1. Particulate air pollution in Central Serbia and some proposed measures for the restoration of degraded and disturbed mining areas

Abstract

Mining causes soil degradation, particle emission, and air and water quality deterioration. This study estimates some health risks in districts of Central Serbia affected by surface mining activities, and proposes measures for land restoration. The epidemiological risk assessment was based on data for seven cancers and three cardiovascular diseases during 2010 - 2020. Results showed a statistically significant increase in the incidence of lung and bronchial cancer in critical districts. Borski district stood out with the highest incidence rates for cardiovascular diseases. The suspected role of particulate air pollution from the mining industry in health deterioration calls for intensified air quality monitoring and development of mitigation technologies. A restoration strategy called the Pan-Life-Carpet technology is proposed for the restoration of mining areas and for air and water pollution control.

2. In-hospital mortality predictors after surgery for Stanford type A aortic dissection - single-center five-year experience

Abstract

Introduction/Objective. Stanford type A aortic dissection is a surgical emergency associated with high mortality. The aim of this study was to determine which group of patients and which characteristics were associated with postoperative, in-hospital mortality. Methods. The retrospective study included 116 patients with type A aortic dissection surgically treated over a five-year period. The association between postoperative, in-hospital mortality and patient characteristics was examined. Results. Total postoperative, in-hospital mortality was 22.4% (26 out of 116 patients). The variables that, after a multivariate analysis, showed a direct correlation with mortality were as follows: admission creatinine value [OR 1.026 (1.006–1.046), p = 0.009], C-reactive protein (CRP) > 10 mg/L [OR 4.764 (1.066–21.283), p = 0.041], and stroke [OR 6.097 (1.399–26.570), p = 0.016]. The receiver operating characteristic (ROC) curve showed that creatinine could be a good predictor of mortality (area under the ROC curve = 0.767; p < 0.0005). The cut-off point was 124.5 μmol/L. The sensitivity was 65% and the specificity was 80%. The cut-off point for CRP was 14.5 mg/L – sensitivity 71.4%, specificity 75% (area under the ROC curve = 0.702, p = 0.021). Conclusion. Surgery for type A aortic dissection is still associated with relatively high mortality. A lower chance of survival may be indicated by elevated admission creatinine and CRP values, as well as stroke.

3. Coronary embolism causing myocardial infarction after heart valve surgery

Abstract

Introduction. Coronary embolism can rarely be a cause of myocardial infarction. It is usually associated with atrial fibrillation, dilated cardiomyopathy, bacterial endocarditis and underlying hypercoagulable state, as well as heart surgery. Case report. We reported a case of a patient with severe mitral and tricuspid regurgitation, with no underlying coronary artery disease. The patient underwent heart valve surgery, and the immediate postoperative course was uneventful. Five days after the operation, the patient sustained cardiac arrest, which was followed by a successful cardiopulmonary resuscitation. Electrocardiography showed atrial fibrillation with a significant ST segment elevation in the inferior leads. Urgent coronary angiography revealed a total occlusion of the right coronary artery, thus percutaneous coronary intervention was performed, after which flow restoration through the artery was achieved. The patient was discharged with triple antithrombotic therapy on the 20th postoperative day. Conclusion. Heart surgery could be followed by unexpected and potentially fatal complications, coronary embolism being one of them. In such case, the prompt and adequate reaction by the whole medical team is crucial for a patient's survival and recovery.

4. Infective endocarditis of partial atrioventricular septal defect: A case report

Abstract

Introduction. Partial atrioventricular septal defect (AVSD) is a form of congenital heart disease (CHD) rarely detected in adults. Infective endocarditis represents a severe complication that carries a substantial risk. Case report. We here reported a case of a 43-year-old female with previously diagnosed adult CHD (partial AVSD and bicuspid aortic valve) presented to the hospital with fever and malaise 14 days prior to admission. On the lung computed tomography scan inflammatory consolidations were found and dual antibiotic therapy (ceftazidime and clarithromycin) was administered without significant regression of pulmonary inflammatory consolidations. The antibiotic treatment was continued with amoxicillin/clavulanic acid combined with levofloxacin and metronidazole. Transthoracic and transesophageal echocardiography revealed a large vegetation (dimension, 3.6 x 1.8 cm) attached to the septal leaflet of the tricuspid valve floating between right atrium and right ventricle through tricuspid valve with high embolic potential. Endocarditis team reached a decision for immediate surgical intervention. The operative findings revealed the partial AVSD, common atrioventricular valve with cleft of the anterior mitral leaflet in the A2 segment and detached and cleft septal leaflet of the tricuspid valve. Vegetation (size 4 x 3 cm) was attached to the septal side of the tricuspid annulus, basal segment of the anterior mitral leaflet and edge of the atrial septal defect freely floating between right atrium, right ventricle and left atrium. Excision of the vegetation and AVSD plastics were done, as well as the reconstruction of the mitral and tricuspid annuli and leaflets. The treatment was continued with antibiotics and completed in 18 days with full recovery. Conclusion. Early and precious diagnosis and optimal management that combines both conventional and surgical approaches are crucial for reducing the risk of complications and mortality in patients with infective endocarditis in grown-up congenital heart disease.

