Blue toe syndrome is a clinical condition characterized by pain and cyanosis in one or more toesor soles of the feet due to microvascular occlusion. The most common cause is atheroembolicdisease or aneurysm. Embolization occurs typically from an ulcerated atherosclerotic plaqueor aneurysm located in the aorto-iliac artery and femoral arteries. Dianosing and treating thesource of embolism in a patient with blue toe syndrome is very important in order to preventrecurrent embolism that may result in limb loss and / or death. On the other hand, palpableperipheral pulses in the first physical examination performed on a patient presenting with theabove-mentioned clinical symptoms excludes vascular pathologies, which may lead to delayin diagnosis. Suspecting microvascular occlusion and using imaging methods to diagnoseintravascular pathologies are quite important in these patients. In our case, there was painand blue / purple discoloration on the soles of both feet, which was aggravated by walking.Peripheral pulses were palpable on physical examination. When CT angiography images andbiopsy results from existing lesions were evaluated in the light of clinical findings, a diagnosis ofblue toe syndrome was made., Blue toe sendromu; mikrovasküler oklüzyona bağlı bir veya daha fazla ayak parmağında veya ayak tabanında ağrı ve siyanoz ile karakterize klinik durumdur. En sık nedeni ateroembolik hastalıklar veya anevrizmadır. Embolizasyon genellikle aorto-iliak, femoral arterlerde bulunan ülsere aterosklerotik plaktan veya anevrizmadan meydana gelir. Blue toe sendromu gelişen bir hastada emboli kaynağını tespit ve tedavi etmek, uzuv kaybı ve/veya ölüm ile sonuçlanabilecek tekrarlayan embolileri önlemek adına oldukça önemlidir. Bununla birlikte yukarıda belirtilen klinik semptomlarla başvuran hastada yapılan ilk fizik muayenede periferik nabızların palpabl olması klinisyenleri vasküler patolojilerden uzaklaştırmakta, buna bağlı tanı koymada gecikme söz konusu olabilmektedir. Bu hastalarda mikrovasküler oklüzyondan şüphelenilmesi ve damar içi patolojileri teşhis için görüntüleme yöntemlerine başvurulması hayati önem arz etmektedir.

Toluene is an aromatic hydrocarbon found in glues, cements and organic solvents. Inhalant abuse or ingestion and industrial exposure may lead toluene toxicity. Toluene exposure may cause serious complications including electrolyte and acide base disturbances, gastrointestinal complaints, neuropsychiatric disorders. However several reversbl cardiotoxicity is rare ocur due to toluen. We report the case with chronic toluene exposure in his workplace who was complicated with reversbl dilated cardiomyopathy and ventricular tachyarrhythmia., Toluen, yapıştrırıcı, sement ve organik solventlerde bulunan bir aromatik hidrokarbondur. İnhalan istismarı veya oral alım ve endüstriyel maruziyeti nedeniyle toluen toksisitesi ortaya çıkabilir. Toluen maruziyetiyle, elektrolit, asit-baz dengesizliği, gastrointestinal komlikasyonlar ve nöropsikiyatrik bozukluklar gelişebilir. Ancak ciddi reversbl kardiyotoksisite nadiren görülür. Bu makalede, işyerinde kronik toluen maruziyeti nedeniyle reversible dilate kardiomyopati ve ventriküler taşiaritmi gelişen bir olgu sunulmuştur.Anahtar kelimeler: Toluen, tilate kardiyomiyopati, ventriküler taşikardi.

The mechanism of neuropathic pain (NP) and osteoarthritic (OA) pain, although different, are both defined as chronic pain, and combinations are possible. In this study, the awareness of both types of chronic pain was investigated among patients and doctors. This prospective study was planned and coordinated by the orthopedic and internal medicine clinics and included 100 patients with a diagnosis of knee OA evaluated with the DN4 questionnaire. Patients with an OA diagnosis included those with NP linked to diabetes and those without NP, and these groups were compared using a visual analogue scale (VAS) and the Western Ontario and McMaster Universities Osteoarthritic Index (WOMAC). Data analysis of both groups focused on descriptive statistics of demographic and clinical characteristics. Twenty-four of the patients with type 2 diabetes mellitus (DM) were diagnosed with NP according to DN4 criteria, and began treatment. Of these patients, 21 (84%) had DM for 10 years or more, while 19 (86.4%) had HbA1c of 7 and above. The average WOMAC pain score for patients with NP was 4.33 ± 1.2 while the score for VAS was 8.25 ± 0.8. Patients without NP had a WOMAC pain score of 2.49 ± 0.6 and a VAS of 6.28 ± 1.8. It was observed that in these two knee OA patient groups, NP had a statistically significant enhancing effect on the WOMAC pain score and the VAS (p < 0.001). As underlying pathophysiological mechanism of pain caused by OA is complex, and OA is considered to have a component of NP, it has been proven to be useful to use drugs apart from conventional treatments for NP. We believe that, as a source of pain that is not relieved after arthroplasty, awareness level of NP among orthopedists should be increased and multidisciplinary studies are required on this topic.

