Objective: Primary percutaneous coronary intervention is the standard treatment for ST-segment elevation myocardial infarction. Although myocardial and epicardial perfusion is usually achieved with primary percutaneous coronary intervention, infarct-related arterial thrombus burden negatively affects the procedural success and clinical outcomes of primary percutaneous coronary intervention. Therefore, we aimed to investigate the association between thrombus burden (calculated before and after initial flow) and clinical consequences in patients with ST-segment elevation myocardial infarction. Materials and Methods: This study retrospectively enrolled 1376 patients who had ST-segment elevation myocardial infarction between May 2012 and November 2015. Patients who had only undergone balloon angioplasty and emergency coronary artery bypass grafting were not included in the study. Data regarding the initial clinical and demographic features of the patients were obtained from their hospital records. Thrombus burden was calculated using baseline and final (after wire inflation or small balloon dilatation) thrombolysis in myocardial infarction thrombus grades. The endpoints of the study were defined as no-reflow development after primary percutaneous coronary intervention and 1-year all-cause mortality. Statistical significance was defined as P < .05. Results: No-reflow was detected in 169 patients (12.3%). The calculated basal thrombus burden was significantly associated with post-procedural no-reflow (P < .001). No-reflow was also associated with advanced age (P < .001), longer pain-to-door time (P < .001), and increased blood glucose levels (P = .032). The calculated final thrombus burden was related to 1-year all-cause mortality (P = .047). One-year all-cause mortality was also associated with advanced age (P < .001), high Killip scores (P = .003), increased white blood cell counts (P = .001), and low estimated glomerular filtration rates (P < .001). Conclusion: Basal thrombus burden was associated with no-reflow, and final thrombus burden was associated with 1-year all-cause mortality. The calculation of thrombus burden before and after initial flow may help to predict clinical outcomes. [ABSTRACT FROM AUTHOR]

6. Acute effect of primary percutaneous coronary intervention on left ventricular dyssynchrony in ST-segment elevation myocardial infarction.

Objective: The aim of this study was to prospectively evaluate the effect of percutaneous coronary intervention in the acute period on left ventricular dyssynchrony in ST-segment elevation myocardial infarction patients by using Tissue Synchronization Imaging. Methods: Forty-four ST-segment elevation myocardial infarction (MI) patients (29 male, 15 female), who were admitted within the first 12 hours of chest pain symptoms, were enrolled in the study. According to the localization of MI, the patients were divided into groups as anterior MI (n=26) and inferior MI (n=18). All echocardiography measurements were taken just before percutaneous coronary intervention (PCI) and follow-ing PCI at a mean of 3-6 days. They were assessed according to the time to reach the peak systolic velocity, which was calculated by the tissue synchronization imaging method from four pairs of non-apical alternate segments. The difference between the duration to reach the peak systolic velocity in alternate segments was regarded as left ventricle dyssynchrony and the results were compared. Results: In the anterior MI group, basal anterior (p<0.01), mid-anterior segment (p<0.01) and basal septal segment (p<0.01); in the inferior MI group, the basal septal segment (p=0.02), mid-septal segment (p=0.02), and basal and mid-inferior segment (p<0.01) values were significantly lower in the post-PCI measurements when compared to the measurements taken prior to PCI. In both groups, the intraventricular dyssyn-chrony indices of the basal anterior-basal inferior (p<0.01), mid-anterior-mid-inferior (p<0.01) segments were found to be significantly lower in the post- PCI period when compared to the pre-PCI period. Conclusion: It was found that percutaneous coronary intervention in patients with ST-elevation significantly decreases the degree of LV dys-synchrony in the acute period. [ABSTRACT FROM AUTHOR]

'Hastalardan elde edilen sonuçlar tamamen sağlıklı 10 kontrol vakasıyla karşılaştırıldı. Hastalardaki kan kortizol,Na, K ve 24 saatlik idrarla itrah edilen 17-KS,N+, K+ miktarları kontrol vakalarına göre istatistik! olarak önemli derecede farklı bulundu »Hastalardaki bulgular sürrenal hipofonksiyon lehine idi. The article presents the case of a 77-year-old woman diagnosed with a single coronary artery originating from left coronary sinus of Valsalva. This condition was associated with serious coronary artery disease. Electrocardiogram showed sinus rhythm, reduction in R wave height and abnormally tall T waves in leads V1-V4. There was a significant stenosis in the proximal in the right coronary artery.

