Objectives: This study aimed to describe the overall characteristics, hospitalization symptoms, treatment, and outcome of patients admitted with zona zoster Methods: In this study, 30 adult patients hospitalized between January 2012 and December 2014 due to zona zoster were evaluated retrospectively. Demographic characteristics, hospitalization symptoms, and treatment of patients of the patients were examined. Results: Among the 30 patients who followed up with zona zoster, 16 (53.3%) were female and the median age was 57.5 (18.0–87.0) years. Twenty-two (73.3%) patients had comorbidities and 7 (23.3%) patients had hypertension. The most common site of involvement was the head-and-neck region in 14 (46.7%) patients. Twenty-nine (96.7%) patients had lesions along a single dermatome and, fever was detected in 5 (16.7%) patients. The duration of hospital stay was 6.0 (2.0–14.0) days. One (3.3%) patient had persistent pain at the site of the lesion after treatment Conclusion: Most patients with zona zoster could be treated in primary care. Primary care physicians should be aware of the characteristics of hospitalized patients with zona zoster to better assist with their follow-up. [ABSTRACT FROM AUTHOR]

9. Scabies: A Growing Concern for Public Health.

Objectives: Scabies, an infestation affecting individuals of all ages, poses significant public health implications due to its potential for epidemics and transmission within households. The aim of this study was to evaluate the demographic data of patients diagnosed with scabies between the years 2022 and 2023. Methods: A retrospective analysis of patients admitted to the Infectious Diseases and Dermatology Outpatient Clinics between January 2022 and November 2023 was conducted, with symptoms of itching and rash, and was diagnosed with scabies during the first admission. The patients of all age groups were included in the study. Results: A total of 1261 patients were included in this study, with a median age of 30.0 (0.0-93.0) years and 669 (53.0%) were male. In 2022, 521 (41.3%) patients were diagnosed with scabies, while in 2023, the number increased to 740 (58.7%). Among the total patients, 167 (13.2%) were admitted from outside the province, with 77 (6.1%) in 2022 and 90 (7.1%) in 2023. Interestingly, 114 (9.0%) patients were diagnosed with scabies while hospitalized for reasons unrelated to scabies itself. This includes 16 (1.3%) patients in 2022 and 98 (7.8%) patients in 2023. Conclusion: Scabies are an escalating public health concern that has the potential to trigger epidemics. Primary health-care institutions, specialty associations, and the General Directorate of Public Health play vital roles in recognizing the disease, coordinating treatment strategies, and disseminating preventive measures to the public. - Abstract
The objective was to apply a population model to describe the time course and variability of serum creatinine (sCr) in (near)term neonates with moderate to severe encephalopathy during and after therapeutic hypothermia (TH). The data consisted of sCr observations up to 10 days of postnatal age in neonates who underwent TH during the first 3 days after birth. Available covariates were birth weight (BWT), gestational age (GA), survival, and acute kidney injury (AKI). A previously published population model of sCr kinetics in neonates served as the base model. This model predicted not only sCr but also the glomerular filtration rate normalized by its value at birth (GFR/GFR0). The model was used to compare the TH neonates with a reference full term non-asphyxiated population of neonates. The estimates of the model parameters had good precision and showed high between subject variability. AKI influenced most of the estimated parameters denoting a strong impact on sCr kinetics and GFR. BWT and GA were not significant covariates. TH transiently increased sCr in TH neonates over the first days compared to the reference group. Asphyxia impacted not only GFR, but also the sCr synthesis rate. We also observed that AKI neonates exhibit a delayed onset of postnatal GFR increase and have a higher sCr synthesis rate compared to no-AKI patients. Our findings show that the use of sCr as marker of renal function in asphyxiated neonates treated with TH to guide dose selection for renally cleared drugs is challenging, while we captured the postnatal sCr patterns in this specific population. - Abstract
Introduction: There is large variability in kidney function and injury in neonates with neonatal encephalopathy (NE) treated with therapeutic hypothermia (TH). Acute kidney injury (AKI) definitions that apply categorical approaches may lose valuable information about kidney function in individual patients. Centile serum creatinine (SCr) over postnatal age (PNA) may provide more valuable information in TH neonates. Methods: Data from seven TH neonates and one non-TH-treated, non-NE control cohorts were pooled in a retrospective study. SCr centiles over PNA, and AKI incidence (definition: SCr ↑≥0.3 mg/dL within 48 h, or ↑ ≥1.5 fold vs. the lowest prior SCr within 7 days) and mortality were calculated. Repeated measurement linear models were applied to SCr trends, modeling SCr on PNA, birth weight or gestational age (GA), using heterogeneous autoregressive residual covariance structure and maximum likelihood methods. Findings were compared to patterns in the control cohort. Results: Among 1,136 TH neonates, representing 4,724 SCr observations, SCr (10th-25th-50th-75th-90th-95th) PNA centiles (day 1-10) were generated. In TH neonates, the AKI incidence was 132/1,136 (11.6%), mortality 193/1,136 (17%). AKI neonates had a higher mortality (37.2-14.3%, p < 0.001). Median SCr patterns over PNA were significantly higher in nonsurvivors (p < 0.01) or AKI neonates (p < 0.001). In TH-treated neonates, PNA and GA or birth weight explained SCr variability. Patterns over PNA were significantly higher in TH neonates to controls (801 neonates, 2,779 SCr). Conclusions: SCr patterns in TH-treated NE neonates are specific. Knowing PNA-related patterns enable clinicians to better assess kidney function and tailor pharmacotherapy, fluids, or kidney supportive therapies.