5. Pregnant woman survives aortic dissection, dies later of coronary embolism - a unique case of a non-compliant patient

Abstract

Introduction. The incidence of aortic dissection ranges 3–5 cases per 100,000 person-years. In women under the age of 40 who acquire aortic dissection, almost half of the cases happen during pregnancy. Case outline. We report a case of a 22-year-old pregnant woman in the 16th gestational week with a history of arterial hypertension and known aortic dilatation, who was admitted with aortic dissection. She underwent the Bentall procedure, in which her aortic valve, ascending aorta, and proximal part of the aortic arch were replaced using valved composite graft. On the next day, the evacuation abortion was made, since the fetus didn’t survive the operation. Six years later, the patient stopped taking anticoagulation therapy and was admitted for prosthetic valve thrombosis, which was successfully treated with intravenous heparin. One year later, the patient died of a myocardial infarction due to coronary thromboembolism, confirmed on autopsy. Conclusion. Preconceptional counseling in women with a known aortic disease is of the utmost importance. Aortic dissection in pregnant women is an acute life-threatening condition for both mother and fetus that should be managed by a multidisciplinary team. After a mechanical heart valve implantation, lifetime oral anticoagulants are mandatory.

Abstract

Medical staff that makes the initial contact with patient with aortic dissection has a goal to rise a suspicion and recognize aortic dissection, as well as to safely transport the patient to a health center capable of providing the definite treatment. First step in the prehospital management is providing oxygen support via breathing mask to prevent organ damage. It is important to monitor all vital functions during transport, including heart rhythm and diuresis, in order to have a continuous insight into the patient's current state and in case of a deterioration react appropriately. Blood pressure control is the main goal of the medical treatment in aortic dissection. Systolic blood pressure should be kept between 100-120 mmHg, whereas the target heart rate is 60 beats per minute. Therapy should be started with intravenous administration of beta-blocker or non-dihydropyridine calcium channel blocker, followed by a vasodilator infusion. For pain management intravenous administration of opiates is recommended. In case of shock, intravenous fluid replacement with crystalloids should be initiated immediately, along with vasopressor therapy with noradrenalin or phenylephrine. In case of shock, the patient should be intubated and mechanically ventilated with the appropriate sedation.

7. Surgical treatment of aortic valve fibroelastoma: A case report

Abstract

A 45-year-old man was admitted to Clinic for the first time, with symptoms of chest pain and fatigue. Computerised tomography (CT) diagnostics of the chest showed a soft tissue vegetation of approximately 5x5 mm on the left aortic coronary cusp. A double-vessel coronary disease was also diagnosed. The patient underwent surgery, a complete resection of the tumour was achieved, which was confirmed by postoperative transoesophageal echocardiography (TEE). Because of the risk of valve damage, it was decided to replace the aortic valve. A bypass from left internal mammary artery (LIMA) to left anterior descending (LAD) coronary artery (LAD-LIMA) and right coronary artery (RCA) was also performed. The patient was discharged on the 14th postoperative day with satisfactory results.

8. Development of cardiopulmonary bypass: A historical review

Abstract

The idea of isolated organ perfusion, a precursor of cardiopulmonary bypass, came by Legalois in 1812. First isolated organ perfusion was described by Loebell in 1849. The first closed system for oxygenation and returning the blood through arteries was created by Frey and Gruber in 1885. Gibbon Jr. is considered the father of extracorporeal circulation. In spring of 1934 he began constructing a machine for extracorporeal circulation in Boston. He published the first description of this system in 1937. Gibbon won the grant of the International Business Machines Corporation for developing the machine in 1947. Together they developed Model I in 1949 and Model II in 1951. After a few unsuccessful attempts in 1952, the first successful surgical intervention on the heart (closure of atrial septal defect) using cardiopulmonary bypass was performed on May 6, 1953. In 1945, Kirklin and his working group reported on a series of eight successfully treated patients in a row who underwent surgery with extracorporeal circulation. First successful valve surgery under the direct vision was performed by Dodrill in 1952, using his “Michigan Heart” machine as a right heart bypass. Using cardiopulmonary bypass, cardiac surgeons can deal with the complex cardiac pathology and save millions of lives.

