Abstract Background The effect of long‐standing prediabetes or its transition on incident cardiovascular disease (CVD) is unclear. This study aimed to evaluate the association of changes in fasting blood glucose (FBG) status with the risk of developing CVD. Methods This research included 12 145 Chinese adults aged 35–74 years and free from diabetes mellitus (DM) at baseline. Study participants were cross‐classified into six categories according to glucose at the first (1998–2001) and the second visit after 8 years: normal fasting glucose (NFG; 50–99 mg/dl), impaired FBG (IFG; 100–125 mg/dl), and DM. Cox proportional hazard regression model was used to estimate the hazard ratio (HR) and 95% confidence interval (CI) for CVD associated with transition of glucose status. Results During a median follow‐up of 5.5 years, 373 incident CVD cases occurred. Compared with participants remaining persistent NFG, a higher risk of developing CVD was identified among those remaining persistent IFG, progressing to DM from NFG or from IFG, with the multivariate‐adjusted HR (95% CI) of 1.792 (1.141, 2.816), 1.723 (1.122, 2.645) and 1.946 (1.120, 3.381), respectively. Furthermore, when stratified by glucose status at baseline, persistent IFG and progression from IFG to DM still increased CVD risk in comparison with reversion from IFG to NFG, with the multivariate‐adjusted HR (95% CI) of 1.594 (1.003, 2.532) and 1.913 (1.080, 3.389). Conclusions Participants with long‐standing IFG and progressing to DM had a higher risk of developing CVD. Further well‐designed studies are warranted to assess the association of other phenotypes or prediabetes duration with CVD.

Background: This study aimed to evaluate the discriminative abilities of glycosylated hemoglobin (HbA1c) and to examine the optimal HbA1c cutoff values for diabetes and prediabetes in Chinese adults. Methods: Data of a population‐based cohort of Chinese adults aged ≥40 years living in Jiading District in Shanghai were used. At baseline, 9389 and 7241 participants were included to identify the optimal HbA1c cutoff values for diabetes and prediabetes, respectively using the 1999 World Health Organization criteria as reference. In addition, the follow‐up data on incident diabetes of 4538 participants were used to determine the HbA1c cutoff value for prediabetes using the development of diabetes as reference. The discriminative abilities of HbA1c were evaluated using receiver operating characteristic (ROC) curves, and the optimal cutoff values were determined by Youden's index. Results: The areas under the ROC curves were 0.849 for diabetes, 0.614 for prediabetes using baseline data, and 0.648 for prediabetes using follow‐up data. An HbA1c cutoff value of 6.0% had the largest Youden's index to diagnose diabetes with a sensitivity of 70.2% and a specificity of 87.4%. An HbA1c cutoff value of 5.6% was indicated for prediabetes using both baseline and follow‐up data. However, the sensitivity and specificity were both low (55.4% and 61.1% using an oral glucose tolerance test as reference, 64.6% and 57.1% using incident diabetes as reference). Conclusions: An HbA1c value ≥6.0% could be used to detect diabetes in Chinese adults aged ≥40 years. However, although an HbA1c value of 5.6% to 5.9% was indicated in this study, the overall discrimination of HbA1c for prediabetes was poor. [ABSTRACT FROM AUTHOR]

Background: Pioglitazone may have a protective effect against cardiovascular disease risk among type 2 diabetes (T2D) patients, but evidence from China is lacking. This study investigated the association using electronic health records (EHR) data from a Chinese cohort of T2D patients. Methods: All T2D patients from the First Affiliated Hospital of Nanjing Medical University who were prescribed at least one oral antidiabetic drug and were aged ≥18 years between 1 July 2005 and 30 June 2017 were eligible for inclusion (n = 71 783). Pioglitazone use was determined in 6‐month study intervals, with outcome events of myocardial infarction (MI), ischemic stroke, and heart failure. Poisson regression was used to estimate adjusted rate ratios (RRs) with 95% confidence intervals (CIs). Results: In multivariable analysis adjusted for potential confounders, pioglitazone use, compared with no use, was associated with a significant 39% decreased risk of MI (RR = 0.61; 95% CI = 0.42‐0.90; P = 0.012). Pioglitazone use was also associated with a non‐significant reduction in risk of heart failure or stroke. When MI, heart failure, and stroke were combined as a composite outcome, pioglitazone use was associated with a 30% decrease in risk (RR = 0.70; 95% CI = 0.56‐0.88; P = 0.002). Conclusions: This study demonstrates that applying informatics tools to a large EHR database could be a good way to efficiently conduct clinical observational research. In addition, the findings validated the favorable effect of pioglitazone on the risk of MI among T2D patients in China, with the use of pioglitazone decreasing the risk of MI among those with T2D. Objective:To survey and analyze the current situation of national mental health in China and to explore the characteristics of national mental health in China by comparing the mental symptoms status before and after 30 years thus provide scientific basis for promoting the national health situation in China. Methods: A total of 12160 residents from 12 provinces and municipalities were selected randomly by using cluster sampling, and their mental health status was measured with the SCL-90 symptom checklist. Results: The average scores of each factor of SCL-90 in China were between 1.23 and 1.66, and no significant peaks were found. The scores of obsessive symptoms, interpersonal relationships, depression, anxiety, hostility, paranoia and psychotic factors were higher in 2015 than in 1986. Except for the paranoid and psychotic factors (P> 0. 05), the scores of other factors were higher in women than in men (P.s <0. 05). The scores of factors in the age 18 - 44-year group were the highest. Conclusion: The overall mental health state of Chinese population SCL-90 is relatively stable after the update of norm, while the female mental health problems are increasingly prominent, and the mental health problems are the most serious among the youth group. Abstract: Background: Diagnostic criteria for metabolic syndrome (MS) proposed by the Chinese Diabetes Society (CDS) may have limitations because an additional 2‐h postprandial plasma glucose (2‐h PPG) test is required to diagnose hyperglycemia. The aim of this study was to revise the CDS‐MS criteria by removing the 2‐h PPG test and determining the optimal fasting plasma glucose (FPG) cut‐off for a diagnosis of hyperglycemia in the Chinese population. Methods: The study population was from the 2007–08 China Diabetes and Metabolic Disorders Study. The optimal FPG diagnostic cut‐off value was determined using receiver operating characteristic curves. Hyperglycemia defined using CDS‐MS criteria was the end‐point. Agreement between different diagnostic methods was assessed using κ values. Results: The study enrolled 46 239 participants (mean age 44.95 years). The optimal diagnostic cut‐off for FPG was found to be 5.62 mmol/L, which showed a sensitivity of 66.92%, a specificity of 89.09%, and an area under the curve of 0.848. Using this diagnostic criterion, the standardized prevalence of MS was similar to that using the CDS‐MS criteria (18.26% vs 17.89%, respectively), with both values being lower than those obtained using conventional international criteria (20.64–26.67%). Compared with the CDS‐MS criteria, the recommended FPG cut‐off showed better agreement (κ) with international criteria: 0.695, 0.774, and 0.730 with the International Diabetes Federation (IDF), revised Adult Treatment Panel III, and Joint Interim Statement of the IDF criteria, respectively. Conclusions: We recommend eliminating the 2‐h PPG blood test and lowering the FPG diagnostic cut‐off value to 5.6 mmol/L in the CDS‐MS diagnostic criteria. [ABSTRACT FROM AUTHOR]