Aim: Transient tachypnea of the newborn (TTN) is the most common respiratory problem encountered by physicians working in the neonatal intensive care unit, and its incidence is higher in late preterm infants compared to term infants. Platelets prevent pulmonary fluid collection and pulmonary edema by an unknown mechanism. Platelet function can be more accurately assessed by platelet mass index (PMI) rather than mean platelet volüme (MPV) or platelet count alone. In our study, it was planned to investigate the duration of need for respiratory support and the relationship between thrombocyte markers, especially PMI, and other respiratory outcomes in late preterm infants with the diagnosis of TTN.Materials and Methods: In this retrospective study, data of all late preterm newborns whose tachypnea lasted for at least 12 hours and who were hospitalized in the neonatal intensive care unit of Buca Seyfi Demirsoy Training and Research Hospital with the diagnosis of TTN between August 1, 2020-July 31, 2021 were analyzed from medical records. The thrombocyte markers, leukocyte and neutrophil values of the infants and respiratory support durations were compared. In addition, these parameters were evaluated in terms of possible differences according to gestational weeks.Results: 52 infants were included in the study. The median gestational week was 35 weeks (34-366/7), and the mean birth weight was 2647474. There was no correlation between total respiratory support duration or >48 hours of respiratory support in infants and platelet count, MPV, PMI, leukocyte and absolute neutrophil count (ANS). However, it was observed that the PMI values were lower in babies who received respiratory support for more than 72 hours, and this was statistically significant. In addition, it was observed that MPV value was significantly higher in babies born at 35th gestational week and ANS value was significantly higher in babies born at 36th gestational week.Conclusion: In our study, it was seen that PMI, one of the platelet markers, was associated with the need for respiratory support for more than 72 hours in late preterm infants with TTN, as the mean PMI values of these infants being lower. However, a similar relationship was not found between other platelet markers and the duration of respiratory support., Amaç: Yenidoğanın geçici takipnesi (YDGT), yenidoğan yoğun bakımda çalışan hekimlerin en sık karşılaştığı solunumsal problem olup geç preterm bebeklerde görülme oranları, term bebeklere göre daha yüksektir. Trombositler, tam olarak bilinmeyen bir mekanizma ile akciğerde sıvı toplanmasını ve akciğer ödemini engeller. Trombositin fonksiyonu, sadece MPV (ortalama trombosit hacmi) ya da sadece trombosit sayısından ziyade, trombosit kitle indeksi ile daha doğru olarak değerlendirilebilir. Çalışmamızda, YDGT tanılı geç preterm bebeklerde sonunum destek ihtiyacı süresini ve diğer solunumsal sonlanımları ile PMI (trombosit kitle indeksi) başta olmak üzere trombosit belirteçlerinin ilişkisinin araştırılması planlanmıştır.Gereçler ve Yöntem: Bu retrospektif çalışmada, 1 Ağustos 2020-31Temmuz 2021 tarihleri arasında, YDGT tanısı ile Buca Seyfi Demirsoy Eğitim ve Araştırma Hastanesi yenidoğan yoğun bakım ünitesinde yatırılmış, takipnesi en az 12 saat sürmüş tüm geç preterm yenidoğanlara ait veriler hastane kayıtlarından incelendi. Bebeklerin trombosit belirteçleri, lökosit ve nötrofil değerleri ile solunum destek süreleri karşılaştırıldı. Ayrıca bu parametreler, gestasyonel haftalara göre olası farklılıklar açısından değerlendirildi.Bulgular: Çalışmaya 52 bebek dahil edildi. Gestasyonel hafta ortancası 35 hafta (34-366/7), ortalama doğum ağırlıkları 2647474 idi. Bebeklerin toplam solunum destek süreleri ya da >48 saat solunum desteği almaları ile trombosit sayısı, MPV, PMI, lökosit ve absolut nötrofil sayısı (ANS) ile ilişkili saptanmadı. Ancak 72 saatten fazla solunum desteği alan bebeklerde PMI değerlerinin daha düşük olduğu ve bunun da istatistiksel olarak anlamlı olduğu görüldü. Ayrıca MPV değerinin 35.gestasyon haftasında, ANS değerinin ise 36. gestasyon haftasında doğan bebeklerde anlamlı olarak yüksek olduğu görüldü.Sonuç: Çalışmamızda, trombosit belirteçlerinden, PMI’nın, YDGT tanılı geç preterm bebeklerin 72 saatten uzun süre solunum desteği almaları ile ilişkili olduğu, bu bebeklerin ortalama PMI değerlerinin daha düşük olduğu görülmüştür. Ancak benzer bir ilişki diğer trombosit belirteçleri ile solunum destek süresi arasında saptanmamıştır.

