Insulinoma is one of the pancreatic neuroendocrine tumors (PanNET) and is exceptionally rare in children. The tumor leads to severe hypoglycemia caused by excessive insulin release. We report a pediatric patient with malignant insulinoma who underwent liver transplantation (LT) due to liver metastases of the insulinoma. A 13-year-old girl presented with symptoms of hypoglycemia due to hyperinsulinism. On computed tomography (CT), a polycystic lesion in the head of the pancreas and enlarged lymph nodes were revealed. A modified Whipple's operation was performed, and histological examination confirmed PanNET. CT also showed an enlarged liver with numerous metastases. Allogeneic LT was carried out successfully. Positron emission tomography-CT using 68Ga-DOTA-labeled somatostatin analogs (SSAs) at the age of 22 years confirmed complete metabolic remission. The patient currently remains under immunosuppressive and anti-proliferative treatment. Multiple surgical interventions, LT combined with SSAs, and immunosuppressive medication proved effective in this case of metastatic malignant insulinoma. [ABSTRACT FROM AUTHOR]

Introduction: Inevitable disease progression in amyotrophic lateral sclerosis (ALS) forces patients and their caregivers (CGs) to reflect on end-of-life treatment. The CGs are often heavily burdened with their role of surrogate decision-makers. The aim of the study was to analyze attitudes of CGs and presumable attitudes of ALS patients from the CGs' perspective towards palliative care in advanced disease stages. Material and methods: One hundred and sixty four CGs from Germany and Poland were interviewed regarding their own preferences and patients' ideational attitudes towards life-prolonging (invasive and non-invasive ventilation, tube feeding) and life-shortening methods (termination of measures, active measures if permitted by law). The data were correlated with patient- and CG-related factors: demographic and clinical data, care commitment, depression and quality of life (QoL). Results: The CGs were mostly female spouses of ALS patients, with secondary/higher education. Nearly 70% (81% in Poland, 57% in Germany; p = 0.0001) reported positive attitudes towards life-prolonging methods, which positively correlated with religiousness and negatively with patients' age. Approximately 40% of CGs (25% and 51% respectively; p = 0.001) reported positive attitudes towards life-shortening methods. It positively correlated with time since diagnosis and negatively with the CG's QoL, religiosity and religious/spiritual faith as factors that significantly influenced end-of-life decisions. There was a strongly positive correlation between CGs' positive attitudes towards life-shortening methods and presumed positive patients' attitudes assessed by their CGs (p < 0.000001). Conclusions: Although attitudes towards treatment differed between countries, the CGs of ALS patients were generally positive towards life-prolonging treatment. A greater acceptance of life-shortening methods in the case of longer disease duration and poorer QoL may indicate worse coping with disease progression and weaker adaptation mechanisms in CGs compared to those previously reported in ALS patients. A close resemblance of the CGs' answers to probable patients' attitudes reported by the CGs indicates that many GCs might actually express their own culturally shaped attitudes towards end-of-life methods. In light of earlier-reported discrepancies between presumed opinions of the CGs and of patients themselves, a greater focus should be placed on thorough discussions on future treatment options with ALS patients in the presence of their CGs, to stay in line with the patient's authentic will. The following text begins with an introduction that discusses (relevant for the subsequent argument) issues related to biotopic and the role of props in the context of the discussed films. The subsequent part of the argument focuses on the significance of props (such as a desk, a quill, a letter, and a book) in Becoming Jane (2007, dir. Julian Jarrold) and Miss Austen Regrets (2008, dir. Jeremy Lovering). The analysis explores the situation where these objects convey specific information encrypted by the directors, while also allowing for viewer interpretations regarding their functions. The text argues that although it is not possible to present the "truth" about the writer's life, one can still attempt to know and understand her world, and the props depicted in the films play a significant role in this process. Objective: To analyze the quality of life (QoL) and frequency of depression in primary caregivers of patients with amyotrophic lateral sclerosis (ALS) in two neighboring European countries. Methods: a cross-sectional study performed in 164 dyads of ALS patients and caregivers followed at clinical ALS centers in Poland and Germany between 2015 and 2018. The quality of life (Anamnestic Comparative Self-Assessment - ACSA, Quality of Life in Life-Threatening Illness - Family Carer Version - QOLLTI-F) and depression (ALS-Depression-Inventory 12-Items - ADI-12) of the caregivers was assessed and correlated with caregivers- and patient-related factors. Patient's clinical status was assessed by ALS Functional Rating Scale – revised and the Behavioral Score of the Edinburgh Cognitive and Behavioral ALS Screen. Results: the caregivers reported a positive QoL associated with functional and behavioral status of the patient, disease duration and caregivers's depression The most impaired domains of the QoL differed depending on the country of provenance, cultural background and/or social support of the caregivers. Depression was present in 1/3 of the caregivers and was significantly more frequent in the Polish group. It positively correlated with female gender, disease duration, sleep disturbances and functional decline. Both QoL and mood were significantly lower in the caregivers more burdened with the functional care of the patients. Conclusions: the wellbeing of caregivers of ALS patients is affected by patient-related and culture-specific conditions. Understanding the needs and background of psychological adaptation of the caregivers from various countries may translate into better QoL and local patient care. [ABSTRACT FROM AUTHOR]

