看護学生を対象に腰痛と精神的ストレスの実態をアンケートにより調査した．腰痛の頻度は49.2％であり，精神的ストレスを自覚する者は48.1％であった（オッズ比：2.46， p ＜ 0.01）. JOABPEQ（Japanese Orthopedic Association Back Pain Evaluation Questionnaire）における心理的障害の項目は腰痛の程度（Visual Analogue Scale: VAS）を反映した（r=－0.290, p ＜ 0.05）．またState－Trait Anxiety Inventory （STAI） における状態不安の評価は平均43.4 ± 9.1 点，特性不安は平均48.8 ± 9.5 点，Self－Rated Questionnaire for Depression （SRQ－D） は平均9.6 ± 5.5 点で, これらの精神的評価も腰下肢症状におけるVAS との関連性があった（いずれもｒ＞ 0.2，p ＜ 0.05）．また腰痛と仮面うつの評価のオッズ比は2.59 であった（p ＜ 0.01）．以上のことから腰下肢症状と精神的状態が呼応することが示された

頚髄症患者における自覚症状とうつ状態との関連性を調べることを目的とした．対象は，26例の頚髄症患者（男15例，女11例，年齢66.2歳）である．疼痛に対するpain drawing chart，30語テスト，visual analogue scale（VAS），およびSelf-Rating Questionnaire for Depression（東邦大式SRQD スコア）について聴取し，麻痺・機能障害などを評価する日本整形外科学会頚髄症判定基準によるJapanese Orthopaedic Association（JOA）スコアによる評価を行った．その結果、頚髄症に由来する痛み（ 4 つのカテゴリーで表現されるVAS およびそれらの合計点）の程度とSRQD による仮面うつ状態について相関関係があることがわかった（いずれもr>0.50）．しかし，他覚所見により判断されるJOA スコアとの関連性はなかった．不定愁訴と混同しやすい頚髄症の痛みやしびれの症状に対して，VAS の分析結果を知ることで，仮面うつ状態への移行が推測可能であることから，看護アセスメントの上でも役立つのではないかと考えられた.We investigated the relationship between the subjective symptom and the status of depression for the 26 patients with cervical myelopathy （male: 15, female: 11）. Pain drawing chart, 30-verbal test, visual analogue scale（VAS）, the evaluation of the masked depression （Self-Rating Questionnaire for Depression: SRQD） and the Japanese Orthopaedic Association（JOA）score were evaluated. As a result, the total pain score（VAS）by 4-subscale categorization, and the evaluation of the SRQD had a statistical correlation. But, SRQD was no relationship with JOA score. The cervical symptoms are similar to the indefinite symptom, and might be mixed up. Therefore, detail analysis of VAS is meaningful to diagnose the possibility to the psychological overlay towards the masked depression, and useful on the nursing physical assessment.

本研究の目的は,対人援助職者のネガティブ感情(NA)およびポジティブ感情(PA)の各々と正の相関関係にあるネガティブ職業生活出来事(NWLE)およびポジティブ職業生活出来事(PWLE)に着目して,対人援助職者のうつ状態を予防する方法について検討することであった.そのため対人援助職者である看護職員,介護職員および児童養護職員を対象にうつ状態の程度を日本語版CES-Dで判定し,さらにNWLEおよびPWLEの体験頻度と,NAおよびPAの体験頻度を測定して,それらの関連性を確認した.その結果,対人援助職者がうつ状態にあるときにはNWLEおよびNAの体験頻度は高くなり,PWLEおよびPAの体験頻度は低くなる傾向が,またうつ状態にないときには,NWLEおよびNAの体験頻度は低くなり,PWLEおよびPAの体験頻度は高くなる傾向が認められた.以上より,特に対人援助職者がうつ状態にないときのPWLEを維持する職場での取組は,対人援助職者のうつ状態に対する第一次予防を効率的に推進すると考えられた.

Few studies have reported the time spent outdoors in patients undergoing long-term oxygen therapy （LTOT）. Also, few have determined whether time spent outdoors was influenced by psychological impairments. The aim of the present study was to examine the usage of oxygen cylinders to assess the time spent outdoors of patients receiving LTOT, and to evaluate the prevalence of dementia and depressive states and determine the correlation between these disorders and time spent outdoors. Fifty-four LTOT patients were enrolled in the present study, and dementia and depressive states were investigated using specific questionnaires. Among them, 23 patients were reassessed one year after the initial evaluation. There was a large variation in time spent outdoors among LTOT patients, and the median number was one hour per day. Measured by the Hasegawa Dementia Scale-Revised（ HDS-R）, dementia was observed in 41% of the patients. Depressive state, measured by the Self-Rating Depression Scale（ SDS）, was observed in 24%. There were no correlations between time spent outdoors and psychological impairments. The percentage of patients in depressive states was statistically higher in the 2nd assessment. More attention must be given to LTOT patients’ usage of oxygen cylinders, and dementia and depressive state should be periodically assessed., 弘前医学. 62, 2011, p.129-137
