Objective: This study was planned to examine the effect of exercise-based cardiac rehabilitation (EBCR) program on functional capacity, kinesiophobia level and quality of life on patients with coronary artery disease.Material and Method: A total of 22 patients with coronary artery disease (the mean age 60.72±5.28 years) were included in the study. Participants were recruited to exercise training programme with cycle ergometer for 5 weeks and 5 sessions/week. Functional capacities of participants were evaluated with the 6-minute walk test (6MWT), kinesiophobia levels with the Tampa Kinesiophobia Scale- Heart version (TKS-H), and quality of life with the Short Form-36 (SF-36) and the Macnew Heart Disease Health-Related Quality of Life Scale (Macnew). Objective: The time in therapeutic range (TTR) of international normalized ratio (INR) is essential for the safety and efficacy of warfarin treatment. In this study, we aimed to determine TTR and the factors that affect TTR in patients using warfarin. Methods: Patients taking warfarin for valvular and nonvalvular atrial fibrillation (AF) or prosthetic heart valves who were admitted to our cardiology outpatient clinic were enrolled. TTR was calculated using the linear interpolation method. The patients were analyzed according to warfarin indications and TTR efficiency (TTR >= 60%). Weekly warfarin dose, the duration of warfarin use, the frequency of INR visits per year, and the awareness of patients regarding target INR were noted. Results: The TTR of 248 patients (aged 57.21 +/- 12.45 years, 33.1% male) was 55.92 +/- 27.84%, and 48.0% patients exhibited efficient TTR. Clinical and demographic characteristics (age, sex, socioeconomic status, and comorbidities) exerted no effect on TTR and TTR efficiency. The frequency of INR visits per year was 10.02 +/- 3.80. TTR was related to the frequency of annual INR visits (r=0.131, p=0.039). Only one-third (30.2%) of patients were aware of their target INR. The literacy of the patients and duration of warfarin use exerted a positive effect on awareness (p=0.011 and p=0.024, respectively). Conclusion: The findings of our study demonstrated that TTR and TTR efficiency were low and not associated with the characteristics of patients or indications. Unfortunately, in patients with valvular AF and prosthetic valves, warfarin is the sole drug that can be used. Thus, awareness and knowledge regarding target INR are essential to overcome poor anticoagulation monitoring with frequent INR visits., Amaç: Uluslararası normalleştirilmiş oranın (INR) terapötik aralıkta geçen zamanı (Time in therapeutic range - TTR) varfarin tedavisinin etkinliği ve güvenliği için zorunludur. Bu çalışmada, varfarin kullanan hastalarda TTR değerlerini ve TTR’yi etkileyen faktörleri belirlemeyi amaçladık. Yöntemler: Kardiyoloji polikliniklerine başvuran valvüler-valvüler olmayan AF veya prostetik kalp kapağı için varfarin kullanan hastalar çalışmaya alındı. Terapötik aralıkta geçen süre doğrusal interpolasyon yöntemi ile hesaplandı. Hastalar varfarin endikasyonlarına ve TTR etkinliğine (TTR ≥%60) göre analiz edildi. Haftalık varfarin dozu, varfarin kullanım süresi, yıllık INR kontrol sıklığı ve hastaların hedef INR değeri konusunda farkındalıkları not edildi. Bulgular: Katılan 248 hastanın (ortalama yaş: 57.21±12.45 yıl, %33.1’i erkek) TTR değeri %55.92±27.84 idi ve hastaların % 48.0’i etkin TTR’ye sahipti. Klinik ve demografik özelliklerin (yaş, cinsiyet, sosyoekonomik durum, komorbiditeler) TTR ve TTR etkinliği üzerinde etkisi gösterilemedi. Yıllık INR kontrolü sıklığı 10.02±3.80 idi. Yıllık INR kontrolü sıklığıyla TTR ilişkili bulundu (r=0.131, p=0.039). Hastaların sadece 1/3’ü (%30.2) hedef INR değerinin farkındaydı. Hastanın okur-yazarlık durumu ve ilaç kullanım süresi farkındalık üzerinde olumlu etki sağladığı görüldü (sırasıyla p=0.011 ve p=0.024). Sonuç: Çalışmamız TTR ve TTR etkinliğinin düşük olduğunu ve bu iki parametrenin hastaların karakteristikleri ve varfarin endikasyonunuyla ilişkili olmadığını gösterdi. Maalesef valvüler AF ve protez kapak varlığında kullanılabilecek tek ilaç varfarindir. Bu nedenle kötü antikoagulasyon izleminin üstesinden gelmek için sık INR kontrolleri ile takip olmak, varfarin hakkında bilgi sahibi olmak ve farkındalık gereklidir.

