Abstract

Chronic kidney disease (CKD) represents a significant global epidemiological challenge, demanding considerable financial resources for treatment. Renal transplantation is the optimal approach for end-stage renal failure, being the most cost-effective option among renal replacement therapies. This narrative review aims to explore clinical conditions associated with excessive healthcare costs among renal transplant recipients, particularly focusing on high-resource users (HRU). We reviewed literature examining conditions generating high costs in kidney transplant patients, including infections, sepsis, pneumonia, antibody-mediated rejection (AMR), graft failure, advanced recipient age, heart failure, and fractures. Immunosuppressive therapies heighten the risk of infections, with sepsis and pneumonia posing significant costs. AMR is a major contributor to healthcare costs, but effective treatment of AMR can extend graft longevity and improve patient outcomes. Graft failure significantly increases medical expenses and adversely affects patient outcomes. Older recipients face higher post-transplant morbidity and mortality rates, though transplantation still offers better long-term survival compared to dialysis. Heart failure and fractures further elevate post-transplant costs and underscore the necessity of targeted interventions to mitigate associated risks. Ensuring kidney transplant care is sustainable and accessible requires a comprehensive strategy. This approach aims to improve patient outcomes while keeping costs reasonable.

Abstract

Introduction: Physical activity is associated with a reduced risk of chronic diseases, including cardiovascular disease, obesity, type 2 diabetes, and cognitive decline. Exercise supports brain health and cognitive function, with potential benefits for neuroplasticity—the brain's ability to reorganize in response to demands and injury. Materials and methods: A literature review was conducted using Pubmed and Google Scholar database with following search terms: “physical activity”, “neurogenesis”, “BDNF”, “neuroplasticity” and “cognitive function”. State of knowledge: Neuroplasticity, the brain’s ability to reorganize and form new connections in response to learning or injury, is supported at the molecular level by various factors (e.g. BDNF or VEGF). Aerobic and resistance exercises have been linked to increased gray and white matter volumes, especially in regions critical to memory and executive function. These effects are valuable in preventing cognitive decline and improving mental health. Current research has also shown that the impact of exercise on neuroplasticity can be augmented through consistent, high-intensity activity, which supports neuroplasticity through hormone modulation. Furthermore, emerging technologies like neuroimaging have advanced our understanding of the mechanisms underlying exercise-induced brain adaptation, which holds promise for personalized interventions. Conclusions: This review focuses on the preventive and therapeutic potential of physical activity for neurological health, highlighting the need for continued research into how different forms of physical activity can influence neuroplasticity.

Abstract

Introduction: The ketogenic diet, characterized by high fat and low carbohydrate intake, has become increasingly popular among athletes and fitness enthusiasts due to its potential for supporting weight loss and enhancing metabolic health. This study aims to analyze the impact of the keto diet on physical and cognitive performance, as well as to understand the mechanisms and challenges associated with its use. Materials and methods: A systematic review of available studies was conducted through extensive searches in databases such as PubMed, NCBI, and Google Scholar. State of knowledge: The ketogenic diet can be beneficial in endurance sports by improving fat utilization as an energy source and increasing energy stability, but its impact on aerobic capacity and time to exhaustion varies. In strength sports that require intense effort, carbohydrate restriction can negatively affect strength and muscle mass development, as glycogen is a key energy source in such activities. The ketogenic diet does not provide full benefits in high-intensity sports, such as sprints or team sports, due to limited glycogen availability. The ketogenic diet can improve body composition and metabolic efficiency in athletes, but its impact on performance depends on individual responses, and the adaptation process may involve challenges such as "keto flu." Ketogenic diet may positively impact mental health by reducing symptoms of anxiety and depression. Additionally, it may enhance sleep quality, particularly by supporting deep sleep, and potentially increase testosterone production. Conclusions: The ketogenic diet can benefit endurance sports by improving fat oxidation and sustaining energy levels, though its effectiveness in high-intensity, glycogen-dependent activities is less certain. Strategies like TKD or CKD, along with performance monitoring, can help athletes optimize results.