9. Surgical treatment of intramural hematoma of the ascending aorta

Abstract

Introduction. Intramural hematoma of the aorta presents potentially fatal condition developing as a result of a vasa vasorum rupture. It is a major risk factor for developing a frank aortic dissection. Case Outline. A 65-year-old woman was admitted to our clinic for the second time, after her symptoms of chest pain and vertigo (with no electrocardiographic signs of myocardial infarction) hadn’t disappeared after several months of medicament treatment (indicated in the first hospitalization). Computed tomography arteriography of the aorta showed no sign of acute aortic dissection, but revealed a contrast depo in the aortic wall of 8 Ч 14 mm dimensions, with no extravasation of contrast. Also, massive pericardial effusion was observed (10-30 mm in thickness). Transesophageal echocardiography confirmed these findings completely. The patient underwent surgery, in which plaque exulceration was detected on the convex side of the ascending aorta, 3 cm above the aortic valve, 1 cm in diameter, with no signs of intimal tear. A resection of the ascending aorta was performed, and the aorta was reconstructed with a 30 mm Dacron tube graft. The patient was discharged on the 14th postoperative day with satisfactory results. Conclusion. Intramural hematoma is not a common event, but it is potentially a fatal one. Open surgery in patients with an intramural hematoma is an effective treatment strategy, although percutaneous endovascular treatment options are being described.

10. Postoperative nonlethal complications following open heart surgery

Abstract

Background/Aim. Postoperative nonlethal complications after open heart surgery are a serious clinical problem causing a considerable engagement of health workers, an augmented use of drugs, and prolonged operation incapacity leading to prolonged hospital stay and increased expenses. The aim of the study was to establish whether there is any correlation between the level of expected operative risk and postoperative nonlethal complications. Methods. A consecutive series of 853 patients subjected to the open heart surgery were investigated, 622 (73%) males and 231 (27%) females. The average age of the patients was 57.2 ± 9.9 (16-81) years. The patients were divided into 3 groups according to the additive EuroSCORE model: groups I, II and III with the expected operative risk of 0%-2%, 2%- 5% and over 5%, respectively. The data were collected prospectively and analyzed retrospectively. Statistical methods of correlation and t-test were used. Results. A high degree of correlation between the operative risk level and frequency of postoperative nonlethal complications (R = 0.98) was found. The average rate of complications was 24% for the whole group of 853 patients. It accounted for 21%, 29% and 47% in the groups I, II and III, respectively. According to the expected operative risk level there was a statistically significant difference in respect of heart arrhythmias (p = 0.02), neurologic complications (p = 0.002), and pulmonary complications (p = 0.009). Conclusion. Our results show a high degree of correlation between the expected level of operative risk according to the EuroSCORE model and the frequency of postoperative nonlethal complications. There is a statistically significant difference in respect to frequency of heart rhythm disturbances, pulmonary and neurological complications and expected operative mortality.

11. Evaluation of results in coronary surgery using EuroSCORE

Abstract

Introduction. The European System for Cardiac Operative Risk Evaluation (EuroSCORE) was developed in order to predict operative risk in cardiac surgery and to assess the quality of the cardio-surgical care. Introduction of the uniform terminology in result evaluation process leads to the significant improvement in measuring and evaluation of surgical treatment quality. Objective. The aim of the study was to evaluate our results in isolated coronary surgery using the EuroSCORE. Methods. The study was done respectively by analyzing predicted mortality according to the EuroSCORE model and observed operative risk in 4,675 coronary patients operated at our Clinic during the period 2001-2008. For statistical analyses, the Pearson, Chisquare and ANOVA tests were used. Results. The total postoperative mortality predicted by the EuroSCORE was 2.9±2.25, while the observed one was 2.2%. When the scoring system and observed results were compared over the years, a considerably lower observed mortality was found during the last 4 years. Overall average number of distal anastomoses was 2.62±0.84. During the period 2004-2008, the average number of coronary anastomoses increased over the years reaching the value of 2.77±0.88. The difference is at the level of statistical significance with the trend of further increase. Percentage of the patients with single or double graft myocardial revascularization decreases, while the number of the patients with triple or more bypasses increases. Conclusion. During the last years, the results in isolated coronary surgery have considerably improved. The EuroSCORE overestimates operative risk. In order to improve its predictive value, the model should be recalibrated.

