Abstract

PURPOSE OF THE STUDY Scapholunate interosseous ligament (SLIOL) tears with accompanying extrinsic ligament rupture have been associated with scapholunate (SL) instability. SLIOL partial tears were examined in terms of tear localization, grade and accompanying extrinsic ligament injury. Conservative treatment responses were scrutinized according to injury types. MATERIAL AND METHODS Patients with SLIOL tear without dissociation were evaluated retrospectively. Magnetic resonance (MR) images were reexamined in terms of tear localization (volar, dorsal or combined volar and dorsal tears), grade of injury (partial or complete) and extrinsic ligament injury accompaniment (RSC, LRL, STT, DRC, DIC). Injury associations were examined with MR imaging. All patients treated conservatively were recalled at their first year for re-evaluation. Conservative treatment responses were analyzed according to pre- and post-treatment first year visual analog scale for pain (VAS), disabilities of the arm, shoulder and hand questionnaire (DASH) and Patient-Rated Wrist Evaluation (PRWE) scores. RESULTS In our cohort, 79% (n: 82/104) of patients had SLIOL tear and 44% (n: 36) of them had accompanying extrinsic ligament injury. The majority of SLIOL tears and all extrinsic ligament injuries were partial tears. In SLIOL injuries, volar SLIOL was most commonly damaged portion (45%, n: 37). DIC (n: 17) and LRL (n: 13) were most frequently torn ligaments, radiolunotriquetral (LRL) injury generally co-existed with volar tears and dorsal intercarpal ligament (DIC) with dorsal tears regardless of injury time. Extrinsic ligament injury accompaniment was associated with higher pre-treatment VAS, DASH and PRWE scores than isolated SLIOL tears. Injury grade, location and extrinsic ligament accompaniment had no significant effect on treatment responses. Test scores reversal was better in acute injuries. CONCLUSIONS On imaging SLIOL injuries, attention should be paid to the integrity of secondary stabilizers. In partial SLIOL injuries, pain reduction and functional recovery can be achieved with conservative treatment. Conservative approach can be the initial treatment option in partial injuries especially in acute cases regardless of tear localization and injury grade if secondary stabilizers are intact. Abstract

Novel distinction drawing is an active process that characterizes mindfulness, which has been associated with an open, creative, and probabilistic mental state, as well as the ability to examine information from new perspectives. The literature review revealed a lack of measurement tools for assessing mindfulness from a social and cognitive perspective in Türkiye. In addition, the frequent use of the Langer Mindfulness Scale (LMS) in educational contexts and its adaptation into many languages highlights the need for a Turkish version of the scale. This study aims to validate the Turkish version of the 14-item LMS. The Turkish version’s factorial structure was tested using the results of confirmatory factor analysis (CFA), which confirmed a three-factor structure that included the engagement, novelty-producing, and novelty-seeking subscales. It was found that measurement invariance based on gender was provided by LMS scores. LMS Turkish version showed satisfying psychometric properties in terms of reliability. Additionally, convergent and discriminant validity were examined in this validation study to provide evidence for criterion-based validity. For this purpose, the relationships between Turkish LMS scores and variables such as positive and negative affect, openness to experiences, self-acceptance, self-defined humor, and health were analyzed. The results showed that self-defined humor, positive affect, openness to experience, and self-acceptance were significantly positively correlated with the Turkish LMS scores, while LMS scores exhibited a significant negative relationship with negative affect. These findings suggest that the Turkish version of the LMS, with its three dimensions, shows acceptable psychometric properties for assessing the state of mindfulness. The Turkish version of the LMS is expected to be used in socio-cognitive mindfulness research in the Turkish cultural context.