Abstract

ABSTRACT Introduction: Digestive system diseases, including IBD, and GERD, are prevalent health concerns worldwide. Physical activity, widely acknowledged for its health benefits, has emerged as a promising area of research for its potential in mitigating symptoms and influencing risk factors associated with digestive system diseases. This study aims to critically assess existing evidence on the role of physical activity in the prevention, symptom management, and progression of prevalent digestive disorders. Materials and Methods: A systematic review of pertinent studies was conducted through comprehensive searches in databases including PubMed, NCBI and Google Scholar. The data were sourced from clinical and experimental studies, evaluated for methodological rigor, participant characteristics, and therapeutic outcomes. State of knowledge: The analysis demonstrated that physical activity can exert beneficial effects on digestive health through mechanisms such as enhanced gut motility, reduced inflammation, and increased microbiota diversity. Moderate-intensity aerobic exercise was associated with reduced risk and symptom improvement in patients with IBS and IBD, while high-intensity exercise showed variable effects, with potential symptom exacerbation in certain cases. Additionally, exercise correlated with a lower incidence of GERD, likely due to benefits in weight management. Despite these positive associations, inconsistencies in study design and exercise protocols underscore the need for standardized guidelines on physical activity tailored to specific digestive conditions. Conclusions: Physical activity appears to play a beneficial role in managing and potentially preventing certain digestive system diseases. While moderate exercise shows promise in symptom alleviation and risk reduction, further research is necessary to establish clear, condition-specific exercise recommendations.

Abstract

We present a determination of the gradient flow scales $w_0$, $\sqrt{t_0}$ and $t_0/w_0$ in isosymmetric QCD, making use of the gauge ensembles produced by the Extended Twisted Mass Collaboration (ETMC) with $N_f=2+1+1$ flavours of Wilson-clover twisted-mass quarks including configurations close to the physical point for all dynamical flavours. The simulations are carried out at three values of the lattice spacing and the scale is set through the PDG value of the pion decay constant, yielding $w_0=0.17383(63)$ fm, $\sqrt{t_0}=0.14436(61)$ fm and $t_0/w_0=0.11969(62)$ fm. Finally, fixing the kaon mass to its isosymmetric value, we determine the ratio of the kaon and pion leptonic decay constants to be $f_K/f_{\pi}=1.1995(44)$., Comment: arXiv admin note: text overlap with arXiv:2104.06747

Abstract

We present a determination of the ratio of kaon and pion leptonic decay constants in isosymmetric QCD (isoQCD), $f_K / f_\pi$, making use of the gauge ensembles produced by the Extended Twisted Mass Collaboration (ETMC) with $N_f = 2 + 1 + 1$ flavors of Wilson-clover twisted-mass quarks, including configurations close to the physical point for all dynamical flavors. The simulations are carried out at three values of the lattice spacing ranging from $\sim 0.068$ to $\sim 0.092$ fm with linear lattice size up to $L \sim 5.5$~fm. The scale is set by the PDG value of the pion decay constant, $f_\pi^{isoQCD} = 130.4~(2)$ MeV, at the isoQCD pion point, $M_\pi^{isoQCD} = 135.0~(2)$ MeV, obtaining for the gradient-flow (GF) scales the values $w_0 = 0.17383~(63)$ fm, $\sqrt{t_0} = 0.14436~(61)$ fm and $t_0 / w_0 = 0.11969~(62)$ fm. The data are analyzed within the framework of SU(2) Chiral Perturbation Theory (ChPT) without resorting to the use of renormalized quark masses. At the isoQCD kaon point $M_K^{isoQCD} = 494.2~(4)$ MeV we get $(f_K / f_\pi)^{isoQCD} = 1.1995~(44)$, where the error includes both statistical and systematic uncertainties. Abstract