Objective: Despite recommendations from heart failure guidelines on the use of pharmacologic and device therapy in patients with heart failure with reduced ejection fraction (HFrEF), important inconsistencies in guideline adherence persist in practice. The aim of this study was to assess ad-herence to guideline-directed medical and device therapy for the treatment of patients with chronic HFrEF (left ventricular ejection fraction ?40%).Methods: The Adherence to guideline-directed medical and device Therapy in outpAtients with HFrEF (ATA) study is a prospective, multicenter, observational study conducted in 24 centers from January 2019 to June 2019.Results: The study included 1462 outpatients (male: 70.1%, mean age: 67±11 years, mean LVEF: 30%±6%) with chronic HFrEF. Renin–angiotensin system (RAS) inhibitors, beta-blockers, mineralocorticoid receptor antagonists (MRAs), and ivabradin were used in 78.2%, 90.2%, 55.4%, and 12.1% of patients, respectively. The proportion of patients receiving target doses of medical treatments was 24.6% for RAS inhibitors, 9.9% for beta-blockers, and 10.5% for MRAs. Among patients who met the criteria for implantable cardioverter–defibrillator (ICD) and cardiac resynchro-nization therapy (CRT), only 16.9% of patients received an ICD (167 of 983) and 34% (95 of 279) of patients underwent CRT (95 of 279).Conclusion: The ATA study shows that most HFrEF outpatients receive RAS inhibitors and beta-blockers but not MRAs or ivabradin when the medical reasons for nonuse, such as drug intolerance or contraindications, are taken into account. In addition, most eligible patients with HFrEF do not receive target doses of pharmacological treatments or guideline-recommended device therapy. (Anatol J Cardiol 2020; 24: 32-40)

Bosna’nın kralları, Macar Krallığı ve papalık gibi Katolik güçlerle Bosna ve Hum bölgesindeki heretikleri ortadan kaldırmak için gizli ittifaklar kurarken derebeyleri yerel halkı ve heretikleri onlara karşı korumaya çalışmaktadırlar. Öte yandan bölgede Bosnalılar ve Osmanlıların ilk temasının vuku bulduğu dönemde soylu aileler arasında siyasal çekişmeler gün yüzüne çıkmıştır. Özellikle I. Kosova Savaşı’nın ardından Bosna ve Hum’da Türklerin etkisi artmış ve bölgedeki siyasi güçlerin Türklerle olan teması daha da sıklaşmıştı. Bu dönemdeki yoğun temasın bir sonucu olarak Türklerden de, idari ve ticari belgelerin metinlerinde bahsedilmeye başlanmıştır. Bu tezde, dönemin belgelerin yardımıyla Türklerin bölge siyasetinde oynadığı rolün ortaya konması amaçlanmaktadır. About one hundred years before the conquest of the Ottomans, the feudal lords of some leading noble families in Bosnia and Hum such as Hrvatinić, Kosača and Radenović were almost as strong as the kings of Bosnia who were members of the dynasty Kotromanić. At times these lords called voivode, could decide who will accede to the throne of Bosnian Kingdom. While Bosnian kings conspiring with Catholic forces like Hungarian Kingdom and Papacy to eliminate the heretics in Bosnia and Hum, the lords were trying to protect the local people and heretics against them. On the other hand, political conflicts among noble families emerged at the time of the first contact of Ottomans and Bosnians in the region. Especially, after the Battle of Kosovo (1389) the effectiveness of Turks in Bosnia and Hum increased and contact of political forces in the region with Turks became more frequent. As a result of intense contact in this period, Turks were also mentioned in the texts of administrative and commercial documents. In this thesis, it is aimed to reveal the role of Turks in the politics of the region by means of these documents. In the first chapter of the study, the historical process of the region is focused on and then the documents mentioning Turks have been scrutinized.