Abstract

PURPOSE OF THE STUDY The purpose of this study is to reveal the waste of water by medical staff hand scrubbing preoperatively in the operation theatre, draw attention to the water consumption with small precautions. The study compares motion -sensitive sensor tap with an uncensored classic tap, to assess the difference of water consumption during hand scrubbing. MATERIAL AND METHODS The presented study compared two groups; hand scrubbing with sensory tap was enrolled as group A and classic tap with running water was enrolled as group B. Three participants were included in each group. Operation faucets with a motion-sensitive sensor and timekeeper were used in group A. Running water from a tap in 10 seconds was measured with the help of a measuring cup. The water amount after 3 minutes of washing was also measured. The difference between the two obtained data was compared. The obtained data were also multiplied to the global surgical data. RESULTS Measurements were performed with a measuring cup and it was measured that 1,250 ml of water is running from faucets in 10 seconds. In group A, faucets were open for 23 seconds whereas in group B faucets were open for 180 seconds. In group A, water utilization was 2,875 ml per person (8,625 ml for three), in group B was the water utilization of 22,500 ml per person (67,500 ml for three). Regarding this data for a single operation, the consumption difference between the two groups was measured as 58,875 ml. The total count of major surgery worldwide per year is reported to 321,500,000. When this number is multiplied with the obtained data, it is calculated that the total amount of water to be saved between the two groups is 18,928,313 m(3). That is equal to one year of water consumption of a city with 125,000 inhabitants. CONCLUSIONS In conclusion, as healthcare professionals, it is possible to save enough water as a medium-sized city by changing our handwashing habits in the operation rooms. As a result, water-conserving systems and hand scrubbing education can be very effective in using the natural resources economically and protecting these resources.

15. A novel chatter detection method for milling using deep convolution neural networks

Abstract

Regenerative chatter is harmful to machining operations, and it must be avoided to increase production efficiency. The recent success of deep learning methods in many fields also presents an excellent opportunity to advance chatter detection and its wider industrial adoption. In this work, a chatter detection method based on deep convolutional neural network (DCNN) is presented. The method uses a cardinal model-based chatter solution to precisely label regenerative chatter levels. During milling, vibration data are collected via a non-invasive data acquisition strategy. Considering nonlinear and non-stationary characteristics of chatter, continuous wavelet transform (CWT) is used as the pre-processing technique to reveal critical chatter rich information. Afterward, the images are used for training and test of the developed DCNN. Abstract

Tool condition monitoring and machine tool diagnostics are performed using advanced sensors and computational intelligence to predict and avoid adverse conditions for cutting tools and machinery. Undesirable conditions during machining cause chatter, tool wear, and tool breakage, directly affecting the tool life and consequently the surface quality, dimensional accuracy of the machined parts, and tool costs. Tool condition monitoring is, therefore, extremely important for manufacturing efficiency and economics. Acoustic emission, vibration, power, and temperature sensors monitor the stability and efficiency of the machining process, collecting large amounts of data to detect tool wear, breakage, and chatter. Studies on monitoring the vibrations and acoustic emissions from machine tools have provided information and data regarding the detection of undesirable conditions. Herein, studies on tool condition monitoring are reviewed and classified. As Industry 4.0 penetrates all manufacturing sectors, the amount of manufacturing data generated has reached the level of big data, and classical artificial intelligence analyses are no longer adequate. Nevertheless, recent advances in deep learning methods have achieved revolutionary success in numerous industries. Deep multi-layer perceptron (DMLP), long-short-term memory (LSTM), convolutional neural network (CNN), and deep reinforcement learning (DRL) are among the most preferred methods of deep learning in recent years. As data size increases, these methods have shown promising performance improvement in prediction and learning, compared to classical artificial intelligence methods. This paper summarizes tool condition monitoring first, then presents the underlying theory of some of the most recent deep learning methods, and finally, attempts to identify new opportunities in tool condition monitoring, toward the realization of Industry 4.0.

Since chatter lowers the performance of the machining operations, it is very crucial to detect and take timely action. In this research, an acoustic emission (AE) sensor was mounted to the milling machine and collects acoustic emissions. Captured data was used for training and testing by the estimation model in order to detect chatter.

Abstract

PURPOSE OF THE STUDY The purpose of this study is to evaluate the results of chronic tendinous mallet finger repair with all soft suture anchors. MATERIAL AND METHODS In this retrospective study we evaluated 14 patients. Data included age, side of surgery, the degree of extensor lag, the ratio of the damaged articular surface, postoperative recovery period and time to return to work. We recorded extensor lag and Crawford scores of all patients at the last follow-up. RESULTS Mean follow-up period was 22.07 months. The preoperative and postoperative extensor lags were 27.2 (15-35) and 3 (0-19), respectively. Crawford scores of patients were moderate in 1 patient, fair in 2 patients and excellent in 10 patients. We observed only one skin complication in the postoperative period. CONCLUSIONS All-soft suture anchors have high patient satisfaction rates and functional results. Thus, they could be used in the treatment of patients with chronic tendinous mallet finger. The low complication rate is a major advantage. [ABSTRACT FROM AUTHOR]

