目的探讨术前益生菌鼠李糖乳杆菌（LGG）灌胃对麻醉手术老年小鼠海马区小胶质细胞及Tau磷酸化的影响。方法18月龄C57BL/6J小鼠30只随机分为3组，10只/组：对照组，麻醉手术组，麻醉手术+鼠李糖乳杆菌组。生理盐水/LGG 109 CFU 150 μL灌胃，每日 1 次，连续20 d后接受异氟醚麻醉+剖腹探查手术，术后12 h免疫荧光染色检测海马区小胶质细胞激活状态，ELISA 检测IL-6的浓度变化，Western blot 检测Tau蛋白磷酸化位点Tau-pS202/pT205和total Tau蛋白表达变化。结果对照组海马区小胶质细胞呈静息状态，炎症因子IL-6浓度为（82.08±12.07） pg/mL。与对照组相比，麻醉手术组海马区小胶质细胞活化增生，胞体变大，突起缩短变粗，炎症因子IL-6上升至（123.7±5.72） pg/mL（P=0.000），磷酸化Tau-pS202/pT205蛋白表达量也明显增加（P=0.002）。而与麻醉手术组相比，麻醉手术+LGG组海马区小胶质细胞增生肥大不明显，炎症因子IL-6分泌减少至（96.68±9.59） pg/mL（P=0.008），磷酸化Tau-pS202/pT205蛋白表达量明显下降（P=0.002）。而3组total Tau蛋白表达水平差异无统计学意义。结论术前服用益生菌鼠李糖乳杆菌减轻麻醉手术导致的老年小鼠海马区小胶质细胞活化、炎症因子分泌增加、以及Tau蛋白磷酸化水平增加。

Objective To observe the effect of Tonifying deficiency and Zhengqi porridge on immune function reconstruction of HIV/AIDS patients with Qi deficiency. Methods Sixty (Human Immunodeficiency Virus/ Acquired Immunodeficiency Syndrome)HIV/AIDS patients admitted to the center for Disease Control and prevention in Huichuan District of Zunyi City were randomly divided into control group and treatment group by random number table. All patients received the highly active anti-retroviral therapy, and those in the treatment group were given Tonifying deficiency and Zhengqi porridge. The CD4+ T-cell count was measured before, 3 months and 6 months after intervention. The Effective reconstitution of immune function was analyzed. Results After 3 and 6 months of treatment, the immune reconstitution in the treatment group was better than that in the control group for patients with CD4+ T-cell count of 100-500/mm3 (P＜0. 05). Conclusion Tonifying deficiency and Zhengqi porridge can promote the reconstruction of immune function in HIV / AIDS patients with Qi deficiency and help to restore immune function. (目的 探讨补虚正气粥对人类免疫缺陷病毒(HIV)感染及艾滋病(AIDS)气虚型患者的免疫功能恢复作用。方法 选择遵义市汇川区疾病预防控制中心60名HIV/AIDS患者, 随机将患者分为治疗组和对照组, 各30例。对照组按传统方法进行HAART(高效抗反转录病毒)治疗, 治疗组则在对照组治疗的基础上加用补虚正气粥。于干预前、干预后3个月和干预后6个月比较两组患者CD4+T淋巴细胞计数值和免疫功能重建有效率。结果 对于干预前CD4+T淋巴细胞计数100～500个/mm3的患者, 在介入3个月和6个月, 治疗组免疫功能的恢复较对照组好(P＜0. 05)。结论 补虚正气粥对HIV/AIDS气虚型患者免疫功能重建有促进作用, 有助于免疫功能的重建。)

Hemodialysis patients are prone to hyperphosphatemia in the process of disease progression, which seriously reduce the patient’s quality of life. In clinical treatment and nursing, dietary nursing intervention can improve the prevention awareness of hyperphosphatemia in patients and their families, improve the level of blood phosphorus and nutritional status of patients, reduce the incidence of hyperphosphatemia in hemodialysis patients, and improve their quality of life under a scientific and reasonable diet planning. The purpose of this paper is to summarize the dietary nursing intervention of hyperphosphatemia in hemodialysis patients, summarize the dietary nursing intervention measures, and provide beneficial guidance for the practice of dietary nursing. Objective: To explore significance of eosinophil and eosinophil/lymphocyte ratio （ELR） in predicting allergic asthma in children in different seasons. Methods: Retrospective analysis of children with asthma who visited pediatric respiratory department outpatient clinic and ward of Shandong Provincial Hospital Affiliated to Shandong First Medical University from January 2021 to December 2021, whose allergen specific IgE （sIgE） and peripheral blood cell analysis were complete. sIgE≥0.35 kUA/L were included in allergic asthma group, sIgE<0.35 kUA/L and total allergen IgE<60 kUA/L were included in non-allergic asthma group. General data and changes in peripheral blood cells of two groups were analyzed. Results: There were 1 378 qualified subjeats, including 999（ 72.5%） in allergic asthma group and 379 （27.5%） in non-allergic asthma group. Number of visits in allergic asthma group varied seasonally, with the most in autumn. Peripheral blood lymphocyte count （LYMPH）, eosinophil count （EOS） and ELR were all higher in children with allergic asthma than in children with non-allergic asthma （P<0.05）, and platelet/lymphocyte ratio （PLR） was lower than that in children with non-allergic asthma （P<0.05）. Peripheral blood LYMPH, PLT, EOS and ELR of children with allergic asthma differed between four seasons, which were higher than those of non-allergic asthma in each season in EOS and ELR, LYMPH was significantly higher than that of children with non-allergic asthma in autumn, and PLT was significantly lower than that of children with non-allergic asthma in spring （P<0.05）. EOS predicted AUC of spring, summer, autumn and winter were 0.79, 0.77, 0.71 and 0.64 in children with allergic asthma, and ELR predicted AUC were 0.72, 0.48, 0.73 and 