Giriş: Aort darlığı (AD) yaygın bir kapak hastalığıdır ve transkateter aort kapak implantasyonu (TAVI), geleneksel cerrahi için yüksek risk altında bulunan hastaları tedavi etmek için giderek daha fazla kullanılmaktadır. İşlemin yaygınlaşması ve vaka sayısının artması ile işlem sırasında ve sonrasında gelişebilecek komplikasyonlara karşı öngördürücü bir skorlama ihtiyacını doğurmaktadır. Biz bu çalışmada koroner arter hastalığının tanı evreleme ve tedavisinde kullanılan Syntax Skor II (SS II) ile geleneksel risk skorlarından EUROSCORE ve STS nin hastane içi mortalite tahmininde ki öngördürücülüğünü karşılaştırdık.Metod: Çalışmaya aort darlığı nedeni ile hastanemizde 2011-2018 yılları arasında TAVİ işlemi uygulanan 70 hasta dahil edildi. Hastalar hastane içi mortalite gelişimine göre iki gruba ayrılarak risk skorlarının mortaliteyle ilişkisi araştırıldı. Bulgular: Hastaların %58,6 ı erkekti ve ortalama yaş 77,4±10.1 idi. Hastalar hastane içinde ölen ve salah ile taburcu edilen iki gruba ayrıldı. Tek değişkenli analizde, STS skoru EUROSCORE ve SS II, sol ventriküler sistolik çap ve mean aortik gradient ölüm grubunda yüksek olarak tespit edildi . postoperatif komplikasyon görülme sıklıkları 2 grupta da benzer oranda izlendi (stroke için p: 0.22, ary için p:0.50, pace implantasyonu için p: 0.49).Regresyon analizinde hastane içi mortalite gelişiminde EUROSCORE TAVİ hastalarında bağımsız öngördürücü olarak tespit edildi (p:0.036). Diğer iki skor da istatistiksel anlamlılık tespit edilmedi. Sonuc: TAVİ uygulanacak hastalarda perioperatif ve hastane içi mortalite tahmininde yaygın kullanılan risk skorlamaları içinden EUROSCORE anlamlı şekilde daha iyi tahmin gücüne sahiptir. Ancak TAVİ ye özgü , daha iyi tahminlerde bulunabilecek yeni risk skorlamalarına ihtiyaç olduğunu düşünmekteyiz.Anahtar kelimeler : TAVİ, STS, EUROSCORE, SYNTAX II, periopertaif mortalite INTRODUCTION: Aortic stenosis (As) is a common valvular disease and transcatheter aortic valve implantation (TAVI) is increasingly used to treat patients at high risk for conventional surgery. As the procedure becomes widespread and the number of cases increases, it necessitates a predictive scoring against complications that may develop during and after the procedure. In this study, we compared Syntax Score II (SS II) which are used in the diagnosis, staging and treatment of coronary artery disease with conventional risk scores ; EUROSCORE and STS, in terms of predicting in-hospital mortality. METHODS: Seventy patients who underwent TAVI between 2011 and 2018 in our hospital for aortic stenosis were included in the study. Patients were divided into two groups according to in - hospital mortality development and the relationship between risk scores and mortality was investigated. FINDINGS: 58.6% of the patients were male and the mean age was 77.4 ± 10.1 years. The patients were divided into two groups who died in the hospital and were discharged safely. In univariate analysis, STS score, EUROSCORE and SS II, left ventricular systolic diameter and mean aortic gradient was high in the group at which patients were death. The incidence of postoperative complications was similar in both groups (p: 0.22 for stroke, p: 0.50 for aki, p: 0.49 for pacemaker implantation).In the regression analysis, EUROSCORE was found to be an independent predictor of in-hospital mortality in TAVI patients (p:0.05). The other two scores were not statistically significant.CONCLUSION: EUROSCORE has significantly better predictive power among the risk scores commonly used in predicting perioperative and in-hospital mortality in