Resumo Fundamentos A determinação precisa do colesterol de lipoproteína de baixa densidade (LDL-C) é importante para se alcançar concentrações de LDL-C recomendadas por diretrizes e para reduzir resultados cardiovasculares adversos em pacientes diabéticos. A equação de Friedewald comumente usada (LDL-Cf) produz resultados imprecisos em pacientes diabéticos devido a dislipidemia diabética associada. Recentemente, duas novas equações – Martin/Hopkins (LDL-CMH) e Sampson (LDL-Cs) – foram desenvolvidas para melhorar a precisão da estimativa de LDL-C, mas os dados são insuficientes para sugerir a superioridade de uma equação sobre a outra. Objetivos O presente estudo comparou a precisão e a utilidade clínica das novas equações de Martin/Hopkins e Sampson em pacientes diabéticos. Método Foram incluídos no estudo quatrocentos e dois (402) pacientes com diabetes. O risco cardiovascular dos pacientes e as metas de LDL-C foram calculadas por diretrizes europeias. As concentrações de LDL-Cmh, LDL-Cs, e LDL-Cf calculadas foram comparadas à concentração de LDL-C direto (LDL-Cd) para testar a concordância entre essas equações e LDL-Cd. Um P valor 400 mg/dl, a concordância com o LDL-Cd foi ruim, independentemente do método usado. Conclusão As equações de Martin/Hopkins e Sampson mostram uma precisão similar para o cálculo de concentrações de LDL-C nos pacientes com diabetes, e ambas as equações são ligeiramente melhores que a equação de Friedewald. Abstract Background The accurate determination of low-density lipoprotein cholesterol (LDL-C) is important to reach guideline-recommended LDL-C concentrations and to reduce adverse cardiovascular outcomes in diabetic patients. The commonly used Friedewald equation (LDL-Cf), gives inaccurate results in diabetic patients due to accompanying diabetic dyslipidemia. Recently two new equations – Martin/Hopkins (LDL-Cmh) and Sampson (LDL-Cs) – were developed to improve the accuracy of LDL-C estimation, but data are insufficient to suggest the superiority of one equation over the other one. Objective The present study compared the accuracy and clinical usefulness of novel Martin/Hopkins and Sampson equations in diabetic patients. Methods This study included 402 patients with diabetes. Patients’ cardiovascular risk and LDL-C targets were calculated per European guidelines. Calculated LDL-Cmh, LDL-Cs, and LDL-Cf concentrations were compared with direct LDL-C concentration (LDL-Cd) to test agreement between these equations and LDL-Cd. A p-value 400 mg/dl, agreement with LDL-Cd was poor, regardless of the method used. Conclusion Martin/Hopkins and Sampson’s equations show a similar accuracy for calculating LDL-C concentrations in patients with diabetes, and both equations were marginally better than the Friedewald equation.

Giriş: Retrospektif bu çalışmanın amaçı, endovasküler abdominal aort onarım (EVAR)ve torasik aortik onarım (TEVAR) erken dönem sonuçlarını değerlendirmek vehastanemizin erken dönem tecrübesini incelemektir.Hastalar veYöntem: Ocak 2009 ile Aralık 2011 tarihleri arasında EVAR veTEVAR işlemi uygulanan tüm hastalar incelenmiştir. İşlem öncesi, işlem ve işlemsonrası veriler retropektif olarak değerlendirildi.Bulgular: Çalışma periyodunda 46 hastaya TEVAR, 25 hastaya EVAR uygulandı.Endovasküler girişim yapılan hastaların ortalama yaşı 63.7 yıldı. Cihazlarıyerleştirme başarısı %100’dü. İlave işlem olarak TEVAR olguları içimsubklaviyen revaskülarizasyonu (1, %4), aortik debranching (3, %12); EVARolguları için femoral revaskülarizasyon (1, %2.1) işlemi uygulanmıştır. Acilolgulardaki hastane içi ölüm oranı, elektif olgulara göre daha yüksek izlendi(%13.3 vs. %5.3).Sonuç: Torasik ve aortik endovasküler onarım erkendönem tecrübe sonuçlarımız umut vericidir. Endovasküler stent greftle aortikpatolojilerin onarımı öğrenme dönemlerinde bile etkili bir işlemdir., Introduction:Thepurpose of this retrospective study was to evaluate the short-term results ofendovascular abdominal aortic repair (EVAR) and thoracic aortic repair (TEVAR)and to present an overview of our early experience at our institution. Patientsand Methods: Data of all patients undergoing EVAR and TEVAR betweenJanuary 2009 and December 2011 were reviewed. Preoperative, intraoperative andearly postoperative data were retrospectively reviewed. Results: During thestudy period, 46 EVARs and 25 TEVARs were performed. The mean age of patientsundergoing endovascular intervention was 63.7 years. The deployment successrates of the procedures were 100%. Additional procedures included subclavianrevascularisation (1, 4%) and aortic debranching (3, 12%) for TEVAR cases andfemoral revascularisation (1, 2.1%) for EVAR cases. In-hospital mortality inemergency cases was higher than the elective cases (5.3% vs. 13.3%).Conclusion: Our early short-term results of endovascularrepair of thoracic and aortic diseases are promising. Endovascular stent graftsare effective procedures for managing aortic diseases in an inexperiencedcentre.