Background/Aim: Coronavirus disease 2019 (COVID-19) cases first emerged in Turkey in March 2020, spreading rapidly and peaking in April and May. This study aimed to assess individuals with loss of taste and smell who were admitted to our hospital with a COVID-19 diagnosis. Methods: Between March and June 2020, we retrospectively assessed 6966 patients who visited Kartal Dr. Lütfi Krdar City Hospital's Infectious Diseases and Clinical Microbiology COVID-19 outpatient clinics; 137 patients with loss of taste and smell were included in the study. We enrolled 18-year-old patients who were admitted to the infection emergency outpatient clinics. Results: Out of the 6966 patients hospitalized with a pre-diagnosis of COVID-19 infection, 137 (0.19%) complained of poor taste and smell. Among these, 69 (50.4%) were female, and 68 (49.6%) were male. Of the 137 patients, 100 (73%) reported a loss of smell, while 94 (68.6%) reported a loss of taste. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) RT-PCR (reverse transcriptase-polymerase chain reaction) was positive in approximately one-third (n=2672, 38.4%) of the 6966 patients and roughly half (n=62, 48.4%) of the patients with loss of taste and smell. The most common symptoms observed in patients with anosmia were fever (n=123, 91%), cough (n=102, 75%), shortness of breath (n=411, 30%), sore throat (n=12, 9%), malaise (n=12, 9%), myalgia (n=11, 8%), nausea/vomiting (n=6, 5%), diarrhea (n=4, 3%), loss of smell (n=2, 2%), and loss of taste (n=2, 2%). Comorbidities included hypertension (n=4, 3%), diabetes mellitus (n=4, 2%), chronic obstructive pulmonary disease (n=2, 2%), and coronary artery disease (n=1, 1%). Conclusion: Patients admitted to our hospital during the initial wave of the pandemic experienced typical and prevalent symptoms such as fever, cough, shortness of breath, sore throat, and loss of taste and smell. Since Coronavirus disease (COVID-19) was first identified as a human infectious agent in December 2019, healthcare professionals have continued to work with professional dedication despite the fear of being infected and infecting their families. In this study, it is aimed to determine the COVID-19 risk factors of healthcare professionals. Between 17 March - 1 June 2020, healthcare professionals who applied to Kartal Dr. Lütfi Kırdar City Hospital Infectious Diseases and Clinical Microbiology emergency outpatient clinic, with a probable or definite diagnosis of COVID-19 were included in the study.Demographic, clinical, laboratory, and thoracic computed tomography (CT) data of patients with positive SARS-CoV-2 RT-PCR tests were evaluated retrospectively. Out of a total of 4200 healthcare professionals working in our hospital, 415 (9.8%) applied to the emergency outpatient clinic due to probable or definite COVID-19 infection. SARS-CoV-2 RT-PCR was positive in 89 (21%) of these patients.The mean age of the patients was 43 years and 64% were female healthcare professionals.40 of the patients declared their colleagues and 7 of them their families as possible transmission routes.The most common symptoms were cough (n=69), sore throat (n=39), respiratory distress (n=30) and fever (n=14).When the underlying diseases were examined, chronic obstructive pulmonary disease was found in 6 patients, diabetes mellitus in one, and malignancy in one.Of the 12 (13.4%) hospitalized patients, five were allied health professionals, five were cleaning staff, and two were doctors.All the patients were treated and none of them were mortal. Protecting healthcare professionals from respiratory pathogens that pose a high threat is important to prevent secondary transmission. To reduce ongoing risks among healthcare professionals, it is important to improve the conditions of the healthcare system and increase compliance during the pandemic. Further studies regarding this subject are needed., Giriş: Coronavirus hastalığı (Coronavirus disease-2019=COVID-19), ilk kez aralık 2019’da insanda enfeksiyon etkeni olarak tanımlandığından bu yana sağlık çalışanları, enfekte olma ve ailelerine bulaştırma korkusuna rağmen profesyonel adanmışlık ile çalışmaya devam etmişlerdir. En sık görülen semptomlar öksürük (n=69), boğaz ağrısı (n=39), solunum sıkıntısı (n=30) ve ateş (n=14) idi.Altta yatan hastalıklar incelendiğinde hastaların altısında kronik obtrüktif akciğer hastalığı, birinde diyabetes mellitus, birinde de malignite saptandı. Hastaneye yatan 12 (%13,4) hastanın beşi yardımcı sağlık çalışanı, beşi temizlik personeli, ikisi ise doktordu. Hastaların tümüne tedavi başlanmış olup hiçbiri mortal seyretmedi.Sonuç: Sağlık çalışanları için yüksek tehdit oluşturan solunum yolu patojenlerinden korumak, ikincil bulaşmayı engellemek için önemlidir. Sağlık çalışanları arasında devam eden riskleri azaltmak için pandemi süresince sağlık sisteminin koşullarının iyileştirilmesi ve buna uyumun arttırılması önemli olup daha çok veriye ihtiyaç vardır.