patients undergoing TAVI. ÖZETGiriş: Koroner kollateral dolaşım (KKD) genellikle kritik bir koroner darlık neticesinde gelişen vasküler yapılardır. Bu yapıların gelişmesinde lezyon ciddiyeti gibi anatomik nedenlerin yanı sıra bazı biyokimyasal ve klinik faktörler de suçlanmıştır. Ancak, KKD gelişmesine katkıda bulunan bu faktörler tam olarak incelenmemiştir. Biz bu çalışmada ST segment yükselmesi olmayan akut sendrom (NSTE-AKS) hastalarında koroner kollateral dolaşım ile klinik-biyokimyasal faktörlerin ilişkisini saptamaya çalıştık.Gereç ve Yöntem: Çalışmamız kesitsel tarzda dizayn edilmiş olup, çalışmaya toplam 100 NSTE-AKS tanısıyla kliniğimize başvuran hastalar alınmıştır. Koroner anjiografide, koroner arterlerin herhangi birinde %80 üzerinde lezyon olması çalışmaya alınma kriteriydi. Koronerlerinde %80' den az lezyonu olan hastalar, KKD gelişimi için yeterli olmadığından çalışma dışı bırakıldı. Koroner kollateral dolaşım Rentrop sınıflamasına göre evrelendirildi. Hastalar kötü KKD olanlar (Grup-1, Rentrop evresi 0–1) ve iyi KKD olanlar (Grup-2, Rentrop evresi 2–3) olarak iki gruba ayrıldı. Tutulan damar sayısı, major epikardiyal arterlerde lezyon olması olarak tanımlanmıştır. TIMI 3 risk skoru daha önce tanımlandığı şekliyle ölçüldü.Bulgular: Çalışmaya toplam 100 NSTE-AKS hastası alınmıştır (yaş ortalaması 61.0±12.6 yıl, %56'sı erkek). Kötü KKD olan hastalarda, iyi KKD olan hastalara göre DM sıklığı daha fazla, Killip evresi daha yüksek, pre-infarkt angina varlığı daha az, bazal CK-MB ve başvuru beyaz küre sayısı daha yüksek ve SV-EF daha düşük saptandı. Diğer değişkenler için gruplar arasında herhangi bir farklılık saptanmadı. Hangi değişkenin KKD'ın bağımsız öngördürücüsü olduğunu tespit etmek için çoklu lojistik regresyon analizi yapıldı. Analiz sonucuna göre sadece DM varlığı (OR: 4,27, %95 GA 1,18 - 15,40, p=0,026) ve yüksek bazal CK-MB düzeyleri (OR:1,047, %95 GA 1,013-1,083) kötü KKD'nin bağımsız öngördürücüleri olarak bulundu. Kötü KKD'nin öngördürülmesinde Bazal CK-MB düzeylerinin en iyi şekilde tahmin edilmesi için ROC curve analizi yapıldı. Bazal CK-MB ≥30,5 U/l olması kötü KKD'nin tahmin edilmesinde %65 duyarlılık %64 özgüllüğe sahiptir (AUC:0.672, %95 GA 0.561-0.783, p=0,004). Sonuç: Çalışmamızda, NSTE-AKS, sorumlu koroner arterde KKD öngördürücülerini araştırdık ve hastaneye başvuru CK-MB düzeyi ile Diyabetes Mellitus varlığını, kötü KKD'nin bağımsız öngördürücüleri olduğunu saptadık. Introduction: Coronary collateral circulation(CCC) develops as a result of critical coronary artery stenosis. In the development of CCC, anatomical lesion severity and some biochemical and clinical factors have been implicated. However, these factors contributing to the development of CCC has not been fully studied. The aim of this study was to assess relationship of coronary collateral circulation and clinical-biochemical factors in patients with acute coronary syndromes without ST-segment elevation (NSTE-ACS) . Materials and Methods: A cross-sectional study has been designed and a total of 100 patients with NSTE-ACS were included in our study. Any coronary artery lesion above %80 on coronary angiography was inclusion criteria. Patiens with coronary artery lesion less than 80% was exluded from the study. Coronary collateral circulation was graded according to Rentrop classification. Patients were separated into two groups: poor CCC group (Group-1, rentrop stage 0-1) and good CCC group (Group-2, rentrop stage 2-3). Number of involved vessels was defined having major epicardial lesion in arteries. Results: A total of 100 NSTE-ACS patient were included