Objective: Pulse pressure (PP) is a significant marker of cardiovascular morbidity.We investigated the relation between aortic PP and the presence and extent of coronary artery disease (CAD) in patients undergoing diagnostic coronary angiography (CAG). Patients and Method: The study group consisted of 550 patients (363 men, 187 women).We evaluated patients in two different groups, PP < 60 mmHg and ? 60 mmHg. Results: In univariate analysis gender and presence of hyperlipidemia showed no statistically significant differences between both groups. However, the ratio of patients having diabetes mellitus, hypertension or smoking were significantly higher in ? 60 mmHg PP group. Introduction: We aimed to evaluate the safety and efficacy of the simpax stent in the treatment of different patient groups. Patients and Methods: Forty-five patients were treated with the simpax stent. Of these patients, 23 patients gave consent for six months of follow-up by quantitative coronary angiography (QSA) and six patients were evaluated by exercise electrocardiographic test. Only the patients having lesions with stenosis > 50% of diameter and lengths > 16 mm with reference diameters < 2.75 mm were included. Results: The device success rate was 100% and procedure success rate was 97.7%. The mean stent length was 24.6 ± 7.3 mm and stent size was 2.54 ± 0.24 mm. The overall six months incidence of major adverse cardiac events (MACE) was 8.8%. MACE was consisted of two cases of non-Q wave myocardial infarction and two cases of repeated revascularization of the target lesion. MACE rate was higher in chronic total occlusion (CTO) group than non-CTO group (respectively 33.3% and 5.1). Also when compared to stent size, MACE rate was 25% in < 2.5 mm, 0% ? 2.5 mm. The QSA results at six months showed in-stent late lumen loss witha diameter of 0.25 ± 0.15 mm in 17 patients. Conclusion: The six month results in this study demonsrated excellent procedural and device success. Simpax stent was associated with a low in-stent late lumen loss. Also this study showed simpax stent was a safe and effective device in non-CTO group with stent size ? 2.5 mm., Giriş: Bu çalışmadaki amacımız farklı hasta gruplarında simpax stentin etkinliği ve güvenilirliğini değerlendirmektir. Hastalar ve Yöntem: Kırk beş hasta simpax stent ile tedavi edildi. Altı aylık takipte 23 hasta kantitatif koroner anjiyografi (QSA) ve altı hasta egzersiz elektrokardiyografi testiyle değerlendirildi. Çalışmaya > %50 çap daralması olan ve uzunluğu > 16 mm ile referans damar çapı < 2.75 mm olan hastalar dahil edildi.Bulgular: Cihaz başarı oranı %100, prosedür başarı oranı %97.7 idi. Ortalama stent uzunluğu 24.6 ± 7.3 mm ve stent çapı 2.54 ± 0.24 mm idi. Altı aylık majör istenmeyen kardiyak olayların (MACE) genel insidansı %8.8 olarak bulundu. MACE oranı kronik total oklüzyon (KTO) grubunda KTO olamayan gruba göre daha fazlaydı (sırasıyla %33.3 ve %5.1). Ayrıca, stent çaplarına göre karşılaştırıldığında, MACE oranı çapı < 2.5 mm olan stentlerde %25; > 2.5 mm olan stentlerde %0 olarak bulundu. On yedi hastadaki altıncı ay QSA sonuçları stent içi geç lümen kaybını 0.25 ± 0.15 mm olarak gösterdi. Sonuç: Çalışmanın altı aylık sonuçlarına göre prosedür ve cihaz başarı oranı mükemmeldi. Introduction: In this study, we investigated the effects of frequent and unnecessary change in medication to control of blood pressure. Also we investigated the role of the regular use of drugs in the treatment of hypertension. Patients and Methods: Nine hundred hypertensive patients (509 males and 391 females; mean age 62.4 ± 12.1 years) were evaluated retrospectively. Only the patients taking anti-hypertensives for at least six months were included. Patients' records were examined in terms of anti-hypertensive treatment. Intra-group exchange and use of regularly in anti-hypertensive drugs were investigated. Effects of the change in blood pressure and patient compliance were investigated. Results: When intra-group drug change was examined, rates of drug changes were as follows; angiotensin receptor blocker group, 62.3%; angiotensin converting enzyme group, 