Background: In recent years, minimally invasive techniques with less tissue excision, rather than surgical treatments including wide tissue excisions, are preferred in the surgical treatment of pilonidal sinus. Crystallized phenol treatment is a widely used minimally invasive method. The aim of this study is to examine the results of applying crystallized phenol in a two-stage manner. Material and Methods: In this study, demographic information, pilonidal sinus characteristics, and results of crystallized phenol treatment of primary pilonidal sinus patients treated with crystallized phenol at Ankara Yıldırım Beyazıt Education and Research Hospital General Surgery Clinic between January 2016 and July 2021 were retrospectively analyzed. Abstract

In the current study, a number of alkaloids including retamine, cytisine, and sparteine (quinolizidine-type), yohimbine and vincamine (indole-type), scopolamine and atropine (tropane-type), colchicine (tropolone-type), allantoin (imidazolidine-type), trigonelline (pyridine-type) as well as octopamine, synephrine, and capsaicin (exocyclic amine-type) were tested in vitro for their inhibitory activity against acetylcholinesterase (AChE) and butyrylcholinesterase (BChE) at 1 mg/ml concentration by the Ellman method using an ELISA microplate reader. Among the alkaloids tested, only capsaicin exerted a remarkable inhibitory effect towards both AChE and BChE [(62.7 ± 0.79)% and (75.3 ± 0.98)%, respectively]. While the rest of the alkaloids did not show any significant inhibition against AChE, three of the alkaloids, namely retamine, sparteine, and yohimbine, exerted a noteworthy anti-BChE effect as compared to galanthamine, the reference drug. © 2007 Verlag der Zeitschrift für Naturforschung.

Abstract

PURPOSE OF THE STUDY We aimed to investigate the functional outcomes and complications of a new approach, the primary UCL repair using JuggerKnot™ Soft Anchor-1.0 mm Mini (Zimmer-Biomet). MATERIAL AND METHODS This study included a total of 12 patients with acute UCL injury who were operated with primary repair with JuggerKnot™ Soft Anchor-1.0 mm Mini between January 2012 and September 2016. All patients were operated on using the same surgical technique. The thumb pinch and grip strengths, articular range of motion, and Glickel functional score were recorded for all patients. The pinch grip strength and articular range of motion were compared with the intact side. Early and late postoperative complications were recorded. RESULTS The mean follow-up time was 22.2 months (range 6-54 months). The grip strength and the pinch strength were 94.3% and 92.27%, respectively, of the contralateral side. Articular range of motion attained the same level as the contralatereal side in all patients at the final visit, and no patient suffered from any complication. The patients returned to work at a mean of 5.45 weeks, and the Glickel score was good in 1 patient and excellent in the remainder 11 patients. CONCLUSIONS Surgical repair using the JuggerKnot™ Soft Anchor-1.0 mm is an effective alternative treatment method for acute total ulnar collateral ligament (UCL) rupture. [ABSTRACT FROM AUTHOR]

Abstract

Tugboats are of vital importance in ports where a significant portion of world trade takes place. Selection of a tugboat that suitable for different operations in a port is a challenging problem that requires many different criteria to be evaluated at the same time. This selection requires high experience as well as technical knowledge of the tugboats and the operations to be carried out. In the present paper, an integrated model for evaluation and selection of tugboats is given. Based on the statistical data available in the study, assessment of the effect of different criteria on different harbour tugboats categorized according to the propulsion systems were carried out. The criteria for the tugboat alternatives were assessed through a questionnaire by subject-matter-experts containing comparative technical, financial and operational questions. The weights of each criteria were calculated using fuzzy Shannon's entropy and fuzzy TOPSIS was utilized to rank the alternatives. Abstract

