精神的あるいは肉体的ストレスに対して.生体がどのように対応しているかをみる指標として,尿中の17-水酸化コルチコステロイド(17-OHCS)(生体の摩耗と断裂)と17-ケトステロイド硫酸抱合体(生体の修復と回復)とを同時に測定することによって把握できる,これらはストレス・バロメーターと呼ばれている.我々は世界で初めてこのストレス・バロメーターの小児の正常値を明らかにし,また小児科領域での各種疾患での臨床応用の可能性についても明らかにした.すなわち,夜尿症では精神的ストレスに関係する二次性夜尿症だけでなく,一次性夜尿症でもストレス・バロメーターが異常を示す症例では抗うつ剤の効果を認めた.また不登校は各種原因で起きるが,いじめが関与しているとストレス・バロメーターは異常を示す.原因が解決され,登校できるようになるとストレス・バロメーターは正常化する.神経性食欲不振症は思春期の女性に特有な疾患である,多くの症例では尿中17-OHCSは低下しているが,逆に高値を示す症例では学校でのいじめや家庭内でのトラブルが継続していることがある.痒みや痛みに対してもストレス・バロメーターは敏感に反応する.このようにストレス・バロメーターを測定することによって,問診ではわからなかったストレスの関与の仕方がわかり,臨床的にストレス・バロメーターを測定することは有用である.

夜尿，吃音等の症状を示す情緒障害児（5才8月の男児）に実施した39回の遊戯療法の過程とその結果についての報告である。患児は，母親の強迫的な性格，厳しいしつけ方，養育態度が異なる同居の5人の成人（父母，祖父母，叔母）による過保護，過干渉などの環境要因がからみあって，情緒障害をきたしたものと考えられ，夜尿と吃音は，患者の精神的緊張や欲求不満に帰因する症状であると推測された事例である。したがって，これらの症状の除去を目的に患者に対して週1回の心理療法と，母親に対する2週に1回のカウンセリングを行うことにした。毎週50分の個別遊戯療法を6カ月（20回）行ったところ，プレイルームにおける患者の行動は，消極的，受動的なものから，抑圧してきた感情を発散し，攻撃行動を経て，自己主張をするようになり，また自信と共に協調性も芽生えてきた。これに伴って，発語の量も多くなり，プレイルーム内での吃音に対する抵抗は消失した。また母親による夜間の排尿訓練を続けさせたところ，夜尿も約3カ月後に殆ど消失した。しかし瀕尿は持続しているようであったので，この時点から母親の介助による夜間の排尿訓練をやめて，代わりに膀胱訓練（キンメル法）を始めさせ排尿の自立を一層促進することにした。患児の場合の夜尿は，二次性の夜尿であり，治療はこの段階から「自発性対罪悪感」の段階への移行を促進するために，21回目から，集団遊戯療法を行うことにした。集団遊戯療法を開始した当初は「引っ込み思案」の状態で，他児との交流は全く希薄であったが，集団遊戯療法を開始して約3カ月が過ぎた頃から，ようやくTh.を煤介としないで他児と交流するようになった。患児は他児や他のThにも話しかけるようになり，縄とびなど自分が得意とする遊びでは自己主張も行うようになった。また積極的に集団の中へ入り込み，集団のなかでも安定してきた。この頃から，患児はプレイルームでの行動を徐々に家庭，幼稚園の順に汎化させていった。吃音は治癒し，よほど緊張する場合を除いては消失した。しかし夜尿は完全には消失しなかった。発症の原因， 自我の発達における攻撃性の役割，道具的条件づけによる排尿訓練の問題点について考察した。, This article is a report of the treatment, by means of both individualand group-play therapy, administered to an emotionally disturbed child with the symptoms of enuresis nocturna and stuttering. The client is a male child (five and eight months old) who has been reared in a family consisting of his parents, grand parents, and his aunt together with his elder sister. The way of rearing this infant by these adult family members was quite different. Since the child was liable to ill until he was 4 years old, the grand parents and his aunt were indulgent to the child. While his father was indifferent to his training, his mother started to severely discipline the child after he became to 4 years old. From that time on, a disagreement of opinions as to the rearing method among his mother and three "substitute mothers" became very evident. His stuttering became severe, and he wet his bed almost every night. The family had not given him toilet training. When the child entered to a kindergarden at the age of 4 and 8 months years, he was completely isolated in the kindergarden. Since the symptoms were conjectured as purely psychogenic and were considered to be caused mental tension and frustration based on a careful study of the case history, we decided to administer psychotherapy to the child (once in a week) and counseling to his mother (once in two weeks). After about one year of psychotherapy (play therapy) and counseling, the stuttering disappeared, the enuresis nocturna has significantly improved and the client has developed an ability for self display. The cause of onset of the symptoms, the role of aggression for the ego-development, and the problems of toilet training by means of instrumental conditioning are discussed.

To clarify the pathophysiology of nocturnal enuresis and to establish its rational classification and treatment, clinical and electroencephalographic studies were made on 404 enuretic children, including an overnight polysomnographic investigation on 55 cases. EEG was normal in only 76 cases(18.8%). Background EEG abnormalities were observed in 226 cases(55.9%). This suggested the presence of brain immaturity or dysmaturity in enuretic children. Epileptic discharges were detected in 94 cases(23.3%), but not concomitant with the enuretic events. The epileptic mechanism may not play a primary role in the manifestation of enuresis. Specific EEG patterns of subcortical origin, i.e., 6 c/s wave-and-spike phantom, 14&6 c/s positive epikes, anterior theta burst, and mu rhythm, were detected in as many as 188 cases(46.5%). This indicates the importance of subcortical brain dysfunction in enuresis. During overnight polysomnographic recordings, 38 enuretic episodes were observed in 30 cases. Twenty eight episodes appeared in light sleep, 8 episodes in deep sleep, but only 2 episodes in REM sleep. According to the arousal response at the time of enuresis, enuretics were properly classified into arousal type(12 cases) and non-arousal type(18 cases). Prognosis was favourable in the cases whose enuresis appeared in light sleep and also in the arousal type enuretics.