Abstract

Hronična bolest bubrega (HBB) je zdravstveni problem koji se javlja širom sveta i povezana je sa visokim kardiovaskularnim komorbiditetom i smrtnošću. Veliki porast broja bolesnika koji imaju terminalnu bubrežnu slabošću (TBS) nastaje kao posledica eksponencijalnog porasta broja bolesnika čija je slabost bubrega posledica hipertenzije i dijabetesa, kao i porasta broja starih sa TBS. Zbog toga više od 50% bolesnika sa HBB umire zbog kardiovaskularnih bolesti i pre započinjanja lečenja metodama za zamenu funkcije bubrega. Utvrditi kliničke karakteristike bolesnika sa i bez kontrastom indukovane nefropatije (pre svega varijable bubrežne funkcije definasane pomoću AKIN i RIFLE kriterijuma) podvrgnutih kardiohirurškim operacijama, potom utvrditi postojanje razlike u mortalitetu i postoperativnom morbiditetu između bolesnika sa i bez kontrastom indukovane nefropatije, a koji se podvrgavaju kardiohirurškoj operaciji i takođe utvrditi prediktore mortaliteta i morbiditeta kod bolesnika sa prethodnom kontrastom indukovanom nefropatijom koji se podvrgavaju kardiohirurškoj operaciji. Studija je koncipirana kao retroprospektivna opservaciona studija u ukupnom trajanju od pet godina retrospektivnog perioda i pola godine prospektivnog perioda kojim su obuhvaćene dve grupe bolesnika: I grupa - pacijenti sa kontrastom indukovanom nefropatijom (CIN) i II grupa - pacijenti bez CIN; koji su podvrgnuti kardiohirurškim operacijama (koronarna, valvularna, kombinovana hirurgija i ostale) na Institutu za kardiovaskularne bolesti Vojvodine u Sremskoj Kamenici. Od ukupnog broja operisanih pacijenata u ovom perioda (oko 5000 bolesnika) u ovu studiju je uključeno 1269 bolesnika. U našoj studiji ukupno je analizirano 1269 bolesnika koji su svrstani u dve grupe. Prvu grupu je činilo 59 (4,6%) pacijenata koji su koronarografisani (dijagnostička, terapijska) i razvili CIN te su upućeni u istoj hospitalizaciji po indikaciji konzilijuma na koronarnu, valvularnu i kombinovanu hirurgiju. Drugu grupu je, Chronic kidney disease (CKD) is a healthcare problem that occurs worldwide and is associated with high cardiovascular comorbidity and mortality. A large increase in the number of patients with terminal renal failure (TRF) occurs as a result of an exponential increase in the number of patients whose renal failure is due to hypertension and diabetes, as well as an increase in the number of elderly with TRF. To determine the clinical characteristics of patients with and without contrast-induced nephropathy (CIN) (renal function parameters defined by AKIN and RIFLE criteria) undergoing cardiac surgery, to determine the difference in mortality and postoperative morbidity between patients with and without CIN who are submitted to cardiac surgery and also to determine predictors of mortality and morbidity in patients with CIN undergoing cardiac surgery. The study was conceived as a retroprospective observational study with a total duration of five years of retrospective period and half a year of prospective period which included two groups of patients: Group I - patients with contrast-induced nephropathy (CIN) and Group II - patients without CIN; who underwent cardiac surgery (coronary, valvular, combined surgery and other) at the Institute for Cardiovascular Diseases of Vojvodina in Sremska Kamenica. Увод: Ризик оперативног лечења у кардиохирургији посматра се са становишта постоперативног морталитет или одређених компликација. Стратификација ризика подразумева преоперативно утврђивање оперативног ризика, у односу на одређен период након операције, на основу тежине стања сваког пацијента посебно. Оно се процењује на основу броја и тежине његових фактора ризика. Предмет истраживања јесте математички статистички модел за предвиђање исхода оперативног ризика у кардиохирургији за период од једне године од датума интервенције. Популација Војводине је, у демографском смислу, специфична. Висока је стопа гојазних, пушача, оболелих од шећерне болести и артеријске хипертензије. Ове специфичности популације, морају се узети у обзир приликом процене ризика од кардиохируршке интервенције. Осим тога, оперативна тактика и техника (примена артеријских графтова, реваскуларизација у ургентним стањима, хибридна хирургија итд.), као и оптимална преоперативна припрема и постоперативни третман имају велики утицај на исход оперативног лечења. Risk stratification involves preoperative determination of operative risk in relation to a certain period after the operation, based on the health status of each patient individually. It is estimated through the number and severity of its risk factors. The subject of research is mathematical statistical model which is able to predict the outcome of operative risk in cardiac surgery for a period of one year from the date of intervention. The population of Vojvodina is, in demographic terms, specific. There is a high rate of overweight people, smokers, patients with diabetes and hypertension. These specifics of the population must be taken into account when assessing the risk of cardiac intervention. In addition, operative tactics and techniques (use of arterial grafts, revascularization in emergency situations, hybrid surgery etc.) as well as the optimal preoperative preparation and postoperative treatment have a major impact on the outcome of operative treatment. The aim of the research is the identification of risk factors that significantly affect the outcome of operative treatment and development of specific models for the prediction of mortality and major cardiac and cerebrovascular complications, as well as their validation. Abstract

