Amaç: Kırım Kongo Kanamalı Ateşi (KKKA), sıklıkla kene tutunması ile bulaşan, ateş, myalji ve kanamalar ile seyreden zoonotik bir hastalık olup önemli halk sağlığı sorunlarına yol açabilmektedir. Klinik belirtiler ve epidemiyolojik öykü yanında, kan sayımı ve biyokimyasal test sonuçları KKKA için ilk ipuçlarını oluşturmaktadır. Bu çalışmada hastanemizde KKKA tanısı ile takip ve tedavi edilen 13 olgunun retrospektif olarak değerlendirilmesi amaçlanmıştır. Materyal ve Metot: Kliniğimizde 2019-2022 yılları arasında KKKA tanısı ile takip ve tedavi edilen olgular, demografik özellikleri, başvuru yakınmaları, kene ile temas öyküsünün varlığı, laboratuvar parametreleri, kan ürünleri replasmanı gerekliliği, hastanede yatış süreleri, ribavirin tedavisi ve olguların sağ kalımları açısından retrospektif olarak hasta dosyalarındaki verilerden incelenmiştir. Bulgular: Kırım Kongo Kanamalı Ateşi tanısı ile 13 olgu [dört kadın, dokuz erkek, ortalama yaş 48,4±16.3 (min: 19-max: 67)] çalışmaya dahil edildi. Olguların başvuru yakınmaları ateş yüksekliği %100, halsizlik %84,6 ve bulantı-kusma %7,7 idi. Sekiz olguda (%61.5) kene temas öyküsü bulunurken beş olguda yoktu. Olguların hepsine destek tedavileri uygulandı, tamamı damlacık ve temas izolasyonu uygulanarak izlendi. Kan ürünleri replasmanı dört olguya (%30.7), ribavirin tedavisi on olguya (%76.9) uygulandı. Takip ettiğimiz olgulardan mortal seyreden olmadı. Sonuç: İlkbahar ve yaz aylarında ateş yüksekliği ile başvuran, hemogramda sitopenisi olan hastalarda kene teması sorgulanmalı ve gerekirse ileri tetkikler yapılmalıdır.

Amaç: Çalışmamızda, Hastanemiz Enfeksiyon Hastalıkları ve Klinik Mikrobiyoloji Kliniği’nde Ekstra Pulmoner Tüberküloz (EPTB) tanısı alan olguların retrospektif olarak irdelenmesi amaçlandı. Gereç ve Yöntemler: Çalışmamız, Van Yüzüncü Yıl Üniversitesi Tıp Fakültesi Dursun Odabaş Tıp Merkezi Enfeksiyon Hastalıkları ve Klinik Mikrobiyoloji Ana Bilim Dalı tarafından 1 Ocak 2015 tarihinden, 6 Haziran 2022 tarihine kadar olan süre içinde takip edilmiş akciğer dışı tüberküloz olgularının tanı, takip ve tedavi bilgilerinin retrospektif olarak incelenmesi ile yapıldı. Bulgular: Çalışmamıza, EPTB tanısı konulmuş olan 142 olgu dahil edildi. EPTB’un en sık görülen tutulum şekli, 61 olgu (%43.0) ile lenf nodu tutulumu (lenfadenit) olarak izlendi. 18 olgunun 15’inde (%83.3) ARB, 7 olgunun 5’inde (%71.4) PCR testi pozitifliği saptandı. 16 olgunun 13’ünde (%81.3) M. tuberculosis üremesi oldu. Histopatolojik inceleme için 106 olgudan (%74.7) biyopsi alındı ve sonuçlarda olguların tamamında kronik granülomatöz inflamasyon tespit edildi. Bunların 46'sında (%43.4) kazeifikasyon nekrozu, 20'sinde (%18.9) nekrotizan granülomatöz inflamasyon, 10 (%9.4) olguda non-kazeifiye granülomatoz inflamasyon ve diğer 30’unda (%28.3) ise granülomatoz inflamasyon mevcuttu. Olgulara standart 6-9 ay tedavi verilirken menenjit, dissemine TB ve osteoartiküler tutulumda tedavi süreleri 9-12 ay olarak uygulandı. Tedavide ilk iki ayda dörtlü anti-TB tedavisi verilirken daha sonra izoniazid (İNH) ve rifampisin (RIF) olarak ikili tedavi ile devam edildi. Sonuç: TB hastalığı ülkemiz için önemli bir sağlık sorunudur. En sık lenf bezi tutulumu şeklinde görülen EPTB olgularında erken tanı ve tedavi mortalite ve morbidite oranlarının azaltılması için önemlidir.