Abstract Background Vascular blow-out syndrome is a life-threatening bleeding condition that usually occurs in the carotid arteries as a result of invasion of head and neck malignant tumors. There are several case reports in the literature on peripheral vascular blow out. To our knowledge, there is no other case report in the literature on blow-out in the femoral artery secondary to skin malignancy. Case presentation A 66-year-old male patient, who was diagnosed with skin squamous cell carcinoma (SCC) as a result of tissue sampling due to redness and stiffness in the left inguinal region, underwent emergency surgery with severe bleeding. Since edema and deformations on the skin did not render the operation possible, the patient was taken to the interventional radiology unit by applying pressure compression onto the bleeding area for urgent endovascular treatment. The patient was treated with a stent-graft. Conclusion The femoral blow-out is an emergency condition that needs to be managed aggressively, and if left untreated, it can cause serious bleeding, limb ischemia, limb loss, or death. Endovascular stent-graft placement is the first method that should be considered, and this method provides fast and effective treatment.

Dental implantlann fiziksel ve biyokimyasal özellikleri basarı oranlan üzerinde önemli bir role sahiptir. Yapılan daha önceki çalışmalar göstermiştir ki, yüzey porözlülüğü arttıkça başarı oranı da artmaktadır. Bundan dolayı poröz yüzey başarı için vazgeçilemez bir unsurdur. Bunun yanı sıra, günümüzdeki çalışmalar implant materyallerinin biyolojik özelliklerini iyileştirmek için yüzey kaplama materyalleri ve metotları üzerine yoğunlaşmıştır. Bu makalede literatürdeki son zamanlarda yapılan çalışma sonuçları derlenerek; gelecekteki çalışmalara hedefler belirleyerek implant kaplama materyal ve metotlarının gelecekteki durumunun profili çıkarılmaya çalışılmıştır. Physical and biochemical properties of dental implants have an important role on their success rate. Previous experiences showed that success rate proportionally increases with surface porosity. Therefore, porous surface is mandatoiy for success. Moreover, recent efforts are focused on surface coating materials and methods to enhance biologic behavior of the implant material. This article reviews recent study results in the literature and tries to profile the future aspect of coating materials and methods of dental implants to address a new target for following studies.