- Abstract
Objective: to investigate the early neonatal outcomes of very-low-birth-weight (VLBW) infants discharged home from neonatal intensive care units (NICUs) in Turkey. Material and methods: a prospective cohort study was performed between April 1, 2016 and April 30, 2017. The study included VLBW infants admitted to level III NICUs. Perinatal and neonatal data of all infants born with a birth weight of.1500 g were collected for infants who survived. Results: data from 69 NICUs were obtained. The mean birth weight and gestational age were 1137 +/- 245 g and 29 +/- 2.4 weeks, respectively. During the study period, 78% of VLBW infants survived to discharge and 48% of survived infants had no major neonatal morbidity. VLBW infants who survived were evaluated in terms of major morbidities: bronchopulmonary dysplasia was detected in 23.7% of infants, necrotizing enterocolitis in 9.1%, blood culture proven late-onset sepsis (LOS) in 21.1%, blood culture negative LOS in 21.3%, severe intraventricular hemorrhage in 5.4% and severe retinopathy of prematurity in 11.1%. Hemodynamically significant patent ductus arteriosus was diagnosed in 24.8% of infants. Antenatal steroids were administered to 42.9% of mothers. Conclusion: the present investigation is the first multicenter study to include epidemiological information on VLBW infants in Turkey. Morbidity rate in VLBW infants is a serious concern and higher than those in developed countries. Implementation of oxygen therapy with appropriate monitoring, better antenatal and neonatal care and control of sepsis may reduce the prevalence of neonatal morbidities. Therefore, monitoring standards of neonatal care and implementing quality improvement projects across the country are essential for improving neonatal outcomes in Turkish NICUs., Turkish Neonatal Society

- Abstract
Background: the aim of this study was to determine the effect of colistin resistance and other predictors on fatality among patients with Klebsiella pneumoniae bloodstream infections (Kp-BSI) and to describe the effect of amikacin and tigecycline on the outcome in an OXA-48 dominant country. Method: This was a retrospective study performed among patients >16 years of age in a tertiary hospital with 465 beds. All cases had ≥1 positive blood culture for K. pneumoniae 48 h after admission. Results: among 210 patients with Kp-BSI, the 30-day mortality rate after isolation of the microorganism was 58%. The rate of carbapenem resistance was higher (64% vs. 38%, p < 0.001) and the colistin minimum inhibitory concentration (MIC) was elevated (7 vs. 4, p < 0.029) among the patients who died. Among the colistin-resistant K. pneumoniae, the rates of OXA-48, ST101, and NDM-1 were 78%, 67%, and 35%, respectively. Amikacin was added to the treatment of 13 patients with carbapenem and colistin-resistant Kp-BSI and 77% survived (p < 0.001). Tigecycline was added to the treatment of 24 patients with carbapenem and colistin-resistant Kp-BSI, and the 30-day mortality rate was 71% (p = 0.576). In the multivariate analysis, carbapenem resistance (odds ratio (OR) 5.2, 95% confidence interval (CI) 2.47–10.9, p < 0.001) and increasing APACHE II score (OR 1.19, 95% CI 1.12–1.26, p < 0.001) were significantly associated with 30-day mortality. The addition of amikacin to the treatment regimen (OR 0.05, 95% CI 0.01–0.23, p < 0.001) was significantly beneficial. Conclusions: Carbapenem resistance, increasing MIC of colistin, and the lungs as the source of the infection were significantly associated with 30-day mortality. The empirical use of combined active aminoglycosides was found to be beneficial in the treatment of colistin-resistant K. pneumoniae infections., NA

- Abstract
Background to evaluate the prevalence, risk factors and treatment of retinopathy of prematurity (ROP) in Turkey and to establish screening criteria for this condition. Methods: a prospective cohort study (TR-ROP) was performed between 1 April 2016 and 30 April 2017 in 69 neonatal intensive care units (NICUs). Infants with a birth weight (BW)=1500 g or gestational age (GA)<= 32 weeks and those with a BW> 1500 g or GA> 32 weeks with an unstable clinical course were included in the study. Predictors for the development of ROP were determined by logistic regression analyses. Results: the TR-ROP study included 6115 infants: 4964 (81%) with a GA <= 32 weeks and 1151 (19%) with a GA>32 weeks. Overall, 27% had any stage of ROP and 6.7% had severe ROP. A lower BW, smaller GA, total days on oxygen, late-onset sepsis, frequency of red blood cell transfusions and relative weight gain were identified as independent risk factors for severe ROP in infants with a BW=1500 g. Of all infants, 414 needed treatment and 395 (95.4%) of the treated infants had a BW <= 1500 g. Sixty-six (16%) of the treated infants did not fulfil the Early Treatment for Retinopathy of Prematurity requirements for treatment. Conclusions: screening of infants with a GA <= 34 weeks or a BW<1700 g appears to be appropriate in Turkey. Monitoring standards of neonatal care and conducting quality improvement projects across the country are recommended to improve neonatal outcomes in Turkish NICUs., NA