in to our study (mean age 61.0 ± 12.6 years, 56% men ) . In Poor CCC group; DM was more frequent , Killip stage was higher , presence of pre- infarction angina was less, basal CK-MB and reference white blood cell count were higher and LV -EF was lower compared with good CCC group. For other variables there were no differences between groups. Multiple logistic regression analysis was performed to detect which variable is the independent predictor of CCC. According to the results of analysis only the presence of DM (OR: 4.27, 95% CI 1.18 to 15.40, p = 0.026 ) and higher baseline CK-MB levels ( OR: 1.047, 95% CI 1.013 to 1.083 ) was found as independent predictors of poor CCC. ROC curve analysis was performed to estimate the basal CK-MB levels, truely. Basal CK-MB ≥ 30,5 U/l has a sensitivity of 65% and specificity of 64% to estimate poor CCC (AUC: 0.672, 95% CI 0561-0783, p = 0.004 ) .Conclusion: In our study, we investigated predictors of poor CCC in responsible coronary artery in patiens with NSTE-ACS and we found that CK-MB levels on admission and presence of DM are independent predictors of poor CCC. 88

4-ÖZETGünümüzde kardiyologlara tipik anginal karakterde göğüs ağrısı ile gelip koroneranjiyografisinde yavaş akım tesbit edilen hastalar oldukça sıktır. Kontrast maddenin koronerarter içerisinde yavaşça ilerlemesi nedeniyle bu klinik tablo koroner yavaş akım olarakadlandırılmıştır.Yapılan çalışmalarda bu hastalarda mikrovasküler düzeyde rezervin anormal olduğutesbit edilmiştir. Mikrovasküler düzeydeki bu anormallik, miyokardın fonksiyonlarındabozukluk meydana getirmektedir. Miyokard fonksiyonlarındaki bu bozukluk çeşitli non-invazifve invazif çalışmalarda gösterilmiştir. Koroner yavaş akımlı hastalarda gelişen diyastolikdisfonksiyon çeşitli çalışmalarla gösterilmiş olmasına rağmen, sistolik fonksiyonlar üzerineetkisini inceleyen çok az çalışma mevcuttur. Çalışmamızda koroner yavaş akımlı hastalarınsistolik fonksiyonlarını, sol ventrikül (SV) fonksiyonlarını kantitatif olarak değerlendirmemizeolanak sağlayan Strain ve Strain Rate ekokardiyografi ile inceledik.Çalışmaya anjiografi sırasında koroner yavaş akım saptanan 25 hasta (Grup 1) ileepikardial koroner arterleri normal bulunan benzer yaş grubundan sağlıklı 20 kontrol grubu(Grup 2) alındı. Hastaların TIMI kare sayıları Gibson ve ark. tarafından tanımlanan yöntemlebelirlendi. Hasta ve kontrol grubunun iki boyutlu ekokardiyografi, PW doku doppler kayıtları,sol ventrikül Strain/Strain Rate kayıtları alındı. Tüm hastalara myokard perfüzyon sintigrafisiyapıldı.İki grubun demografik özellikleri arasında bir fark yoktu, iki boyutlu ekokardiyografibulgularında da gruplar arasında fark yoktu. Ill ÖZET PRIMER PERKÜTAN TRANSLUMINAL KORONER ANJİYOPLASTİ UYGULANAN AKUT MİYOKARD İNFARKTÜSLÜ HASTALARDA SOL VENTRİKÜL BÖLGESEL SİSTOLİK FONKSİYONUNUN LONGITUDINAL STRAİN/STRAİN RATE İLE DEĞERLENDİRİLMESİ Bölgesel myokardial fonksiyonların non-invaziv olarak kantitifiye edilmesi Klinik Kardiyoloji'nin önemli bir hedefidir. Strain (S)/strain rate (SR) görüntüleme bölgesel deformasyonu tanımlamakta olup kardiyak translasyon ve tethering'e daha az duyarlı olduğundan bölgesel fonksiyon değerlendirmelerinde doku Doppler görüntüleme'den üstündür. SR lokal deformasyon hızını ölçerken, S deformasyon yüzdesini ifade etmektedir. Anterior akut miyokard infarktüslü (AAMİ) hastalarda klinik gidişat sol ventrikül (SV) fonksiyon bozukluğunun derecesi ile ilişkilidir. Deneysel çalışmalarda olduğu gibi klinik çalışmalarda da iskemik miyokardın erken reperfüzyonu ile tehdit altındaki miyokardiyal dokunun kurtarıldığı gösterilmiştir