BACKGROUND: Previous studies on the effects of valgus and varus angles of tibial component on short-term postoperative outcomes after mobile bearing unicompartmental knee arthroplasty have been reported in and outside China. However, there are few reports on the effect of the valgus and varus angles of tibial component on short-term postoperative outcomes after fixed bearing unicompartmental knee arthroplasty. OBJECTIVE: To investigate the effect of valgus and varus angles of tibial component on short-term clinical outcomes in patients with medial knee osteoarthritis undergoing fixed bearing unicompartmental knee arthroplasty. METHODS: 120 patients (122 knees) who underwent fixed bearing unicompartmental knee arthroplasty for medial knee osteoarthritis in Department of Orthopedic Surgery, Affiliated Hospital of Xuzhou Medical University from August 2020 to January 2023 were selected as the study subjects. Two physicians measured the varus angle of femoral prosthesis, valgus and varus angles of tibial prosthesis, flexion and extension angles of femoral prosthesis, and posterior inclination angle of tibial prosthesis after unicompartmental knee arthroplasty based on postoperative X-ray. After excluding the influence of the other three angles, the measurement results of the tibial component varus angle were divided into three groups: <-2°,-2° to 2°, >2°, which were denoted as groups 1, 2, and 3, respectively. The range of knee motion, the hospital for special surgery knee score, the American knee society score, and forgotten joint score were recorded and compared before and after the operation. RESULTS AND CONCLUSION: (1) A total of 120 patients (122 knees) were enrolled in this study. They were divided into three groups according to the size of the valgus and varus angles of the tibial prosthesis after operation: 37 patients in the first group, 60 patients in the second group, and 23 patients in the third group. There was no significant difference between the three groups in terms of baseline information such as age, gender, and side of surgery (P > 0.05). (2) Patients were followed up for 3-30 months after arthroplasty. (3) The hospital for special surgery knee score of the second group was higher than that of the first group (P=0.015) and the third group (P=0.012). The American knee society score of the second group was significantly higher than that of the first group (P=0.014) and the third group (P < 0.001). The forgotten joint score of the second group was higher than that of the first group (P=0.033) and the third group (P=0.016). BACKGROUND: Thromboelastography plays an important role in identifying the hypercoagulable state of blood and thrombosis in humans. Recent studies have shown a correlation between an increase in mean platelet volume and thrombosis. We can therefore ask whether the combined diagnosis of thromboelastography and mean platelet volume is a more accurate predictor of thrombosis. OBJECTIVE: To predict the status of blood and the occurrence of thrombosis after total knee arthroplasty by means of mean platelet volume combined with thromboelastography. METHODS: One hundred and twenty patients who underwent unilateral total knee arthroplasty between May 2015 and March 2022 were collected. Patients were divided into 60 patients in the thrombosis group and 60 patients in the control group based on ultrasound findings on postoperative day 7. Whole blood cell and thromboelastography were performed 1 day before, 1 and 7 days after surgery, respectively. Multifactorial analysis was used to investigate independent predictors of thrombosis after total knee arthroplasty. The receiver operating characteristic curve and area under the curve were measured in the subjects. RESULTS AND CONCLUSION: Mean platelet volume correlated most strongly with maximum amplitude, followed by coagulation angle. Mean platelet volume and coagulation angle on postoperative day 1 were independent predictors of thrombosis. Mean platelet volume tended to rise and then fall in patients with thrombosis. The best critical value for mean platelet volume to predict thrombosis was 10.73 fL. The area under the receiver operating characteristic curve of subjects was 0.665 (95%CI: 0.568-0.762, P < 0.05]; whereas the area under the receiver operating characteristic curve for subjects using mean platelet volume combined with coagulation angle was 0.815 (95%CI: 0.750-0.879, P < 0.05). In addition, the maximum amplitude, coagulation angle, coagulation index and mean platelet volume were significantly higher in the thrombosis group than in the control group postoperatively (P < 0.05). The results suggest that the mean platelet volume can reflect the hypercoagulable state of blood after surgery, and the combination of mean platelet volume and coagulation angle on day 1 after total knee arthroplasty can improve the prediction of thrombosis. At present, chemical science is facing unprecedented opportunities and challenges due to the technological changes brought by artificial intelligence. In order to promote the paradigm shift in chemical research, this study proposes the construction plan of the robotic AI-chemist cloud facility. This system realizes a new paradigm of scientific research by collecting multi-channel data to build a database, developing large scientific models enhanced by chemical knowledge, constructing clusters of robotic facilities, and building an intelligent management decision system, which will dramatically improve the efficiency of scientific research and solve scientific problems in terminal applications. BACKGROUND: The probability of meniscal tears is gradually increasing. However, the relationship between meniscal tears and the anatomical morphology of the tibial plateau has been poorly studied and controversial so far. OBJECTIVE: To measure the posterior tibial slope, medial tibial plateau depth, and lateral tibial plateau height in patients with meniscal tears and to investigate their association