37. HIV ile Enfekte Hastalarda AIDS Tanımlayıcı Hastalık ile Başvuran Hastaların Değerlendirilmesi.

- Abstract
Giriş: Bu çalışmada insan bağışıklık yetmezliği virüsü (HIV) nedeniyle polikliniğimizde takip edilen hastalarda AIDS tanımlayıcı hastalık sıklığını görmek amaçlandı. Gereç ve Yöntem: 2018-2023 yılları arasında hastanemiz enfeksiyon hastalıkları ve klinik mikrobiyoloji polikliniğine başvuran ve takipleri yapılan HIV pozitif hastalar geriye yönelik olarak incelendi. Hastaların ilk başvuru sırasında AIDS tanımlayıcı hastalık belirtileri, HIV-RNA ve CD4 sayıları, eşlik eden komorbid durumlar irdelendi. Bulgular: Yaşları 21 ile 66 arasında değişen, 5 (%10) kadın ve 45 (%90) erkek, HIV tanımlayıcı hastalık olan 50 hasta çalışmaya dahil edildi. En sık görülen AIDS tanımlayıcı hastalık Pneumocystis jirovecii (n=19, %38) pnömonisiydi. Bunu sırasıyla lenfoma (n=9, %18), sifilitik üveit (n=7, %14), Kaposi sarkomu (n=7, %14), kandida özefajiti (n=6, %12), toksoplazma keratit ve ensefaliti (n=2, %4), kriptokok menenjiti (n=2, %4), sitomegalovirüs retiniti (n=2, %4) ve bir hastada progresif multifokal lökoensefalopati idi. Hastaların 5'inde (%10) birden fazla HIV tanımlayıcı hastalık mevcuttu. HIV-RNA düzeyi 0-14093638 kopya/ml (ortalama: 1158750 kopya/ml) arasında ve CD4 sayısı 1-1314/μl (356) arasında değişmekteydi (hastalık gruplarına göre HIV-RNA ve CD4 sayıları ve ortalamaları Tablo 1'de özetlenmiştir). Sonuç: AIDS tanımlayıcı hastalıklar neoplazmlar, viral, bakteriyel, parazitik, fungal enfeksiyonlar şeklinde karşımıza çıkabilmektedir. Farklı şikayetlerle başvuru yapan hastalarda tanımlayıcı hastalıkların varlığı HIV enfeksiyonunu düşündürmeli ve gerekli tetkikleri istenmelidir. [ABSTRACT FROM AUTHOR]

Merkez Bankasının temel hedefi fiyat istikrarının sağlanması olduğundan, üretim açığının asgari hata payı ile tahmin edilmesi, uygulanacak para politikasının etkinliği açısından kritiktir. Çalışmamızda üretim açığı hesaplanmıştır. Bu doğrultuda yapılan hesaplamalar 4 farklı yöntem ile Türkiye özelinde 2002-2014 dönemi için gerçekleştirilmiştir. Bu hesaplamalardan ikisi istatistiksel tekniklerden Doğrusal Trend Modeli ve Hodrick-Presscot (HP) Filtresi iken diğer ikisi yapısal yöntemlerden Çok Değişkenli Yapısal Model ve Yapısal Otoregresyon (SVAR) modeli olarak seçilmiştir. Hesaplamaların sonucunda enflasyonu en iyi açıklayan üretim açığının, SVAR modeli ile tahmin edilen üretim açığı tahmini olduğu sonucuna varılmıştır. HP filtreleme tekniği ise SVAR modelini izleyen en iyi ikinci teknik olarak karşımıza çıkmaktadır.