Generative adversarial networks (GANs) provide state-of-the-art results in image generation. However, despite being so powerful, they still remain very challenging to train. This is in particular caused by their highly non-convex optimization space leading to a number of instabilities. Among them, mode collapse stands out as one of the most daunting ones. This undesirable event occurs when the model can only fit a few modes of the data distribution, while ignoring the majority of them. In this work, we combat mode collapse using second-order gradient information. To do so, we analyse the loss surface through its Hessian eigenvalues, and show that mode collapse is related to the convergence towards sharp minima. In particular, we observe how the eigenvalues of the $G$ are directly correlated with the occurrence of mode collapse. Finally, motivated by these findings, we design a new optimization algorithm called nudged-Adam (NuGAN) that uses spectral information to overcome mode collapse, leading to empirically more stable convergence properties.

33. Amnion as an Innovative Antiseptic Carrier: A Comparison of the Efficacy of Allogeneic and Xenogeneic Transplantations in the Context of Burn Therapy

Abstract

Background and Objectives: The amniotic membrane is widely used in the treatment of chronic wounds, in toxic epidermal necrolysis (TEN), and in the treatment of burns. In our clinical practice, we use amniotic dressings on shallow skin wounds caused by burns. Counteracting infections is an important aspect of working with burn wounds. Therefore, the main goals of this work are to demonstrate the usefulness of amniotic membrane soaked in antiseptics for the prevention of wound infections and to compare the antibacterial efficacy of selected variants of allogeneic and xenogeneic amniotic membrane grafts soaked in specific antiseptic agents. Materials and Methods: The studied material consisted of human and pig placenta. The human and animal amnions were divided in two parts. The first part consisted of amniotic discs placed on rigid mesh discs and preparing the fresh amnion. The second part of the amnion was frozen at a temperature of −80 °C for 24 h. Then, it was radio-sterilized with a dose of 35 kGy. The amniotic discs were placed on rigid mesh to prepare the radiation-sterilized amnion. The amniotic discs were placed in a 12-well plate and immersed in 3 mL of the appropriate antiseptic solutions: Prontosan, Braunol, Borasol, Microdacyn, Octenilin, Sutrisept, and NaCl as a control. The amniotic discs were incubated in antiseptics for 3 h. The microbiological tests were conducted by placing the antiseptic-infused amniotic discs on microbiological media inoculated with hospital strains. Results: The largest average zone of growth inhibition was observed in dressings soaked with Sutrisept, Braunol, and Prontosan. The greatest inhibition of bacterial growth was achieved for radiation-sterilized porcine amnion impregnated with Braunol and Sutrisept, as well as for radiation-sterilized human amnion impregnated with Braunol. Conclusions: Human and porcine amniotic membrane is effective in carrying antiseptics. Radiation-sterilized amnion seems to inhibit the growth of microorganisms better than fresh amnion.

We investigate the asymptotic safety scenario for a scalar-gravity system. This system contains two avatars of the dynamical Newton coupling, a gravitational self-coupling and a scalar-graviton coupling. We uncover an effective universality for the dynamical Newton coupling on the quantum level: its momentum-dependent avatars are in remarkable quantitative agreement in the scaling regime of the UV fixed point. Abstract

The human brain displays a complex network topology, whose structural organization is widely studied using diffusion tensor imaging. The original geometry from which emerges the network topology is known, as well as the localization of the network nodes in respect to the brain morphology and anatomy. One of the most challenging problems of current network science is to infer the latent geometry from the mere topology of a complex network. The human brain structural connectome represents the perfect benchmark to test algorithms aimed to solve this problem. Coalescent embedding was recently designed to map a complex network in the hyperbolic space, inferring the node angular coordinates. Here we show that this methodology is able to unsupervisedly reconstruct the latent geometry of the brain with an incredible accuracy and that the intrinsic geometry of the brain networks strongly relates to the lobes organization known in neuroanatomy. Furthermore, coalescent embedding allowed the detection of geometrical pathological changes in the connectomes of Parkinson's Disease patients. The present study represents the first evidence of brain networks' angular coalescence in the hyperbolic space, opening a completely new perspective, possibly towards the realization of latent geometry network markers for evaluation of brain disorders and pathologies.
