设计了10个高强钢T形件的单向拉伸试验,分别研究了螺栓直径、螺栓线距、螺栓端距和钢材强度对T形件受拉力学性能的影响,试验结果表明:在试验参数范围内,T形件共产生螺栓破坏、联合屈服和翼缘屈服三种破坏模式;增大螺栓直径会显著提高T形件的初始刚度、屈服荷载、极限承载力和极限变形;增大螺栓线距有利于提高T形件的变形能力,但初始刚度、屈服荷载和极限承载力会有所降低;增加螺栓端距能小幅提高T形件的屈服荷载、极限承载力和极限位移,但对初始刚度影响不大;钢材强度对T形件的屈服荷载和极限承载力影响较大。另外,在ABAQUS软件中建立了T形件的有限元分析模型,利用10个高强钢T形件和其他学者的试验数据验证了模型的可靠性,最后研究了翼缘厚度、单侧螺栓间距、螺栓规格以及预紧力等参数对高强钢T形件受拉力学性能的影响,参数分析结果表明翼缘厚度是影响T形件受拉力学性能的关键参数,而单侧螺栓间距和螺栓规格的影响较小,预紧力对T形件的初始刚度有较大影响。研究结果可为高强钢T形件的设计施工、规范制定提供数据参考和设计指导。

4. 司美格鲁肽联合二甲双胍治疗2型糖尿病 合并多囊卵巢综合征1例.

- Abstract
报道1例2型糖尿病合并多囊卵巢综合征患者使用司美格鲁肽联合二甲双胍的诊疗过 程。患者为中年女性,主因“多尿、多饮2个月,发现血糖升高20 d”入院。入院后完善相关检查,结果 显示空腹血糖为11.5 mmol/L,糖化血红蛋白为12.5%,新诊断为2型糖尿病,同时合并肥胖、多囊卵巢 综合征、脂肪肝、肝功能异常,予以司美格鲁肽联合二甲双胍治疗。患者血糖控制良好,体重明显下 降,代谢指标改善,胰岛素抵抗较前减轻,月经频率增加,性激素水平有所恢复。司美格鲁肽是一种可 与其他降糖药物联合使用的人胰高糖素样肽‑1受体激动剂。对合并多囊卵巢综合征的患者,司美格 鲁肽联合二甲双胍治疗除了可以降糖、减重,改善胰岛素抵抗外,还有助于改善患者代谢紊乱及生殖 障碍。 [ABSTRACT FROM AUTHOR]

Cassava cultivation is low-cost and high-yielding, and tubers, stems and leaves can be widely utilized in the food medicine and light industry. However, cassava planting is mainly manual work at present. There is an urgent need to develop precision planting machines suitable for the agronomic requirements of cassava planting. A seeder is the core component of a precision planting machine, including the real-time and pre-cut seed-cutting type. Among them, the real-time seed cutting cannot realize the automatic sowing, due to the long and complicated shape of the cassava seed rods, while the low persistence of manual seed feeding and serious leakage. A seed cutter can be used to cut the cassava seed stems into seed stems of about 150 mm in length in pre-cut seeding. An automatic continuous and controlled seeding of cassava seed stems can be achieved through the seed discharge mechanism after cleaning. The lifting type seed rower has been improved to add the gravity seed cleaning mechanism, while the structure of the clamping plate has been optimized for the seed rowing effect. But the leakage filling still exists so far. Some research has been conducted on the seeders, such as slotted wheels, and single roller types. However, it is very necessary to improve the seed filling performance and qualification index, when seeding cassava seed stems with the existing pre-cut seed sower, because the cassava seed stems are cylindrical woody stalks of a certain length with surfaces of complex physical characteristics. Particularly, the performance of cassava precision seeding needs to be improved, due to a complex process with the multi-factor change. This study aims to improve the seed filling effect and low qualification index of pre-cut seeding type cassava precision seeder. A precision cassava seeding mechanism was designed with pre-cut seeding and friction belt, consisting of seed drop slide, active roller, type hole friction belt, support roller group, driven roller, and seed storage box. The basic structure and working principle of the seed-rowing mechanism were described to determine the parameters of key components. The main factors were then determined with the seed-filling performance of the seeding mechanism, according to the shape, number, installation inclination, and speed of the typed hole on the friction belt, as well as the thickness of the seed stem layer. The discrete element method (DEM) was used to establish a simulation model of the "seed stem group-hole friction zone". The single-factor simulation was realized to clarify the influence of each factor on the seed-filling performance. The optimal combination of factors and parameters was determined after quadratic regression and orthogonal rotational simulation. Taking the installation inclination angle of the seeding hole friction zone, the thickness of the seeding stem layer, and the speed of the seeding hole friction zone as the factors, the mathematical regression model of each factor was established for the evaluation index. The results showed that the influencing factors of the seed filling index and the leakage index were ranked in descending order of the speed of the type hole friction belt, the installation angle of the type hole friction belt, and the thickness of the seed stem layer. The optimal combination of factors and parameters was achieved in the bench test. The seed filling qualification index was 94.13% for the precut cassava-type friction belt precision seeding mechanism, and the leakage index was 3.77% when the thickness of the seed stem layer was rounded to 220-280 mm, the speed of the friction belt was 0.6 m/s, the installation inclination of the friction belt was 45°, the shape of the friction belt was C-shaped and the number of holes was 12. A better performance was achieved, where the seed filling index and the leakage index were 94.13% and 3.77%, respectively. 8. 2型糖尿病合并暴发性1型糖尿病一例 并文献复习:双重糖尿病?.