55.1%; betablockers group, 39.9%; calcium channel blocker group, 20.9%. While the ratio of non-compliance was 28.7% in the patient group with drug change, it was only 9.3% in the group without change in treatment. Average blood pressure difference was found 6.1 ± 7.6 mmHg and 7.6 ± 7.5 mmHg respectively (p < 0.001). The average number of drug usage with and without change of antihypertensive drug were 2.5 ± 0.87 and 2.2 ± 0.89 (p < 0.001) respectively. Conclusion: In patients under angiotensin converting enzyme inhibitors and angiotensin receptor blocker treatments, intra-group change of medication is common. Frequent intra-group change of anti-hypertensive drugs has negative effects on blood pressure control., Giriş: Bu çalışmada sık ve gereksiz ilaç değişiminin kan basıncı kontrolü üzerindeki etkisini araştırdık. Ayrıca, hipertansiyon tedavisinde düzenli ilaç kullanımının rolü çalışıldı. Hastalar ve Yöntem: Dokuz yüz hipertansif hasta (509 erkek ve 391 kadın; yaş ortalaması 62.4 ± 12.1 yıl) geriye dönük olarak değerlendirildi. En az altı aydır antihipertansif ilaç alan hastalar çalışmaya dahil edildi. Hasta kayıtları antihipertansif tedavi yönünden incelendi. Grup içi değişim ve antihipertansif ilaçların düzenli kullanımı araştırıldı. Değişimin kan basıncı ve hasta uyumuna etkisi araştırıldı. Bulgular: Grup içi ilaç değişim oranları; anjiyotensin reseptör bloker grubunda,%62.3; anjiyotensin dönüştürücü enzim grubunda,%55.1; beta-bloker grubunda,%39.9; kalsiyum kanal bloker grubunda,%20.9 olarak bulundu. Düzensiz ilaç kullanım oranı ilaç değişikliği olan hasta grubunda %28.7 iken, ilaç değişikliği olmayan hasta grubunda sadece %9.3 idi. Ortalama kan basıncı farkı sırasıyla 6.1 ± 7.6 mmHg ve 7.6 ± 7.5 mmHg idi (p < 0.001). Antihipertansif ilaç değişimi olan ve olmayan gruplarda kullanılan ortalama ilaç sayısı sırası ile 2.5 ± 0.87 ve 2.2 ± 0.89 idi (p < 0.001). Sonuç: Anjiyotensin dönüştürücü enzim inhibitörleri ve anjiyotensin reseptör bloker tedavisi altındaki hastalarda grup içi ilaç değişimi yaygındır. Antihipertansif ilaçların grup içi sık değişimi kan basıncı kontrolünü olumsuz etkiler.

Background: We investigated the relationship between brain natriuretic peptide (BNP), a marker for congestive heart failure, and plasma homocysteine levels. Methods: The study included seven patients (3 females, 4 males) with congestive heart failure and 36 patients (17 females, 19 males) without a history of previous myocardial infarction or congestive heart failure. Plasma BNP and homocysteine levels were measured. All the patients were assessed by transthoracic echocardiography.Results: In 19 patients (44%) plasma BNP and homocysteine levels were lower than 15 pg/ml and 7.91 mol/l, respectively, whereas in 24 patients, the corresponding levels were higher than 68.4 pg/ml and 8.84 mol/l. There was a significant positive correlation between plasma BNP and homocysteine levels (r=0.92, p, Amaç: Konjestif kalp yetersizliğini gösteren bir parametre olarak kabul edilen BNP (B-tipi natriüretik peptid) ile plazma homosistein düzeyi arasındaki ilişki araştırıldı. Üretilen kompozitlerin mekanik ve tribolojik özellikleri incelenmiştir. %30 ve %35 oranlarında iki ayrı Cam Elyaf takviyeli, Polyamid-6 matrisli kompozit malzeme ilk olarak 120ºC sıcaklıkta 2 saat süre ile fırında ön ısıtmaya tabi tutulmuştur. 1gr Bor Oksit-10ml ispirto olacak şekilde ayarlanarak Bor Oksit ispirtoda çözülmüştür. %30 ve %35 oranlarında iki ayrı Cam Elyaf takviyeli, Polyamid-6 'ya hacimce Bor Oksit oranı %1 ve %2 olacak şekilde Bor Oksit -ispirto karışımı ilave edilmiştir ve karıştırılmıştır. The addition of B²O³ to glass fiber-reinoforced Polyamide 6 matrix causes the decrease of strength and stiffness values. B²O³ addition has much more effect on decreasing of strength than decreasing of hardness. On the other hand the addition of %1 B²O³ to 30% glass fiber reinforced Polyamide 6 matrix increases the strain value at the failure (rupture) point. Furthermore, it was seen that coefficient of thermal conductivity increased with increasing ratio of B²O³ in %30 and %35 glass fiber reinforced composite specimens.
