Biceps tendon disorders are substantial cause of intense shoulder pain, and thus they require a careful medical attention. We compared the clinical outcomes of chronic rupture, tenotomy and tenodesis of proximal biceps tendon of patients underwent arthroscopic shoulder surgery. We included 98 patients over 50 years-old who underwent arthroscopic surgery for rotator cuff tears and grouped as tenotomy, tenodesis and chronic rupture of biceps tendon. Preoperative and postoperative 24-months follow-up Visual Analog Scale (VAS) score and the University of California–Los Angeles (UCLA) Shoulder Scale scores were compared. Chronic biceps rupture was the most common pathology in massive rotator cuff rupture group (75.9%). As tenotomy, tenodesis and chronic rupture groups were compared in terms of VAS and UCLA score improvements (preoperative and postoperative change); tenotomy showed better outcomes and there was a statistically significant difference between tenotomy and tenodesis groups in terms of improvements in VAS and UCLA scores (p = 0.036, p = 0.010, respectively). No statistically significant difference between chronic rupture and tenodesis in terms of VAS and UCLA scores improvements were observed (p = 1.000, p = 0.250, respectively). Also, tenotomy showed superior VAS score improvement than chronic tendon rupture (p=0.024) however no statistically significant difference was observed in UCLA score improvement between tenotomy and chronic rupture (p = 0.527). According to our study, tenotomy seems to be a more appropriate surgical method than tenodesis in massive and full-thickness rotator cuff tears with severe biceps tendon degeneration, and also it is not necessary to perform tenodesis in cases with chronic biceps rupture.

ÖZET Tahılın nem içeriği, depolama, işleme ve taşıma gibi çeşitli tarımsal faaliyetlerde, tahılın fiziksel özelliklerini etkileyen en önemli parametrelerden biridir. Tahıl depolamanın iyileştirilmesinde ve depolama süresinin uzatılmasında, nem miktarını azaltmak için kurutma makineleri önemli bir rol oynamaktadır. Kurutma makinelerinde sürecin dinamik takibi ve kapalı döngü kontrolü için, doğru geri bildirim verilerini dijital ve gerçek zamanlı olarak aktaran nem sensörlerine ihtiyaç vardır. Tahıldaki nem, dielektrik katsayısını ölçülebilir miktarda değiştirmektedir. Bu tezde dielektrik katsayısının değişimini algılayabilen, kapasitif yöntemle gerçek zamanlı nem ölçümü yapabilen, elektromekanik bir tahıl nem sensörü tasarımı yapılarak tahıl kurutma makinesinin otomasyon sistemine entegre edilmiştir. Tasarlanan elektromekanik nem sensörü; tahılın içerisine konulduğu, akış hızının ayarlandığı ve paralel plakalı kapasitör şeklinde olan ölçüm kabı ile değişen kapasite miktarını ölçerek nem değerine çeviren mikrodenetleyici kontrollü sensör elektronik kartı olmak üzere iki kısımdan oluşmaktadır. Geliştirilen tahıl nem sensörünü kalibre etmek için mısır ve buğday numuneleri kullanılmıştır. Öncelikle mısır ve buğday için ayrı ayrı nem içeriklerine bağlı olarak kapasitanslarının değişimi, ikinci dereceden polinomlar ile modellenmiştir. Daha sonra sıcaklığın mısır ve buğday nemi üzerindeki etkisi de ikinci dereceden polinomlar ile modellenmiştir. Son olarak kapasitans değişimine bağlı model ile sıcaklık değişimine bağlı model birleştirilerek nihai hibrit model oluşturulmuştur. İyileştirilen sensör sürekli tahıl akışında ölçüm yapabilmektedir. Verilerin gerçek zamanlı olarak iletilebilmesi için, geliştirilen sensör tahıl kurutucu boşaltma sisteminin çıkışına uyarlanmıştır. Kurutucu kontrol sisteminin otomasyonunda, elde edilen veriler PLC cihazına ve HMI paneline modbus RTU haberleşme yöntemi ile aktarılmıştır. Tasarlanan elektromekanik tahıl nem sensörü; HMI dokunmatik panel cihazı, PLC cihazı, VFD inverter, 3 fazlı asenkron motor ve kurutucu tahliye sistemi alt bileşenlerinden oluşan özgün tasarımlı bir otomasyon sisteminde test edilmiştir. Test sonuçları enerji verimliliği, ölçüm doğruluğu, kullanım kolaylığı gibi parametreler referans alınarak yorumlanmıştır. Tez sonucunda; 90x210x260 mm boyutlarında, 304L mat krom malzemeden, IP67 koruma sınıfında, -40°C ile +105°C sıcaklık aralığında çalışan, tahıl sıcaklığının neme etkisi modellendiği için sensör ölçüm değeri dış ortam sıcaklığından etkilenmeyen, %7- %25 Rh ölçüm aralığında ve ±% 0,3 doğrulukta ölçüm yapabilen, 1saniye örnekleme zamanı ile sürekli tahıl akışında, kapasitif yöntemle ölçüm yaparak modbus haberleşme ile otomasyon sistemine aktarabilen elektromekanik bir tahıl sensörü başarıyla geliştirilmiştir. SUMMARY Moisture content of grain is one of the most important parameters affecting the physical properties of grain in various agricultural activities such as storage, processing and transportation. Dryers play an important role in improving grain storage and