with meniscal tears. METHODS: From September 2019 to June 2022, 164 patients with meniscal tears and undergoing arthroscopic treatment were selected from Affiliated Hospital of Xuzhou Medical University. These patients were divided into medial meniscal tears group (n=92), lateral meniscal tears group (n=45) and medial and lateral meniscal tears group (n=27). Totally 78 patients with knee pain without meniscal tears during the same period were selected as the control group. The medial tibial slope, the lateral tibial slope, the medial tibial plateau depth and the lateral tibial plateau height in each group were carefully measured and compared. RESULTS AND CONCLUSION: (1) The medial tibial slope in the medial meniscal tears group was significantly steeper than that in the control group (P < 0.05), and the lateral tibial slope in the lateral meniscal tears group was steeper than that in the medial meniscal tears group and the control group (P < 0.05). The medial tibial slope of the medial meniscal tears group and the control group was significantly greater than their lateral tibial slope (P < 0.05). The medial tibial plateau depth in the medial meniscal tears group was significantly shallower than that in the lateral meniscal tears group and control group (P < 0.05), and the lateral tibial plateau height in the lateral meniscal tears group was higher than that in the medial meniscal tears group and control group (P < 0.05). (2) These results indicate that steep posterior tibial slope is a risk factor for meniscal tears. BACKGROUND: The technique of oblique lumbar interbody fusion has become increasingly mature, but when pedicle screw internal fixation is performed through posterior approach, the fluoroscopy-assisted pedicle screw insertion using a traditional open technique still brings greater trauma to the patient. With the advancement of technology, robot-assisted surgery systems have provided clinicians with new options. OBJECTIVE: To explore the clinical outcomes of percutaneous robot-assisted minimally invasive pedicle screw insertion in an oblique lumbar interbody fusion procedure. METHODS: Totally 78 patients with lumbar degenerative disease who attended Department of Spinal Surgery, Beijing Jishuitan Hospital were treated with oblique lumbar interbody fusion. When lumbar pedicle screw internal fixation was performed through posterior approach, 36 cases (trial group) underwent percutaneous robot-assisted minimally invasive pedicle screw insertion, and 42 cases (control group) underwent fluoroscopy-assisted open invasive screw insertion. The difference in operation time, visual analogue scale score, Oswestry disability index, intraoperative and postoperative blood loss was compared between the two groups. RESULTS AND CONCLUSION: The operative time of trial group was shorter than that of control group (P < 0.05). The postoperative visual analogue scale score of the trial group was lower than that of the control group (P < 0.05). The amount of intraoperative and postoperative blood loss in the trial group was less than that in the control group (P < 0.05). It is concluded that robot-assisted pedicle screw insertion in oblique lumbar interbody fusion can shorten the operative time, reduce intraoperative blood loss, lessen risk of surgical complications, and reduce the soft tissue damage of lumbar spine. BACKGROUND: For patients with Crowe II and III developmental dysplasia of hip combined with severe hip osteoarthritis, total hip arthroplasty is still controversial whether to choose acetabular anatomical reconstruction or high hip rotation center reconstruction. OBJECTIVE: To compare the differences in gait characteristics between the reconstruction technique of the high hip center and the anatomical hip center during total hip arthroplasty. METHODS: Forty patients with Crowe II-III developmental dysplasia of hip with unilateral hip disease from January 2014 to July 2017 were selected. All patients received total hip arthroplasty, of which 20 cases underwent acetabular anatomical reconstruction, and another 20 patients underwent high hip center reconstruction. The patients were followed up for more than 2 years after arthroplasty for gait analysis. The study was approved by Ethics Committee of Affiliated Hospital of Xuzhou Medical University. RESULTS AND CONCLUSION: (1) Spatiotemporal parameters: The pace, stride frequency, step length, single leg support time, single leg swing time, and double leg support time of the two groups of surgical side and non-surgical side were similar. There was no significant difference in pace, stride frequency, step length, one-leg support time, one-leg swing time, and two-leg support time between the two groups (P > 0.05). (2) Kinematic parameters: The maximum extension of the hip joint on the surgical side of the high reconstruction group was lower than that on the non-surgical side (P < 0.01). The maximum hip flexion and hip flexion and extension range of the two groups on the surgical side were lower than those on the non-surgical side (P=0.01). The maximum extension of the hip joint and hip flexion and extension range in the high reconstruction group were lower than those in the anatomical reconstruction group (P < 0.05). The maximum flexion of the hip joint, maximum knee flexion, knee flexion and extension range, and maximum dorsiflexion of the ankle, the maximum plantar flexion of the ankle, and the range of ankle dorsiflexion were not significantly different between the two groups (P > 0.05). (3) Kinetic parameters: The longitudinal peak of the plantar reaction force on the surgical side of the high reconstruction group was higher than that of the anatomical reconstruction group (P < 0.05). There was no significant difference between the internal and external peaks of the plantar reaction force and the front and back peaks of the two groups on the surgical side (P > 0.05). (4) The results show that in patients with developmental dysplasia of hip, total