- Abstract
Objective To investigate and compare clinical features between type 2 diabetes mellitus (T2DM) combined with fulminant type 1 diabetes mellitus (FT1DM) and simple FT1DM. Methods A case of T2DM combined with FT1DM was reported, and the clinical features of T2DM combined with FT1DM were summarized and analyzed. The clinical characteristics between 21 T2DM combined with FT1DM patients and 161 simple FT1DM patients from Japan and 70 simple FT1DM patients from China were compared, and a t test was used for the comparisons. Results (1) Most patients with T2DM combined with FT1DM were male (male∶female was 14∶7) and old. The course of FT1DM was (4.4±1.7) days. All patients had negative islet autoantibodies. (2) Compared with simple FT1DM from Japan, patients with T2DM combined with FT1DM were older [(60.1±11.8) vs (39.1±15.7) years, P<0.001], had higher glycated hemoglobin A1c (HbA1c) levels (8.2%±1.6% vs 6.4%±0.9%, P<0.001), and were more obese [body mass index (BMI) was (23.7±5.7) vs (20.7±3.9) kg/m², P=0.032]. (3) Compared with domestic FT1DM, the difference was similar, patients with T2DM combined with FT1DM were older[(60.1±11.8) vs (31.7±11.4) years, P<0.001], had higher HbA1c levels (8.2%±1.6% vs 6.8%±1.1%, P<0.001), and had longer course of disease[(4.4±1.7) vs (3.1±2.4) d, P=0.032], but there was no statistically significant difference in BMI [(23.7±5.7) vs (21.1±3.7) kg/m², P=0.068]. Conclusion Double diabetes can be manifested as T2DM combined with FT1DM, which is characterized by a longer course of disease, older age, higher BMI and HbA1c level compared with simple FT1DM. 9. 特纳综合征患者糖代谢异常的特点及其与染色体核型之间的关系.

- Abstract
Objective To investigate the glucose metabolism disorder status and clinical features in patients with Turner syndrome (TS). Methods A total of 61 patients with TS was included in this study. Oral glucose tolerance test (OGTT), glycated hemoglobin A1c (HbA1c) and autoimmune antibodies, including glutamic acid decarboxylase antibody (GADA), islet cell antibodies (ICA) and islet cell antigen 2 antibody (IA-2A) were measured in these patients. The t test and Logistic regression were used for statistical analysis. Results (1) The mean age of all the patients was (15.2±4.9) years, and mean body mass index (BMI) was (20.5±3.5) kg/m2. (2) HbA1c was 5.4%±1.4%. About 31.5% (17/54) of the patients had abnormal glucose metabolism: 4 patients were diagnosed as diabetes, 4 cases were isolated impaired fasting glucose (IFG), 6 cases were impaired glucose tolerance (IGT), and 3 cases were combined IFG and IGT. Compared to healthy female adolescents (n=40), patients with TS had higher homeostasis model assessment of insulin resistance (HOMA-IR) (3.4±2.7 vs 2.0±0.7, t=3.321, P=0.001), larger areas under glucose curve [(22.3±8.8) vs (17.7±2.4) mmol·h/L, t=2.339, P=0.022] and insulin curve [(284±178) vs (175±86) mU·h/L, t=3.533, P= 0.001] during OGTT. Only one patient had one test positive for IA-2A while her glucose tolerance test was normal. (3) Patients were classified into three subgroups according to their karyotypes: 45, XO group (n=23); isoXq group (n=17), including patients with 46X, i(X)(q) and 45, XO/46X, i(X)(q); and other karyotype type group (n=14). The percentage of abnormal glucose metabolism was 47.8% (11/23), 23.5% (4/17), and 14.3% (2/14) in three groups, respectively. There was no statistical significance among the three groups (χ2=3.764, P>0.05). Conclusion Compared to healthy female adolescents, young patients with TS have more severe insulin resistance and higher risks for abnormal glucose metabolism. Patients with 45, XO karyotype have higher inclination for abnormal glucose metabolism than patients with other karyotypes. [ABSTRACT FROM AUTHOR]