Abstract

Objectives: Epithelium at the apex of a nonvital tooth can be presumably stimulated by inflammation to form a true epithelium-lined cyst, or radicular cyst. The source of the epithelium is usually a rest of Malassez but also may be traced to sinus lining or epithelial lining of fistulous tracts. Cyst development is common; the reported frequency varies from 7% to 54% of periapical radiolucencies. The incidence of radicular cysts is greater in the third to sixth decades and shows a slight male predominance. Most of the radicular cysts are found in the maxilla, especially around incisors and canines.Methods: A 38-year-old male patient was referred to the Department of Oral Diagnosis and Radiology, Faculty of Dentistry, Marmara University, with a complaint of bilateral painless swelling on the anterior of hard palate. The swelling was smooth-surfaced and measured roughly 6 cm. in size. There were no filled teeth, but right maxillary canine was nonvital. Panoramic radiography showed radiolucency with partially sclerotic border from right premolar to left canine in alveolar process. The area was examined with conventional computed tomography to evaluate bone involvement. Coronal and axial computed tomography images showed expansive process occupying most of right maxillary sinus with a very thin sclerotic border. Cone-beam computed tomography image reconstructions showed aggressive destruction and extension into the maxillary sinus. The patient’s operation was carried out under general anesthesia. Enucleation after incision and mucoperiosteal flap elevation was performed for both cysts in bilateral locations. In order to avoid postoperative hemorrhage and edema antrotomy was performed. Operation side was sutured primarily. Conclusion: This presentation describes a relatively rare case of giant radicular cyst occurrence in maxillary sinus and its treatment. Epitelin kaynağı Malassez artıklarıdır; ancak sinüs sınırı veya fistül yollarının epitelinden de kaynaklanabilir. Radyografik olarak saptanan radyolusent alanların %7-%54’ünü kistler oluşturmaktadır. Radiküler kistler; 30-60’lı dekatta ve erkeklerde daha fazla görülmektedir.Metodlar: 38 yaşında erkek hasta Marmara Üniversitesi Diş Hekimliği Fakültesi Oral Diagnoz ve Radyoloji Anabilim Dalı’na palatinal anterior bölgede çift taraflı ağrısız şişlik şikayeti ile başvurmuştur. Şişlik yüzeyi pürüzsüz ve boyutu yaklaşık 6 cm olarak ölçüldü.. Vitalometrik muayenede sağ maksiller kaninin devital olduğu belirlenmiştir. Panoramik radyografide sağ 1. Premolardan sol kanine doğru kısmen sklerotik sınır ile çevrili radyolusent alan izlenen olgunun koronel ve aksiyel bilgisayarlı tomografi görüntülerinde de çevresi ince bir sklerotik sınır ile çevrili sağ maksiller sinüsün büyük bölümünü kaplayan ekspansif proçes izlenmiştir. Volumetrik Bilgisayarlı Tomografi rekonstrüksiyonunda agresif tarzda destrüksiyon ve maksiller sinüse doğru geniş yayılım saptanmıştır. Hasta genel anestezi altında opere edilmiş, insizyon ve mukoperiostal flap kaldırıldıktan sonra kist bilateral olarak enükle edilmiştir. Post-operatif kanama veya ödemi engellemek için antrotomi yapılarak, operasyon sahası primer olarak suture edilmiştir. Bu çalışmada farklı mikroJprojeler kapsamında Adana ilinde dağıtılan ve yetiştirici koşullarında yetiştirilen saf Saanen keçileri ile yine aynı koşullarda yetiştirilen yerli Kıl keçilerinin sıcak ve nemli subtropik iklim koşullarındaki adaptasyon yetenekleri ile sıcaklık stresi oluşması durumunda hormonal mekanizmada oluşabilecek değişimler, verim düzeyleri ile ilişkilendirilerek ortaya konulmaya çalışılmıştır. Adana ili Karaisalı ilçesinde halk elinde yetiştirilen 20 baş Saanen ile 20 baş Kıl keçinin temmuz ayındaki süt verimi, sıcaklık stresi ile T3, T4 hormon düzeylerindeki değişimleri incelenmiştir. Çalışmada fizyolojik adaptasyon mekanizmaları ile serum T3 ve T4 hormon düzeyleri birlikte ele alındığı zaman Çukurova bölgesinin subtropik iklim koşullarına getirilen Saanen ırkı keçilerde yerli ve bölgenin ırkı olan Kıl keçi ile kıyaslandığı durumda termal stres oluştuğu, belirlenmiştir. Bu bulgunun süt verimi açısından değerlendirildiği zaman Saanen keçilerinin kendilerinde beklenen performansı gösteremediği söylenebilir. Dolayısı ile yürütülecek çevresel iyileştirme çalışmalarında bölgesel koşulların dikkate alınmasının önemi bir kez daha bu çalışma ile ortaya konmuştur. In this study, thermal stress adaptation capabilities in the event of changes resulting from hormonal mechanism, yield levels of Saanen goats and native Hair goats, were determined in humid subtropical climate conditions. The both genotypes were kept in same conditions at breeders’ condition. The Saanen goats were distributed within the microJprojects in the province of Adana. To find out thermal stress effects on T3, T4 hormone levels and milk production, 20 heads Saanen goats and 20 heads native Hair goats of Karaisalı district of the province of Adana were investigated in this study during July.In this study, when the mechanisms of physiological adaptation and serum T3 and T4 hormone levels taken together to consideration, the stress conditions of imported Saanen goats occurred when they were compared to indigenous Hair of goats. Thus it could be said that, Saanen goats did not show the expected performance. Therefore, as a conclusion of this study, it should be underlined that, the importance of environmental and regional conditions for this kind of improvement studies should have been taken into considerations.

Abstract

This study aimed to postoperatively evaluate the effects of intraoperative neural monitoring (IONM) on muscles and nerves in patients who underwent modified radical mastectomy (MRM). In the 11 patients included in the study, nerves were determined and protected by nerve monitoring during the axillary dissection (IONM group). In another 11 patients, nerve monitoring was not performed; however, protection of the same nerves was attempted through careful nerve dissection (cautious nerve dissection [CND] group). The control group consisted of 22 healthy subjects. Muscle and nerve functions were blindly evaluated by an experienced physical therapy and rehabilitation specialist using electromyography (EMG) and ultrasonography (US) methods. Abstract