WOS:000540091500001 PubMed: 32449420 Objective: We aimed to detect the malignant arrhythmic potential of COVID-19 with surface electrocardiographic (ECG) markers. Material and method:Of the ECG parameters PR, QT, QTc, QTd, TPe, and Tpe/QTc were measured in 51 COVID-19 patients and 40 in control subjects. Results: Compared to control group mean QTc (410.8 +/- 24.3 msec vs. 394.6 +/- 20.3 msec,p < .001) and Tpe/QTc (0.19 +/- 0.02 vs. 0.18 +/- 0.04,p= .036) and median QTd (47.52 vs. 46.5) values were significantly higher in COVID-19 patients. Troponin levels were significantly correlated with heart rate (r = 0.387,p = .006) but not with ECG parameters. Conclusion:Several ventricular arrhythmia surface ECG predictors including QTc, QTd, and Tpe/QTc are increased in COVID-19 patients. Since medications used in COVID-19 patients have the potential to affect these parameters, giving importance to these ECG markers may have a significant contribution in decreasing disease-related arrhythmias.

Twohundred Intensive Care Unit (ICU) patients are consulted by our department in between October1997 to December1998 for infectious diseases. A prospective study is performed for the reasons of hospitalization, underlying diseases, risk factors, etiology, antibiotic susceptibility, type of infection and prognosis. Patients were consisted of 44% females and 56% males with a mean age of 42.2. Sixty five percent of the patients were directly admitted to the ICU where as 35% were transferred either from clinics or hospitals. 55% of the patients already had primary infections before admitted to the ICU; however, 64% of these patients were transferred to the ICU either from another clinic or hospital. In 39% of the patients, infection has developed while hospitalization in the ICU. The most common pathogens isolated from ICU patients were Staphylococcus aureus (32%), Acinetobacter spp. (16%) , coagulase negative Staphylococcus (13%), Pseudomonas spp (13%), Klebsiella spp. (10%), Escherichia coli (9%), Enterococcus spp. (5%), Candida spp. (4%), Streptococcus pneumoniae (3.7%) and Enterobacteriaceae spp. (2.6%). However for nosocomial ICU infections (n= 78) the most common pathogens were Acinetobacter spp. (25.6%), S. aureus (21.7%) and Pseudomonas spp. (%16.6). In the patients with community acquired infection (n= 40), pathogens were described as S. aureus (37.5%), E. coli (22.5%) and S. pneumoniae (12.5%). In patients transferred from another clinics or hospitals (n= 70), the most common pathogens were S. aureus (40%), coagulase negative staphylococci (18.5%) and Acinetobacter spp. (15.7%). Rate of meticillin resistant S. aureus isolated from ICU infections was 90%. Vancomcyin was the most effective antibiotic for Staphylococcus (100%) in this study, followed by trimethoprim-sulphamethoxazole. Imipenem (85%) and amikacin (60%) were the most effective antibiotics for Acinetobacter spp. Whereas ceftazidime (70%) and imipenem (60%) were the most effective agents for Pseudomonas spp. Nosocomial infection types of our patients were as follows; respiratory tract infections (27%), urinary tract infections (24.3%), bacteremia (20.5%), post-operative surgical infections (13%) and others (15%). Mortality rate for ICU patients was 44% (n= 88). It is observed that the mortality rate was significantly high for the patients having more than 20 days of hospitalization time and for the patients with APACHE II score > 15 (p < 0.05).