This chapter presents a case study of a toxic Planktothrix rubescens bloom in the Serbian reservoir Vrutci used as the water supply for the city of Užice. Chemical cyanotoxin analyses were supplemented by Artemia salina bioassays, a questionnaire directed to the inhabitants of Užice, and epidemiological work. The overall aim of the studies was to assess the potential human health impacts of the toxic bloom.

Abstract

Introduction: Quality of life assessment and survival analysis are a "gold standard" in assessing treatment quality in general. In the absence of a universally accepted integral index, we developed our own QOLi-NS (Quality of Life Index - Novi Sad) as a disease specific index. The aim of this study was to investigate long-term survival and quality of life after myocardial revascularization with respect to sex and age. Material and methods: The study included 563 coronary patients operated on at the University Clinic of Cardiovascular Surgery in Sremska Kamenica, in the period from September 1, 1995 to June 1, 1998. The patients were divided into two groups with respect to sex and age; two age categories were defined: younger and older that 65 years of age. Quality of life was evaluated using Integral Quality of Life Index QOLi - NS. Actuarial life table was used to establish the survival curve. Results: After myocardial revascularization, all investigated patients demonstrated a considerable improvement in quality of life with statistically significant differences with respect to age and sex. The highest postoperative quality of life (QOLi-NS), and the best survival rate were found in male patients and in patients younger than 65 years. Conclusion: The gathered data confirmed a significant improvement in quality of life and survival rate after myocardial revascularization in all groups of patients. A statistically significant difference in survival rate was determined with respect to the patients age. Improved quality of life in patients older than 65 years proved that myocardial revascularization is a good therapeutic option for these patients. [ABSTRACT FROM AUTHOR]

Abstract

Introduction. Prosthetic valve thrombosis is usually a subacute or chronic condition, although it may also present with a fresh thrombus. It occurs in two forms: obstructive and non-obstructive thrombosis. Case Report. We present a case of a female patient who underwent mitral valve replacement with mechanical prosthesis due to severe mitral stenosis. The postoperative course was uneventful and the patient was discharged on vitamin K antagonist therapy with international normalized ratio target 3.0. Five months later, the patient was admitted with severe shortness of breath and signs of acute heart failure. International normalized ratio at that moment was 2.3. Transthoracic echocardiography indicated severely raised gradient across the prosthetic valve and mechanical valve malfunction was suspected. Cinefluoroscopy showed that one of the prosthetic valve leaflets was completely immobile. Transesophageal echocardiography definitely confirmed thrombosis of the prosthetic valve with large multiple thrombi that completely fixed one leaflet in closed position, and partially limited the motion amplitude of the other leaflet. There were thrombi floating between the left ventricle and left atrium. Thrombectomy of the prosthetic valve was performed, which was sufficient for the complete restoration of the mechanical valve function. The vitamin K antagonist dosage was carefully up-titrated in order to reach and maintain the target international normalized ratio of 3.0. Conclusion. Prosthetic valve thrombosis is a serious and life-threatening condition that requires urgent management. Coordination and cooperation of the whole heart team is necessary for optimal choice of treatment, which primarily includes surgery or fibrinolysis. [ABSTRACT FROM AUTHOR]