Amaç: Epilepsi tanısını son 3 ay içinde alan ve daha önce nörolojik herhangi bir problemi olmayan çocuklara ve ebeveynlerine epilepsi hakkında verilecek standart eğitim ile çocuğun yaşam kalitesinde ve anksiyetesinde, ebeveynin ise anksiyetesindeki olası değişimin saptanması, diğer yandan eğitim verilen grubun eğitim sonrası sonuçlarını, poliklinikte en az 1 yıldır izlenen, ancak standart eğitimi almamış olgular ve yine polikliniğimizde dirençli epilepsi tanısı ile izlenmekte olan olgular ile karşılaştırılması ve olası farklılıkların saptanmasıdır.Gereç ve Yöntem: Çalışmaya Ege Üniversitesi Tıp Fakültesi Pediatrik Nöroloji Bilim Dalı polikliniğinde 1 Ocak-31 Aralık 2010 tarihleri arasında izlenen toplam 72 epilepsi tanılı olgu alındı. Hastalar üç gruba ayrıldı. I.Grup, daha önce nörolojik herhangi bir problemi olmayan, okul çağı (6-18 yaş), en az iki afebril nöbeti olan ve antiepileptik tedavi başlanacak hastalardan (rolandik nöbet/çocukluk çağı idiyopatik epilepsileri), II.grup poliklinikte epilepsi tanısı ile izlenen, en az 1 yıldır tedavi gören, 1 veya daha fazla antiepileptik ilaç kullanan, son 3 ayda en az bir nöbet geçiren hastalardan, III.grup ise dirençli epilepsi tanısı ile izlenmekte olan hastalardan oluşmaktaydı. I.grup hastalara standart eğitim öncesi ve bu eğitimden 6 ay sonra testler ve anketler uygulandı. Uygulanan anket ve testler; epilepsi hakkında genel bilgi anketi, anksiyete ölçekleri (ebeveyn için `Durumluluk-Süreklilik Kaygı Envanteri', çocuk için `Çocuklar İçin Anksiyete Ölçeği'), çocuğun kendisi için doldurduğu `Yaşam Kalitesi Ölçeği', son olarak da ebeveynin çocuğun özelliklerini göz önünde bulundurarak doldurduğu `Yaşam Kalitesi' ve `Çocuk Davranış Değerlendirme Ölçeği' idi. II. ve III.grup hastalara ise tüm bu bahsedilen testler, rutin poliklinik izlemleri esnasında uygulandı.Sonuçlar: Uygulanan epilepsi hakkında genel bilgi anket puanları karşılaştırıldığında, I.grubun eğitim sonrası değerlerinin eğitim öncesine ve diğer 2 grubun puanlarına göre yüksek olduğu görülmüştür. I.grup için bakıldığında, eğitim sonrası bilgi puanlarının; gelir, ailede birey başına düşen gelir ile pozitif, nöbet sıklığı, ebeveyn durumluluk ve süreklilik anksiyetesi ile negatif korelasyonu olduğu saptanmıştır. II. grubun ise, eğitim sonrası bilgi puanlarının; gelir ve ailede birey başına düşen gelir ile pozitif korelasyonu dışında diğerleri ile korelasyonu olmadığı görülmüştür.Ebeveyn anksiyetesini ölçmek amacı ile uygulanan Durumluluk-Süreklilik Kaygı Envanteri puanları I.grup için eğitim öncesi ve sonrası karşılaştırıldığında, hem durumluluk hem süreklilikte puanların anlamlı olarak düştüğü görülmüştür. 3 grubun puanları birbiri ile karşılaştırıldığında ise hem durumluluk hem süreklilik anksiyetesinde yine fark saptanmış ve bunun 3.grubun anksiyete puanlarının yüksekliği ile ilişkili olduğu görülmüştür.Çocuk anksiyetesini ölçmek amacı ile uygulanan `Çocuklar İçin Anksiyete Ölçeği' puanlarından, I.grubun eğitim sonrası anksiyete puanlarının düştüğü görülmüş ve bu istatistiksel olarak anlamlı bulunmuştur.Çocuk Yaşam Kalitesi Ölçek puanlarından, I.grup kendi içinde değerlendirildiğinde, `Ölçek Toplam Puanları' (ÖTP) ve `Psikososyal Toplam Puanlarının' (PSTP) eğitim sonrası arttığı görülmüş ve istatistiksel açıdan anlamlı olarak değerlendirilmiştir. 3 grubun skorları birbiri ile karşılaştırıldığında ise, III.grubun ÖTP ve PSTP puanlarının her 2 gruptan belirgin olarak düşük olduğu görülmüştür. Çocuk yaşam kalitesi ile ebeveyn anksiyetesi arasındaki korelasyon araştırıldığında, I.grup için, çocuk tarafından doldurulan testlerin `Fiziksel Sağlık Toplam Puanları' (FSTP) ve ebeveyn tarafından doldurulan testlerin ÖTP'leri ile ebeveyn durumluluk anksiyetesi arasında negatif korelasyon olduğu ancak diğer yaşam kalitesi puanları ile ilişkili olmadığı görülmüştür.Epilepsi başlama yaşı ile çocuk yaşam kalitesi ilişkisine bakıldığında ise, I.grupta çocuk yaşam kalitesi ebeveyn ölçeği ÖTP ile negatif ilişki dışında, anlamlı farklılık gözlenmemiştir.Tartışma: Epilepsi hakkında bilgi vermek, nöbet kontrolü kadar önemlidir. Epilepside çocuk ve ebeveyn anksiyetesi bazı demografik değişkenlerden etkilenmektedir. Ayrıca ailenin hastalık ile ilgili yetersiz bilgilerinden dolayı aşırı korumacı davranışları, yüksek anksiyete düzeyleri ve bunun gibi birçok faktörün, hem çocuğun hem de ebeveynin hayat kalitesini olumsuz olarak etkilediği diğer çalışmalarla gösterilmiştir. Ailelerin bu hastalık konusunda sözel ve görsel yolla bilgilendirilmeleri, zaman içinde hem anksiyetelerinin azalmasına, ilaç kompliansının artmasına ve belki de nöbet kontrolünün daha başarı ile sağlanmasına faydalı olacaktır.Anahtar sözcükler: Epilepsi, çocuk, anksiyete, eğitim, ebeveyn, yaşam kalitesi Objectives and Hypotheses: Determining