Bu çalışmanın amacı primer perkütan transluminal koroner anjiyoplasti (PTKA) endikasyonu konulmuş akut miyokard infarktüslü (AMİ) hastalarda sol ventrikül bölgesel sistolik fonksiyonlarında girişim sonrası meydana gelen değişimlerin longitudinal strain/strain rate kullanılarak tespiti ve bu değişimin sol ventriküler yeniden biçimlenme ve mortalite üzerine 30. gündeki etkilerinin belirlemesidir. Bu çalışmaya AAMİ' nün ilk 6 saati içinde müracaat eden ardışık 21 hasta (yaş ortalaması: 60.5±11.3 yıl) ve koroner angiogarfisi normal sağlıklı 20 kişi (yaş ortalaması: 49.4±9.1 yıl) alındı. Geçirilmiş iskemik kalp hastalığı öyküsü olanlar, miyakardiyal hastalığı olanlar, atriyal fibrilasyon ve dal bloğu olanlar çalışmaya alınmadı. Ekokardiyografik kayıtlar girişimden hamen once (invaziv prosedürünIV gecikmesine neden olmayacak şekilde), hastalığın 1. haftasında ve 1. ayında alındı. Girişim sonrası dönemde apikal görüntülemeden sol ön inen arterin beslediği segmentlerin bazal, mid ve apikal segmentlerinden S,SR ve velosite (V) ölçümleri yapıldı. Hastalarda tüm işlemler başarılı kabul edildi. Tekrarlayan iskemi ve miyokard infarktüsü gözlenmedi. Hastalarda AAMİ sırasında Sol ön inen arterin beslediği segmentlerde sistolik V, S ve SR değerlerinin kontrole kıyasla düşük olduğunu gözlemledik. AMİ sırasında sistolik S ve SR değerlerinin apikal segnıentten bazale kadar olan homojen dağılımı kayboldu. Velositeler üzerine özellikle apikal segmentler başta olmak üzere reperfüzyonun etkisi gözlenmedi. Başarılı primer PTKA' nın S/SR üzerine önemli bir etkisi bu indekslerin sol ventrikül duvarları boyunca homojen dağılımını sağlamasıydı. Hastalarda SV sistolik fonksiyonlar bozulmadı. Takiplerde S/SR değerleri ile duvar hareket skor indeksi arasında güçlü negatif korelasyonlar, EF ile pozitif korelasyonlar tespit edildi. Bu çalışmada başarılı primer PTKA işlemi hastalan remodellingden korudu. Primer PTKA işleminin yeniden şekillenme üzerine olan etkileri renkli Doppler miyokardiyal görüntüleme kaynaklı S/SR ekokardiyografi ile değerlendirildi. SV duvarlarından elde edilen S ve SR verileri AAMİ'nün erken döneminde korunan global SV fonksiyonlarının tahmininde değerli bilgiler sağladı. Bu çalışmada, S/SR uygulamasının geçerliliği ekokardiyografi ile S/SR değerleri ve sistolik fonksiyonlar arasındaki iyi korelasyonun gösterilmesi ile desteklendi. Bizim sonuçlarımız başarılı primer PTKA yapılan AAMİ'lü hastalarda akut iskemi ve remodelling' in belirlenmesinde velositelerden üstün olan S ve SR verilerinin kullanılmasının klinik olarak avantaj lannı gösterdi. SUMMARY THE ASSESSMENT OF LEFT VENTRICULAR REGIONAL SYSTOLIC FUNCTION BY LONGITUDINAL STRAIN / STRAIN RATE IN ACUTE MYOCARDIAL INFARCTION PATIENTS UNDERGOING PRIMARY PERCUTANEOUS TRANSLUMINAL CORONARY ANGIOPLASTY The non-invasive quantification of regional myocardial functions is an important goal in clinical cardiology. Strain (S)/strain rate (SR) imaging that depicts regional deformation is less susceptible to cardiac translation and tethering and may be superior to TDI for regional functional analysis. While regional systolic SR measures the rate of local deformation, regional systolic S expresses the percentage of deformation. The clinical course of patients with AAMI is related to the degree of left ventricular functional impairment. Experimental as well as clinical studies have demonstrated that early reperfusion of the ischemic myocardium can salvage jeopardized myocardial tissue. The aim of this study was the identification of the changes in regional systolic function in the left ventricle using longitudinal