Amaç: Klinik hipotiroidi de dislipidemi görüldüğü bilinmektedir. Ama subklinik hipotiroidiyle ilgili literatür bilgileri çelişiktir ve çoğu total kolesterolle sınırlıdır. Bu çalışmayla, dislipideminin tüm alt gruplarında subklinik hipotiroidi sıklığını saptamayı amaçladık. Gereç ve Yöntemler: Çalışmayı, dahiliye polikliniğine başvuran 277'si kadın, 79'u erkek toplam 356 hastayla yaptık. Bunun için tüm hastaların lipid profilleri ve tiroid fonksiyon testlerine baktık. Daha sonra da her bir alt grup için dislipidemisi olanlarla olmayanlarda subklinik hipotiroidi prevalansına bakıp, farkın anlamlı olup olmadığını inceledik. Bulgular: Subklinik hipotiroidi sıklığını, hiperkolesterolemi saptananlarda %5.9, saptanmayanlarda %6.6; LDL kolesterol düzeyi yüksek olanlarda %4.7, olmayanlarda %7.2; hipertrigliseridemisi olanlarda %7.4, olmayanlarda %5.0 ve HDL düzeylerinde azalma olanlarda %2.6, olmayanlarda %7.1 bulduk. Bu sonuçlara göre, dislipidemili hastalarla, normal lipid profiline sahipler arasında, istatistiksel olarak anlamlı fark yoktu. Benzer şekilde, subklinik hipotiroidili hastalarla ötiroidlerin lipid profilleri arasında da anlamlı fark saptamadık. Sonuç: Bu sonuçlar ışığında, subklinik hipotiroidinin, dislipidemi açısından belirgin bir risk faktörü olmadığını söyleyebiliriz. Objective: It is well-established that dyslypidemia occurs in clinical hypotyhroidism. However, data on subclinical hypothyroidism is controversial and are limited to total cholesterol. In this study, we aimed to determine the prevalence of subclinical hypothyroidism in all subgroups of dyslipidemia. Material and Methods: The study group consisted of 356 patients (277 females, 79 males) who presented to the internal medicine outpatient clinic. Thus, we examined the lipid profile and thyroid function tests in all patients. Then, for all groups with or without dyslipidemia, we examined the prevalence of subclinical hypothyroidism and compared the groups for statistical significance. Results: The frequency of subclinical hypothyroidism was 5.9% in cases with hypercholesterolemia and 6.6% in those without, 4.7% in patients with high levels of LDL-cholesterol and in 7.2% in those with normal levels, in 7.4% of hypertriglyceridemia patients and in 5.0% in those without and in 2.6% cases with low levels of HDL-cholesterol level and in 7.1% in those with normal levels. According to these results, there was no statistically significant difference between patients with and without dyslipidemia. Similarly, there was no significant difference between subclinical hypotyhroid patients and the eutyhroid ones. Conclusion: Based on these results, we suggest that subclinical hypothyroidism is similar to euthyroidism with regard to dyslipidemia. 39

Tirotoksik hipokalemik periyodik paralizi THPP , hipertiroidizmin nadir görülen bir komplikasyonu olup, tekrarlayan hipokalemi ile birlikte akut kas güçsüzlüğü ile karakterize bir hastalıktır. Aşırı karbonhidrat alımı, stres veya ağır fiziksel aktivite atağın ortaya çıkmasına yol açar. Hastalığın oluşum mekanizması, tirotoksikozda artan beta adrenarjik stimulasyonun Na+-K- ATPaz pompasını aşırı aktivasyonuyla, iskelet kası hücresine akut K+ iyonunun geçişidir. Atakların süresi ortalama 2-36 saat kadar olup gerekli durumlarda K replasmanı ile bu süre daha da kısalmaktadır. Sonuç olarak hastalarda ataklar halinde kas güçsüzlüğü veya paralizi izlenmektedir. Bu yazıda, yeni başlayan paralizi atakları tarif eden ancak ailede benzer bir tablonun görülmediği 23 yaşında bir erkek hasta sunulmaktadır, Thyrotoxic hypokalemic periodic paralysis THPP is a rare complication of hyperthyroidism which is characterized by recurrent hypocalemia with acute muscle weakness. Excessive carbohydrate intake, stress or intensive physical activity leads to the occurance of symptoms. In thyrotoxicosis, beta adrenergic impulses stimulate Na-K ATPase pump and this leads to increased entry of acute K+ ions through the skeletal muscle cells. The duration of attacks range from 2-36 hours and can be shortened by K supplementation in appropriate situations. As a result, episodes of muscle weakness and paralysis are observed in these patients. Our case is a 23 years old male who describes recurrent muscle weaknesses since a week without a family history.