the possible change in anxiety and quality of life of the child and parental anxiety with a structured education program about epilepsy for parents and children who were diagnosed as epilepsy at most 3 months ago. On the other hand, we aimed to determine all these parameters in children and families who did not have the standard education program and who were followed up in outpatient clinic at least for one year. comparing and determining all these after education parameters with the results of tests applied to parents and children who are on follow-up for at least 1 year in the Pediatric Neurology outpatient clinic and did not have the standard education program and ones who are on follow-up as intractable epilepsy.Material and method: 72 epileptic children and their parents who were on follow-up in the Ege University Medical Faculty Pediatric Neurology outpatient clinic between 1 January- 31 December 2010, were included in the study. Patients were divided into three groups. I.group was composed of school age (6-18 years old) patients without any neurologic abnormality (rolandic epilepsy/idiopathic childhood epilepsy), who had at least two afebrile seizures and were planned to start antiepileptic drugs. II.group was composed of patients on follow-up in the outpatient clinic, having antiepileptic therapy for at least 1 year, using at least 1 antiepileptic drug, with at least one seizure during the last 3 months period and III.group was composed of patients with intractable epilepsy. Questionnaires were applied to patients and their parents in the I.group before and 6 months after the education. Applied questionnaires and tests were; questionnaire on general knowledge about epilepsy, anxiety scales (`State-Trait Anxiety Inventory? for the parent, `Anxiety Scale for Children? for the child), `Quality of Life Scales? which were applied to parent and the child and at last, `Child Behavior Check List? which was applied to the parent. All these tests were applied to the II. and III.group patients during their routine outpatient clinic visit.Results: When `questionnaire on general knowledge about epilepsy? scores were compared, post-education scores of the I.group were higher than the pre-education scores and scores of the other 2 groups. For the I.group, post-education scores were in positive correlation with family income, family income per person; in negative correlation with seizure frequency, parent state and trait anxiety.For the II.group,except positive correlation with family income, post-education scores were not correlating with the other variables.When State-Trait Anxiety Inventory which was applied to evaluate parental anxiety scores, were compared for the I.group as pre and post-education, a significant decrement was seen both in state and trait scores. When scores of this test were compared for the 3 groups, differences were seen in both state and trait scores and these differences were seen to be due to the high anxiety scores of the III.group.`Anxiety Scale for Children? which was used to determine the child?s anxiety, was also compared for the I.,II. and III.groups and decrement was seen for the I.group after the education that was statistically significant.When Child Quality of Life Scales were evaluated, for the I.group, `Scale Total Scores? and `Psychosocial Health Total Scores? were significantly higher in the post-education scales when compared for the I.group. When 3 groups were compared, `Scale Total Scores? and `Psychosocial Health Total Scores? of the III.group were significantly lower than the other 2 groups. When correlation between child?s quality of life and parental anxiety was analysed, a negative correlation between parental `State Anxiety? and `Physical Health Total Score? of the scales filled by the children and `Scale Total Score? of the scales filled by the parents were determined but no similar correlation was seen with the other health quality subscores.When correlation of child?s quality of life and the age of diagnosis was analysed, no relation was identified except the negative correlation in the `Scale Total Score? filled by the parent.Conclusion: Giving information about epilepsy is as important as seizure control. Child and parent anxiety in epilepsy is affected by several demographic variables. Besides, it is shown in many studies that over protective behaviors of the family due to lack of enough knowledge about the disease, cause high anxiety levels and this affect the quality of life of the child and the parent in a negative way. Giving information to the families about this disease with oral and visual ways, will decrease the anxiety level in time, increase drug compliance and possibly provide a better seizure control.Keywords: Epilepsy, child, anxiety, education, parent, quality of life. 92