S/SR indices at the post interventional period and the determination of the impact of these changes on mortality and on remodelling at 30 days in acute anterior myocardial infarction patients (AAMI) undergoing primary percutaneous transluminal coronary angioplasty (PTCA). Twenty-one consecutive patients (mean age: 60.5±11.3 years) within the first 6 hours of AAMI and 20 healthy control subjects (mean age: 49.4±9.1 years) who had normal coronary angiogram were included in this study. Patients with previous coronary ischemic heart events, myocardial disease, valvular heart disease, atrial fibrillation and bundle branch block were excluded. Echocardiographic recordings were performed justVI before intervention (without any delay for invasive procedure), at the 1st week and the 1st month of the disease. Longitudinal myocardial S/SR and velocity (V) interrogated using apical views were postprocessed from basal, mid and apical segments which supplied by the left anterior descending artery (LAD). All of the procedures were accepted successful in patients. Recurrent ischemia and MI were not observed. We have observed a decrease of systolic V, systolic S and SR values in the segments supplied by the LAD during AAMI in patients compared to controls. The homogeneous distribution of systolic S and SR indices from apical to basal segments was lost during myocardial infarction. The impact of reperfusion on myocardial systolic velocities could not demonstrated, especially apical segments. One of the significant effects of successful primary PTC A on S/SR indices was that these indices provide a homogenous distribution of deformation throughout the left ventricular walls. Left ventricular systolic functions were not deteriorated in the patients. The strong negative correlations were observed between S/SR indices and wall motion score index and positive correlations between S/SR indices and EF in the serial follow ups. The successful primary PTCA procedure was prevention of the remodeling in patients at this study. The impact of primary PTCA on remodeling was to evaluate by the color Doppler myocardial imaging derived S and SR echocardiography. The parameters derived from S/SR indices of LV walls provide valuable information to predict preserved global LV function during the early period of an AAMI. In the present study, the validity of this approach (S/SR technic) was supported by the demonstration of a good correlation between the S/SR and systolic function by echocardiography. Our results demonstrate the potential clinical advantages of using S and SR parameters over velocity in the detection of acute ischemia and remodeling in patients with AAMI undergoing successful primary PTCA. 80

III ÖZET Saf mitral darlığı miyokardiyal ve fonksiyonel faktörlerin bir sonucu olarak sol ventriküler performansı etkilemektedir. Bu durum bazı hastalarda klasik ekokardiyografi teknikleriyle tespit edilemese bile TDI (Tissue Doppler Imaging) ile uzun aks fonksiyonlarında bozulma olarak gösterilebilmektedir. Mitral darlığına sıklıkla eşlik eden bir aritmi olan atrial fibrilasyonun sol ventrikül üzerindeki etkilerini değerlendiren bir çalışmaya ise literatürde rastlanmamıştır. Bu çalışmada; atrial fibrilasyonun eşlik ettiği mitral darlıktı hastalarda standart ekokardiyografik yöntemlerle birlikte TDI görüntüleme yöntemini kullanarak sol ventrikül sistolik fonksiyonlarını değerlendirmek ve elde edilen parametrelerin atrial fibrilasyon ile ilişkisini araştırmak amaçlandı. Çalışmaya 30'u sinüs ritminde, 25'i atrial fibrilasyonlu toplam 55 mitral darlıklı hasta ve 30 normal kontrol