [ABSTRACT FROM AUTHOR]

Gentiopicroside (GPS),the main active ingredient of Gentianace which is a kind of pretty popular Chinese traditional herb drug, will be widely distributed in human's tissue and organs (including the lungs, liver, heart, brain, etc) as being used by oral or intravenous administration. GPS always has efficacy of anti-inflammatory, anti-fibrosis, anti-oxidative stress, anti-apoptosis, or inhibition of tumor cell proliferation. Recently, many researches revealed it was wealthy exploring the usage of GPS on alcoholic/non- alcoholic liver diseases, drug/toxicant induced liver diseases,and fulminant hepatitis. To investigate the effect of intravitreal injection of bone marrow mesenchymal stem cells(BMSCs) cultured in vitro on the retinal ganglion cells after optic nerve injury in cats and its possible mechanisms. A cat model of right optic nerve injury was established and divided into four groups: (1)Group A: Right eye BMSCs injection transplantation group, intravitreal injection transplantation BMSCs concentration of 1× 105 cells/mL single cell suspension 0.1 mL;(2)Group B: right eye PBS injection group, intravitreal PBS buffer 0.1 mL;(3)Group C: Sham-injured control group, BMSCs left eye group, only exposed optic nerve without injury, no treatment; (4)Group D: normal control group, PBS left eye group, normal eyes, without any treatment. The survival rate of RGCs isolated from retina was observed by immunofluorescence staining with dicarboxyl tetramethylindole carbonyl cyanide perchlorate staining at 3, 7, 14 and 28 days after transplantation. The content of brain-derived neurotrophic factor in isolated retina was detected by double antibody sandwich ELISA. At 3, 7, 14 and 28 days after operation, the RGCs density in peripheral and central retina decreased significantly (peripheral area: P3d = 0.0446, P7d = 0.0011, P14d < 0.001, P28d < 0.001; central area: P3d = 0.0437, P7d = 0.0067, P14d < 0.001, P28d < 0.001). After 7 days, 14 days and 28 days, RGCs density and BDNF content in group A were significantly higher than those in group B (P < 0.05). BMSCs transplantation can slow down the apoptosis of RGCs in cats with traumatic optic nerve injury, which may be related to the increase of BDNF expression. [ABSTRACT FROM AUTHOR]

11. HBV DNA 低水平患者肝组织炎症活动度和纤维化程度的影响因素分析.

- Abstract
Objective To investigate liver pathological features and related factors in patients with low-level HBV DNA, and to provide a basis for evaluating the conditions of such patients. Methods A total of 137 patients with an HBV DNA level of <2000 IU/ml and >20 IU/ml who attended The Third People′s Hospital of Kunming from January 2014 to December 2017 were enrolled. Liver pathological examination was performed for all patients. Of all patients, 44 had grade 1 (G1) liver inflammation, 84 had grade 2 (G2) liver inflammation, and 9 had grade 3 (G3) liver inflammation; as for fibrosis stage, 2 had S0 fibrosis, 67 had S1 fibrosis, 56 had S2 fibrosis, 9 had S3 fibrosis, and 3 had S4 fibrosis. The correlation of liver inflammation grade and fibrosis stage with age, sex, duration of HBV infection, blood biochemistry, HBsAg level, and HBV DNA was analyzed. The Kruskal-Wallis H test was used for comparison of continuous data between multiple groups, the chi-square test was used for comparison of categorical data between groups, and univariate and multivariate one-way non-conditional logistic regression analyses were used to investigate the influencing factors for ≥G2 liver inflammation and ≥S2 fibrosis. ResultsThere were significant differences in HBsAg, platelet count (PLT), diameter of the splenic vein, and spleen thickness between the G1, G2, and G3 groups (H=7.135, 7.458, 7.588, and 10.150, all P<0.05), and there were also significant differences in HBsAg, PLT, diameter of the portal vein, diameter of the splenic vein, and spleen thickness between the S0, S1, S2, S3, and S4 groups (H=20.564, 12.065, 26.171, 14.720, and 13.725, all P<0.05). Of all 137 patients, 44 had no or mild inflammation and necrosis (<G2 group) and 93 had moderate or severe inflammation and necrosis (≥G2 group); age, alanine aminotransferase, alkaline phosphatase (ALP), white blood cell count, diameter of the splenic vein, and spleen thickness, with statistical significance determined by the univariate logistic regression analysis, were included in the multivariate analysis (α=0.15), and the results showed that age (odds ratio [OR]=1.045, P<0.05), ALP(OR=1.019, P<0.05), and spleen thickness (OR=1.150, P<0.05) were independent risk factors for ≥G2 liver inflammation in the patients with low-level HBV DNA. Of all 137 patients, 69 had no or mild liver fibrosis (<S2 group) and 68 had moderate or severe liver fibrosis (≥S2 group); total bilirubin, HBsAg, PLT, and diameter of the portal vein, with statistical significance determined by the univariate logistic regression analysis, were included in the multivariate analysis (α=0.15), and the results showed that HBsAg (OR=2.065), PLT(OR=0.988), and diameter of the portal vein (OR=2.464) were independent risk factors for ≥S2 liver fibrosis in the patients with low-level HBV DNA. Conclusion The majority of patients with low-level HBV DNA have the indication of antiviral therapy. Age, ALP, and spleen thickness are closely associated with liver inflammation grade in patients with low-level HBV DNA, and HBsAg, PLT, and diameter of the portal vein are closely associated with liver fibrosis. - Abstract
Objective: To establish a method to isolate and culture cat bone marrow mesenchymal stem cells in vitro and analyze their phenotypical properties after catture expansion as well as preliminary identification, so as to estabiish foundation for the further study of bone marrow mesenchymal stem cells transplantation. Methods: Bone marrow mesenchymal stem cells from cats were isolated, cultured and purified by the whole bone marrow adherence method. The bone mim-ow mesenchymal stem cells were purified by discard- ing suspended cells through exchanging medium. The morphology of cultured bone marrow mesenchymal stem cells was observed under inverted phase contrast microscope. The growth curve of cell proliferation was obtained based on the observation of the proliferation sta- tus of 1st, 3rd, 5th, 7th, and 9th generation. The expressions of CD34 and CD44 and CD90 of cells were analyzed by using flow cytome- try in order to identify bone marrow mesenchymal stem cells. Results: Bone marrow mesenchymal stem cells had spindle shape under in- verted phase contrast microscope. The primary cells were confluent in single layer after being plated for 5-7 days, and then the cells were passaged. After extending to the third passage, cells appeared relatively uniform appearance with fusiform or flattened, which were rapid proliferated as arranged with swirl observed under inverted phase contrast microscope. Bone marrow mesenchymal stem cells of 3rd, 5th generation proliferated more rapidly than those of 7th and 9th generation. Flow cytometric analysis demonstrated the positive rate of CD34, CD44, CD90 were 17.5%, 97.9%, 91%, respectively, which was consistent with the surface antigen of bone marrow mesenchymal stem cells. Conclusion: Purified bone marrow mesenchymal stem cells can be obtained via this protocol. Cells of 3rd, 5th generation with high proliferation are fit to the further experiment.