hip arthroplasty using high hip center reconstruction can obtain gait parameters similar to anatomical hip center reconstruction. However, compared with patients undergoing anatomical hip rotation center reconstruction, patients with high hip center reconstruction had limited hip flexion and extension range movement and greater longitudinal force on the hip. BACKGROUND: It has been controversial whether the early analgesic effect and patient satisfaction of cocktail periarticular injection in total knee arthroplasty are better than femoral nerve block. OBJECTIVE: To compare the early clinical effect with cocktail periarticular injection and femoral nerve block in total knee arthroplasty. METHODS: From March to December 2019, 70 cases with chronic knee osteoarthritis were collected from Affiliated Hospital of Xuzhou Medical University. The patients were randomly divided into two groups. In the observation group (n=35), patients received total knee arthroplasty. Cocktail solution was injected into the posterior capsule, bilateral collateral ligament areas, and the subcutaneous tissue of the incision. After the incision was closed, cocktail solution was injected into the drainage tube. In the control group (n=35), patients received total knee arthroplasty. The femoral nerve block was performed after the incision was closed. Postoperative visual analogue scale scores for pain, knee active activity and analgesic satisfaction of patients were assessed within 14 days after surgery. The study was approved by ethics committee of Affiliated Hospital of Xuzhou Medical University. RESULTS AND CONCLUSION: (1) At 2, 3 and 5 days postoperatively, the visual analogue scale pain scores of the observation group were significantly lower than those of the control group (P < 0.05); there was no significant difference at other time points (P > 0.05). (2) The knee flexion of observation group was statistically greater than control group at 2 and 3 days postoperatively (P < 0.05); there was no significant difference at other time points (P > 0.05). (3) Totally 23 of 35 patients in the observation group were satisfied with the analgesic effect and 10 of the 35 patients in the control group were satisfied with the analgesic effect. The differences between the groups were significant (P < 0.05). (4) Acute analgesia was observed 20 times in the observation group after operation, and 36 times in the control group. (5) The results showed that cocktail periarticular injection analgesia was superior to femoral nerve block analgesia in knee arthroplasty, which could improve the active motion and satisfaction of the knee in the early postoperative period. Halogenated hydrocarbon organic tracer is a kind of chemical tracer, which has the characteristics of less adsorption on the bottom surface and easy to be detected. In this resear ch, gas chromatography-mass spectrometry (GC/MS) was applied to analyse p-fl uoro bro mo benzene, bromobenzene, bromocyclohexane, p-dichlorobenzene, m-dichlorobenzene, o-dichlorobenzene, p -bromochlorobenzene, 1, 3, 5-trichlorobenzene, 1, 2, 4-trichloro benzene, p-di bromobenzene, 1, 2, 3-trichloro benzene and bromododecane as t welve halogenated hydrocarbon organic petroleum tracer. The separation was performed on a Kromasil HP-5 column (phenyl met hyl siloxane, 60 m X 320 µm X O. 25 µm). The carrier gas (N2 ) was at a flow rate of 1. 0 mL/min. Quantitative analysis of halogenated hydrocarbon organic petroleum tracer was carried out by electron ionizatio n (El) source and selective ion monitoring (SIM) mode, the sample inject volume was 2 µL, the ion source temperature was 260 °C and the auxiliary heating temperature was 280 °C. The results showed that twelve halogenated hydrocarbon organic petroleum tracer had good linear relationship in the range of 10-10 000 µg/L, the limit detection was 10-4 µg/L, and the standard addition recovery rate was less than 3% . This method has good precision, sensitive, simple and rapid pretreatment, which is suit able for determine the content of petroleum tracer, also can provide a technical way for petroleum tracer detection in the process of petroleum development. BACKGROUND: Total hip arthroplasty has been widely used in clinical practice, and achieved good clinical efficacy. However, with the yearly increased number of patients receiving primary arthroplasty, the number of revision hip arthroplasty patients has increased. However, hip arthroplasty is one of the most difficult hip surgery, and it is challenging. OBJECTIVE: To review the causes of femoral revision of the hip arthroplasty, the classification of the femoral bone defect, and the type of prosthesis available in the clinical revision, and the clinical effect of the different prosthesis, so as to provide reference for femoral revision. METHODS: CNKI, Web of Science, and PubMed were searched for the literatures published between January 1975 and August 2017 with the key words of “Revision total hip arthroplasty, Femur revision, Cemented femoral revision, Cemenless femoral revision Reoperation, Porous-coated stems, Modular, tapered stem, Wagner SL revision Stem”. RESULTS AND CONCLUSION: (1) A total of 1 041 articles were retrieved. After reading the abstract, unrelated and repeated studies were excluded. Finally, 109 studies were included. Of them, there were 106 English studies and 3 Chinese studies. Femoral revision was analyzed and summarized. (2) Femoral revision of the hip is still the future challenges facing the joint surgeon. We continue to strive to overcome the direction. Although the current material science and surgical technology are now rapidly developed, only the analysis of the reasons for the revision of the femoral side, identifying the femoral side bone defect classification, mastering the treatment of different defects and the application of the principle of prosthesis type, making reasonable femoral revision program is the key to success according to the patient's age and activity.
