12. SCOLIOSIS SCREENING FROM PLAIN RADIOGRAPHS INCLUDING CHEST AND ABDOMINAL X RAYS BY USING THE DEEP LEARNING METHOD: IS IT WORTH?

Abstract

Objective: This study aimed to use deep learning techniques to discriminate different degrees of scoliosis on plain radiographs. Materials and Methods: The study was performed on 1006 standing plain abdominal and chest radiographs (age range 10-18 years) obtained from the archive of our institution. The radiographs were divided into three groups according to the degree of scoliosis: normal (0-9°), mild (10-29°), and moderate/advanced (30° and above). The data were randomly selected and 15% were used for testing, 15% for validation, and the remaining 70% for training. Due to the limited data, the transfer learning (TL) method was used. Pre-trained VGG-16, ResNet-101, and GoogLeNet networks were used for TL. The original classifier was replaced with a new one. Geometric transformations of the radiographs were used for data augmentation. Rotation (-30, 30 degrees), translation (-30, 30 pixels), and scaling (0.9, 1.1 pixels) were applied to the images. The performance of the networks was evaluated using the performance parameters of accuracy, sensitivity, specificity, precision, and F1 score. Results: Overall accuracy after testing the models was determined to be 90.1% for VGG-16, 86.1% for ResNet-101, and 85.5% for GoogLeNet. The accuracy, sensitivity, specificity, precision, and F1 score were 90.1%, 90.7%, 95.0%, 89.9%, and 90.1% for VGG-16, respectively. The VGG-16 values were determined to be higher than those of the ResNet-101 and GoogLeNet networks. Conclusion: The results showed favorable results for deep TL methods in the assessment of normal, mild, and moderate/advanced scoliosis. [ABSTRACT FROM AUTHOR]

15. SHOULD PELVIC FIXATION BE INCLUDED IN NEUROMUSCULAR SCOLIOSIS SURGERY?

Abstract

Objective: The aim of the current study was to compare the activity levels and radiological outcomes of patients who underwent neuromuscular scoliosis (NMS) surgery with and without pelvic fixation. Materials and Methods: Thirty-three NMS patients aged 10-20 years with a pelvic obliquity (PO) of 15° or more and a follow-up of at least 24 months who underwent posterior surgery for NMS at two different centers were included in the study. Out of the 33 patients, 16 without pelvic fixation (WoPF) and 17 with PF (WPF) underwent posterior spinal surgery. Radiological results and independent movement levels according to the Gross Motor Function Classification System (GMFCS) were compared in the two groups. Results: The follow-up period of the patients was 46.69±21.95 months in WoPF and 43.88±20.05 months in WPF, and there was no significant difference between the two groups in postoperative radiological values (p=0.763). In the PO values, postoperative improvement was more pronounced in the WPF group (WoPF: 14.31°±8.292; WPF: 9.35°±5.338), but there was no statistically significant difference between the two groups (p=0.087). Patients' GMFCS levels were higher in the WPF group than in the WoPF group (WoPF: 2.75±1.29; WPF: 3.76±1.03). GMFCS levels of patients in both groups did not change and were similar to pre-operative levels. Conclusion: The study demonstrated that NMS surgery with PF was not significantly different clinically and radiologically from surgery without PF. Considering PF-related complications in NMS surgery, surgery without PF may be an option in NMS patients with PO. [ABSTRACT FROM AUTHOR]