extending the storage period to reduce the amount of moisture. For dynamic monitoring and closed loop control of the process in dryers, humidity sensors that transmit accurate feedback data digitally and in real time are needed. Moisture in the grain changes the dielectric coefficient measurably. In this thesis, an electromechanical grain moisture sensor, which can detect the change in dielectric coefficient and measure moisture in real time with capacitive method, has been designed and integrated into the automation system of the grain dryer. Designed electromechanical humidity sensor; It consists of two parts, a measuring cup in the form of a parallel plate capacitor, in which the grain is placed, the flow rate is adjusted, and a microcontroller-controlled sensor electronic card that measures the changing capacity amount and converts it to moisture. Corn and wheat samples were used to calibrate the developed grain moisture sensor. First of all, the variation of capacitances for corn and wheat, depending on their moisture content, is modeled with quadratic polynomials. Then, the effect of temperature on corn and wheat moisture is also modeled with quadratic polynomials. Finally, the final hybrid model was created by combining the model depending on the capacitance change and the model depending on the temperature change. The improved sensor can measure in continuous grain flow. In order to transmit the data in real time, the developed sensor is adapted to the output of the grain dryer unloading system. In the automation of the dryer control system, the obtained data is transferred to the PLC device and the HMI panel with the modbus RTU communication method. Designed electromechanical grain moisture sensor; It has been tested in a uniquely designed automation system consisting of HMI touch panel device, PLC control, VFD inverter, 3-phase asynchronous motor and dryer evacuation system sub-components. Test results are interpreted with reference to parameters such as energy efficiency, measurement accuracy, and ease of use. As a result of the thesis; With dimensions of 90x210x260 mm, 304L matte chrome material, IP67 protection class, operating in the temperature range of -40°C to +105 °C, the sensor measurement value is not affected by the outdoor temperature since the effect of the grain temperature on humidity is modeled, in the measurement range of 7%-25% Rh and an electromechanical grain sensor that can measure with an accuracy of ± 0.3% and transfer it to the automation system via modbus communication by measuring with a capacitive method in a continuous grain flow with a sampling time of 1 second has been successfully developed. Background In this study, we determined that among patients who had been operated upon for hip fractures at our hospital, prognostic factors for mortality and functional recovery in the preoperative period were indicated via laboratory parameters using the International Falls Efficacy Scale (FES-I) and Berg Balance Scale (BBS) scores. Methodology Between January 2020 and January 2021, the results of 64 patients who had been surgically treated for a hip fracture and 57 patients who had scheduled elective surgery were compared retrospectively. The groups' demographic data and blood parameters were compared. We used the FES-I and BBS scores to determine patients' physical functional status and fear of falling. Results The case group's statistically significant FES-I score was high, and its BBS score was low (p = 0.001/0.001). As expected, the case group's D-dimer measurement was higher than the control group's (p = 0.001). In addition, hemoglobin, platelet, lymphocyte, albumin, total protein, and calcium levels were lower in the case group (p = 0.001 for all levels). No significant difference was found for other parameters. Conclusions The scales are used by physical therapy, neurology, and orthopedics professionals to evaluate the geriatric population's physical functional status and fear of falling. We believe prevention and cost-effective treatments for hip fractures can be achieved by determining geriatric patients' hemoglobin, platelet, lymphocyte, albumin, total protein, and calcium levels upon hospital admission and by directing these patients to relevant clinics using the fear-of-falling and balance scales.

According to both of Einstein's theories, some physical events cause time shortening, but only in the theory of general relativity, space-time changes. This change is the warping of space-time. If the bending of space-time were applied to the theory of special relativity, the point position of the observer would have to be bent. The bending of a single point would break the continuity of space-time. Therefore, a new modeling that fits both theories is needed. This modeling is a shortened space-time modeling.
