Objective To explore the predictive value of the acute physiology and chronic health evaluationⅡ（APACHEⅡ) combined with serum procalcitonin（PCT) and hypersensitive C-reactive protein（hs-CRP) in severe pneumonia complicated with respiratory failure.Methods A total of 120 patients with severe pneumonia who were treated in the hospital from February 2020 to February 2023 were selected and divided into the control group（85 cases) and the observation group（35 cases) according to the combined respiratory failure.The APACHEⅡ score, serum PCT and hs-CRP levels between two groups were compared, and the influencing factors of severe pneumonia combined with respiratory failure and the predictive efficacy of related indicators were analyzed.Results There were statistically significant differences in terms of smoking history, chronic obstructive pulmonary disease（COPD) and oxygenation index between the two groups（P＜0.05) .The APACHEⅡ score, serum PCT and hs-CRP levels of the observation group were higher than those of the control group, and the differences were statistically significant（P＜0.05) .Smoking, concomitant COPD, APACHEⅡ score, PCT, hs-CRP were risk factors for severe pneumonia with respiratory failure（P＜0.05), while oxygenation index was a protective factor（P＜0.05) .The predictive sensitivity and area under the receiver operating characteristic curve of APACHEⅡ score, serum PCT and hs-CRP were higher than those when each indicator was used alone（P＜0.05) .However, there was no statistically significant difference in specificity between the two groups（P＞0.05) .Conclusion The APACHEⅡ score combined with serum PCT and hs-CRP has a high predictive efficiency for severe pneumonia complicated with respiratory failure. Objective To investigate the effects of lncRNA NKILA on epithelial-mesenchymal transition and radia⁃ tion resistance of nasopharyngeal carcinoma cells and its mechanism. Methods Nasopharyngeal carcinoma cells CNE2 were cultured in vitro and the radio-resistant cell models were constructed. Radio-resistant CNE2 cells were randomly divided into the blank group, NC group, and transfection group. Cells in the transfection group and NC group were transfected with NKILA-mimic and NKILA-NC plasmid, respectively； cells in the blank group were not transfected. LncRNA NKILA and miR-21 expression levels were detected by RT-qPCR, cell proliferation was detected by MTT, and cell invasion and migration were detected by Transwell assay. The expression of E-cadherin and N-cadherin proteins was detected by Western blotting. The targeted relationship between lncRNA NKILA and miR-21 was predicted by TargetScan, and the targeted relationship between lncRNA NKILA and miR-21 was verified by double luciferase reporter gene experiment. Ten SD nude mice were randomly divided into the observation group and control group, with 5 in each group. Radio-resistant CNE2 cells transfected with lncRNA NKILA and simple radio-resistant CNE2 cells were injected into the armpit of the right forelimb, respectively. The mice were killed in routine feeding for 4 weeks, and the transplanted tumor volume was separated and measured. Results The expression levels of lncRNA NKILA and E-cadherin in the transfection group were significantly higher than those in the blank group and NC group, while the expression of miR-21, cell proliferation, invasion, migration and N-cadherin protein in the transfection group were significantly lower than those in the blank group and NC group (all P<0. 05). There was no significant difference in the above indexes between the blank group and the NC group (all P>0. 05). TargetScan prediction showed that lncRNA NKILA had targeted binding sites with miR-21. The results of double luciferase reporter gene experiment showed that the luciferase activity of CNE2 cells co-transfected with NKILA-mimic plasmid and miR-21-3'-UTR-WT was significantly lower than that of CNE2 cells co-transfected with NKILANC plasmid and miR-21-3'-UTR-WT (P<0. 05). There was no significant difference in the luciferase activity between CNE2 cells co-transfected with NKILA-mimic plasmid and miR-21-3'-UTR-MUT and those co-transfected with NKILA-NC plasmid and miR-21-3'-UTR-MUT (P>0. 05). The transplanted tumor volume of the observation group was significantly lower than that of the control group (P<0. 05). Conclusion LncRNA NKILA can inhibit the proliferation, invasion and migration of nasopharyngeal carcinoma cells through targeted inhibition of miR-21 expression, and then inhibit the radiore⁃ sistance and epithelial mesenchymal transformation of nasopharyngeal carcinoma cells. A new pretreatment technology of phenolic compounds analysis in water was established. When the pH value of the water sample was 3-4, the extraction of phenolic compounds in 500 mL water sample could be completed in 5 min by using SDB-XC solid phase extraction membrane. Then the folded membrane was put into a reaction bottle, in which five fluorinated benzyl bromide solution was used as the derivatization reagent to complete the elution and derivatization of phenolic compounds on the membrane by20 min ultrasound. HP-5 MS column(30 m×0.25 mm×0.25μm)was selected as analytical column, the detection could be completed within 10 min by using negative chemical ionization source. In this method, the selection of membrane, the control of acidity, derivation condition, ultrasonic condition and interference situation were analyzed and optimized. The results showed that the correlation coefficient of each component was greater than 0.997 in the linear range of 10-200μg/L. The recoveries of different concentrations were 70%-107%, and the relative standard deviations were 3.8%-10.4%. Some phenolic compounds were detected in actual samples by this method. The method can achieve the rapid extraction, synchronous elution, low pollution and high sensitivity detection of samples. [ABSTRACT FROM AUTHOR]

