It has been shown that the in situ hybridization (ISH)method for the detection of phagocyted bacteria in polymorphonuclear neutrophils is more sensitive than conventional blood culture in patients with sepsis.The present study was designed to further evaluate the clinical utility of the ISH method, using a commercially available kit (Hybrizep(R)), for detecting pathogens in patients with severe respiratory infections. Peripheral blood was taken from patients with severe respiratory infections for both routine blood culture and the ISH method,respectively.In addition,focal samples including sputum,bronchoalveolar lavage,central and thoracic catheter, etc. were simultaneously examined for bacterial culture. A total number of 22 specimens was examined.The positive cases by ISH were 50.0% in the respiratory infections,which were significantly higher than those in blood cultures (9.1%).However, identical pathogens in the ISH method were not isolated from blood and/or other sources in respiratory sites. These findings suggest a possibility of high frequency of bacteremia or multifactorious pathogens in severe respiratory infections.The ISH method may provide additional information on serious respiratory infections for the detection of bacteremia. 金沢大学医薬保健研究域医学系, 症例は66歳,女性.主訴はチアノーゼ.他疾患治療中にチアノーゼを指摘され当院紹介となった.胸部造影CT検査では右下肺静脈へ直接流入する右A^8に加え右S^6胸膜直下に拡張した動静脈を認めた.肺動脈造影検査では右A^8から左房へのシャント血流を認めた.肺血流シンチでは肺外臓器への核種分布が確認され,右左シャントの存在は明らかでありシャント率は42%と計算された.以上より多発肺動静脈瘻と診断し右下葉切除術を行った.術前に45.0mmHgであった動脈血酸素分圧が術後には90.6mmHgと著明なる改善を認めるなど術後経過は良好であり,19病日に退院した. The patient was a 66-year-old female. In the course of therapy for compression fracture, cyanosis was pointed out. Chest CT scan revealed abnormal vessels in the right S^6 sub-pleura and the direct connection between the right A^8 and right inferior pulmonary vein. Pulmonary angiography showed shunt flow into the atrium. Perfusion scintigraphy revealed extrapulmonary activity due to the right-to-left shunt and a high-level shunt ratio. The diagnosis of pulmonary arterio-venous fistula was made on the basis of these findings. A right lower lobectomy was performed via video-assisted thoracic surgery. Severe hypoxemia was improved after surgery. The postoperative course was uneventful, and the patient was discharged on the 19^ POD.

慢性心不全(CHF)患者の重症度による夜間の酸素飽和度(SaO_2)と無呼吸の比較について検討を行うために安定した慢性軽症心不全患者6例(男4例,女2例63±5.2歳,左室駆出率:49.8±3.4%,NYHA class:IかII,mild-CHF群)と安定慢性重症心不全患者11例(男9例,女2例,62±11.9歳,左室駆出率:25.6±8.6%,NYHA class:III,severe-CHF群)患者を対象とし,両群を比較することにより検討を行った.全例において室内空気下にパルスオキシメーターを用いて24時間のSaO_2と脈拍数を連続記録した.そして夜間のSaO_2が3%あるいは4%以上低下した1時間あたりの回数(3%ODI,4%ODI),SaO_2の最低値を各々分析した.さらにポリソムノグラフィーを用いてSaO_2と睡眠に関する全てのデータを連続的に記録分析した.その結果,severe-CHF群のODIはmild-CHF群に比して有意に高値であった(4%ODI;5.8±5.1 vs 0.6±0.5,p, Background : Patients with chronic heart failure (CHF) commonly experience Cheyne- Stokes respiration, central apnea, or obstructive apnea during sleep associated with oxygen desaturation. Nocturnal oxygen therapy and nasal continuous positive airway pressure (NCPAP) reduce sleep-disordered breathing in stable CHF. However, the relation between sleep apnea and nocturnal desaturation inpatients with severe, stable CHF in Japan is unknown. Objectives : To examine nocturnal oxygen saturation (SaO_2) and sleep apnea in Japanese patients with severe, stable CHF. Methods : The subjects were 11 patients with severe, stable CHF (9 men and 2 women, LVEF=25.6±8.6%, NYHA class=III, severe CHF group) and 6 with mild, stable CHF (4 men and 2 women, LVEF=49.8±3.4 %, NYHA class=I or II, mild CHF group). SaO_2 was continuously recorded with a pulse oximeter under room air, and 4 % and 3 % SaO_2 dip rate per hour (GDIs) and the SaO_2 nadir were analyzed. 人工骨頭置換術患者16名において周術期の動脈血血液ガスの変化を測定した。手術は硬膜外麻酔で行い、ルームエア自発呼吸下での血液ガスを、術前から術後2日目にかけて8回測定した。また、Pa02に対する骨セメン卜の関与について調べるため、骨セメントを使用した群(セメント群n= 8)としなかった群(セメントレス群n=8)に患者を分けてデータを比較した。その結果、両群とも術後数時間後からPa02が低下し始め、その傾向はセメント群で著しいことが示された。また、セメン卜群ではその回復が遅延した。PaC02は両群とも術後は上昇を認めなかった。遅発性低酸素血症の原因としては、骨髄内操作により血管内に流入した骨髄脂肪による肺塞栓、あるいは凝固系の賦活による微少血栓が疑われる。セメン卜群ではステム打ち込み時に骨髄内圧が上昇するため塞栓が多量となり、低酸素血症が重症化したものと思われる。

クラーレ化,人工呼吸(25回/min)下のネコ(体重2.5～4kg)を用い,左室心尖部付近の心筋中間層に,ワイヤー型交叉熱電対を刺入して,心筋局所血流量(以下心筋血流とする)を測定し,平均血圧(以下血圧とする)とともに持続的に記録した。実験操作は,1)水圧バルーンによる左冠動脈前下行枝(LAD)の段階的圧迫,2)1分間の呼吸停止,過呼吸(50回/min),100%N_2吸入,及び100% O_2吸入,3)adrenaline(Ad,0.01-10μg/kg),noradrenaline(NA,0.01-10μg/kg),isoproterenol(IPN,0.005-5μg/kg),papaverine(Pap,0.01-2mg/kg),nitroglycerin(NG,0.1-100μg/kg),diltiazem(Dil,0.1-200μg/kg),及びdipyridamole(Dipy,0.01-5mg/kg)の静注を行った。LADの圧迫により心筋血流は減少し,血圧は変化しなかった。バルーンの水圧を調節するこことにより数段階の血流変化をもたらすことが出来,LADの完全閉塞後にもある程度の心筋血流が観察された。呼吸停止とN_2吸入は,心筋血流増加と血圧上昇をもたらしたが,N_2吸入による変化のほうが著明であった。過呼吸とO_2吸入は心筋血流減少と血圧下降を起こしたが,O_2吸入による変化は軽度であった。AdとNAは0.02μg/kgより心筋血流増加と血圧上昇を,IPNは0.005μg/kgより心筋血流増加と血圧下降を起こした。この所見ならびにα及びβ遮断薬を用いた結果から,心筋血流変化には少なくともadrenergic-β型の血流増加が見られることがわかった。Papの0.05mg/kg,NGの0.5μg/kg,Dilの5μg/kg,Dipyの0.1mg/kg以上で心筋血流増加と血圧下降が見られた。とくに,Dipyの心筋血流増加作用は著明で持続も長く,血圧下降が軽度であることから,その冠血管選択性が示唆された。, Myocardial blood flow (BF) was measured by the thermoelectrical method in curarized cats (2.5-4.0kg) under controlled ventilation (25/min).