Background: the aim of this study was to determine the effect of colistin resistance and other predictors on fatality among patients with Klebsiella pneumoniae bloodstream infections (Kp-BSI) and to describe the effect of amikacin and tigecycline on the outcome in an OXA-48 dominant country. Method: This was a retrospective study performed among patients >16 years of age in a tertiary hospital with 465 beds. All cases had ≥1 positive blood culture for K. pneumoniae 48 h after admission. Results: among 210 patients with Kp-BSI, the 30-day mortality rate after isolation of the microorganism was 58%. The rate of carbapenem resistance was higher (64% vs. 38%, p < 0.001) and the colistin minimum inhibitory concentration (MIC) was elevated (7 vs. 4, p < 0.029) among the patients who died. Among the colistin-resistant K. pneumoniae, the rates of OXA-48, ST101, and NDM-1 were 78%, 67%, and 35%, respectively. Amikacin was added to the treatment of 13 patients with carbapenem and colistin-resistant Kp-BSI and 77% survived (p < 0.001). Tigecycline was added to the treatment of 24 patients with carbapenem and colistin-resistant Kp-BSI, and the 30-day mortality rate was 71% (p = 0.576). In the multivariate analysis, carbapenem resistance (odds ratio (OR) 5.2, 95% confidence interval (CI) 2.47–10.9, p < 0.001) and increasing APACHE II score (OR 1.19, 95% CI 1.12–1.26, p < 0.001) were significantly associated with 30-day mortality. The addition of amikacin to the treatment regimen (OR 0.05, 95% CI 0.01–0.23, p < 0.001) was significantly beneficial. Conclusions: Carbapenem resistance, increasing MIC of colistin, and the lungs as the source of the infection were significantly associated with 30-day mortality. Material and methods A prospective cohort study was performed between April 1, 2016 and April 30, 2017. the study included VLBW infants admitted to level III NICUs. Perinatal and neonatal data of all infants born with a birth weight of.1500 g were collected for infants who survived. Results Data from 69 NICUs were obtained. the mean birth weight and gestational age were 1137 +/- 245 g and 29 +/- 2.4 weeks, respectively. During the study period, 78% of VLBW infants survived to discharge and 48% of survived infants had no major neonatal morbidity. VLBW infants who survived were evaluated in terms of major morbidities: bronchopulmonary dysplasia was detected in 23.7% of infants, necrotizing enterocolitis in 9.1%, blood culture proven late-onset sepsis (LOS) in 21.1%, blood culture negative LOS in 21.3%, severe intraventricular hemorrhage in 5.4% and severe retinopathy of prematurity in 11.1%. Hemodynamically significant patent ductus arteriosus was diagnosed in 24.8% of infants. Antenatal steroids were administered to 42.9% of mothers. Conclusion the present investigation is the first multicenter study to include epidemiological information on VLBW infants in Turkey. Morbidity rate in VLBW infants is a serious concern and higher than those in developed countries. Implementation of oxygen therapy with appropriate monitoring, better antenatal and neonatal care and control of sepsis may reduce the prevalence of neonatal morbidities. Background To evaluate the prevalence, risk factors and treatment of retinopathy of prematurity (ROP) in Turkey and to establish screening criteria for this condition., Methods A prospective cohort study (TR-ROP) was performed between 1 April 2016 and 30 April 2017 in 69 neonatal intensive care units (NICUs). Infants with a birth weight (BW)=1500 g or gestational age (GA) 1500 g or GA> 32 weeks with an unstable clinical course were included in the study. Predictors for the development of ROP were determined by logistic regression analyses., Results The TR-ROP study included 6115 infants: 4964 (81%) with a GA 32 weeks. Overall, 27% had any stage of ROP and 6.7% had severe ROP. A lower BW, smaller GA, total days on oxygen, late-onset sepsis, frequency of red blood cell transfusions and relative weight gain were identified as independent risk factors for severe ROP in infants with a BW=1500 g. - Abstract
Central nervous system nocardiosis is rare but has high morbidity and mortality. Immunocompromised patients who have malignancies such as lymphoma, infections such as human immunodeficiency virus (HIV), and bone marrow or solid organ recipients are particularly vulnerable to infection. However, here, we present a patient who developed nocardiosis and recurrent brain abscesses despite having no immunodeficiency problems. The abscess recurred despite total abscess excision and parenteral therapy. After nocardiosis was diagnosed, intravenous treatment with imipenem and amikacin was initiated. The patient was discharged on oral doxycycline.