Purpose: In this study we aimed to evaluate thyroid pathologies in patients with primaryhyperparathyroidism who underwent surgery for parathyroidectomy.Methods: Patients who underwent parathyroidectomy for primary hyperparathyroidism betweenthe years of 2006-2012 were included in this retrospective study. Before and after surgery,hormonal and biochemical examinations of the patients and results of preoperative imagingstudies were recorded.Results: Of the patients included in the study, 86 were female and 12 were male. Preoperativemean serum calcium level was 12.25 ± 1.48 mg/dL (normal range 8.5-10.1 mg/dL), meanparathyroid hormone (PTH) level was 425.09 ± 440.47 pg/mL (normal range 11-65 pg/mL).The thyroid ultrasound records of 93 patients were available. Of these patients, 26 (28%) hadnormal ultrasound, 18 (19.4%) had solitary nodule, 43 (46.2%) had multinodular goiter and 4(4.3%) had thyroiditis (without nodule) and 2 (% 2.2) had solitary nodule with thyroiditis.Adenoma was detected by ultrasonography in 45.2 % of patients, and by scintigraphy in 72.7of patients. Parathyroidectomy and thyroidectomy were performed at the same time in 54 of 98patients (65.1%). Of these patients Seven (7.1%) had papillary thyroid cancer, 30 (30.6%) hadbenign multinodular goiter, 8 patients (8.2%) had benign solitary nodule, 2 patients (2%) hadlymphocytic thyroiditis, and 7 (7.1%) had normal thyroid tissue.Conclusion: Thyroid diseases are frequently encountered with primary hyperparathyroidism.Therefore, before performing parathyroidectomy with minimally invasive procedure, patientsmust be evaluated for thyroid disorders, Amaç: Primer hiperparatiroidi (PHPT) nedeniyle paratirodektomi yapılan hastalarda eşlik edentiroid patolojilerinin değerlendirilmesi amaçlandı.Gereç ve Yöntem: Bu retrospektif çalışmaya 2006-2012 yılları arasında PHPT tanısı nedeniyleparatiroidektomi yapılan hastalar dahil edildi. Operasyon öncesi ve sonrası hastalarınbiyokimyasal ve hormonal tetkikleri, preoperatif görüntüleme tetkiklerinin sonuçları kaydedildi.Bulgular: Çalışmaya dahil edilen PHPT hastaların 86’sı kadın, 12’si erkekti. Genel olarak hastaların%72’sinde tiroid patolojisi mevcuttu. Hastaların %45.2’sinde ultrasonda ve %72.7’sindeparatiroit sintigrafisinde paratiroit adenomu tespit edildi. Doksan sekiz hastanın 54’üne (%65.1)paratiroidektomi ile birlikte tiroidektomi de yapıldı. Yedi (%7.1) hastada papiller tiroid kanseri,30 hastada (%30.6) benign multinodüler guatr, 8 hastada (%8.2) benign soliter nodül, 2 hastada(%2) lenfositik tiroidit ve 7 hastada (%7.1) normal tiroid dokusu tespit edildi. Sonuç: Tiroid hastalıklarının PHPT ile birlikteliği sık görülebilmektedir. Bu nedenleparatiroidektominin minimal invaziv yöntemle yapılmasına karar vermeden önce hastalarmutlaka tiroid patolojisi açısından değerlendirilmelidir

Amaç: Subklinik ve aşikar hipertirodileri olan hastaların serum sklerostin seviyeleri kontrol grubunun serum sklerostin seviyeleri ile karşılaştırmayı amaçladık. Bu parametrenin hipertroidi süresi, tedavi süresi, tedavisiz kalan süre, kadın hastalar için menopoz durumu, gebelik sayısı, sigara ve alkol kullanımı, VKİ ile ilişkisini araştırdık.Yöntem: Çalışma için ÇOMÜ Tıp Fakültesi Uygulama ve Araştırma Hastanesi Endokrinoloji ve Metabolizma Polikliniğine başvurarak hipertiroidi tanısı alan 60 hasta seçildi. Kontrol grubu, hasta grubu ile benzer özelliklere sahip gönüllü 27 kişiden oluşturuldu. Serum TSH, sT3, sT4 değerleri ECLIA yöntemi ile Cobas e601 analizöründe Roche kitleri (Roche Diagnostics GmbH) kullanılarak ve serum sklerostin düzeyi ELISA yöntemine dayanan ticari kitler kullanılarak ölçüldü.Bulgular: Hasta grupları ve kontrol