0.68 in children with allergic asthma. Conclusion: Allergic asthma in children is seasonally variable and peaks in autumn. EOS and ELR in peripheral blood cells in children with allergic asthma are higher than in children with non-allergic asthma in each season of year, LYMPH is significantly higher than children with nonallergic asthma in the fall, and PLT is lower than in children with non-allergic asthma in spring, suggesting that allergic asthma type Ⅱ inflammation persists, and EOS and ELR have predictive value for children's allergic asthma. As a cation with abundant intracellular contents and extensive functions, magnesium plays an active role in immune function and captivates widespread attention. Under the influence of multiple factors, such as use of calcineurin inhibitors, hypomagnesemia post-kidney transplantation is not uncommon. Infection is a common complication post-kidney transplantation and one of the main causes of death of kidney transplant recipients. Recent clinical studies have shown that hypomagnesemia post-kidney transplantation is closely associated with the risk of infection posttransplantation. Emphasizing and monitoring magnesium concentration in kidney transplant recipients may help prevent infection and improve clinical prognosis of both recipients and grafts. Therefore, research progress in magnesium and immune response, the causes of hypomagnesemia post-kidney transplantation and hypomagnesemia and infection postkidney transplantation was reviewed, aiming to provide reference for the prevention and treatment of infection post-kidney transplantation. Objective To investigate the effect of self-management-oriented 5A nursing mode on postoperative self-care ability and follow-up compliance of glaucoma patients. Methods A total of 87 glaucoma patients admitted to our hospital from November 2021 to November 2022 were selected as the study objects. They were divided into the observation group(44 cases) and the reference group(43 cases) by random envelope method. The reference group received routine nursing mode, and the observation group received self-management-oriented 5A nursing mode on the basis of routine nursing. Both groups continued to be intervened for six months. The self-management ability [adult health self-management ability scale(AHSMSRS)], sleep quality [Pittsburgh sleep quality index scale(PSQI)], quality of life[quality of life scale for patients with visual impairment(SQOL-DVI)], self-care ability [self-care ability scale(ESCA)], anxiety and depression [Beck anxiety inventory(BAI), Beck depression inventory(BDI)] were compared and analyzed between the two groups before and after follow-up compliance, knowledge mastery, postoperative recovery(visual acuity, intraocular pressure, recurrence rate, complication rate). Results The follow-up compliance rate of the observation group [100.00%(44/44)] was higher than that of the reference group [86. 05%(37/43)], and the difference was statistically significant(χ²=4. 600, P =0.032). The scores of disease, medication, correct eye knowledge and postoperative precautions in the observation group were higher than those in the reference group, and the differences were statistically significant(P <0.001). The recurrence rate of the observation group[2. 27% (1/44)] was lower than that of the reference group[18. 60%(8/43)], and the difference was statistically significant( χ²=4. 617, P =0.032). The incidence of complications in the observation group [4. 55%(2/44)] was lower than that in the reference group[23. 26%(10/43), and the difference was statistically significant(χ²= 6. 403, P =0.011). The improvement of visual acuity and intraocular pressure in the observation group after nursing was greater than that in the reference group, and the differences were statistically significant(P < 0.05). After nursing, the AHSMSRS and ESCA scores of the observation group were higher than those of the reference group, and the differences were statistically significant(P < 0.05). After nursing, the scores of PSQI, SQOL-DVI, BAI and BDI in the observation group were lower than those in the reference group, and the differences were statistically significant(P <0.05). Conclusion Self-management-oriented 5A nursing model can improve the self-management ability and self-care ability of glaucoma patients after surgery, relieve their anxiety and depression, and improve their compliance. Objective To investigate the application of magnetic resonance imaging (MRI) diffusion tensor imaging (DTI) in the assessment of white matter injury (WMI) and