Bir Slav dili olarak Boşnakça, Slav dillerinin üç kolundan biri olan Güney Slav dilleri kolu altında yer alır. Boşnakçanın kökeni, diğer Slav dilleri için de ana dil konumunda olan Eski Slavcaya dayanır. Boşnakçanın ad durumlarını konu edinen çalışmamız, bu dildeki ad durumu çekim sistemi ile Türkçedeki çekim sitemini karşılaştırarak tanımlamayı amaçlamaktadır. Tezimizin Giriş bölümünde çalışmamızın konusu, sınırları ve yöntemi ortaya konmuştur. Bu bölümün ardından Boşnakçanın Tarihsel Gelişimi hakkında bilgi verilmiştir. Bir diğer bölümde Boşnakçadaki Adların Özellikleri anlatılmıştır. Bu bölümü takiben, çalışmamızın ana kısmını oluşturan Boşnakçadaki Ad Durumları incelenmiştir. Son olarak değerlendirmelerimizi içeren Sonuç bölümü ve çalışmamızda yararlandığımız kaynakların yer aldığı Kaynakça ve Ekler bölümü bulunmaktadır. Bosnian as a Slavic language ranks in the South Slavic languages which are one of three branches of the Slavic Languages Group. The origins of Bosnian language depend on Old (Church) Slavic language which was a primitive language of all current Slavic languages. The aim of this dissertation is to make descriptive comparison of the declension system in Bosnian language and Turkish language. In the introduction part of the dissertation, the subject, limitations and method of the study have been presented. The first chapter deals with historical development of Bosnian language. In the second chapter, features of nouns in Bosnian language have been mentioned. The following chapter analyses the cases of nouns in Bosnian language. Finally, the dissertation includes summary, bibliography and appendix sections. 345

Giriş:Bu çalışmanın amacı, kardiyak konsültasyonesnasında tanı ve tedavi üzerinde ekokardiyografi sonuçları ile kardiyologtarafından gerçekleştirilen fizik muayene sonuçlarını karşılaştırmaktır.Hastalarve Yöntem: Kardiyak olmayan birimlerdenyatırılan ardışık seçilmemiş 265 hasta danışman kardiyolog tarafından tamkardiyovasküler muayene sonrası çalışmaya alındı. Fizik muayene sonrasıdanışman kardiyolog taşınabilir kardiyak ultrason kullanarak her hastayıgörüntülenmiştir. Tüm hastalar daha sonra bir üst platformda gerçekleştirilenstandart ekokardiyografik cihazla altın standart olarak değerlendirildi. Kesintanı, tedavi ve tanısal testlerdeki değişiklikler değerlendirildi.Bulgular:Bu hasta popülasyonunda standart ekokardiyografikcihazla 196 kardiyovasküler bulgu saptanmıştır. Fizik muayene tüm bulguların%41’ini ve taşınabilir ultrason ise %23’ünü tespit etmede başarısız olmuştur.Taşınabilir kardiyak ultrason cihazı 149 hastanın tedavi kararında etkiliolmuştur ve bunların %42’sinde tedavi değişirken %21’inde tanısal testlerdeğişmiştir (hastaların çoğunda ikisi de değişmiştir).Sonuç: Konsültasyonlar esnasında taşınabilirkardiyak ultrason ile değerlendirme önemli kardiyovasküler patolojilerintespitini sağlamaktadır. Ayrıca, rutin fizik muayeneye ek olarak deneyimlikardiyologlar tarafından taşınabilir cihazla yatak başı kardiyak fonksiyon veanatominin direkt değerlendirilmesi tedavi ve tanısal testlerde önemlideğişiklikler ile sonuçlanmaktadır., Introduction:The purpose of this study was to compare theresults of physical examinations (PEs) performed by a cardiologist with theresults of point-of-care echocardiography for the diagnosis and management ofpatients during cardiac consultation rounds.Patientsand Methods: In all, 265 hospitalized consecutive patientsfrom non-cardiac units (age, 62 ± 11; male, 56%) were included after completecardiovascular (CV) examination by a consulting cardiologist. After the PE, theconsulting cardiologist imaged each patient using a hand-carried cardiacultrasound (HCU). All patients subsequently underwent a study with a standardechocardiographic device (SED) as the gold standard, performed on an upper-endplatform. Definitive diagnosis, management change, and modifying diagnosticworkup were also assessed.Results:There were 196 CV findings detected with an SED inthis patient population. Of these, PE failed to detect 41% of the overall CVfindings and HCU missed 23% of the overall CV pathology. Overall, HCU had aneffect on patient treatment decisions in 149 patients (56%); 42% had a changein medical therapy and 21% had a change in their diagnostic workup (most withchanges in both).Conclusion: HCU echocardiographicassessment during consultation rounds improved the detection of significant CVpathology. Also, a direct assessment of cardiac function and anatomy at thebedside by an experienced cardiologist with HCU as a routine adjunct to PEresults in an important change in clinical management and diagnostic workupduring consultation.