vakası alındı. Hasta ve kontrol grubunun standart ekokardiyografi yöntemi ile kardiyak çapları, mitral kapak alanları, sol ventrikül ve sol atrium ejeksiyon fraksiyonları hesaplandı. Triküspit kaçağı olanlarda CW doppler ile pulmoner arter basıncı hesaplandı. M-mod ile septal ve lateral mitral annulus yerdeğiştirmeleri hesaplandı. İİİ ÖZET Konjestif kalp yetersizliği tüm dünyada en önemli ölüm nedenlerinin başında gelmektedir. Ölüm; genellikle hastalardaki aniden ortaya çıkan aritmi ve tromboembolik komplikasyonlar nedeniyle olmaktadır. Konjestif kalp yetersizliğinin tedavisi nörohumoral mekanizmaların daha iyi anlaşılması ile bu sürecin üzerine yönlendirilmiş ve yapılan çalışmalarla tedavide verilen B blokerlerin mortaliteyi azaltıcı etkileri gösterilmiştir. Bu çalışmada, konjestif kalp yetersizlikli hastalarda nonselektif (Bı ve B2) bir B bloker olan, aynı zamanda ctı bloker etkisi de bulunan karvedilol tedavisinin sol atriyal appendiks fonksiyonları üzerindeki etkilerinin belirlenmesi amaçlandı. Çalışmaya semptomatik konjestif kalp yetersizliği olan 16' sı erkek 4'ü kadın 20 hasta alındı. Hastalar uzun süredir klasik kalp yetersizliği tedavisi almaktaydı. Hastaların bazalde transözefajiyal ekokardiyografi ile sol atriyal appendiks fonksiyonları, transtorasik ekokardiografi ile sol ventrikül fonksiyonları değerlendirildi. Hastalara minimal dozda karvedilol başlanıp 15 günde bir doz artırımı yapılarak tolere edebildikleri maksimal doza çıkıldı. Her hastaya karvedilol tedavisinin 3. ayının sonunda transözefajiyal ekokardiyografi ile sol atriyal appendiks fonksiyonları, transtorasik ekokardiografi ile de sol ventrikül fonksiyonları tekrar değerlendirildi. Sol atriyal appendiks fonksiyonlarının göstergesi olarak sol atriyal appendiks geç boşalma velositesi, velosite time integrali, sol atriyal appendiks ejeksiyon fraksiyonu ve sol atriyal appendiks ve sol atriyumdaki spontan ekokardiyografik kontrast yoğunluğu, sol ventrikül performansını değerlendirmek için sol ventrikül diyastol sonu çapı, sol ventrikül sistol sonu çapı,sol ventrikül ejeksiyon fraksiyonu,sol ventrikül diastol sonu volümü ve sol ventrikül sistol sonu volümü ölçüldü. 38 ÖZET Çalışmamızda, rutin koroner anjiografi ve PTCA yapılan hastalarda işlemden sonraki plazma ADM seviyelerinin artıp artmadığını göstermeyi amaçladık. Çalışmaya koroner anjiografi endikasyonu konulan toplam 89 hasta alınmıştır. Koroner anjiografi sonucuna göre hastalar, normal, KAH olup PTCA yapılmayan ve PTCA yapılan olgular olarak üç gruba ayrılmıştır. Koroner anjiografisi normal bulunan 21 hasta dışında diğer olgulara (n=68) sol ventrikülografı yapıldı ve LVEDP'leri ölçüldü. Her üç grubun periferik venlerinden, koroner anjiografi öncesi ve sonrası plazma ADM seviyelerine bakıldı. Koroner anjiografisi normal olan grupta (n=21) işlemden önceki plazma ADM seviyeleri ortalama 13,9+1,7 p.g/ml, sonraki ise 13,7+1,7 p.g/ml bulundu. Aralarındaki fark istatistiksel olarak önemli değildi (P>0,05). Koroner arter hastalığı olup PTCA yapılmayanlarda (n=22) işlem öncesi plazma ADM değerleri 15,5±1,6 p.g/ml, sonrası 16,5+1,7 p.g/ml bulundu. Her iki grup arasında istatistiksel olarak anlamlı fark yoktu (P>0,05). PTCA yapılan hastaların (n=46) işlem öncesi plazma ADM seviyeleri 17,1+1,8 p.g/ml, sonrası 27,9±2,5 p.g/ml bulundu. Her iki grup arasındaki fark istatistiksel olarak önemliydi (P0.05). These levels were 15,5 ±1.6 p.g/ml and 16,5±1.7 p.g/ml in CAD patients without PTCA. - Abstract