Large-diameter steel casing is widely used as pile-foundation during design and construction of open sea bridges, and its wave force problem is one of hot issues in ocean engineering. Most of the existing results are based on numerical simulations and model tests, however results on in-situ monitoring are fewer. Relying on" One Belt One Road" landmark project China-Maldives Bridge, we installed multiple sets of pressure sensors on No.20 pier column of main bridge, and carried out field monitoring of environmental loads. The methods of Fast Fourier Transform and filtering are adopted to analyze measured data, separate the different frequency components, convince the period of tide and extract wave pressure which the maximum wave pressure is about 4 kPa. Objective: To analyze the clinical effects of mifepristone and guizhi fuling capsules on the endometriosis and serum levels of cancer antigen 125 (CA125) and cancer antigen 199 (CA199). Methods: 106 cases with endometriosis who were treated in our hospital were selected and randomly divided into the control group and the experimental group with 53 cases in each group. The patients in the control group were treated with mifepristone, while the patients in the experimental group were treated with guizhi fuling capsule on the basis of the control group. Then the clinical curative effect, serum levels of CA125, CA199, vascular endothelial growth factor (VEGF), superoxide dismutase (SOD), interleukin-6 (IL-6), interleukin-8 (IL-8), tumor necrosis factor-α (TNF-α), hypersensitive c-reactive protein (hs-CRP), whole blood viscosity, plasma viscosity and erythrocyte aggregation in the two groups were observed and compared before and after the treatment. Results: The effective rate of the experimental group was significantly higher than that of the control group(P<0.05); The serum levels of CA125, CA199, VEGF, SOD, IL-6, IL-8, TNF-α，hs-CRP, whole blood viscosity, plasma viscosity and erythrocyte aggregation indexes of the experimental group were significantly lower than those of the control group(P<0.05). Conclusion: Mifepristone and guizhi fuling capsule had better clinical effects on the endometriosis, which could reduce the serum levels of CA125 and CA199. [ABSTRACT FROM AUTHOR]

The composite bridge with corrugated steel webs, which adopted the technology of segmental prefabrication and assembly, is a new composite structure. Since concrete webs replaced by corrugated steel webs, the positioning and connecting process of corrugated steel webs is added. we studied on the key factors influencing the construction efficiency during the segmental prefabrication and assembly for corrugated steel webs in order to adapt the characteristics of the factory manufacture and the mechanical installation. At the same time, we carried out a scale model test to verify the structural performance. The results show that the precast segmental box girders with corrugated steel webs has the same properties with the in-situ structures during normal use, it can meet the engineering application. Objective To explore the differential expression of microRNAs (miRNAs) in steroid-induced osteonecrosis (ON). Methods Bone tissues were selected from patients with steroid-induced ON between the necrotic zone of femoral heads and its femoral neck to analyze the miRNA expression profile using the microarray. The most differentially expressed miR-125a-3p and miR-17-5p in microarray analysis were further confirmed by real-time quantitative PCR. Results According to the microarray screening, 8 miRNAs were upregulated and 3 miRNAs were downregulated in the necrotic zone of femoral heads samples (Fold>2, P<0.05). Results of real-time PCR revealed that miR-125a-3p was upregulated and miR-17-5p was downregulated in the necrotic zone of femoral heads samples, which were in agreement with the microarray data. Conclusion MiRNA’s differential expression profile of human steroid-induced ON samples was obtained. Among the differentially expressed miRNAs, miR-125a-3p and miR-17-5p are the most apparently differentially expressed miRNAs, which may be related to the pathogenesis and development of steroid-induced ON. Objective To investigate the regulatory effects of glucocorticoids on the local renin-angiotensin system in osteoblasts by using MC3T3-E1 cells. Methods Cellular immune histochemistry was carried out to observe the renin-angiotensin system in osteoblasts. And then after 12-hour culture in serum-free solution, MC3T3-E1 cells were divided into four groups: control, dexamethasone (DXM), dexamethasone + mifepristone (DXM+MIF) and mifepristone (MIF). Components of the renin-angiotensin system including angiotensin type 1 receptor (AT1R), angiotensin type 2 receptor (AT2R) and angiotensin converting enzyme (ACE) in osteoblasts were detected at the mRNA and protein levels using Western blot and PCR. The activity of ACE was also measured after 36-hour intervention. Results The results of immunohistochemistry showed that AT1R, AT2R and ACE were all expressed in osteoblasts. The activity of ACE increased obviously after dexamethasone intervention compared with that in the control group, which was blocked by mifepristone. The mRNA levels of AT1R, AT2R and ACE were increased by dexamethasone compared with those in the control group, which was inhibited by mifepristone. The protein levels of AT1R, AT2R and ACE were enhanced by dexamethasone compared with those in the control group, which was blocked when the cells were co-intervened with mifepristone. However, ACE activity and the mRNA and protein levels of AT1R, AT2R and ACE did not change when the cells were intervened with mifepristone alone (P>0.05). Conclusion The local renin-angiotensin system in osteoblasts is activated by dexamethasone through glucocorticoid receptors on osteoblasts, which may be one of the pathogenesis of glucocorticoid-induced osteoporosis.