Background Idiopathic Granulomatous Mastitis (IGM) is a benign chronic inflammatory breast disease that mimics breast cancer, and the etiopathogenesis has not yet been fully evaluated. Autoimmunity has received the most focus as a possible etiology. Our aim in this prospective clinical study was to investigate the possible association between the cytokines, interleukin IL-17, IL-22, IL-23 and IGM. Materials and Methods The current study was conducted in 26 women with histopathologically diagnosed IGM, and 15 control women of reproductive age having no breast disease history. Blood samples were collected, and serum concentrations of IL-17, IL-22, and IL-23 were determined. Results In the analysis of variables, the patients with IGM and the control group had statistically significant differences between serum IL-22 titers (p = 0.0378) and IL-23 titers (p = 0.0469. No statistically significant difference was found between IGM patients and the control group in serum IL-17 titers (p = 0.9724). Conclusion The results of the current study, especially pertaining to serum IL-22 and IL-23 levels, support the etiopathogenesis of IGM in favor of the autoinflammatory thesis. Nevertheless, this thesis should be supported by a large case number and prospective clinical studies.

Abstract

ÖzetÇoğu antİneoplastİk ajanların yaygın yanetkilerinden biri olan oral mukositis kanser kemoterapisinde en sık metotreksat (MTX)+5florourasil (5FU)*e bağlı olarak ortaya çıkmaktadır. Bu karşılaştırmalı klinik çalışmada MTX+5FU kullanımına bağlı olarak gelişen oral muko-sitisin ortaya çıkışını önelemede ağız bakımının etkisi araştırılmıştır. MTX+5FU ile antineoplastik kemotera-pi alan 30 hasta çalışma için seçildi. 15 kişiye kemotera-pi öncesi standart bir ağız bakımı protokolü verilerek, tedavileri sırasında hijyen motivasyonları yapıldı. Geri kalan 15 kişi kontrol grubuydu. Tedavileri sırasında tüm hastalar belirli günlerde kontrol edilerek çeşitli kriterler bazında değerlendirildi, iki grup arasında istatistiksel olarak anlamlı bir 1 arkın olup, olmadığı incelendi. Çalışmanın sonuçlarına göre, ağız bakımı yapan hasta grubunda kontrol grubuna oranla hem objektif, hem de sübjektif oral mukositis indekslerinde ve diğer bulgularda anlamlı bir azalma ve daha kısa süre içerisinde değerlerde düzelme olduğu bulundu.Anahtar sözcükler: Kemoterapi, oral komplİkasyon, ağız bakımı, oral mukositis.AbstractOral mucositis, which is a common side-effect of most antineoplastic agents, occur especially due to methotrexate (MTX) + 5fluorouracil (5FU) usage. In this comparative clinical study, effect of oral hygiene on development oj MTX+5FU4nduced oral mucositis was investigated. Tliirty patients undergoing antineoplastic chemotherapy with MTX-5FU combination were icluded to this study. Prior the chemotherapy standart oral hygiene was instructed to 15 to these patients and they were also motivated during the ir therapy. The rest 15 were the control group. On predetermined criteria basis all die patients were followed-up during their therapy. Differences between the results of both groups were evaluated statistically. According to the results, the patients, to whom die oral hygiene was instructed, the objective and subjective oral mucositis indexes and the other local scores reduced significantly, and recovered in a shorter period when compared with the control group.Keywords: Chemotherapy, oral complicat on, oral hygiene, oral mucositis.

Abstract

Objective: We aimed to compare the effect of bupivacaine and levobupivacaine which are intrathecally used during the elective cesarean section under spinal anesthesia. Material and Method: Eighty patients were included in the study groups who were managed with a cesarean section under spinal anesthesia. Randomly selected fourty pregnant were administered with 12.5 mg bupivacaine+20 ?g fentanyl and the other 40 pregnant were administered with 12.5 mg levobupivacaine+20 ?g fentanyl. The basal heart rate and arterial pressure values were preoperatively obtained. These values were recorded by one minute intenals till the baby came out and then measured by 3 minutes interval. After the 20 minutes, measurements were recorded by 5 minutes interval till the block level regressed to T10. Additionally, sensorial and motor block, satisfaction of patient and surgeon, and analgesia quality were evaluated. Results: The Bromage scores at first minute in the levobupivacaine group were significantly lower compared with the bupivacaine group's (p0.05). Conclusion: Levobupivacaine is a good alternative for bupivacaine in equal doses during the cesarean section under the spinal anesthesia.