Objective: This study aimed to define the prevalence and predictors for pseudoaneurysm after coronary angiography, cardiac catheterization and percutaneous coronary interventions (PCIs) performed via the femoral artery., Methods: The study included 8469 patients enrolled between January 2007 and December 2009 on whom cardiac catheterization, coronary and/or peripheral angiography and PCIs via the femoral artery were performed. All data, including clinical characteristics and complications, were obtained retrospectively from patient chart records., Results: Pseudoaneurysm was detected in 65 (0.76%) patients. Pseudoaneurysm was ascertained more frequently in patients with a history of coronary artery disease (0.9% vs. 0.4%; p=0.012), in females than in males (1.4% vs. 0.5%; p<0.001), in patients older than 65 years (1.2% vs. 0.6%; p=0.002), in patients with a history of femoral artery intervention (1.2% vs. 0.6%; p=0.01), in hypertensives than in normotensives (1.3% vs. 0.5%; p<0.001), in patients taking low molecular weight heparin (1.0% vs. 0.2%; p<0.001), in patients taking clopidogrel (1.0% vs. 0.4%; p=0.007), and in patients with chronic renal disease (3.8% vs. 0.7%; p<0.001). There was no statistically significant trend (1.2% vs. 0.7%; p=0.053) towards more pseudoaneurysm formation in emergent interventions than in elective procedures., Conclusion: Patients with a higher risk of pseudoaneurysm development following intervention via the femoral artery should be specified and extra attention given during the intervention. These patients should be informed of the increased risk of this complication and its results, and should be under close follow-up concerning development of iatrogenic femoral pseudoaneurysm.

26. Acute effect of primary percutaneous coronary intervention on left ventricular dyssynchrony in ST-segment elevation myocardial infarction.

Objective: The aim of this study was to prospectively evaluate the effect of percutaneous coronary intervention in the acute period on left ventricular dyssynchrony in ST-segment elevation myocardial infarction patients by using Tissue Synchronization Imaging., Methods: Forty-four ST-segment elevation myocardial infarction (MI) patients (29 male, 15 female), who were admitted within the first 12 hours of chest pain symptoms, were enrolled in the study. According to the localization of MI, the patients were divided into groups as anterior MI (n=26) and inferior MI (n=18). All echocardiography measurements were taken just before percutaneous coronary intervention (PCI) and following PCI at a mean of 3-6 days. They were assessed according to the time to reach the peak systolic velocity, which was calculated by the tissue synchronization imaging method from four pairs of non-apical alternate segments. The difference between the duration to reach the peak systolic velocity in alternate segments was regarded as left ventricle dyssynchrony and the results were compared., Results: In the anterior MI group, basal anterior (p<0.01), mid-anterior segment (p<0.01) and basal septal segment (p<0.01); in the inferior MI group, the basal septal segment (p=0.02), mid-septal segment (p=0.02), and basal and mid-inferior segment (p<0.01) values were significantly lower in the post-PCI measurements when compared to the measurements taken prior to PCI. In both groups, the intraventricular dyssynchrony indices of the basal anterior-basal inferior (p<0.01), mid-anterior-mid-inferior (p<0.01) segments were found to be significantly lower in the post-PCI period when compared to the pre-PCI period., Conclusion: It was found that percutaneous coronary intervention in patients with ST-elevation significantly decreases the degree of LV dyssynchrony in the acute period.