neurodevelopmental prognosis of preterm infants. Methods A total of 149 preterm infants were included, all of whom underwent head MRI+DTI examination, and the values of fractional anisotropy (FA) and apparent diffusion coefficient (ADC) were collected. According to the examination results, preterm in‐ fants were divided into the WMI group (40 cases) and nWMI group (109 cases). For further analysis, the WMI and nWMI groups were classified based on postmenstrual age (PMA) during MRI+DTI examination (33-34 weeks group, 35-36 weeks group, and ≥37 weeks group). We compared FA and ADC values between WMI and nWMI groups. The Gesell Develop‐ mental Scale was used to evaluate the development of 39 preterm infants at 6 months of corrected age. Among them, there were 18 cases of adaptive behavioral development retardation, 16 cases of individual-social behavior developmental retarda‐ tion, 21 cases of gross motor performance developmental retardation, 25 cases of fine motor behavior developmental retarda‐ tion and 19 cases of speech behavior developmental retardation. Spearman correlation was used to analyze the correlations between the FA and ADC values and the Gesell Developmental Scale. Results The FA values of each ROI in the nWMI group were higher than those of the WMI group at different corrected age (all P<0. 05). ADC values of periventricular white matter and genu of corpus callosum in the WMI group at the corrected age of 33-34 weeks were higher than those of the nW‐MI group (all P<0. 05). At the corrected age of 35-36 weeks, ADC values in genu of corpus callosum and posterior limbs of internal capsule in the WMI group were higher than those in the nWMI group (all P<0. 05). At the corrected age of ≥37 weeks, ADC values in semiovale center, periventricular white matter, genu of corpus callosum and posterior limbs of internal capsule in the WMI group were higher than those in the nWMI group (all P<0. 05). There was a negative correlation between the ADC value of periventricular white matter and Gesell Developmental Scale score in premature infants with gross motor de‐ velopment delay (r=-0. 594, P<0. 05). The ADC value of the forelimb of the internal capsule was negatively correlated with the Gesell Developmental Scale score in preterm infants with cognitive behavior developmental retardation (r=-0. 524, P< 0. 05). Conclusions FA values in anterior and posterior limbs of internal capsule and paraventricular white matter can re‐ veal WMI in preterm infants. Objective: To investigate the effect of ultrasound-guided high iliac fascia block on blood coagulation in elderly patients with femoral head replacement. Methods: Eighty elderly patients undergoing femoral head replacement under intravenous general anesthesia were selected. The patients were divided into two groups according to the random number table, with 40 cases in each group: the observation group underwent ultrasound-guided high fascia iliac block followed by intravenous general anesthesia 30 min before anesthesia induction; the control group only received intravenous general anesthesia. Venous blood samples were collected at admission (baseline), 1 h and 24 h after operation for thromboelastography analysis. Serum was separated by centrifugation, and the levels of serum cortisol, C-reactive protein (CRP), interleukin (IL)-6 and tumor necrosis factor (TNF)-α were determined. Numerical rating scale (NRS) scores were recorded at 6 h (T1), 24 h (T2) and 48 h (T3) after operation. Postoperative adverse reactions and patient satisfaction scores were recorded. Results: Compared with the control group, the reaction time (R) value of the observation group was increased at 24 h after operation, and maximum amplitude (MA), α and coagulation index (CI) were decreased (P＜0.05). Compared with those at admission, the R value of the control group at 24 hours after operation was decreased, and MA, α, and CI were increased (P＜0.05). Compared with the control group, the contents of cortisol, CRP, IL-6 and TNF-α at 1 h and 24 h after operation in the observation group were decreased (P＜0.05), the NRS scores at rest and during exercise at T1-T3 after operation were lower (P＜0.05), the satisfaction degree was higher (P＜0.05), and the incidence of restlessness, nausea and vomiting during the recovery period was lower (P＜0.05). Conclusion: Ultrasound- guided high fascia iliaca block combined with general anesthesia can reduce the stress response and hypercoagulable state during and after femoral head replacement in elderly patients, and provide better postoperative analgesia.
