Objective: To study the clinical value of the collapse angle of flow ⁃ volume (F ⁃ V) curve in assessment of clinical phenotype and severity of chronic obstructive pulmonary disease (COPD) patients. Methods: A total of 101 subjects who underwent pulmonary function tests from December 2021 to December 2022 at the First Affiliated Hospital of Nanjing Medical University were selected for this study. Subjects in the angle group were 33 stable COPD patients with collapse angle of F ⁃V curve. The non ⁃angle group including 38 cases of stable COPD patients without collapse angle, who matched with the angle group in forced expiratory volume in the first second as a percentage of predicted value (FEV1%pred) . The control group including 30 subjects without previous cardiopulmonary disease and had normal lung function. Basic information, clinical symptom scores (CAT score, mMRC score), pulmonary function parameters and daily exercise finger pulse oxygen parameters were collected and compared among the groups. Multifactorial logistic regression was used to analyze the factors associated with the collapse angle of F⁃V curve. The predictive value of the collapse angle of F ⁃ V curve for acute exacerbation of COPD within one year of follow ⁃ up was analyzed by receiver operating characteristic (ROC) curve. Results: Pulmonary function was severely impaired in the angle group, with FEV1 and forced vital capacity (FVC) of 0.91±0.24 and 2.11±0.63, respectively. CAT score, mMRC score, and ΔSpO2 of the angle group were higher than those of the non⁃angle group and the control group. SpO2L of the angle group after walking exercise was lower than that of the non⁃angle group and the control group (P < 0.05) . CAT score ≥12, mMRC score ≥2, and ΔSpO2≥13% were main related factors of the emergence of the collapse angle of F⁃V curve. The area under the ROC curve of the collapse angle of F⁃V curve for predicting the acute exacerbation of COPD was 0.777. The sensitivity and specificity of prediction were the best when the angle was <129.1°, which were 72.73% and 67.35%, respectively. Conclusion: When the collapse angle of F⁃V curve appears in patients with COPD, their lung function is often severely impaired. They are more prone to post⁃activity hypoxemia and acute exacerbations. Therefore, attention should be paid to the presence of the collapse angle of F ⁃V curve in the pulmonary function report of COPD, in order to recognize the high ⁃ risk group of COPD as early as possible. Based on the first-principles calculations of density functional theory, the stability, electronic structure, and optical properties of single layer GeC, N-doped, As doped, and N-As doped GeC systems were systematically studied. The results show that the single layer GeC is a direct bandgap semiconductor with the bandgap of 2. 10 eV. Compared with the single layer GeC, the bandgap and work function of the doped system decrease, indicating that the required energy for electronic transition is relatively small in our doped system. Moreover, the light absorption coefficient of the doped system improves, and the absorption band edge has also undergone a red shift, effectively expanding the response range of the system to light and improving the absorption ability of the system to photons. In addition, the As doped GeC system not only exhibits impurity levels near the Fermi level, but also shows the optimal optical properties such as absorption coefficient, static dielectric function, and extinction coefficient in the low energy region. Breast cancer is the most prominent malignant tumor in women and the most common cause of cancer-related deaths in women worldwide. Breast cancer is a focus of cancer prevention and treatment programs in China. With the progress of breast cancer treatment, traditional Chinese medicine( TCM) has played a unique role in the integrated TCM and western medicine treatment model for breast cancer, and has become one of the main ways to treat breast cancer. Here, the recent clinical research, experience, diagnosis, and treatment consensus regarding breast cancer treated with TCM are summarized to review the current situation and prospects of TCM in treating breast cancer, analyze the problems existing in clinical research, and put forward suggestions to provide direction and ideas for better future clinical treatment and research. [Objective] Evaluation indicators have functions such as goal orientation, assessment, monitoring and correction. However, the rural revitalization strategies implemented in different regions have unique features and priorities. Therefore, constructing a flexible evaluation indicator system that reflects regional characteristics is crucial for scientifically evaluating and identifying the development level and obstacles of rural revitalization in different types of regions. [Method] Based on the practical implementation of Chongqing' s rural revitalization strategy, a flexible evaluation approach was proposed. Empirical evaluation analysis was conducted using methods such as coefficient of variation, comprehensive evaluation and obstacle degree model. [Result] (i) The flexible evaluation indicator system for rural revitalization at the county scale in Chongqing consisted of ' 17 basic indicators + 6 characteristic indicators'. (ii) In general, the rural revitalization level of the three types of counties in Chongqing, including the pilot demonstration type, actively promoted type and key supported type, was not high by the end of the 13th Five-Year Plan period. The comprehensive evaluation values of the 20 sample counties ranged from 0. 22 to 0. 