Abstract

Hronična bolest bubrega (HBB) je zdravstveni problem koji se javlja širom sveta i povezana je sa visokim kardiovaskularnim komorbiditetom i smrtnošću. Veliki porast broja bolesnika koji imaju terminalnu bubrežnu slabošću (TBS) nastaje kao posledica eksponencijalnog porasta broja bolesnika čija je slabost bubrega posledica hipertenzije i dijabetesa, kao i porasta broja starih sa TBS. Zbog toga više od 50% bolesnika sa HBB umire zbog kardiovaskularnih bolesti i pre započinjanja lečenja metodama za zamenu funkcije bubrega. Utvrditi kliničke karakteristike bolesnika sa i bez kontrastom indukovane nefropatije (pre svega varijable bubrežne funkcije definasane pomoću AKIN i RIFLE kriterijuma) podvrgnutih kardiohirurškim operacijama, potom utvrditi postojanje razlike u mortalitetu i postoperativnom morbiditetu između bolesnika sa i bez kontrastom indukovane nefropatije, a koji se podvrgavaju kardiohirurškoj operaciji i takođe utvrditi prediktore mortaliteta i morbiditeta kod bolesnika sa prethodnom kontrastom indukovanom nefropatijom koji se podvrgavaju kardiohirurškoj operaciji. Studija je koncipirana kao retroprospektivna opservaciona studija u ukupnom trajanju od pet godina retrospektivnog perioda i pola godine prospektivnog perioda kojim su obuhvaćene dve grupe bolesnika: I grupa - pacijenti sa kontrastom indukovanom nefropatijom (CIN) i II grupa - pacijenti bez CIN; koji su podvrgnuti kardiohirurškim operacijama (koronarna, valvularna, kombinovana hirurgija i ostale) na Institutu za kardiovaskularne bolesti Vojvodine u Sremskoj Kamenici. Od ukupnog broja operisanih pacijenata u ovom perioda (oko 5000 bolesnika) u ovu studiju je uključeno 1269 bolesnika. U našoj studiji ukupno je analizirano 1269 bolesnika koji su svrstani u dve grupe. Prvu grupu je činilo 59 (4,6%) pacijenata koji su koronarografisani (dijagnostička, terapijska) i razvili CIN te su upućeni u istoj hospitalizaciji po indikaciji konzilijuma na koronarnu, valvularnu i kombinovanu hirurgiju. Drugu grupu je činilo 1210 (95,4%) bolesnika kod kojih nakon koronarografije nije razvijena kontrastom indukovana nefropatija, a takođe su tokom iste hospitalizacije operisani. Kriterijumi za uključivanje pacijenata u studiju su svi punoletni bolesnici koji su upućeni na kardiohirurške operacije (koronarna, valvularna, kombinovana i ostale). CIN je definisan kao porast vrednosti kreatinina unutar pet dana nakon koronarografije za 25% u odnosu na vrednost kreatina pre koronarografije. Praćene su preoperativne, operativne i postoperativne karakteristike bolesnika sa CIN i bolesnika bez CIN. U disertaciji su korišćene mere deskriptivne statistike: aritmetička sredina, standardna devijacija, medijana, kvartili, frekvence i procenti. Za poređenje srednjih vrednosti varijabli dve populacije primenjen je test za nezavisne uzorke i Man-Vitnijev test. Povezanost kategorijskih varijabli ispitana je pomoću Hi-kvadrat testa za tabele kontigencije ili pomoću Fišerovog testa. Određivanje uticaja promenljivih na ishod lečenja izvršen je primenom univarijantne i multivarijantne binarne logističke regresije, koja je poslužila i za pravljenje nove varijable (modela) za procenu ishoda lečenja. Prediktivni kvalitet varijabli na ishod ocenjen je pomoću ROC krivih. Za određivanje dužine preživljavanja primenjena je Kaplan-Meier analiza preživljavanja. Criteria for inclusion of patients in the study are: all adult patients who are referred for cardiac surgery (coronary, valvular, combined and other). CIN was defined as a at least 25% increase in creatinine value within five days after coronary angiography compared to creatine value before coronary angiography. Preoperative, operative and postoperative characteristics of patients with CIN and patients without CIN were analyzed. Statistical analyses included measures of descriptive statistics: arithmetic mean, standard deviation, median, quartiles, frequencies and percentages. To compare the mean values of the variables of the two populations, t-test for independent samples and the Mann-Whitney test were applied. The correlation of categorical variables was examined using the Chi-square test for contingency tables or using the Fisher test. The influence of variables on the treatment outcome was determined by applying univariate and multivariate binary logistic regression, which also served to create a new variable (model) for assessing the treatment outcome. Abstract