The region of Upper Silesia, located in southern Poland, is characterised by very high emissions of carbon dioxide into the air—the annual emission exceeds 33 Mt CO2 and the emission 'per capita' is 7.2 t/y in comparison to the EU average emission per capita 6.4 t/y and 8.4 t/y for Poland in 2019. Although in the region there are over 100 carbon dioxide emitters covered by the EU ETS, over 90% of emissions come from approximately 15 large hard coal power plants and from the coke and metallurgical complex. The CCUS scenario for Upper Silesia, which encompasses emitters, capture plants, transport routes, as well as utilisation and storage sites until 2050, was developed. The baseline scenario assumes capture of carbon dioxide in seven installations, use in two methanol plants and transport and injection into two deep saline aquifers (DSA). The share of captured CO2 from flue gas was assumed at the level of 0.25–0.9, depending mainly on the limited capacity of storage. To recognise the views of society on development of the CCUS technologies in Upper Silesia, thirteen interviews with different types of stakeholders (industry, research and education, policy makers) were conducted. The respondents evaluated CCU much better than CCS. The techno-economic assessment of CCUS carried out on a scenario basis showed that the economic outcome of the scenario with CCUS is EUR 3807.19 million more favourable compared to the scenario without CO2 capture and storage. Objectives: Ectopic adrenocorticotropic syndrome (EAS) causes approximately 10–18% of cases of Cushing's syndrome (CS) in adults, while in children it occurs much less frequently. Case presentation: We report two cases of neuroendocrine tumors (of the thymus and the appendix) in a 12-year-old boy and a 15-year-old girl who presented with the clinical features of CS. Elevated serum cortisol, ACTH, and chromogranin levels were observed in both patients. Diagnoses were made on the basis of a mass in the thymus/appendix region visualized with chest/abdominal CT scan and radiotracer accumulation in scintigraphy in the same areas. Histopathological examinations confirmed the diagnoses of NET. Conclusion: EAS is an extremely rare endocrine disorder. However, it should be taken into consideration in the diagnostic process of every case of ACTH-dependent CS. [ABSTRACT FROM AUTHOR]

This work is a continuation and extension of previous socio-economic analyses of hard coal mines, which were conducted at the Central Mining Institute in the years 2013-2015. The paper presents the results of the economic evaluation of the hard coal mining sector in the years 2016-2018 using the Cost-Benefit Analysis (CBA) methodology. Used for the socio-economic assessment of hard coal mining, the CBA methodology enables the comprehensive evaluation of the functioning of this sector of the economy in Poland. In addition to financial aspects, which are important from the point of view of coal companies, it also included the social and environmental influence resulting from the impact of mines on the environment. Direct data of operating costs and payments (including pub lic-law payments), incurred by the hard coal mining industry in Poland, was used. This data is obtained by Industrial Development Agency JSC, Branch Office Katowice as part of the “Program of statistical surveys of official statistics” – statistical survey “Hard coal and lignite mining industry”. They were supplemented with data coming from commonly available public statistics. For the analysed period the presented results indicate that the financial and social benefits resulting from the hard coal mining activity in Poland outweighed the financial, social and environmental costs generated by this industry. This confirms the desirability of further functioning of the hard coal mining industry in Poland, however, assuming effective restructuring activities that will result in lower costs of coal production. The application of Fourier transform infrared spectroscopy to the simultaneous analysis of heavy metal concentration in soil samples was demonstrated in this paper. Two spectral techniques, namely, attenuated total reflectance (ATR) and diffuse reflectance (DRIFT), were applied and the whole infrared spectral region, i.e., far IR, mid IR, and near IR were considered in this work. Spectral data with reference to the results of laboratory analysis enabled the development of calibration partial least squares (PLS) models. The PLS models for the ATR near IR were characterized by a good fit and good prediction abilities. According to the results obtained, the most accurate description and prediction were realized in the case of mid/far and near IR for the mercury and nickel concentration in soil. Application of far IR slightly improved the prediction possibilities of the model. The construction of PLS models based on the Fourier-transform infrared (FT-IR) spectra enables the perception of FT-IR spectroscopy as a supplementary method that is useful in the estimation and monitoring of the contamination level in soils. A universal innovative burner with vertical feeding of fuel was developed. The burner allows for installation in coal-fired boilers of various manufacturers and to meet the requirements related to emission limits and thermal efficiency. The paper presents the results of thermal and emission tests after the application of innovative construction solutions of the retort burner in a water boiler using eco-pea coal and alternative fuel produced in the form of pellets from fine coal and coal sludge. Obtained results were applied to current requirements related to emissivity and thermal efficiency for boilers with automatic fuel hoppers equipped with a retort burner. Combustion in the developed innovative retort burner for eco-coal and alternative fuel pellets allowed compliance with the highest emission classes for gas and dust substances as well as high combustion efficiency. The article focuses on two productions (the TV series Upstart Crow and the feature film Shakespeare in Love), which present the life of the famous playwright in an unusual way. It also looks at matters related to the practice of writing biographies of famous people, as well as biographism, which is treated by the makers of the aforementioned productions in a deceptive and hypothetical way. Finally, the question is asked regarding the function of deception and mystification, which were consciously used in the process of creating Shakespeare's biographies in the two examples analysed.