67, and there were significant differences among the three types of counties, with the key supported type having the lowest level of development. (iii) The development levels of the various dimensions of rural revitalization were uneven. The realization rates are 54% for affluent living, 47% for civilized rural customs, 47% for effective governance, 43% for ecological livability, 42% for thriving industries, and 40% for characteristic development. At the same time, there were significant differences in the development levels of each dimension among the three types of counties, especially in terms of thriving industries, ecological livability, affiuent living and characteristic development, with the maximum gap being 21 %, 23%, 46% and 20%, respectively. (iv) The rural revitalization level of the three types of counties was constrained by five obstacle factors, including the proportion of demonstration villages for garbage classification, the proportion of strong collective economy villages, the ratio of agricultural product processing output value to total agricultural output value, agricultural labor productivity, and the proportion of civilized villages at or above the county level. However, the degree of obstacle varied among different types of counties. Additionally, each type of county was also constrained by different indicators, such as the number of platforms for integrating industries, the proportion of agricultural products with ' Two products and one standard', and the ratio of disposable income of urban and rural residents. [Conclusion] The rural revitalization level of the three types of counties in Chongqing is not high, and the development levels of the various dimensions are uneven, and there is a significant gap between the three types of counties, and the rural revitalization level is mainly constrained by eight obstacle factors in six dimensions. Objective To investigate the effect of hydrogel dressing change in the nursing care of exposed’s Kirschner needle pin path after supracondylar fracture of humerus in children. Methods A total of 183 children with supracondylar fracture of humerus admitted to the Department of Pediatric Orthopedics of this hospital from September 2018 to May 2020 were selected, including 101 males and 82 females. Their ages ranged from 1 to 13 years, with an average age of (7. 32±4. 38) years. BMI ranged from 18. 71 to 22. 54 kg/m²,with an average of (20. 37±1. 08) kg/m2. All the children underwent emergency closed reduction Kirschner needle fixation. The number of Kirschner needles ranged from 3 to 4,with an average of (3. 32±0. 18). Diameter ranged from 1. 5 to 2. 0 mm, average (1. 63±0. 90) mm. According to different nursing methods, they were divided into the hydrogel group (using hydrogel to nail the way nursing, 81 cases) and the control group (using conventional method to nail the way nursing, 102 cases). The score of Visual Analogue Scale, healing of nail, superficial and deep infection, infection strains and so on were recorded in the two groups. Results In the hydrogel group, the score of Visual Analogue Scale and the infection rate of superficial and deep nails were significantly lower than those in the control group at week one to four, and the healing rate of grade A and C were significantly higher than those in the control group, with statistical significance (P＜0. 05). There was no significant difference in grade B healing rate between the two groups (P＞0. 05). The types of bacteria from deep infection were the same as those from superficial infection in the two groups. Conclusion In children with supracondylar fracture of humerus, the use of hydrogel in the nailing nursing can improve the healing degree of the nailing, and then reduce the nailing pain and bacterial infection, especially the incidence of deep infection. 本研究探究不同族群師生之教室言談。採質性研究法進入兩個場域觀察：漢人教師與原住民幼兒、原住民教師與漢人幼兒。研究發現，漢人教師在原住民區教導的原住民幼兒，在教室之口說回答簡短、常穿插族語且在團體中交叉共構。漢人教師在原住民區之提問以封閉性問題居多，原住民幼兒被要求來前面單獨回答時則常靜默，但如有媒介物就能敘說；原住民幼兒口說日記遊走在真實和「編說」中，與漢人教師所欲討論真實事件形成雙軌平行之對答。漢人區任教之原住民教師口語表達常重複、補充說明和插入臺語；原住民教師與漢人幼兒之對話以開放性提問居多，提問的內容若為讓幼兒探索教師意圖，則常缺乏線索而造成幼兒隨意猜測；若是提問的內容為幼兒的經驗事件，則提供幼兒社會建構、同儕鷹架之對話關係。此外，在漢人區的原住民教師之言談常過度延伸或轉換主旨，以至於延宕提問結論或沒有結論。 This study explored classroom discourses between teachers and young children with different ethnic groups, adopting an ethnographic approach as the qualitative method. Data were collected from (1) a Han teacher and aboriginal children in a Paiwan tribe; and (2) a Painanese teacher and Han children in a Han area. The results indicated that in the class of the Han teacher, the discourse styles of the aboriginal children included short sentences and interpolating Paiwanese nouns. The aboriginal children were unfamiliar with speaking alone in front of people, and multiple children spoke over each other in group situations. If the aboriginal children had varied media, such as their own drawings, they could express themselves; when they discussed their daily lives based on these drawings, other children frequently participated, generating a story that varied between reality and fabrication. However, the Han teacher tended to pull the children back to reality. By contrast, the discourse style of the aboriginal teacher in the Han area included unique syntax, a topic-associating style, and interpolated Hokenese. Most of the questions asked were open-type questions. It was unclear if those questions were meant to allow the children to explore the intentions of the teacher; little context and few hints were provided for such questions, causing the children to randomly guess the answers. If the questions related to the experiences of the children, the aboriginal teacher provided them opportunities for social construction and peer scaffolding. Chinese teaism was formed during the historic times, and its stereotyping was influenced by Buddhism, especially the Zen School. However, the influence had two sides: on the one hand, Zen School had made itself sinicized by virtue of the tea party and tea event rituals; on the other hand, Chinese tea culture had been upgraded through absorbing Buddhist teachings with an explicit formal expression as well as a unique spiritual connotation, developing to teaism by throwing off tea’s materiality.