Introduction. If young women with congenital heart disease need heart valve surgery, it is necessary to thoroughly consider the choice of the valve and the risks of serious complications during pregnancy. Case Report. We report a case of a woman who presented with a pregnancy complicated by mechanical aortic valve thrombosis at the end of the first trimester. After a thorough evaluation by a multidisciplinary team, the patient underwent surgical thrombectomy and normal mechanical valve function was restored. At 36 weeks of gestation, planned cesarean section was performed. The mother and the child remained well during the 5-year follow-up. Conclusion. Mechanical heart valves in pregnancy carry a very high risk of complications. An individualized approach is needed in the management of women with mechanical valves, as well as uniform antenatal care in centers that provide complete care from pregnancy planning to delivery. Abstract

Background/Aim. Postoperative nonlethal complications after open heart surgery are a serious clinical problem causing a considerable engagement of health workers, an augmented use of drugs, and prolonged operation incapacity leading to prolonged hospital stay and increased expenses. The aim of the study was to establish whether there is any correlation between the level of expected operative risk and postoperative nonlethal complications. Methods. A consecutive series of 853 patients subjected to the open heart surgery were investigated, 622 (73%) males and 231 (27%) females. The average age of the patients was 57.2 ± 9.9 (16--81) years. The patients were divided into 3 groups according to the additive EuroSCORE model: groups I, II and III with the expected operative risk of 0%-2%, 2%-5% and over 5%, respectively. The data were collected prospectively and analyzed retrospectively. Statistical methods of correlation and t-test were used. Results. A high degree of correlation between the operative risk level and frequency of postoperative nonlethal complications (R = 0.98) was found. The average rate of complications was 24% for the whole group of 853 patients. It accounted for 21%, 29% and 47% in the groups I, II and III, respectively. According to the expected operative risk level there was a statistically significant difference in respect of heart arrhythmias (p = 0.02), neurologic complications (p = 0.002), and pulmonary complications (p = 0.009). Conclusion. Our results show a high degree of correlation between the expected level of operative risk according to the EuroSCORE model and the frequency of postoperative nonlethal complications. There is a statistically significant difference in respect to frequency of heart rhythm disturbances, pulmonary and neurological complications and expected operative mortality. Увод: Објективна процена оперативног ризика кардиохируршких болесника и процена потребне постоперативне ангажованости медицинских сестара у јединици интензивног лечења могу допринети брзом увиду у тежину здравственог стања болесника , осигурању оптималног броја сестара, омогућавању квалитетне здравствене неге болесника, олакшању организације рада , аргументованом уговарању потреба за сестринским кадром. Током 2010. године, за процену ризика у кардиохирургији, креиран је модел European System for Cardiac Operative Risk Evaluation (EuroSCORE II). Циљеви истрађивања: 1.Испитати повезаност EuroSCORE II, и активности медицинских сестара у јединици интензивног лечења. 2.Утврдити утицај релевантних фактора ризика из EuroSCORE II, на активности медицинских сестара у јединици интензивног лечења. 3.Испитати повезаност EuroSCORE II, и специфичних интервенција медицинских сестара јединице интензивног лечења. Методе:У студију je била укључена консекутивна серија од 809 болесника, оперисаних на Клиници за кардиоваскуларну хирургију Института за кардиоваскуларне болести, у периоду од 01.02. 2014-30.11.2014. године. Подаци о вредностима EuroSCORE II и релевантним факторима ризика преузети су болничког информационог система.Подаци о сестринским активностима у јединици интензивног лечења, након кардиохируршке интервенције, регистровани у одговарајућим листама. Испитивање повезаности нивоа оперативног ризика и активност медицинских сетра вршено је помоћу линеарне корелације. Утицај појединих фактора из модела EuroSCORE II на активност медицинских сестара испитиван је мултиваријантном линеарном регресијом. Зависност специфичних интервенција медицинских сестара од нивоа EuroSCORE II и других параметара оцењивана је помоћу бинарне логистичке регресије. Квалитет предиктивног модела, одређиван је помоћу ROC кривих, укључујући одређивање оптималног пресека, сензитивности и специфичности. Abstract