grupları arasında serum sklerostin seviyeleri açısıdan anlamlı bir fark saptanmadı. Tüm gruplarda baktığımızda serum sklerostin ile sT3 ve sT4 arasında pozitif yönde kuvvetli bir korelasyon saptadık (p=0268; p=0,227). Serum sklerostin seviyeri ile yaş, cinsiyet, VKİ, sigara ve alkol kullanımı arasında anlamlı bir ilşki saptamadık (p=-0,101, p=0,276, p=-0,182, p=0,402, p=0,451). Serum sklerostin ile hastalık süresi ve tedavi süresi arasında anlamlı ilişki kuramadık (p=-0,020, p=-0,069). Çalışmaya aldığımız kadın hastaların gebelik sayıları ve menopoz durumları ile serum sklerostin seviyelerini karşılaştırdığımızda anlamlı bir ilişki saptamadık (p=-0,034, p=0,386).Sonuç: Tüm gruplara baktığımızda sT3 ve sT4 yüksek olan hastalarda serum sklerostin seviyesini daha yüksek bulduk ama alt gruplara baktığımızda aşikar hipertiroidi ve subklinik hipertiroidi grupları ile kontrol grubu arasında serum sklerostin seviyleri arasında herhangi bir fark tespit edemedik. Literatüre baktığımızda bu konuda yapılmış benzer bir çalışma bulunmamaktadır. Yaptığımız çalışma Serum sklerostinin hipertiroidide meydana gelen osteoporozda etkili olduğunu düşündürmektedir. Osteoporozun erken tanısında serum düzeyi bakılarak ve osteoporoz tedavsinde anti sklerostin antikorları kullanılarak, sklerostinden yararlanılabilir. Bu konuda kesin kararlar alabilmek için daha fazla sayıda hasta grubu ile yapılmış gelecek çalışmalara ihtiyaç vardır. Introduction: We aimed to compare the levels of serum sclerostin in patients that have subclinic and overt hyperthyroidism to the control group. We examined this paradigm's relation to duration of hyperthyroidism, duration of treatment, duration without treatment, the status of menopause for female patients, number of pregnancies, alcohol and tobacco consumption, and VKI.Methods: For this study, 60 individuals are selected from the patients that applied to the Departments of Endocrinology and Metabolism, Faculty of Medicine, Canakkale Onsekiz Mart University and diagnosed with hyperthyroidism. The control group is composed of 27 volunteer individuals that have similar characteristics to the patient group. Serum TSH, sT3, sT4 values are measured by the ECLIA method using Roche kits (Roche Diagnostics GmbH) in Cobas e601 analyser; and the level of serum sclerostin is measured by the ELISA method by using commercial kits. Results: No significant difference is observed between the patient group and the control group. In all groups, we found a strong positive correletation between serum sclerostin and sT3-sT4 (p=0268; p=0,227). We did not observe any correlation between serum sclerostin levels and age, sex, VKI, tobacco and alcohol consumption (p=-0,101, p=0,276, p=-0,182, p=0,402, p=0,451). We compared the serum sclerostin levels with the women patients' number of pregnancy and menopause status, and found no correlation (p=-0,034, p=0,386).Discussion: We found that in all groups the patients with high sT3 and sT4 have higher serum sclerostin levels, but in sub-groups we did not find a difference between the serum sclerostin levels of the patient group with overt hyperthyroidism and subclinic hyperthyroidism and the control group. To the best of author's knowledge, there is not any similar study conducted in this particular subject. This study makes us consider that serum sclerostin is effective in the osteoporosis that occurs in hyperthyroidism. Sclerostin can be used in the early diagnosis of osteoporosis by examining the serum levels, and in the osteoporosis treatment by using anti sclerostin anticors. In order to reach certain conclusions in this subject, future studies with larger number of patients are required. 59