Abstract

Giriş: Konjenital skolyoz hastalarında tedaviden amaç deformite gelişmesini engellemek ve gelişmiş ise düzeltmek, omurganın büyüme potansiyelini korumak ve büyümesini sağlamak, göğüs kafesi gelişimini sağlamak ve akciğer fonksiyonlarını korumakla beraber hastanın yaşam kalitesini iyileştirmektir. ‘Spinal Appearence Questionnaire(SAQ)’ skolyoza özgü yaşam kalitesi ölçeği geliştirilmiştir. SAQ anketinin başka dillerde AİS hastaları için geçerlilik güvenirlik çalışmaları mevcuttur. Literatürde konjenital skolyoz hastalarının beden algılarını ve beklentilerini değerlendirmek için geliştirilmiş hastalık spespik bir ölçeğe rastlanmamıştır. Bunlara dayanarak bu çalışmada 10 yaş üzeri konjenital skolyoz tanısı alan Türk hastalarda SAQ anketinin Türkçe versiyonunun geçerliliği ile güvenilirliğini değerlendirmeyi ve elde edilen sonuçları diğer dillerdeki çalışmalarla karşılaştırmayı amaçladık.Gereç ve Yöntem: SAQ anketinin Türkçe versiyonu literatürde yayınlanmış ve kabul görmüş kurallara uygun olarak geliştirilmiştir. Takibinde Gazi Üniversitesi Tıp Fakültesi Hastanesinde, cerrahi tedavi uygulanan veya korseli ve korsesiz takip edilen 52 konjenital skolyoz hastasına yaklaşık iki hafta arayla Tr-SAQ ve SRS-22 anketleri uygulanmıştır. Hastaların her iki ölçekten aldıkları puanlar arasındaki ilişki ‘Spearman Korelasyon’ analizi ile incelenmiştir. Anketin geçerliliğine faktör analizi ve benzer ölçek geçerliliği ile bakılmıştır. Anketinin güvenirliliğini belirlemek için Cronbach alfa değeri ile iç tutarlılık analizleri66yapılmıştır. Test-tekrar test güvenirliliği için sınıf içi korelasyon katsayısı incelenmiştir.Bulgular: Çalışmaya dâhil edilen 52 hastanın %73,1’i kadın, katılımcıların yaş ortalaması 17,3±6,1 (min:10; maks:35) ve Cobb açısı ortalaması 42,9±12,6 (min: 20,00; maks: 76,00) derecedir. Anket faktör analizinde ‘Görünüş (Soru 1-10)’ ve ‘Beklenti (Soru 12-15)’ alt grupları olmak üzere iki faktörlü bir yapı göstermiştir. Sınıf içi korelasyon katsayısı (ICC) tüm anket için 0,991 gibi yüksek bir değer bulunmuştur. Anketin iç tutarlılığında, Cronbach alfa değeri oldukça yüksektir (0,883). Anketin Türkçe versiyonun görünüş skoru ile SRS-22 kendi imaj/görünüş skoru arasında anlamlı negatif yönlü kuvvetli bir ilişki saptanmıştır (Spearman rho, -0,516).Sonuç: SAQ anketinin Türkçe versiyonu konjenital skolyoz tanılı hastalar için geçerli ve güvenilir bulunmuştur. Konjenital skolyozlu Türk hastalarda kullanılabilir. Introduction: The main goals of treatment in congenital scoliosis patients are prevention of deformity progression and correction of deformity if advanced, protection of growth potential of the spine and providing its growth, providing development of the thorax and protecting lung functions while improving the quality of life of patients. The "Spinal Appearance Questionnaire (SAQ)" scoliosis-specific quality of life scale has been developed. There are validity and reliability studies of the SAQ questionnaire for AIS patients in other languages. In the literature, there are no disease-specific scales developed to evaluate the body perception and expectations of congenital scoliosis patients. Based on these, we aimed to evaluate the validity and reliability of the Turkish version of the SAQ questionnaire in Turkish patients over 10 years of age with congenital scoliosis, and to compare the results with studies in other languages.Material and methods: The Turkish version of the SAQ questionnaire has been published in the literature and has been developed in accordance with accepted guidelines. Afterwards, with about two weeks apart, Tr-SAQ and SRS-22 questionnaires were applied to 52 congenital scoliosis patients who had undergone surgical treatment or were being followed-up with or without orthoses at Gazi University Medical Faculty Hospital. The relationship between the scores of the patients on both scales was tested by 'Spearman Correlation' analysis. The validity of the questionnaire was examined by factor analysis and similar scale validity. Internal consistency analyzes were performed with the Cronbach alpha value to68determine the reliability of the questionnaire. For test-retest reliability, the intraclass correlation coefficient was examined.Results: Of the 52 patients in the study, 73.1% were female, the mean age of participants were 17.3 ± 6.1 (10-28), and the mean Cobb angle was 42.9° ± 12.06° (Range, 20°-76°). The SAQ showed a two factor structure being, 'Appearance (Items 1-10)' and 'Expectation (Items 12-15)' in the factor analysis. The intraclass correlation coefficient (ICC) was 0.991, which translates to a high correlation. The Cronbach alpha value for internal consistency of the SAQ was significantly high (0.883). There was a significantly negative correlation between the appearance score of the Turkish version of the SAQ and the SRS-22 self image score (Spearman's rho, -0,516).Conclusion: The Turkish version of the SAQ was found to be reliable and valid for patients with congenital scoliosis. It can be used in Turkish congenital scoliosis patients safely.