Amaç: Bir üniversite hastanesinde dört yoğun bakım ünitesinde yatan hastalarda nozokomiyal infeksiyonların sıklığı ile izole edilen patojenlerde antimikrobiyallere direnci belirleme. Yöntem ve Gereç: Medikal, cerrahi ve reanimasyon yoğun bakım ünitelerinde yatan hastalar prospektif olarak izlendi. İzolatlar CDC kritelerine göre belirlenen nozokomiyal infeksiyonlardan toplandı. İzolatların tanımlanması ve antibiyotiklere duyarlılığı Sceptor (Becton Dickinson) otomatize sistemle CCLS kriterlerine göre yapıldı. Bulgular: YBÜ’lerinde yatan 3962 hastada, nozokomiyal infeksiyon insidansı % 6,9; en yüksek oran reanimasyon YBÜ’nde (% 14,7), en düşük oran cerrahi YBÜ’nde (% 2,5) olarak belirlendi. 272 hastada 492 Nİ epizodu belirlendi. Nİ insidansı 1000 hasta gününe 13,1 olarak saptandı. Nİ oranları, üriner sistem (% 32), primer bakteriyemi (% 24), pnömoni (% 20), cerrahi alan (% 13) ve diğer infeksiyonlar % 11 bulundu. Pnömoni Reanimasyon YBÜ’nde en sık saptanan (% 54) Nİ idi. Etkenlerin % 39’u gram pozitif, % 52’si gram negatif bakteri, % 9’u Candida türleri idi. En sık izole edilen bakteri S. aureus (% 18); bunu P. aeruginosa (% 16), Acinetobacter spp. (% 10), koagülaz negatif stafilokok (KNS) (% 10), Klebsiella (% 9), E. coli (% 9), ve Enterococcus spp. (% 9) izlemekteydi. S. aureus türlerinde metisilin direnci % 90, KNS larda % 95, S. aureus ve KNS larda vankomisine direnç saptanmadı. Enterokokların % 8’i vankomisine dirençli idi. Gram negatiflere en etkili antibiyotik karbapenemi, amikasin ve siprofloksasin izlemekteydi. P. aeruginosa suşlarının % 5’i ile Acinetobacter türlerinin % 15’i tüm antibiyotiklere dirençli bulundu. Acinetobacter türlerine en etkin antibiyotikler imipenem (% 73), tobramisin (% 46); Pseudomonas türlerine tikarsilin/klavulanat (% 56), piperasilin (% 44), imipenem (% 33); Klebsiella türlerine imipenem (% 92), E. coli suşlarına imipenem (% 94), amikasin (% 94), siprofloksasin (% 74); Enterobacter türlerine siprofloksasin (% 89), imipenem (% 78) idi. Aim: To determine the frequency of nosocomial infections (NIs) and the antimicrobial resistance of pathogens isolated from patients admitted to 4 intensive care units (ICUs) at a university hospital. Materials and Methods: The patients were prospectively followed over 2 years in medical, surgical, and reanimation ICUs. The isolates were collected from patients with NI determined by the Center for Diseases Control and Prevention criteria. The identification was performed and susceptibility to antibiotics was determined in an automated system Sceptor (Becton Dickinson) as described by the CCLS. Results: Among the 3962 patients in the ICUs, total NI incidence was 6.9%, with the highest rate in the reanimation ICU (14.7%) and the lowest rate in the surgical ICU (2.5%). In all, 492 NI episodes were diagnosed in 272 patients. The incidence of NIs was 13.1 per 1000 patient days. The NIs were urinary tract infections (32%), primary blood stream infections (24%), pneumonia (20%), surgical-site infections (13%), and other infections (11%). Pneumonia was the most common NI in the reanimation ICU (54%). The pathogens were 39% gram-positive bacteria, 52% gram-negative bacteria, and 9% Candida species. S. aureus (18%) was the most frequently isolated bacteria, followed by P. aeruginosa (16%), Acinetobacter spp. (10%), coagulase negative staphylococcus (CNS) (10%), Klebsiella (9%), E. coli (9%), and Enterococcus spp. (9%). Resistance to methicillin was 90% in S. aureus isolates and was 95% in the CNS isolates; no resistance was detected to vancomycin in S. aureus and CNS isolates. Eight percent of Enterococcus isolates were resistant to vancomycin. Against the gramnegative bacteria the carbapenems were most active, followed by amikacin and ciprofloxacin. Five percent of P. aeruginosa and 15% of Acinetobacter spp. were resistant to all antibiotics. The most active antimicrobial agents against Acinetobacter spp. were imipenem (73%) and tobramycin (46%); against Pseudomonas spp. were ticarcillin/clavulanate (56%), piperacillin (44%), and imipenem (33%); against Klebsiella spp. was imipenem (92%); against E. coli were imipenem (94%), amikacin (94%), and ciprofloxacin (74%); and against Enterobacter spp. were ciprofloxacin (89%) and imipenem (78%).
