Objective To apply the existing best evidence of the prevention and management of febrile neutropenia after chemotherapy to nursing practice in order to reduce the incidence of complications in patients with breast cancer. Methods Using the JBI clinical evidence practice application system, based on the existing best evidence, six review criteria were developed and applied to nursing practice. The quality review and process improvement were carried out in one ward for 6 months. In the baseline review process, 349 times of chemotherapy in 128 cases were statistically reviewed. In the second-round review process, 349 times of chemotherapy in 128 cases were statistically reviewed. Fifty patients were followed up and their self-efficacy was evaluated by using General Efficacy Scale (GES). Results After the second round of examination, all the examination standards reached 100. 00% except the seventh article(patients had to follow the doctor's advice to complete medical examination and recoding) , which rose to 88. 2%. According to the follow-up outcomes, patients had achieved higher GES scores after implementation of evidence-based prevention and management. Conclusion The evidence-based quality review program has significantly improved nursing practice in the prevention and management of febrile neutropenia in breast cancer patients undergoing chemotherapy, but it still needs to be reviewed and improved to improve the quality of nursing. (目的 将现有的化疗后粒细胞减少性发热的预防与治疗的最佳证据应用到护理实践中, 以减少化疗患者并发症的发生。方法 采用JBI临床证据实践应用系统, 基于现有的最佳证据, 制定9条审查标准, 应用于护理实践, 在1个病区开展6个月的质量审查和流程改进。基线审查中, 选取128名乳腺癌化疗患者, 对其中的349次化疗进行统计审查; 第2轮审查中, 选取128名乳腺癌化疗患者, 对其中的349次化疗进行统计审查。另外, 随访50名患者, 分别在改善前和改善后采用一般自我效能量表(GES)评估患者自我效能。结果 经过第 2轮审查, 除第7条(患者均遵医完善检查和记录)上升至88. 2%, 其余审查标准均达到100. 00%。随访结果显示, 改善后患者GES量表评分较改善前得到有效提升(P＜0. 05)。结论 基于循证的质量审查项目改善了乳腺癌化疗患者粒细胞减少性发热的预防与管理的护理实践, 但仍需不断审查改进, 以提高护理质量。)

Objective: To compare the effect of two laser initiators on the thermal performance of Denlaser 800 laser and cautery effect of oral mucosa and soft tissue. Methods: The two laser initiators were dropped on the slide in equal amounts and the galvanometer was installed at the bottom of the slide to measure the temperature. Laser irradiation for ten seconds, at the end of ten seconds to record the temperature of the galvanometer. The same amount of methyl violet liquid and ink juice were coated on the front 1/2 of the back of the tongue of experimental mice, and the same laser was used to irradiate the experimental part coated with light sensitive agent under the same parameter conditions. The tissue computer of the experimental area was used to measure the burning area. Results: The average temperature increase of methyl violet absorption Denlaser 800 laser was 101.4°C, and the average temperature increase of ink was 86.8°C. The increase temperature of methyl violet absorption Denlaser800 laser was higher than that of ink (P<0.05). The cautery area of coated methylene violet mucosa was significantly larger than that of ink juice and laser mucosa. The cautery area of the mucous tissue irradiated by methyl violet liquid combined with low dose laser was significantly larger than that of the mucous tissue irradiated by high dose laser alone, and slightly larger than that of the mucous tissue irradiated by high dose laser of ink juice. Conclusion: Methylene violet is better than ink initiator for Denlaser800 laser thermal performance, photoinitiator can increase the effect of laser on oral mucosa, methylene violet is better than ink juice, it is an ideal light sensitive agent in stomatology. BACKGROUND: Posterior wall damaged vertebral osteoporotic compression fracture with spinal canal occupying is often treated by open surgery or conservative therapy at home and abroad. For elderly patients with osteoporosis, with the external spinal fixation device, the axial distraction of the spinal canal occupying the posterior bone block reduction, expansion of the spinal canal, and percutaneous