Увод: Ризик оперативног лечења у кардиохирургији посматра се са становишта постоперативног морталитет или одређених компликација. Стратификација ризика подразумева преоперативно утврђивање оперативног ризика, у односу на одређен период након операције, на основу тежине стања сваког пацијента посебно. Оно се процењује на основу броја и тежине његових фактора ризика. Предмет истраживања јесте математички статистички модел за предвиђање исхода оперативног ризика у кардиохирургији за период од једне године од датума интервенције. Популација Војводине је, у демографском смислу, специфична. Висока је стопа гојазних, пушача, оболелих од шећерне болести и артеријске хипертензије. Ове специфичности популације, морају се узети у обзир приликом процене ризика од кардиохируршке интервенције. Осим тога, оперативна тактика и техника (примена артеријских графтова, реваскуларизација у ургентним стањима, хибридна хирургија итд.), као и оптимална преоперативна припрема и постоперативни третман имају велики утицај на исход оперативног лечења. Циљ истраживања јесте идентификација свих фактора ризика који значајно утичу на исход оперативног лечења и развој сопствених модела за предикцију морталитета и значајних кардијалних и церебрoваскуларних компликација, као и њихова валидација. Методе: Узорак је чинило 2664 консекутивних болесника оперисаних на Клиници за кардиоваскуларну хирургију, Института за кардиоваскуларне болести Војводине у периоду од 01.07. 2011. до 21.12.2013. године. Анализирано је укупно 48 потенцијално релевантних фактора ризика. Подаци о морталитету и компликацијама у периоду хоспитализације и о рехоспитализацијама преузимани су из болничког информационог система (БИС). Болесници који, 365 дана од операције, нису били регистровани у БИС-у, позивани су телефоном како би се добио увид у њихово стање. Модел за процену једногодишњег оперативног ризика креиран је помоћу мултиваријантне бинарне логистичке регресије. Дискриминативна моћ модела испитана је помоћу Receiver Operating Characteristic (ROC) кривих, при чему су одређени гранични пресек, сензитивност и специфичност одговарајуће варијабле. Резултати: Добијени модел је добар маркер за предикцију морталитета годину дана од операције (area = 0,712: р < 0,0005). Вредност граничног пресека је 3,04, сензитивност је 0,700; специфичност је 0,626. Хосмер - Лемешов тест за креирани модел износи 0,125 (р > 0,05). На узорку на коме је направљен модел, у односу на значајне, неповољне кардијалне и цереброваскуларне догађаје вредност површине испод ROC криве су: area = 0,713; p < 0,0005 , вредност граничног пресека 7,87, сензитивност 0,667, специфичност 0,635. Када се модел примени на контролну групу, вредност површине испод ROC криве су следеће: area = 0,518, p = 0,581. Хосмер - Лемешов тест за креирани модел показује да је вредност р = 0,007. Закључак: Модел за предикцију морталитета је прецизан како у односу на целу групу болесника, тако и у односу на тип кардиохируршке инервенције. Креирани модел има добру моћ дискриминације. In addition, operative tactics and techniques (use of arterial grafts, revascularization in emergency situations, hybrid surgery etc.) as well as the optimal preoperative preparation and postoperative treatment have a major impact on the outcome of operative treatment. The aim of the research is the identification of risk factors that significantly affect the outcome of operative treatment and development of specific models for the prediction of mortality and major cardiac and cerebrovascular complications, as well as their validation. Methods: The sample was comprised of 2664 consecutive patients who underwent surgery at the Clinic for Cardiovascular Surgery at the Institute of Cardiovascular Diseases Vojvodina in the period 01.07. 2011 - 21.12.2013. A total of 48 potentially relevant risk factors were analyzed. Data on mortality and complications during hospitalization, and the rehospitalization rates were obtained from the Hospital Information System (BIS). Patients not registered within the BIS during 365 days from the operation day, were contacted by phone in order to gain insight about their status. A model for one-year operative risk assessment was created using multivariate binary logistic regression. The discriminative power of the model was tested using the Receiver Operating Characteristic (ROC) curves, with determination of the following parameters: cut-off value, sensitivity and specificity of the response variables. Results: The model is a good marker for the prediction of mortality one year after the operation (area = 0.712: p 0.05). Patients not registered within the BIS during 365 days from the operation day, were contacted by phone in order to gain insight about their status. A model for one-year operative risk assessment was created using multivariate binary logistic regression. The discriminative power of the model was tested using the Receiver Operating Characteristic (ROC) curves, with determination of the following parameters: cut-off value, sensitivity and specificity of the response variables. Results: The model is a good marker for the prediction of mortality one year after the operation (area = 0.712: p 0.05).