18. Recommendations for Surgical Interventions during COVID-19 Pandemic

Abstract

Throughout human history, many pandemics have broken out and caused great numbers of deaths. As the technological development made quite a huge advancement by the twentieth century, the medical treatment armamentarium had widened its facilities and resources. This brought with sophisticated and competent health care services for people which provided the medical practitioners with higher success rates when fighting against diseases and improved recovery and survival rates. However, during the COVID-19 pandemic, the high transmission rate of the novel SARS-Cov-2 virus has led to the paralysis of the health system and running out of the resources and healthcare workers. Thus, local government healthcare service regulators have begun building up plans to create management algorithms to use the limited resources more efficiently. This paper presents preliminary practical instructions for healthcare providers as a basic foundation for COVID-19 and potential future pandemics.

19. The implant density does not change the correction rate of the main and the accompanying curves: A comparison between consecutive and intermittent pedicle screw constructs

Abstract

Objective: The aim of this study was to evaluate the clinical outcomes and the coronal correction rate of the main and accompanying curves of adolescent idiopathic scoliosis (AIS) corrected with pedicle screws inserted consecutively or intermittently. Methods: The prospectively collected data of 60 patients (8 men and 52 women; mean age: 14.6±2.5 years) who underwent corrective surgery for AIS between January 2010 and December 2015 were reviewed retrospectively. Two groups were constituted according to the pedicle screw construct type: consecutive pedicle screw construct (CPSC) and intermittent pedicle screw construct (IPSC) groups. The preoperative, early postoperative, and 24-month follow-up radiographs and the Scoliosis Research Society-22 (SRS-22) scores were reevaluated. The Cobb angle of the main and accompanying curves, the correction rate, and the flexibility of the curves were calculated. Results: The mean preoperative Cobb angles were 57.03° and 57.46°, the mean postoperative Cobb angles were 14.93° and 14.4°, and the mean correction rates were 76.22% and 75.31% in IPSC and CPSC groups, respectively (p>0.05). The preoperative and postoperative accompanying curve magnitudes and correction rates were similar (p>0.05). These radiographic outcomes were also consistent with the SRS-22 scores. Conclusion: Both the pedicle screw constructs had satisfactory outcomes following the surgery, which were confirmed by both the SRS-22 scores and radiographs taken perioperatively and at follow-ups. The IPSC and CPSC groups did not demonstrate a significant change in the correction rate of the main and minor or major accompanying structural and nonstructural curves, and also in the SRS-22 scores. 25. Excision and short segment fusion of a double ipsilateral lumbar hemivertebrae associated with a diastematomyelia and fixed pelvic obliquity

Abstract

We report the surgical treatment course of a 4-year-old girl with congenital scoliosis, diastematomyelia and double adjacent hemivertebrae. She had a lumbar curve with an apparent pelvic obliquity. Simultaneous excision of double segmented sequential hemivertebra at the L3-L4 level and fusion with shortsegment instrumentation was performed via a posterior approach. Intraoperative radiographs revealed satisfactory curve correction and 0 degrees pelvic obliquity. Following the excision of double adjacent hemi-vertebrae, three adjacent nerve roots were placed in one intervertebral foramen bilaterally. Nevertheless, no neurological deficit was developed, and the patient was able to ambulate with a brace at day one. Pelvic balance and deformity correction were maintained with no implant failure at the fifth year followup. Excision of two ipsilateral adjacent hemivertebra and short-segment posterior fusion performed via posterior-only approach simultaneously is an effective, safe, and less invasive technique for the treatment of the described case.