vertebroplasty to solidify the vertebral body achieved good clinical results. OBJECTIVE: To observe the clinical effect of percutaneous vertebroplasty of anterior and posterior longitudinal ligaments for osteoporotic compression fracture of vertebral body with posterior wall damage under external spinal fixation. METHODS: A total of 52 aged patients with osteoporotic compression fracture of vertebral body with posterior wall damage were selected, aged from 61 to 86 years (mean 78.62±5.20 years). The patients were treated with external spinal fixation combined with percutaneous vertebroplasty. X-ray and CT were used to observe the anterior margin height loss, kyphosis angulation, vertebral stenosis, and bone cement distribution in the fractured vertebrae before, 1 day and 3 months postoperatively, during final follow-up (12 months postoperatively). Visual analogue scale score and Oswestry dysfunction index were also evaluated. RESULTS AND CONCLUSION: Fifty-two cases were followed up for 3-12 months. X-ray films exhibited that preoperatively and 1 day, 3 months postoperatively, at final follow-up, the loss rates of anterior vertebral body height were (34.5±2.2)%, (3.5±1.3)%, (4.0±0.6)%, and (4.4±1.1)%; the kyphosis angles were (12.1±1.7)°, (4.0±0.8)°, (3.5±0.5)°, and (4.4±0.2)°. CT showed that the spinal stenosis rates were (40.9±7.2)%, (10.7±2.5)%, (9.7±1.1)%, and (9.8±0.7)%. The distribution range of bone cement filling in fractured vertebral bodies was all ≥75%. There were significant differences in visual analogue scale score and Oswestry disability index before and 1 day after treatment (P < 0.05). There was no significant difference in visual analogue scale score and Oswestry disability index between 3 months after surgery and last follow-up (P > 0.05). The results have shown that percutaneous vertebroplasty under anterior posterior longitudinal ligament stretch of the vertebral body not only significantly improves the clinical effect of osteoporotic vertebral compression fracture repair, but also improves the vertebral body solidification rate and relieves pain, which is a safe and effective minimally invasive combination of new technology. 采用响应面法优化氧化葡萄糖杆菌甘油脱氢酶的反应及稳定性条件，以提高二羟基丙酮的发酵浓度.结果表明甘油脱氢酶在pH 4.61，温度33.3 ℃，甘油质量浓度80.0 g • L-1时有最高的酶活性；但该酶仅在pH 4. 0〜5.0很窄范围内及温度低于25 ℃以下时稳定；100 g • L-1甘油或者50 g • L-1 二羟基丙酮对该酶的稳定有利.由于氧化葡萄糖杆菌的生长条件和甘油脱氢酶催化条件差异较大，为提高二羟基丙酮的转化效率，研究了 一种新的二羟基丙酮的转化方法.该方法将二羟基丙酮的发酵过程分成菌体培养期和甘油转化期2个阶段，分别控制其最优条件.实验中利用一个5-L的发酵罐的进行转化，结果显示在136 h的转化过程中甘油脱氢酶的稳定性保持良好，最终发酵液中的二羟基丙酮质量浓度达到286.2 g • L-1.

The melanoma-differentiation-associated gene 5（Mda5）of DF-1 was knocked down using CRISPR/Cas9 technology to explore the effect of the Mda5 gene on the replication of Newcastle disease virus（NDV）and infectious bursal disease virus（IBDV） replication. CRISPR/Cas9 gene editing technology was applied to construct Mda5 knockout DF-1 cell lines, and real-time fluorescence quantitative PCR and immunofluorescence were used to detect the RNA levels of viruses, cytopathic conditions and mRNA levels of antiviralrelated genes after IBDV and NDV infection of Mda5 knockout cells. Mda5 knockout DF-1 cells were obtained ；there were no significant differences in cell morphology, apposition ability and proliferation ability compared to control cells. Compared to control cells, Mda5 knockout DF-1 cells infected with IBDV showed significant down-regulation of IFN-β and PKR expression, no significant differences in CH25H expression and viral replication levels. After infection with NDV, IFN-β expression and viral replication levels were significantly downregulated, CH25H expression was significantly up-regulated, and PKR expression was not significantly different after infection with NDV. In DF-1 while applying CRISPR/Cas9 technology to knockout Mda5, though IFN-β significantly responded to IBDV and NDV infection, failed to significantly inhibit viral replication, suggesting that Mda5 is not a key pattern recognition receptor for antiviral replication.
